Obstructive sleep apnea (OSA) syndrome is a condition characterized by the presence of repeated complete or partial collapse of the upper airways during sleep associated with episodes of intermittent hypoxia, leading to fragmentation of sleep, sympathetic nervous system activation, and oxidative stress. To date, one of the major aims of research is to find out a simplified non-invasive screening system for this still underdiagnosed disease. The Berlin questionnaire (BQ) is the most widely used questionnaire for OSA and is a beneficial screening tool devised to select subjects with a high likelihood of having OSA. We administered the original ten-question Berlin questionnaire, enriched with a set of questions purposely prepared by our team and completing the socio-demographic, clinical, and anamnestic picture, to a sample of Italian professional nurses in order to investigate the possible impact of OSA disease on healthcare systems. According to the Berlin questionnaire, respondents were categorized as high-risk and low-risk of having OSA. For both risk groups, baseline characteristics, work information, clinical factors, and symptoms were assessed. Anthropometric data, work information, health status, and symptoms were significantly different between OSA high-risk and low-risk groups. Through supervised feature selection and Machine Learning, we also reduced the original BQ to a very limited set of items which seem capable of reproducing the outcome of the full BQ: this reduced group of questions may be useful to determine the risk of sleep apnea in screening cases where questionnaire compilation time must be kept as short as possible.

Objective: This study was undertaken to assess the impact of immunosuppressive and immunomodulatory therapies on the severity of coronavirus disease 2019 (COVID-19) in people with multiple sclerosis (PwMS). Methods: We retrospectively collected data of PwMS with suspected or confirmed COVID-19. All the patients had complete follow-up to death or recovery. Severe COVID-19 was defined by a 3-level variable: mild disease not requiring hospitalization versus pneumonia or hospitalization versus intensive care unit (ICU) admission or death. We evaluated baseline characteristics and MS therapies associated with severe COVID-19 by multivariate and propensity score (PS)-weighted ordinal logistic models. Sensitivity analyses were run to confirm the results. Results: Of 844 PwMS with suspected (n = 565) or confirmed (n = 279) COVID-19, 13 (1.54%) died; 11 of them were in a progressive MS phase, and 8 were without any therapy. Thirty-eight (4.5%) were admitted to an ICU; 99 (11.7%) had radiologically documented pneumonia; 96 (11.4%) were hospitalized. After adjusting for region, age, sex, progressive MS course, Expanded Disability Status Scale, disease duration, body mass index, comorbidities, and recent methylprednisolone use, therapy with an anti-CD20 agent (ocrelizumab or rituximab) was significantly associated (odds ratio [OR] = 2.37, 95% confidence interval [CI] = 1.18–4.74, p = 0.015) with increased risk of severe COVID-19. Background In Crohn's disease (CD) patients, stress is believed to influence symptoms generation. Stress may act via central nervous system pathways to affect visceral sensitivity and motility thus exacerbating gastrointestinal symptoms. The neural substrate underpinning these mechanisms needs to be investigated in CD. We conducted an explorative functional magnetic resonance imaging (fMRI) study in order to investigate potential differences in the brain stress response in CD patients compared to controls. Methods 17 CD patients and 17 healthy controls underwent a fMRI scan while performing a stressful task consisting in a Stroop color-word interference task designed to induce mental stress in the fMRI environment. Key Results Compared to controls, in CD patients the stress task elicited greater blood oxygen level dependent (BOLD) signals in the midcingulate cortex (MCC). Conclusions & Inferences The MCC integrate “high” emotional processes with afferent sensory information ascending from the gut. In light of these integrative functions, the stress-evoked MCC hyperactivity in CD patients might represent a plausible neural substrate for the association between stress and symptomatic disease. The MCC dysfunction might be involved in mechanisms of central disinhibition of nociceptive inputs leading to amplify the visceral sensitivity. Finally, the stress-evoked MCC hyperactivity might affect the regulation of intestinal motility resulting in exacerbation of disease symptoms and the autonomic and neuroendocrine regulation of inflammation resulting in enhanced inflammatory activity.

Ulcerative colitis (UC) is associated with psychological stress and poor emotional functioning. The neural emotional processing involves the complex integration of several cortical and subcortical brain structures. The amygdala plays a fundamental role in the neural processing of emotional stimuli and is a core structure of the brain-gut axis (BGA) that represents the anatomo-functional substrate for the bidirectional influences between emotions and gastrointestinal functions. The aim of this study was to investigate the brain emotional processing in UC patients compared to healthy people. METHODS: Ten UC patients in remission and 10 matched healthy controls underwent a functional magnetic resonance imaging (fMRI) scan while performing a task involving emotional visual stimuli. A set of negative, positive, and neutral pictures were used to study brain-related emotional responses. RESULTS: A significantly reduced blood oxygen level-dependent (BOLD) signal in UC patients relative to controls was found in the amygdala, thalamic regions, and cerebellar areas (P < 0.05 corrected for multiple comparisons). The group-related differences were detected in the brain activity in response to positive emotional stimuli. CONCLUSIONS: UC is associated with an emotional dysfunction characterized by decreased sensitivity to emotions with a positive content. The previous intestinal inflammatory activity in UC patients might have contributed to determine the functional changes of the amygdala that we found. On the other hand, the dysfunction of the amygdala may influence the course of the disease.

The management of public transport for rebalancing the use of transportation systems is a useful tool for reducing negative externalities without excessively affecting zone accessibility. In this context, a rail or metro system can be a key element for producing a high-quality supply of public transport. Obviously, due to the great vulnerability of rail technology to system failures, it is necessary to develop suitable tools to identify rapidly, even with off-line procedures, the best operational strategies which minimise user discomfort produced by such failures. Hence, the authors proposal is to extend previous models proposed in the literature by considering travel demand as an outcome of a random variable and not only in terms of average values. The proposed approach is applied in the case of a real dimension metro network, considering a wider class of failure contexts.

There is a strong association between nose morphology and personal identity, and we must bear in mind that in many patients, the most important expectation from rhinoplasty is not only an improved facial harmony but an existential change. The authors discuss the psychological aspects of rhinoplasty including expectations and motivations, body image disorders, short- and long-term outcome of rhinoplasty for medical or cosmetic indication, and psychosocial outcomes for patients seeking cosmetic surgery. The general body of research seems to confirm that in general, there is a positive outcome for patients’ psychological well-being and psychosocial functioning after undergoing elective cosmetic surgery; however, there are also multiple indications of poor outcomes that cannot be ignored.

Background Crohn’s disease (CD) is a chronic intestinal disorder characterized by overproduction of inflammatory cytokines and recurrent abdominal pain. Recently, brain morphological abnormalities in the pain matrix were found in patients with chronic pain disorders including irritable bowel syndrome. To investigate potential structural brain changes associated with CD, we used magnetic resonance imaging (MRI). Furthermore, we tested whether in patients gray matter (GM) volumes correlated with disease duration. Methods Eighteen CD patients in remission and 18 healthy controls underwent structural MRI. Voxel-based morphometry (VBM) is a fully automated technique allowing identification of regional differences in the amount of GM enabling an objective analysis of the whole brain between groups of subjects. VBM was used for comparisons and correlation analysis. Key Results With respect to controls, CD patients exhibited decreased GM volumes in portion of the frontal cortex and in the anterior midcingulate cortex. Disease duration was negatively correlated with GM volumes of several brain regions including neocortical and limbic areas. Conclusions & Inferences Crohn’s disease is associated with brain morphological changes in cortical and subcortical structures involved in nociception, emotional, and cognitive processes. Our findings provide new insight into the brain involvement in chronic inflammatory bowel disorders

Se l’esplorazione della Grotta del Re Tiberio, la più importante cavità carsica della Vena del Gesso Romagnola, può dirsi praticamente conclusa, praticamente nulla è stato ancora fatto per definirne l’evoluzione nel tempo di alcune sue forme e sedimenti peculiari. L’analisi di dettaglio delle forme erosivo-dissolutive e dei sedimenti chimici e fisici presenti in due piccole gallerie relitte all’interno della Cava di Monte Tondo e facenti parte del sistema carsico del Re Tiberio, ha permesso di ricostruire la loro storia evolutiva, che ha comportato anche lo sviluppo di alcuni rari speleotemi, tra cui i poolfinger di calcite, i cristalli di gesso epitassiali e gli orli di ossidi di ferro e di gesso secondario che bordano i cristalli della matrice gessosa esposti nelle pareti della grotta, nonché la presenza di cristallizzazioni effimere di epsomite.

Dal 2010 è in corso il Progetto Life+ 08NAT/IT/000369 “Gypsum” finalizzato alla tutela e gestione dei principali ambienti gessosi dell’Emilia-Romagna e coofinanziato dall’Unione Europea. Nell’ambito dell’Azione A3 è previsto un monitoraggio pluriennale dei principali acquiferi carsici sotto l’aspetto chimico e microbiologico. Vengono brevemente illustrati i primi risultati delle analisi chimiche effettuate sulle acque della Risorgente del Re Tiberio e quelle della Risorgente di Ca’ Boschetti, oltre che quelle della piccola risorgente di Caronte, situata in riva sinistra del Senio.

Se l’esplorazione dei fenomeni carsici profondi di Monte Tondo (Vena del Gesso Romagnola) può dirsi praticamente conclusa, poco invece è stato fatto per definirne globalmente l’evoluzione speleogenetica nel tempo. La presenza di alcuni livelli suborizzontali ben sviluppati, non solo all’interno della Grotta del Re Tiberio, ma anche in molte altre cavità facenti parte dello stesso sistema, ha permesso, per la prima volta, di tentare la correlazione di questi livelli con i terrazzi fluviali della valle del Senio (corrispondenti ai livelli di base carsici del periodo) e quindi di definire a grandi linee l’evoluzione temporale di questo carsico, che, se si tralasciano i piccoli e rari fenomeni intramessiniani, è di gran lunga con i suoi oltre 150.000 anni, la più antica cavità nei gessi dell’Emilia Romagna. Lo studio ha permesso anche di evidenziare come, nelle grotte in gesso, la velocità di adattamento delle forme carsiche ipogee alle variazioni climatiche esterne sia molto rapido e in particolare le gallerie antigravitative si possano correlare con i periodi di aggradazione dei terrazzi nel periodo immediatamente successivo ad una fase fredda, di cui quindi possono essere considerati degli indicatori.

In patients with Crohn's disease (CD) stress is believed to increase the incidence of disease relapse. The brain processes stressful stimuli and triggers the stress-evoked responses. Habituation to stress is an adaptive process that allows minimizing these responses. We hypothesized inadequate habituation to stress in CD patients. The aim of this study was to compare the neural habituation between CD patients and controls. Twenty CD patients and eighteen controls underwent a functional magnetic resonance imaging while performing two repeated runs of a stress-evoking task. The task elicited different neural activity between the groups across runs in (1) amygdala, hippocampus, (2) insula, putamen (3) cerebellar regions, suggesting altered habituation to stress in patients. These structures regulate the neuroendocrine and autonomic stress-evoked responses that control the proinflammatory responses. The inadequate habituation to stress that we found in patients could play a role in the relationship between stress and inflammatory exacerbations in CD.

Aim: The purpose of this study is to assess past and present attachment relationships in outpatients affected by Inflammatory Bowel Disease (IBD) as Ulcerative Colitis and Crohn’s Disease. Methods: 664 subjects were divided in a experimental sample of 314 IBD outpatients (157 males, age from 16 to 78, m: 43.14) and a control sample of 350 non-pathological subjects, similar for sex and age, without IBD-affected relatives. In all cases the following questionnaires were submitted: 1) Parental Bonding Instrument (PBI) by Parker, et al. (1979) for the study of parental relationships in the first 16 years; 2) Attachment Style Questionnaire (ASQ) by Feeney et al.(1994) to assess some dimensions of present attachment style. Recent studies have shed light on the biological foundations of Basic Emotional Systems that can be found both in animals and in human beings. The aim of our study is to reveal the associations that exist between the Basic Emotions making up part of the personality structure, the relationship-based emotional regulatory system, i.e. Attachment Style, and the influence of these on the manifestation and expression of psychological distress. Our sample consisted of 110 subjects (M = 34, F = 76) , of whom 50 suffered from a stroke and 60 suffered from bone fractures; the average age was 72.67 (SD = 11.24). Participants were administered the following questionnaires: Feeney et al.’s Attachment Style Questionnaire (Fossati et al., 2003, Jour. Soc. Pers. Rel. 20(1), 55-79) in order to assess adult attachment styles; Zigmond and Snaith’s Hospital Anxiety and Depression Scale (Costantini et al., 1999, Support Care Cancer. 7:121–127) in order to assess the intensity of anxiety and depression; the Affective Neuroscience Personality Scales (Davis et al., 2003, Neuro-Psychoanalysis, 5(1), 57-69) in order to evaluate the Basic Emotional Systems. Notwithstanding the advanced age of the subjects, the analysis indicated the existence of a statistically significant relationship between the Attachment Style (ASQ) and the Basic Emotional Systems of the personality structure (ANPS). In particular, we found a positive correlation between Secure Attachment Style and positive emotions, while the Avoidant Attachment Style seemed to modulate only partly adaptive emotions and psychological distress (HADS).

30 subjects with BJHS (Hypermobile) ( M age = 32.3 yr., SD = 10.4) and two control groups, 25 healthy subjects (Healthy) ( M age = 33.9 yr., SD = 93) and 30 fibromyalgic patients (Fibromyalgic) ( M age = 32.2 yr., SD = 9.4), were given the Symptom Checklist-90-R, the Illness Behavior Questionnaire, the Female Functional Symptoms Frequency (FFF) and the Male Functional Symptoms Frequency (MFF), derived from the DSM-III–R, evaluating functional somatic disturbances and their intensity, and the Symptom Questionnaire. The mean scores for the Hypermobile group showed significant psychological distress and increased frequency and intensity of somatic symptoms. Scores on Anxiety and Somatic Symptoms, General Hypochondriasis, Disease Conviction, Affective Disturbance, Denial, and Irritability were significantly higher in the Hypermobile than in the Healthy group. Elevated scores were found for the Fibromyalgic group on the Illness Behavior Questionnaire subscales for Psychological vs Somatic Focus, Disease Affirmation, and Discriminating Factors when compared with the Hypermobile group. Considerable emotional symptoms were detected which should not be underestimated by physicians when establishing an integrated biopsychosocial therapy.

Una stretta relazione lega attaccamento, malattia, e disagio psicologico: il pattern di attaccamento influisce su: percezione dei sintomi, ricerca di assistenza sanitaria (Ciechanowsky, 2002), vulnerabilità alla malattia, comportamento di malattia (Maunder e Hunter, 2001) e modo di affrontare crisi e nuovi adattamenti manifestati dal paziente. Soggetti sicuri, con schemi di Sé e degli atri positivi, hanno una migliore capacità di resilience rispetto agli insicuri (Bartholomew e Horowitz, 1991). Nella clinica riabilitativa, il familiare di riferimento svolge un ruolo attivo e partecipa dei processi mentali del paziente. Esistono diverse modalità di caregiving (Feeney, 1996): soggetti con stile sicuro tendono ad essere più empatici, responsivi e attivi nel fornire aiuto degli insicuri. Ci si propone di indagare con appositi strumenti psicometrici lo stile di attaccamento, il disagio psichico, il recupero funzionale e le loro relazioni, in pazienti con esiti di ictus ospedalizzati e relativi familiari di riferimento rispetto ad un gruppo di controllo costituito da pazienti con esiti di frattura e loro familiari di riferimento. Si ipotizza che i soggetti con stile di attaccamento sicuro presentino livelli di disagio psicologico inferiori rispetto a quelli con stile insicuro, e livelli di indipendenza funzionale maggiori; inoltre, si ipotizza che una relazione in cui paziente e familiare manifestano strategie di attaccamento sicuro sia correlata ad un esito migliore del ricovero rispetto a quelli con stile insicuro; infine possono esserci differenze tra pazienti con lesioni all’emisfero destro, sinistro e con i pazienti ortopedici. In the practice of clinical rehabilitation, the caregiver plays an active and participating role in the patient’s mental processes. Moreover, the attachment style of the caregiver is associated with a particular type of care (Feeney, 1996). Caregivers with a secure style tend to be empathic and to actively offer assistance, while those with an insecure style exhibit different types of non-responsive caregiving, depending on their particular type of insecurity. We hypothesise that subjects with a secure attachment style will exhibit lower levels of psychological distress and higher levels of functional independence compared to those with an insecure style. We further expect that a relationship between patient and caregiver characterised by secure attachment strategies will be correlated with a more positive outcome of recovery compared to relationships based on an insecure style. Finally, differences may be found between patients with lesions of the right hemisphere, those with lesions of the left hemisphere and orthopaedic patients. Description of the sample: The study excludes patients with severe cognitive deficits. The sample consists of 35 stroke patients (M = 66%, F = 34%; average age = 69 with SD = 9.7; married = 66%) and 30 patients with fractures (M = 27%, F = 73%; average age = 75 with SD = 9.3; married = 50%), as well as the patients’ caregivers. In the neurological patients, the cerebral lesion is located in the right hemisphere in 60% of the cases, while most of the strokes are ischaemic in nature (66%). A share of 70% of the patients with right-hemisphere lesions exhibits modifications in cognitive functioning (neglect: 38%). Among the patients with lesions of the left hemisphere, around 60% show alterations in the cognitive profile (aphasia: 43%). A share of 50% of the orthopaedic patients (74% of the cases with femoral fractures) exhibits modifications of the cognitive profile. The caregivers of the post-stroke patients (spouse: 43%, son or daughter: 46%) are more frequently women (57%) with an average age of 53; 74% are married. Men and women are equally represented among the caregivers of the orthopaedic patients (spouse: 47%, son or daughter 50%); these caregivers have an average age of 61, and 80% are married. Tools and methodology: The HAD (Hospital Anxiety and Depression scale by Zigmond and Snaith, 1983), with separate subscales for anxiety and depression, was used to assess the psychological distress of the patients on admission and on discharge. The patients’ level of functional independence was also tested on admission and discharge by means of the FIM scale (Functional Independence Measurement, by Dodds et al., 1993). The CES-D scale (Center of Epidemiologic Studies Depression scale, by Radloff, 1977 in the Italian form validated by Fava, 1982) was used to measure the presence and degree of depression in the caregivers, likewise on admission and discharge. The attachment style of both patients and caregivers was identified using RQ (Relationship Questionnaire by Bartholomew and Horowitz, 1991) and ASQ (Attachment Style Questionnaire by Feeney, 1996 in the Italian version validated by Fossati et al., 2003). The ASQ evaluates attachment style according to five key factors: “confidence discomfort with closeness”, “need for approval”, preoccupation with relationships” and “relationships as secondary”. The preliminary findings appear to confirm most of the hypotheses formulated above.

Lo studio dell’attaccamento, offre un interessante spunto di approfondimento per l’analisi dei vissuti psicologici dei pazienti in riabilitazione a causa di deficit cognitivi e motori e dei loro familiari, poiché in condizioni emotive come quelle descritte, gli individui tendono a riattivare e ad accentuare l’insieme dei comportamenti tipici del legame di attaccamento (ricerca di vicinanza, bisogno di sicurezza e di protezione, protesta per la separazione). Pertanto l’attaccamento, che si manifesta negli stili rappresentazionali connessi al mantenimento dei sentimenti di sicurezza e protezione, contribuisce a comprendere l’adattamento globale del soggetto a tale condizione di bisogno e di pericolo.

Il lavoro si pone l’obiettivo di: 1) analizzare le caratteristiche e i cambiamenti nelle condizioni cliniche, in particolare ansia e depressione, di pazienti in riabilitazione e relativi familiari di riferimento, e se queste sono correlate con il recupero funzionale dei pazienti stessi; 2) stabilire la presenza o meno di una sofferenza psicologica maggiore nel gruppo di pazienti con esiti di ictus e dei loro familiari rispetto ai pazienti ortopedici e relativi congiunti; 3) approfondire la distribuzione dei diversi stili di attaccamento nei soggetti del campione, e la loro influenza sull’entità del disagio psichico di pazienti e familiari.

La ricerca esplora le dinamiche triadiche nelle famiglie con figli nati pretermine. Diversi studi hanno analizzato gli stili di attaccamento in campioni di madri-bambini prematuri con risultati discordanti e difficilmente comparabili. In particolare i dati disponibili relativi ai padri sono molto scarsi. Questo studio indaga lo sviluppo dell’attaccamento genitore-bambino e la presenza di fattori di rischio psicologico in famiglie con nati pretermine. I risultati evidenziano all’interno delle famiglie con nati pretermine lo sviluppo dell’attaccamento significativamente a rischio fin dalle prime settimane di vita. La ricerca evidenzia la necessità di un intervento psicologico sull'intera famiglia del neonato e non solo sulla diade madre-bambino.

Questo studio intende valutare la diffusione sul territorio italiano dei gruppi di auto-aiuto come tecnica di intervento per i Disturbi del Comportamento Alimentare (DCA). Da un’indagine telefonica condotta su 90 Centri Ospedalieri, Cliniche Private o Pubbliche e Associazioni che si occupano di terapia dei DCA, è risultato che solo 9 strutture (10% del campione) hanno attivato dei gruppi di self-help (19 in totale), per questa tipologia di disturbi. I facilitatori individuati hanno compilato un questionario appositamente approntato e successivamente sono stati sottoposti a un’intervista. Secondo i dati raccolti i gruppi sono costituiti da 5-14 persone (quelli con pochi partecipanti sono ritenuti i più funzionanti), prevalentemente femmine di età dai 14 ai 70 anni, ogni incontro dura mediamente un ora e mezza, con frequenza settimanale. La comunicazione viene descritta come libera, empatica e partecipata con una leadership democratica. Lo scopo è la riduzione della sintomatologia attraverso la condivisione delle informazioni. Gli elementi caratterizzanti sono: la presenza dell’helper, il senso di appartenenza, quello di fiducia e il gruppo come fonte di sostegno emotivo. Quasi l’80% dei gruppi collabora con il sistema sanitario pubblico. In conclusione, in Italia, i gruppi di auto-aiuto non sono utilizzati frequentemente nel trattamento dei DCA, anche se possono risultare una metodologia utile all’interno di protocolli terapeutici (medici e psicologici) più complessi.

La sindrome OSAS è un disturbo frequente caratterizzato dall’insorgere di apnee durante il sonno. Questa ricerca ipotizza che tali pazienti presentino livelli elevati di sonnolenza diurna, di percezione dello stress, di depressione e un’alterazione del comportamento di malattia rispetto ai controlli. Il campione studiato è composto da 96 soggetti (32 pazienti ospedalizzati con diagnosi OSAS; 32 soggetti ricoverati nello stesso reparto per un disturbo differente e 32 soggetti sani). Gli strumenti utilizzati sono: l’ESS per misurare la sonnolenza diurna; l’IBQ per il comportamento di malattia; il CES-D per la depressione, il PSQ per lo stress percepito e un Questionario Clinico appositamente approntato per la ricerca. I pazienti OSAS hanno presentato livelli significativamente elevati di sonnolenza diurna e stress percepito rispetto ad entrambi gli altri gruppi e maggiori sintomi depressivi rispetto al solo gruppo dei pazienti ricoverati. All’IBQ i pazienti OSAS hanno mostrato, rispetto a quelli ospedalizzati, punteggi significativamente elevati alle scale di Irritabilità, Affermazione di Malattia e Funzione Discriminante, e si sono differenziati dai soggetti sani rispetto alla Convinzione di Malattia. Il questionario clinico ha evidenziato che questi pazienti lamentano, con l’insorgere del disturbo, un calo delle capacità di attenzione, concentrazione e abilità manuale, difficoltà in ambito lavorativo, sociale, familiare e sessuale e un incremento nell’uso di sostanze eccitanti.

Questo studio si propone di valutare l’impatto della patologia oncologica pediatrica sulla relazione Padre-Madre-Figlio. La ricerca, svolta su un campione di 37 famiglie, ha confrontato 3 gruppi: 8 famiglie con un bambino affetto da patologia oncologica, 15 con un figlio affetto da diabete e 14 con figli sani. Per la valutazione è stato utilizzato il test FACES III di Olson, Portner e Lavee (1982), che descrive il funzionamento della famiglia attraverso le percezioni che le persone hanno delle proprie relazioni familiari. L’analisi dei dati ha evidenziato che i padri dei bambini affetti da patologie oncologiche sono quelli maggiormente soddisfatti del funzionamento del proprio nucleo familiare, mentre quelli con figli diabetici sono risultati i più insoddisfatti. Analizzando la struttura familiare interna sono emerse alcune tendenze significative: la coppia genitoriale con bambini oncologici appare, rispetto ai controlli, maggiormente “divisa”; in tali nuclei si riscontra minore contrapposizione tra genitori e figlio, maggiore vicinanza del bambino ad uno solo dei due coniugi, maggiore dispersione dei componenti e maggiore distanza madre-padre nell’analisi dei Punteggi di Discrepanza. I risultati di questo studio evidenziano l’importanza della funzione di “base sicura” che sembra svolgere il padre nei confronti della condizione di pericolo rappresentata dalla malattia del figlio. Ne consegue la necessità di sostenere entrambi i genitori nel difficile ruolo di contenimento delle angosce del piccolo paziente.

L’intervento psicoeducazionale consiste nel fornire informazioni specifiche in aree considerate importanti nel processo di guarigione allo scopo di incrementare la consapevolezza di malattia e la motivazione al cambiamento. Questa ricerca ha valutato l’utilizzo in Italia dei gruppi psicoeducazionali nel trattamento dei Disturbi del Comportamento Alimentare (DCA). Sono stati contattati telefonicamente 75 centri ospedalieri pubblici e privati specializzati: il 20% ha dichiarato di utilizzare questa metodologia. Abbiamo somministrato ai conduttori dei gruppi un questionario self-report di 25 item appositamente approntato per l’occasione. I risultati indicano che i gruppi sono sorti per soddisfare esigenze di ordine pratico o economico e in riferimento a esperienze precedenti oppure su proposta individuale. Si riscontrano differenze di ordine organizzativo e strutturale (numero di partecipanti, durata degli incontri, caratteristiche del conduttore), ma anche similitudini importanti: modello teorico di riferimento (cognitivo-comportamentale), argomenti affrontati, inserimento all’interno di programmi terapeutici integrati. Le informazioni di follow-up (fornite da oltre la metà dei gruppi contattati) indicano una buona risposta terapeutica quando l’intervento è utilizzato all’interno di un trattamento di più ampie dimensioni (psicoterapia individuale o familiare, terapie mediche, ricovero) oppure nell’ambito di protocolli complessi quali i stepped-care, i decision tree. Un ulteriore ambito applicativo è quello preventivo.

Esposizione di una ricerca sulle alterazioni del comportamento di malattia e sulle manifestazioni di disturbi affettivi nei pazienti adulti affetti da dermatite atopica

A number of destructive and nondestructive tests have been carried out on 15 t slender-type concrete armour blocks from three Italian breakwaters, in order to investigate the material properties after a long-term exposure at sea. Measured surface hardness from rebound hammer tests in situ appears to be correlated to the results of laboratory breakage tests on core samples. The core position on the unit shows to have an influence on the concrete strength, with weaker points generally occurring at legs' intersections. Further laboratory results from porosity, carbonatation and ultrasonic pulse velocity tests and finally from destructive split-out tests on core samples generally show the good quality of aged pozolanic marine concretes, as typically used in Italian coastal structures. Despite the data scatter, also due to the necessarily limited number of cores, the average value of the tensile strength achieves 4 MPa (with a variation coefficient around 20%) and increases with concrete density (mean value of 24.5 kN with variation coefficient of 2%). These test results from prototype armour blocks can be useful for the application of advanced probabilistic methods for the design of rubble mound breakwaters.
