To analyze the perception of Nurses about the Systematization of Perioperative Nursing Care (SAEP) and its implementation in a reference hospital in a city in the state of Ceará - Brazil. The study was carried out with ten nurses from that hospital. Meetings were held and questions were raised on the topic addressed and educational activities on SAEP were carried out. It was shown that SAEP was not performed in the routine of the unit; however, according to the nurses, they had knowledge about systematization and knew the importance of its implementation. The results obtained with the questionnaires were satisfactory in terms of the possibility of implementing the systematization. It was observed that the accomplishment of the educational activity reduced doubts of the professionals on the subject. In this study it was possible to notice that even with the professionals' knowledge regarding systematization, this process is not applied. [ABSTRACT FROM AUTHOR]

Background: Preoperative information provision is a strategy used by nurses to control elective surgical patients' anxiety levels. A tool for assessing this information will improve the quality of nursing care. Objective: To develop and validate a preoperative information assessment scale. Methodology: Psychometric study. A Likert-type 15-item scale was developed based on a literature review and the researchers' experience. The scale was applied to 200 patients undergoing elective surgery in the preoperative period. Construct validity was assessed through exploratory factor analysis, principal components method, Varimax orthogonal rotation. Results: The KMO measure (0.92) and Bartlett's test of sphericity confirm the model's suitability. Two factors were extracted with the exploratory factor analysis, explaining 53.48% of the total variance. Cronbach's alpha for the total scale was 0.903. The theory of transpersonal caring is useful in understanding critical moments in the health-disease process such as the experience of major surgery. This study aims to evaluate the religiousness and spiritual well-being of patients hospitalized in the preoperative period of heart surgery from the perspective of Jean Watson’s transpersonal caring. Materials and methods: A cross-sectional study was conducted between January and June 2017 at the surgical wards of a university cardiology hospital in northeastern Brazil. 174 participants were evaluated in the preoperative period of cardiac surgery using the Duke University Religion Index and the Spiritual Well Being Scale. Results: The results showed that patients made significant use of religion to maintain their spiritual well-being in the preoperative period. The analysis of the results found was carried out based on Jean Watson’s theory of transpersonal caring. Discussion and conclusion: Existential crisis experienced by patients in the face of cardiac surgery, which involves restrictions, imposed changes and uncertainty, has an impact on the maintenance of well-being and its spiritual dimension which, in a transpersonal analysis, is a dimension of the individual’s integral health. Objetivo: Evaluar una intervención enfermera sobre el nivel de conocimientos de los pacientes sobre los cuidados postquirúrgicos en un hospital de II nivel de complejidad. Materiales y métodos: Proyecto de gestión del cuidado utilizando como referente metodológico el marco lógico adaptado. Se formuló una estructura conceptual-teórico-empírica para la investigación, que artículo los conceptos de Adaptación de Roy, educación de la teoría de la incertidumbre en la enfermedad de Mishel y se formularon los indicadores empíricos a partir de los diagnósticos, clasificación de resultados y las intervenciones enfermeras para dar solución al problema de la práctica. La muestra estuvo conformada por 101 pacientes adultos, con riesgo quirúrgico I y II, que fueron intervenidos quirúrgicamente y permanecían hospitalizados. Se aplicaron como instrumentos de recolección de información previo y posterior a la intervención una valoración verbal de los sentimientos sobre la cirugía y evaluación de conocimientos sobre los cuidados posquirúrgicos. La intervención incluyó educación personalizada, proyección de videos y explicación con folletos. Resultados: el uso de la intervención permitió mejorar los resultados con relación al nivel de conocimientos en actividad, dieta, cuidados de la herida y los signos y síntomas de infección, pasando de un nivel de conocimientos inicial nulo o bajo (1, 2) a un nivel sustancial o extenso (4 o 5), según la escala de resultados. Aim: to evaluate the quality of the preoperative nursing visit check list data in a university hospital in northeastern Brazil. Method: cross-sectional study from July to December 2017. The parameters non-fulfillment and reliability of data were used. For the calculation of incompleteness, the scoring system proposed by Romero and Cunha was adopted. Reliability was measured by the Kappa indicator. Results: 203 preoperative visit sheets were analyzed; 68.2% of the variables (15 of 22 variables analyzed) had a percentage of non-fulfillment ranging from >5% to 9.9%, classified as very low/low nonfulfillment. Considering the Kappa values, 81.8% of the studied variables had data reliability considered almost perfect. Conclusion: The quality of the preoperative visit check list data is satisfactory, since the non-fulfillment of most of the analyzed variables is very low/low and the agreement analysis indicated that the information is robust and reliable. Objective: to assess the prevalence, intensity and discomfort of the thirst of the orthopedic surgical patient and the correlations with the demographic and clinical-surgical variables. Method: cross-sectional, quantitative and analytical study with 78 patients in the Post-Anesthetic Recovery Room of a teaching hospital. Results: eight patients (10.3%) reported thirst spontaneously. Its mean intensity was 4.8, and the discomfort, 5.0. The attribute dry mouth was the most prevalent, followed by desire to drink water and dried lips. Intensity correlated significantly with discomfort of thirst in the group of patients from 21 to 40 years. There was a relationship of prevalence between presence of thirst and all the attributes of the Perioperative Thirst Discomfort Scale, with the exception of the attribute bad taste in the mouth. Conclusion: the thirst of the orthopedic surgical patient is prevalent in the postoperative period, with moderate intensity, but considerably uncomfortable. [ABSTRACT FROM AUTHOR]

Objective: to evaluate the perceived stress of relatives of patients in the waiting room of a Surgical Center and to relate it to sociodemographic variables. Methodology: a cross-sectional study with 104 relatives of patients submitted to surgery who were in the waiting room of a Surgical Center. The Sociodemographic Characterization Form and Perceived Stress Scale were used. Results: The highest averages of the Perceived Stress Scale belong to women over 40 years of age, without partners, with children, who receive less than three minimum wages and who attended high school, with a statistically significant difference observed for sex exclusively. In general, the family members were in a low level of stress. Conclusion: The waiting room in Surgical Center can contribute with the emergence of feelings like anxiety, nervousness and consequently stress. Objective: To analyze the factor structure of the version of the Safety Attitudes Questionnaire/Operating Room Version that has been translated and culturally adapted to the Brazilian context. Method: This was a methodological study about a questionnaire. The questionnaire was administered to 412 health professionals who worked in operating rooms. The factor structure was tested with confirmatory factor analysis (CFA) and Cronbach's alpha. Results: The total score for Cronbach's alpha was 0.912; and the scores for the domains ranged from 0.56 and 0.85. The domain with the worst performance was communication in the surgical setting. The results of CFA showed that the score for SRMR was 0.052, RMSEA, 0.031, and CFI, 0.95. These scores indicate the reliability and acceptability of the Brazilian adaptation of the questionnaire. Conclusion: The factor structure demonstrated the validity and reliability of the Brazilian version of the questionnaire for measuring the patient safety climate as perceived by healthcare professionals who worked in surgical settings [ABSTRACT FROM AUTHOR]

Introduction: Hypothermia is a common event in the intraoperative period, it triggers consequences in the recovery of the patient, with complications in several systems of the organism, such as cardiac, respiratory, integumentary, digestive, immunological and also the coagulation system. Objective: To analyze the complications presented by the patient in the postoperative period related to intraoperative hypothermia. Methods: A retrospective cohort study was carried out in a sample composed of 54 patients' files, from a previous experimental study, in which they were submitted or not to warmed intraoperative intravenous infusion and anesthetic recovery. The variables were analyzed at 4 different times, upon arrival at the hospitalization unit, after 17, 32 and 108 hours postoperative. The model used was the marginal logistics. Results: The majority of patients 40 (74.07%) were female, mean age of 47.06 years, 42 (77.78%) were normothermic patients from the Post Anesthesia Recovery Room, with a mean temperature of 36.2ºC. Regarding the comparison between variables and groups of normothermic and hypothermic patients, over time, the variables that presented statistical significance were the time of hospitalization, pain, nausea, evacuation and surgical wound with presence of secretion, with a p-value less than 0.05. Conclusion: In view of the complications found in this study, it is necessary to develop preventive and control actions for intraoperative hypothermia aiming at a better recovery of the patient in the postoperative period. RESUMO Objetivo: investigar os estressores percebidos pelos pacientes no pós-operatório imediato de cirurgia cardíaca e sua relação com características sociodemográficas e clínicas. Método: estudo correlacional, prospectivo, desenvolvido no interior paulista, entre agosto/2013 e dezembro/2014. Uma amostra não probabilística foi constituída por pacientes submetidos à primeira cirurgia de revascularização do miocárdio e/ou correção de valvulopatias. Utilizamos a "Escala de Avaliação de Estressores em Unidade de Terapia Intensiva". Resultados: participaram 105 pacientes. O item avaliado como mais estressante foi "ter sede", e o menos estressante foi "membro da equipe de enfermagem não se apresentar pelo nome". Das variáveis sociodemográficas e clínicas investigadas (sexo, idade, tipo e tempo de cirurgia, dor, tempo de entubação, uso de psicotrópico e tempo na unidade de terapia intensiva), apenas dor apresentou relação significativa com os estressores. Conclusão: conhecer os estressores pode auxiliar na implementação de práticas relacionadas à sua redução, favorecendo a recuperação dos pacientes.

RESUMO Objetivo: verificar a ocorrência de complicações intraoperatórias e pós-operatórias em pacientes submetidos a procedimentos cirúrgicos de urgência e emergência entre janeiro e dezembro de 2012, com tempo de jejum inferior a oito horas. Método: conduziu-se um estudo quantitativo, tipo coorte retrospectivo, por meio da análise de prontuários médicos. Resultados: foram incluídos 181 prontuários de pacientes submetidos a procedimentos cirúrgicos com duração média de 59,4 minutos, destacando-se a cirurgia de correção de fraturas em 32% dos casos. Foram observadas complicações em 36 (19,9%) dos pacientes, destacando-se o vômito (47,2%); seguido de náuseas (16,7%); necessidade de transfusão sanguínea (13,9%); infecção do sítio cirúrgico (11,1%); e óbito (11,1%). O tempo médio de jejum foi de 133,5 minutos. O tempo de jejum não apresentou correlação estatisticamente significante com as complicações investigadas. Conclusão: as complicações intraoperatórias e pós-operatórias estiveram associadas às condições clínicas dos pacientes e não ao tempo de jejum.

Abstract OBJECTIVE To analyze nurses' competencies with regard to their work in post-operative heart surgery and the strategies implemented to mobilize these competencies. METHOD This was an exploratory study with a qualitative approach and a methodological design of collective case study. It was carried out in three post-operative heart surgery units, consisting of 18 nurses. Direct observation and semi-structured interviews were employed to collect data. Data were construed through thematic analysis. RESULTS nine competencies were found, as follows: theoretical-practical knowledge; high-complexity nursing care; nursing supervision; leadership in nursing; decision making; conflict management; personnel management; material and financial resources management; and on-job continued education. Organizational and individual strategies were employed to develop and improve competencies such as regular offerings of courses and lectures, in addition to the individual pursuit for knowledge and improvement. CONCLUSION the study is expected to lead future nurses and training centers to evaluate the need for furthur training required to work in cardiac units, and also the need for implementing programs aimed at developing the competencies of these professionals.

ABSTRACT Objective: to analyze the evidence available in the literature on the alterations in the sexuality of candidates and recipients of liver transplantation. Method: integrative review of the literature with search for primary studies in the databases MEDLINE (via PUBMED), CINAHL e LILACS, published in English, Portuguese and Spanish. Results: the 16 primary studies included were grouped into three categories: 1) female sexuality (n=5), 2) male sexuality (n=5) and 3) male and female sexuality (n=6). In category 1, the subjects investigated were contraception, pregnancy, sexual dysfunction, presence of gynecological symptoms and sexually transmitted infections. In category 2, the main focus of the studies was erectile dysfunction, sexual desire and satisfaction, and consequences of the immunosuppressive regimen with mycophenolic acid in men. In category 3, the evaluation of sexual function was the main topic. Conclusion: the scientific evidence generated provides support to encourage health professionals to incorporate the topic of sexuality in the routine of care. Knowledge gaps were identified and new studies should be conducted in order to implement interventions to prevent, minimize and/or control changes related to the patient’s sexuality.

Abstract OBJECTIVE To search for the scientific evidence available on nursing professional actions during the anesthetic procedure. METHOD An integrative review of articles in Portuguese, English and Spanish, indexed in MEDLINE/PubMed, CINAHL, LILACS, National Cochrane, SciELO databases and the VHL portal. RESULTS Seven studies were analyzed, showing nurse anesthetists' work in countries such as the United States and parts of Europe, with the formulation of a plan for anesthesia and patient care regarding the verification of materials and intraoperative controls. The barriers to their performance involved working in conjunction with or supervised by anesthesiologists, the lack of government guidelines and policies for the legal exercise of the profession, and the conflict between nursing and the health system for maintenance of the performance in places with legislation and defined protocols for the specialty. Conclusion Despite the methodological weaknesses found, the studies indicated a wide diversity of nursing work. Furthermore, in countries absent of the specialty, like Brazil, the need to develop guidelines for care during the anesthetic procedure was observed.

Abstract OBJECTIVE To identify the occurrence of surgical site infection (SSI) and its risk factors in patients undergoing colon surgery in a tertiary hospital located in the countryside of the state of São Paulo. METHOD Retrospective cohort study, with collection of information contained in the medical records of patients undergoing colon surgery in the period between January 2010 and December 2013. The studied variables were the possible risk factors related to the patient, to demographic characteristics and the surgical procedure. RESULTS In total, were evaluated 155 patients with an overall SSI incidence of 16.7%. A statistically significant association was found both in the univariate as in the multivariate analysis between the SSI and the following variables: male gender, Charlson index and mechanical bowel preparation. CONCLUSION The understanding of health professionals about the factors that influence the incidence of SSI in colon surgery may contribute to the quality of care provided to surgical patients, from effective actions to minimize the risk of infections.

Resumo Objetivo: Identificar os métodos de aquecimento para prevenir hipotermia em pacientes adultos no intraoperatório de cirurgia abdominal eletiva com exposição visceral. Métodos: Estudo quantitativo, exploratório e descritivo, prospectivo, realizado num hospital público da região sul do Brasil. Constituiu a amostra 63 pacientes. Observaram-se os métodos de aquecimento utilizados e foi aferida a temperatura timpânica. Para tratamento dos dados, aplicou-se a estatística descritiva por meio do software SEstatNet. Resultados: Foram utilizados métodos de aquecimento ativo e passivo. A infusão de fluidos aquecidos para irrigação da cavidade abdominal foi a medida mais empregada (nº 63; 100%) do método ativo, enquanto que a manutenção do sistema de refrigeração desligado até o início da cirurgia foi a medida mais utilizada (nº 57; 90,5%) do método passivo. Conclusão: Com os métodos de aquecimento empregados não houve hipotermia grave, porém, existem métodos atuais mais eficientes que poderiam prevenir a hipotermia leve e moderada encontradas.

RESUMO Objetivo Identificar os fatores associados à carga de trabalho de enfermagem no cuidado a pacientes no pós-operatório de cirurgia cardíaca. Método Estudo de coorte prospectivo, conduzido com 187 pacientes da Unidade de Terapia Intensiva Cirúrgica (UTI) do Instituto do Coração. Os dados foram coletados nas primeiras 24 e 72 horas do paciente na UTI. A variável dependente foi a carga de trabalho calculada por meio do Nursing Activities Score (NAS) e as independentes foram de natureza demográfico-clínicas e escores de morbimortalidade. Para análise dos dados utilizou-se os testes de Wilcoxon-Mann-Whitney e de correlação de Spearman, e a regressão linear com modelo de efeitos mistos. Resultados A maioria dos pacientes era do sexo masculino (59,4%), com média de idade de 61 anos (±12,7) e 43,9% desenvolveram algum tipo de complicação no pós-operatório. Nas 24 horas, a carga de trabalho foi de 82,4% (±3,4) e foi de 58,1% (±3,4) nas 72 horas. AbstractOBJECTIVEEvaluate pre- and intraoperative practices adopted by medical and nursing teams for the prevention of surgical infections.METHODA prospective study carried out in the period of April to May 2013, in a surgical center of a university hospital in Belo Horizonte, Minas Gerais.RESULTS18 surgeries were followed and 214 surgical gloves were analyzed, of which 23 (10.7%) had postoperative glove perforation detected, with 52.2% being perceived by users. Hair removal was performed on 27.7% of patients in the operating room, with the use of blades in 80% of the cases. Antibiotic prophylaxis was administered to 81.8% of patients up to 60 minutes prior to surgical incision. An average of nine professionals were present during surgery and the surgery room door remained open in 94.4% of the procedures.CONCLUSIONPartial adhesion to the recommended measures was identified, reaffirming a need for greater attention to these critical steps/actions in order to prevent surgical site infection.

Objective: Evaluate the security climate in an ophthalmic surgical hospital through the perception of its professionals. Method: Exploratory, descriptive and transversal study of quantitative nature. Developed from October to January 2016 through the application of the Safety Attitudes Questionnaire (SAQ) to 61 different categories of health professionals being only considered for 27 of the questionnaires applied analysis. Results: The average score obtained through the six domains of SAQ-climate of teamwork, safety Climate, job satisfaction, stress Recognition, perception of management and working conditions-showed concordance index below 75 points from the range of Likert, indicating a negative perception of safety climate. Conclusions: The study pointed out weaknesses in six areas, suggesting the development of actions focusing on improving security climate as a priority measure in ensuring the safety of the surgical patient. A theoretical study aimed to analyze the existing knowledge in the literature on the perioperative thirst symptom from the perspective of Symptom Management Theory, and supplemented with the experience of the study group and thirst research. Thirst is described as a very intense symptom occurring in the perioperative period, and for this reason it cannot be ignored. The Symptom Management Theory is adequate for understanding the thirst symptom and is a deductive theory, focused on the domains of the Person, Environment and Health / Illness Status, as well as on the dimensions of Experience, Management Strategies and Symptom Outcomes. Using the theory leads us to consider perioperative thirst in its multifactorial aspects, analyzing the interrelation of its domains and dimensions in order to draw attention to this symptom that has been insufficiently valued, recorded and treated in clinical practice.

Este artigo teve como objetivo construir e apresentar um plano de cuidados pré e pós-operatórios para pacientes submetidos à cirurgia de cortico-amígdalo-hipocampectomia (AHC) para ser implementado em uma unidade de internação cirúrgica de um Hospital Universitário do Sul do Brasil. Trata-se de um relato de experiência desenvolvida durante o Estágio Supervisionado de Enfermagem por graduandos de enfermagem. Construiu-se um plano com 26 cuidados de enfermagem para pacientes submetidos à cirurgia de AHC, que foi validado por profissionais experts da área. Realizaram-se encontros expositivos e dialogados com a equipe de enfermagem para esclarecer e desmistificar a epilepsia, suas diferentes apresentações de crises e possibilidades de tratamento, bem como apresentar o plano de cuidados elaborado. Essa experiência contribuiu com a formação profissional dos estudantes, assim como forneceu subsídios para a implementação da sistematização da assistência de enfermagem a este paciente específico, considerando o cuidado sistematizado uma ferramenta fundamental no trabalho do enfermeiro.

Objective To develop a safety protocol for the management of thirst in the immediate postoperative period. Method Quantitative, methodological, and applied study conducted in April-August 2012. An extensive literature search and expert consultation was carried out to develop the protocol and its operating manual. Theoretical and semantic analyzes were carried out by experts. Results Assessment of level of consciousness, reflexes of protection of the airways (cough and swallowing), and absence of nausea and vomiting were selected as safety criteria. These criteria were grouped and formatted in a graph algorithm, which indicates the need to interrupt the procedure if a security criterion does not reach the expected standard. Conclusion The protocol was elaborated to fill in the gap in the literature of a specific model concerning nursing actions in the safe management of thirst in the immediate postoperative period.

Estudo de caso que objetivou desvelar, a partir da adoção da Teoria do Cuidado Transpessoal, os significados do relacionamento interpessoal terapêutico entre enfermeiro e usuário, sobre a visita pré-operatória de enfermagem após a vivência do processo cirúrgico. A amostra, de conveniência, foi constituída por três enfermeiros e três usuários de uma instituição que possui cadastro atualizado para realizar cirurgia de alta complexidade cardiovascular, perfazendo nove combinações de interações terapêuticas. Foram utilizados impressos estruturados segundo a teoria de Jean Watson e as taxonomias North American Nursing Diagnosis Association, Nursing Intervention Classification e Nursing Outcome Classification. Os aspectos éticos e legais de pesquisa envolvendo seres humanos foram assegurados. Nos resultados, identificaram-se três clusters para apreender a significação da visita pré-operatória pelos usuários e cinco clusters para apreender a percepção dos enfermeiros ao vivenciarem esta experiência clínica.

O estudo teve por objetivos caracterizar o ensino de Enfermagem em Centro Cirúrgico em relação à carga horária, conteúdo e componente curricular específico; e identificar as opiniões e vivências dos docentes no ensino desse conteúdo. Realizou-se estudo descritivo de abordagem quanti-qualitativa que utilizou para a coleta de dados um instrumento semiestruturado, com perguntas relacionadas ao ensino da disciplina, enviado por e-mail aos coordenadores dos Cursos de Bacharelado em Enfermagem de 219 escolas do estado de São Paulo, cadastradas no portal eletrônico do Ministério da Educação. Dos 52 respondentes, 90,4% eram de escolas privadas, localizadas principalmente no interior do estado (71,1%). Dos cursos, 53,8% ministravam o conteúdo de centro cirúrgico no interior de outra disciplina. A mediana da carga horária do conteúdo foi de 64h (40-300h). Os sujeitos comentaram o conteúdo extenso para uma carga horária pequena, tanto prática quanto teórica, e sua importância para a formação do enfermeiro generalista.

Objective:to describe the behavior of children during the instructional session of therapeutic play (ITP) in the preoperative period and to verify the behavior presented by them during the perioperative period. Methods: Exploratory descriptive study with a quantitative approach, developed in a large private hospital in Sao Paulo, in which the behaviors presented by 30 children between three and five years old, undergoing minor surgery were seen at the hospital during the session of TP and in the surgical center, from admission to awaken from anesthesia. Results: Most children participated in the ITP session (21; 70%), entered the surgical room spontaneously (22; 73.3%) without resisting mother separation (24; 80%), collaborating with the anesthetic procedure (16; 53.3%) and quietly awakening from anesthesia (26; 87%). Conclusion: The use of ITP led the child to understand the surgical procedure, making it less traumatic.

Fundamento: la superación profesional constituye una prioridad de la política de salud en Cuba, como una vía para propiciar la calidad en la atención de los pacientes. Objetivo: confeccionar una guía metodológica para la superación profesional del personal de enfermería que labora en Unidades Quirúrgicas de Urgencias. Métodos: estudio descriptivo realizado durante los años 2009 y 2010 en el Hospital Gustavo Aldereguía Lima de Cienfuegos. Se seleccionó una muestra de 18 enfermeros de la Unidad Quirúrgica de Urgencias, para la aplicación del diagnóstico de las necesidades formativas. Se diseño la guía y se sometió a criterio de expertos. Resultados: se precisó la insuficiente preparación profesional del personal de enfermería en relación con el perioperatorio y se evidenciaron las necesidades formativas. La guía fue diseñada en función de la planificación, ejecución y evaluación de talleres para preparar el personal de enfermería. Los especialistas declararon la calidad de la propuesta para contribuir a resolver la problemática planteada.Conclusiones: la aplicación de la guía metodológica para la superación del personal de enfermería que labora en las Unidades Quirúrgicas de Urgencias debe prepararlos para perfeccionar la calidad en la atención de los pacientes.

Background: professional development is a priority in Cuban health policy, as a way to promote the quality care of patients.Objective: to design a methodological guide for the professional development of nurses working in Surgical Emergency Units.Methods: a descriptive study was conducted during 2009 and 2010 at the Gustavo Aldereguía Lima Hospital in Cienfuegos. A sample of 18 nurses of the Surgical Emergency Unit was selected for assessing their training needs. The guide was designed and subjected to expert judgment.Results: the insufficient professional preparation of nurses regarding the perioperative period was determined and their training needs were shown. The guide was designed according to the planning, implementation and evaluation of workshops to train the nursing staff. Specialists stated the quality of the proposal to help resolve the problems mentioned.Conclusions: implementation of the methodological guide for professional development of the nurses working in the Surgical Emergency Units should prepare them to improve care of patients.
