Background: Environmental habitat may play a role in clinical disparities of primary hyperparathyroidism (pHPT) patients. Aims: To compare preoperative clinical symptoms and associated conditions and surgical findings in patients with pHPT, living in different geographical regions from the Black Sea, Mediterranean and Anatolia regions. Study Design: Retrospective, clinical-based multicentric study of 694 patients with pHPT. Methods: Patients from 23 centers and 8 different geographical regions were included. Data related to baseline demographics, clinical, pathologic and treatment characteristics of 8 regions were collected and included age, gender, residential data, symptoms, history of fracture, existence of brown tumor, serum total Ca and p levels, serum parathormone (PTH) levels, serum 25-OH vitamin D levels, bone mineral density, size of the resected abnormal parathyroid gland(s), histology, as well as the presence of ectopia, presence of dual adenoma, and multiple endocrine neoplasia (MEN)- or familial-related disease. Results: The median age was 54. Asymptomatic patient rate was 25%. The median PTH level was 232 pg/mL and serum total Ca was 11.4 mg/dL. Eighty-seven percent of patients had an adenoma and 90% of these had a single adenoma. Hyperplasia was detected in 79 patients and cancer in 9 patients. The median adenoma size was 16 mm. Significant parameters differing between regions were preoperative symptoms, serum Ca and p levels, and adenoma size. All patients from South-East Anatolia were symptomatic, while the lowest p values were reported from East Anatolia and the largest adenoma size, as well as highest Ca levels, were from Bulgaria. Conclusion: Habitat conditions vary between geographical regions. This affects

Bupropion is a new-generation antidepressant, while its sustained release (SR) formulation is used in smoking cessation for a long time successfully (1). Urticaria, pruritus and rashes can be seen in 1-4% of the patients and anaphylactoid reactions like angioedema and dyspnea can be seen in 0,1-0,3% of the patients using bupropion. Herein, a case with urticaria and angioedema due to SR bupropion treatment has been reported., Bupropion yavaş salınımlı formu ile sigara bırakma tedavisinde uzun süredir başarı ile kullanılan yeni kuşak bir antidepresandır.

In military aviation, the use of flight simulators has proliferated recently in order to train fifth generation fighter pilots. With these simulators, pilots can carry out real-time flights resulting in seeing their faults and can perform emergency drills prior to real flights. Since we cannot risk losing the aircraft and the pilot himself/herself in the flight training process, flight simulators are of great importance to adapt the fighter pilots competently to real flights aboard the fifth generation aircraft. The real flights are impossible to simulate thoroughly on the ground. To some extent, the fixed-based simulators may assist the pilot to steer aircraft technically and visually but flight simulators can't trick the pilot's vestibular, sensory, and perceptual systems without motion platforms. This paper discusses the benefits of motion simulators for fifth generation fighter pilots' training in preference to the fixed-based counterparts by analyzing their pros and cons.

In patients with Chronic Kidney Disease (CKD) cardiovascular disease risk is increased and major cause of mortality in these patients is cardiovascular diseases. Heart rate recovery (HRR) index is a method used in demonstrating the cardiac autonomic dysfunction and predicting mortality in recent years and there has been many studies about this method. In this study, we aimed to show the presence of cardiac autonomic dysfunction in patients with CKD with HRR index. 120 patients with stage 1-4 CKD who admitted to Celal Bayar University Hafsa Sultan Hospital Internal Medicine or Nephrology clinics were included in this study. 30 patients without any comorbidities who admitted to Celal Bayar University Hafsa Sultan Hospital were included as control group. Patients were categorized into 4 groups and each group had 30 patients.Exercise stress test was conducted with Bruce protocol in patients without any cardiac disease according to their history, physical examination, ECG and echocardiography. Heart rate recovery index in 1st, 2nd, 3rd and 5th minutes were evaluated.Average of 1st, 2nd, 3rd and 5th minute HRR in stage 3 and 4 CKD patients and average of 1st and 5th minute HRR in stage 2 CKD patients were significantly lower than control group. Comparison between all stages had shown that in every HRR phase, stage 4 had lower HRR index than stage 1. Also, there was a correlation between GFR and HRR index in 1st, 2nd, 3rd and 5th minutes.According to these data, it can be presumed that cardiac autonomic dysfunction starts in early stages of CKD and dysfunction gets more significant in advanced stages. This study also shows that HRR index can be used as a cardiac autonomic dysfunction indicator in CKD patients. However, further studies are required about using this method in routine evaluation. Our study will contribute to further studies and literature about this subject. 48

- Abstract
In this issue of The Turkish Journal of Gastroenterology, Bülbüller et al. (1) presented their experience on the results of surgical treatment of hepatic Echinococcal (hydatid) cysts. Echinococcal disease continues to be a major health problem in developing countries in sheep-raising areas, which makes a large portion of the world. Public education directed at the proper disposal of cyst containing viscera of slaughtered animals (preventing their consumption by carnivores) can play an important role in the eradication of the disease. However, public health measures do not seem to work in a lot of countries and second to the fact that large populations of humans are on the move, physicians everywhere nowadays are faced with patients with the disease having to decide on a management strategy. The liberal use of sonography, CT and MRI has also increased the number of asymptomatic patients who come to the attention of physicians. A review of recent medical literature discloses 3 alternative strategies for the management of cystic echinococcosis of the liver. The proposed modalities are; surgery, percutaneous drainage and chemotherapy. A follow-up without intervention strategy can also be implemented on asymptomatic patients based on the follow-up findings by Frider et al on a small group of patients (2)

Gastrointestinal stromal tumors (GISTs) are the most common mesenchymal tumors of the gastrointestinal tract. They can arise from any part of the tract as well as the omentum, mesentery or retroperitoneum. In this study, we present a case of a GIST originating from an extraordinary site, the pancreas. Evaluation of 30-year-old man with complaints of abdominal distension revealed a cystic, distal pancreatic lesion 13 cm in diameter. There were no intra-abdominal or distant organ metastases. The patient was operated with a diagnosis of cystic pancreas tumor, distal pancreatectomy and splenectomy was performed. The lesion was diagnosed as gastrointestinal stromal tumor upon histopathological examination. He was discharged on the sixth postoperative day without any complications and is being followed up for 21 months without systemic or local recurrence. Extragastrointestinal GISTs are reported rarely. To our knowledge, only one pancreatic GIST has been reported previously in the English literature.

ultrasonography-guided excision ,breast cancer ,nonpalpable breast mass - Abstract
WOS: 000317861600006, Aim With the improvement of expertise in technique and skills of ultrasonography (US), many surgeons now perform image-guided procedures for breast masses themselves. To minimize operative time, and to achieve adequate surgical margins with an acceptable cosmetic outcome, we designed a safe and simple hybrid technique to excise suspicious nonpalpable masses, which is described herein for the first time. Patients and Methods Intraoperatively-marked incision is performed, and a needle is advanced at a 0.5-cm distance to the lesion according to real-time ultrasound measurements. Four deep sutures are passed to fix and hang the predicted specimen adjacent to the needle in four directions. En bloc excision with aimed surgical margins of approximately 0.5cm is achieved by means of retracting sutures, and confirmed by specimen and tumour bed US. Results In the present study, there were 25 women (17 with malignant and 8 with benign lesions), with a mean age of 50.88 years (range: 3069) and mean tumour diameter of 10.6mm (range: 615). All lesions were correctly identified and localized by intraoperative US, and free margins of excision were obtained in all malignant lesions by means of the presented technique. The combined operative technical approach with surgeon-performed intraoperative US-guided needle placement and retracting sutures were feasible, simple and beneficial. We achieved complete tumour removal in all dimensions with no reexcision. The procedure was performed under local anaesthesia in an outpatient fashion with no complications. Conclusion Our report of the operative technique demonstrates that a combination of surgeon-performed image-guided localizations, together with a suture-oriented fashion to assure negative surgical margins in all dimensions, improves margin clearance rate at the time of first surgical intervention. This method can be performed with operative and cost efficiency, and might become a valuable tool to minimize operative time and yield minimal sacrifice of normal breast tissue with maximal cosmetic outcome.

WOS: 000313708600017, PubMed: 22766655, Background Surgeon-performed ultrasonography (US) of thyroid nodules might serve as a potential therapeutic guide to designate accurate surgical or clinical intervention. Objective To evaluate the diagnostic adequacy of surgeon-performed ultrasonography guided fine needle aspiration biopsy (FNAB) of thyroid nodules, the factors responsible for diagnostic adequacy and the impact of surgeon-performed US on treatment approach. Methods Retrospective review of a single surgeon performed 621 US-guided FNABs without on-site cytological specimen assessment. Outside US findings were compared to the surgeon-performed US. Measured variables and outcomes for the study included diagnostic adequacy rates and the effects of detected differences between US reports on treatment variability. Results Diagnostic adequacy rate of surgeon-performed US-guided FNAB was determined to be 94.52% without on-site specimen evaluation by cytologist. Non-diagnostic specimens occurred in 34 of 621 (5.48%) nodules. The differences detected between the outside US and surgeon-performed US altered invasive treatment algorithm in 30 (5.47%) patients. FNAB was avoided for 15 (2.7%) patients. Total thyroidectomy became the preferred surgical option in 15 (2.7%) patients after the discovery of additional nodules in the contralateral lobe. Conclusion Surgeon-performed US offers clear clinical benefits in terms of diagnostic yield of FNAB with providing valuable additional data that might alter surgical treatment approach. J. Surg. Pheochromocytomas can be a part of familial neoplastic syndromes, in which case they tend to be multiple and involve both adrenal glands. Therefore, sparing adrenocortical function represents a major concern while dealing with these hereditary lesions. Herein, we describe the clinical characteristics and the management strategy of a patient with von Hippel-Lindau (VHL) disease who had multiple, bilateral pheochromocytomas as well as bilateral renal masses, pancreatic masses, and a paracaval mass. Only a portion of the left adrenal gland has remained in situ after two consecutive open surgeries and a percutaneous radiofrequency ablation which have been performed to treat the various components of this syndrome. No adrenal or extra-adrenal pheochromocytoma recurrences have been detected during a follow-up period of more than 2 years. Pancreatic and adrenal functions were normal throughout the postoperative period and never necessitated any replacement therapy. Adrenal cortex-sparing surgery is a valid option for VHL disease patients who present with synchronous bilateral adrenal pheochromocytomas.

The technological developments that increased in recent years also increased the speed of globalization and all kinds of distances between countries decreased. Thus, it has been thought that the borders were eliminated and integration between nations was maintained. However, the countries have embraced differentiation ways with several methods in order to maintain advantage with different aspects in the global market. After the increasing competition conditions, the discourse integration has brought the efforts for differentiation, the process of external presentation has accelerated and the borders have become clearer as a result of these efforts. During the external presentation process, the most important instrument of countries is branding, being brand countries.Just like products, the countries are positioned as a result of the image and definitions they leave impact on people and other countries. Image has become a very important instrument for presentation and competition. Therefore, in order to continue their existences and adapt to competition, countries have speed the image and competition activities up and even they have started to take a step to the path of branding. Thus, the brand of country phenomenon has arisen. The positions of the countries that building brand in the field of national competition, affect the economical and political relationships dramatically. The countries that want to become a brand must attract the attention of national media, other countries and the governments. In order to generate a positive image, evoke a sense of respect and trust before the intended audience, the candidate countries are in competition permanently. For this purpose, the public institutions and foundations and private sector of countries are planning to take the biggest portion in this competitive field by achieving sustainable and successful presentation activities. This research is a qualitative research. After the literature investigation as a scope of this research, the public institutions? presentation strategies and implementations of Turkey, which is in progress of becoming a ?brand country?, have been searched and the contributions of these institutions have been displayed. Beside the literature scanning, meetings with the employees of institutions have been made by asking half structured questions thoroughly. Institutions surveyed, respectively, the Ministry of Culture and Tourism, Ministry of Foreign Affairs, Prime Ministry Promotion Fund, the General Directorate of Press and Information, the Anatolia News Agency(TIKA) and Turkey Radio and Television Corporation(TRT). 209

Bu çalışmada sektör yöneticileri ile yarı yapılandırılmış görüşme tekniği kullanılarak Türkiye’de bölgesel havayolu taşımacılığının geliştirilmesi için neler yapılması gerektiği ortaya konulmuştur., Regional airline transportation is the one of the fastest growing part of the air transportation system. The regional airlines which give the service with small size capacity and generally between large and the small cities, on short and medium range routes make a contribution to social and economic life of the regions they serve. Regional airlines also provide easily access to that regions. In Turkey, which has been in a large geography, mostly the passenger transportation is providing through the highways, the percentage of air transportation relatively little. This small proportion of air transportation causes some problems. Legal procedures and regulations are usually not enough about the regional air transportation in Turkey. In addition the concept of regional air transportation usually deals with in different ways. In this study through the semi-structured interviews, industry managers are interviewed on what can be done on the issue of developing the regional airline transportation in Turkey.

AMAÇ Bu çalışmanın amacı, karın içi kullanılan Ankaferd Blood Stopper’in (ABS) periton içi yapışıklık oluşumunu arttırıp arttırmadığını ve in vivo kullanıma bağlı majör toksik etkilerini irdelemektir. İntraperitoneal ABS kullanıma bağlı olarak herhangi bir toksik etki saptanmamakla birlikte bazı minör değişiklikler gözlenmiştir., BACKGROUND The aim of this study was to investigate whether intraabdominal Ankaferd Blood Stopper (ABS) causes increased intraabdominal adhesion formation and to determine any side effects of ABS in vivo. METHODS The present experimental study was designed to examine the effects of Ankaferd solution on peritoneal adhesion formation in a rat model of cecal abrasion. Intraperitoneal adhesions were assessed macroscopically and histopathologically on the 10th postoperative day. The possible adverse affects of ABS on liver and lung tissues were analyzed histopathologically, and blood chemistry was also evaluated. RESULTS Our study revealed that ABS reduced intraperitoneal adhesion formation in an experimental rat model. The blood chemistry was not disturbed due to ABS administration. Intraperitoneal administration of ABS led to some minor changes in the lungs and serosal surfaces of the intestines, with minor architectural changes in the liver that were not considered as toxic. Further studies with various application doses and routes with more detailed cellular analysis are thus warranted to clarify the possible pleiotropic and adverse effects of this new agent away from hemostasis. CONCLUSION There was less intraperitoneal adhesion formation in the ABS group than in the control group and saline group. Intraperitoneal administration of ABS has no toxic effects on blood chemistry or the lungs, kidneys and the liver, but it has some minor adverse effects.

Background/Aim: The excessive apoptosis of intestinal epithelial cells (IECs) partly accounts for the development of colonic inflammation and eventually results in ulcerative colitis (UC). Humanin, an endogenous anti-apoptotic peptide, has previously been shown to protect against Alzheimer's disease and a variety of cellular insults. The present study aimed to investigate the effects of glysin variant of humanin (HNG) on 2,4,6-trinitrobenzene sulphonic acid (TNBS)-induced colitis in rats. Materials and Methods: Rats were divided into four groups as follows: Group 1 (n = 8): control; isotonic saline solution 0.1 ml/rat rectally, Group 2 (n = 8): TNBS colitis; 0.1 ml of a 2.5% (w/v) TNBS solution in 50% ethanol rectally, Group 3 (n = 8): 10 µM HNG, and Group 4 (n = 8): 20 µM HNG intraperitoneal (ip) on day 2 and 6 after rectal TNBS administration. Rats were sacrificed 7 days after the induction of colitis. Blood and tissue samples were harvested for biochemical and histopathological analysis. Results: HNG treatment significantly ameliorated weight loss and macroscopic and microscopic scores. TNBS-induced colitis significantly increased the colonic mRNA expression of tumor necrosis factor-alpha (TNF-α), interleukin-1beta (IL-1β), and caspase-3 activities in group II in comparison to the group I. HNG treatment was associated with an inhibition of mRNA expression of TNF-α and IL-1β, and a decrease in caspase-3 activities in colon tissues in group III and IV when compared to group II. Conclusion: The results of this study indicate that HNG treatment may exert beneficial effects in UC by decreasing inflammatory reactions and apoptosis. [ABSTRACT FROM AUTHOR]

Bu nedenle, dokuda NO üretimini artırmak amacıyla, NO prekursörü L-Argininin kullanıldığı çalışmalarda kısmen iyi sonuçlar alınmıştır. 39 44

- Abstract
A 55-year-old man was investigated for right upper abdominal quadrant pain. He had No history of abdominal trauma or surgery. Imaging studies showed a common hepatic artery aneurysm involving the gastroduodenal artery. Following aneurysmectomy, examination with a hand Doppler apparatus yielded clear arterial signals from the liver surface. Therefore, vascular reconstruction was not performed. He had an uneventful postoperative course.
