Abstract

Abstract Background Combined prevention interventions, including early antiretroviral therapy initiation, may substantially reduce HIV incidence in hyperendemic settings. Our aim was to assess the potential short-term impact of combined interventions on HIV spreading in the adult population of Mbongolwane and Eshowe (KwaZulu-Natal, South Africa) using sex- and age-specific scenarios, and age-targeted interventions. Methods A mathematical model was used with data on adults (15–59 years) from the Mbongolwane and Eshowe HIV Impact in Population Survey to compare the effects of various interventions on the HIV incidence rate. These interventions included increase in antiretroviral therapy (ART) coverage with extended eligibility criteria, increase in voluntary medical male circumcision (VMMC), and implementation of pre-exposure prophylaxis (PrEP) among women. Abstract

Patients with a Klinefelter syndrome (KS), defined by a 47 XXY karyotype, were long considered infertile. Testicular sperm extraction (TESE) now allows them to access fatherhood. We will present the data of studies since first experiment of TESE. Several factors influencing TESE outcome were proposed in these different studies. Among them, clinical and hormonal parameters have reported by few studies, age has been one of the most discussed prognostic factor of positive sperm retrieval rate. Data seems to show that TESE carried out before an age greater than 30 has a poorer prognosis for positive sperm retrieval. In few studies performed in younger patient, before 20 years, SRR was closed to result for 20 to 30 year old patients. Offering a TESE before 16 years old does not improve positive sperm extraction rate. In fact, the few studies carried out before the age of 16 were of poorer prognosis, most often linked to insufficient maturation of the residual gametes. In addition, androgen therapy, frequently prescribed in case of Klinefelter syndrome, did not seem to show any effect on sperm retrieval but only few studies were interested in the possible impact of this treatment. In conclusion, further studies are necessary to determine the interest of new markers to predict the chance of sperm retrieval, taking into account age, hormonal therapy.

Abstract

Although animal trials are the most accurate approach to determine the metabolisable energy (ME) content of pet food, these are expensive and labour-intensive. Instead, various equations have been proposed to predict ME content, but no single method is universally recommended. Data from canine and feline feeding studies, conducted according to Association of American Feed Control Officials recommendations, over a 6-year period at a single research site, were utilised to determine the performance of different predictive equations. Predictive equations tested included the modified Atwater (MA equation), NRC 2006 equations using both crude fibre (NRC 2006cf) and total dietary fibre (NRC 2006tdf), and new equations reported in the most recent study assessing ME predictive equations (Hall equations; PLoS ONE 8(1): e54405). Where appropriate, equations were tested using both predicted gross energy (GE) and GE measured by bomb calorimetry. Associations between measured and predicted ME were compared with Deming regression, whilst agreement was assessed with Bland-Altman plots. 335 feeding trials were included, comprising 207 canine (182 dry food; 25 wet food) and 128 feline trials (104 dry food, 24 wet food). 75. Random effect models in the statistical analysis of human fecundability data : application to artificial insemination with sperm from donor

Abstract

The main aim of this dissertation is to explore methodological approaches to correlated binary data and to assess their suitability for the analysis of data on human fertility. The dataset concerns a study of Artificial Insemination by Donor (AID). AID represents an unusual research opportunity to study both male and female fecundability simultaneously. In each attempt to conceive, artificial insemination is carried out in consecutive ovulatory cycles until conception or change of treatment. The probability of conception may differ between women, so that the data are discrete time survival data with censoring and between-subject heterogeneity. There is also potential heterogeneity between donors. Non-systematic allocation of the donor to recipient ensures that the same woman receives semen from several donors, This added heterogeneity as well as other cycle dependent covariates have to be taken into account. The analysis must also take account of covariates, most of them time-varying. Our dataset have a crossed hierarchical structure due to the presence of both, female and male factors. The rather complicated "design" calls for unit specific regression models. These models are presented as well as their lack of tractability except in some rather specific cases. The motivation for choosing Gaussian random effects in unit specific regression models is discussed. We demonstrate the use of an approximate inference method (Penalized Quasi Likelihood). This method is shown to be a useful and practical way of carrying out preliminary data analysis. Finally a Bayesian procedure (Gibbs sampling) provides validation and more accurate results despite the intensive computation it needs. The main substantive finding of the analysis is the unexpectedly pronounced heterogeneity of donor fecundability, even after inclusion of conventional measures of sperm quality into the model. These measures were shown to be predictive at the donor level but not at the level of individual donation.

High-throughput single-molecule techniques are expected to challenge the demand for rapid, simple, and sensitive detection methods in health and environmental fields. Based on a single-DNA-molecule biochip for the parallelization of tethered particle motion analyses by videomicroscopy coupled to image analysis and its smart combination with aptamers, we successfully developed an aptasensor enabling the detection of single target molecules by a sandwich assay. One aptamer is grafted to the nanoparticles tethered to the surface by a long DNA molecule bearing the second aptamer in its middle. The detection and quantification of the target are direct. The recognition of the target by a pair of aptamers leads to a looped configuration of the DNA-particle complex associated with a restricted motion of the particles, which is monitored in real time. An analytical range extending over 3 orders of magnitude of target concentration with a limit of detection in the picomolar range was obtained for thrombin.

The participants underwent serial ovarian ultrasound examinations.Main Outcome Measure(s): Four outcome measures were analyzed: short luteal phase, low mid-luteal phase PDG level (mPDG), normal then low luteal PDG level, low then normal luteal PDG level.Results: A long preovulatory phase was a predictor of short luteal phase, with or without adjustment for other variables. A high periovulatory PDG level was a predictor for short luteal phase as well as normal then low luteal PDG level. A low periovulatory PDG level predicted low mPDG and low then normal luteal PDG level, with or without adjustment for other variables. A small maximum follicle predicted normal then low luteal PDG level, with or without adjustment for other variables. The relationship between small maximum follicle size and short luteal phase or small maximum follicle size and low mPDG was no longer present when the regression was adjusted for certain characteristics. A younger age at menarche and a high body mass index were both predictors of low mPDG.Conclusion: Luteal phase abnormalities exist over a spectrum where some ovulation disorders may exist as deviations from the normal ovulatory process.This study confirms the negative impact of a small follicle size on the quality of the luteal phase. The occurrence of normal then low luteal PDG level is confirmed as a potential sign of luteal phase abnormality.

Abstract

Background The aim of this ancillary study of the SARAH trial is to compare health-related quality of life (HRQoL) in patients with locally advanced or inoperable hepatocellular carcinoma (HCC) treated with transarterial radioembolisation (TARE) or sorafenib. Methods This study included randomised patients who received either TARE or at least one dose of sorafenib with no major deviation in the protocol and who had at least one QoL follow-up assessment in addition to the baseline evaluation. QoL was assessed from the date of randomisation using the European Organisation for Research and Treatment of Cancer QLQ-C30 questionnaire, until disease progression or other reasons for stopping study participation. Data were analysed using linear mixed and time-dependent models. Results A total of 285 patients were included (122 and 163, in the TARE and sorafenib groups, respectively). Questionnaire completion rates were similar (77.5% versus 80.4%, in the TARE and sorafenib groups, respectively, p = 0.25). Longitudinal HRQoL analysis showed a significant treatment and time effects for fatigue and global health status, and significant treatment, time and treatment by time interaction effects for appetite loss, diarrhoea and social functioning. The median time to deterioration for the global health status was 3.9 months (95% confidence interval [CI] 3.7–4.3) versus 2.6 months (95% CI 2.0–3.0) in the TARE and sorafenib groups, respectively. Conclusions HRQoL was preserved longer with TARE than with sorafenib in locally advanced HCC. These data could be used to optimise management of patients with advanced or inoperable HCC.

88. Le vécu de la prise en charge en salle de naissance : une étude qualitative comparative

Abstract

Le but de cette étude était d’étudier le vécu des mères insatisfaites de la prise en charge en salle de naissance en le comparant avec celui de mères satisfaites, à partir des données recueillies par évocations hiérarchisées dans l’Étude du Vécu de l’Accouchement (EVA). Il s’agissait d’une étude qualitative, contrôlée par témoins, en population, réalisée au sein de 25 maternités françaises d’un même territoire (Réseau AURORE). Les expressions recueillies par l’évocation hiérarchisée, appelées verbatim, ont été regroupées en sous-thèmes, thèmes et méta-thèmes. Le critère principal de jugement était le pourcentage de patientes ayant cité au moins un mot appartenant à un sous-thème. Six cent quarante-deux mères ont été incluses dans l’étude EVA. Soixante et onze mères ont répondu « insuffisamment » et/ou « pas du tout » à au moins un des items du questionnaire de satisfaction. Elles ont toutes été incluses dans notre étude et constituaient le groupe de cas (insatisfaites). Le groupe témoin (satisfaites) appariés comprenait 144 mères. Le sous-thème de la peur a été significativement plus évoqué par le groupe de mères insatisfaites que par le groupe témoin (42 versus 22 %, p = 0,02). Concernant le sous-thème de la douleur, aucune différence significative n’a été observée entre les deux groupes (45 versus 33 %, p = 0,09). Les résultats incitent à porter plus d’attention à la réassurance des mères en salle de naissance. Identifier les mères inquiètes en cours de grossesse et au moment de l’accouchement doit être une priorité pour les soignants.

Abstract

During normal menstrual cycles, serum levels of progesterone vary widely between cycles of same woman and between women. This study investigated the profiles of pregnanediol during the luteal phase. Data stemmed from a previous multicenter prospective observational study and concerned 107 women (who contributed 326 menstrual cycles). The study analyzed changes in observed cervical mucus discharge, various hormones in first morning urine, and serum progesterone. Transvaginal ultrasonography and cervical mucus helped identifying the day of ovulation. Changes in pregnanediol glucuronide levels during the luteal phase were examined and classified according to the length of that phase, a location parameter, and a scale parameter. Associations between nine pregnanediol glucuronide profiles and other hormone profiles were examined. Low periovulatory pregnanediol glucuronide levels and low periovulatory luteinizing hormone levels were associated with delayed increases in pregnanediol glucuronide after ovulation. That 'delayed increase profile' was more frequently associated with cycles with prolonged high LH levels than in cycles with rapid pregnanediol glucuronide increases. A 'plateau-like profile' during the luteal phase was associated with longer cycles, cycles with higher estrone-3-glucuronide and pregnanediol glucuronide during the preovulatory phase, and cycles with higher periovulatory pregnanediol glucuronide levels. Distinct profiles of urinary progesterone levels are displayed during the luteal phase. These profiles relate to early hormone changes during the menstrual cycle. In everyday clinical practice, these findings provide further evidence for recommending progesterone test seven days after the mucus peak day. Abstract

Context: Determination of steroid levels in the amniotic fluid gives some insight on fetal adrenal and gonadal functions. Objective: Our objectives were to establish reference ranges of 12 steroid levels throughout pregnancy and to compare them with steroid levels from pregnancies with fetuses presenting with 21-hydroxylase deficiency (21OHD). Methods: Liquid chromatography coupled with tandem mass spectrometry (LC-MS/MS) was applied to 145 “control” amniotic fluid samples from gynecology activity (12 + 6 to 32 + 4 gestational weeks, GW). The following steroids were analyzed according to gestational age and compared to 23 amniotic fluid samples from fetuses with classic 21OHD confirmed by molecular studies: delta-4-androstenedione (D4), dehydroepiandrosterone (DHEA), 17-hydroxyprogesterone (17OHP), 11-deoxycortisol (11OH), 21-deoxycortisol (21OH), corticosterone, deoxycorticosterone (DOC), testosterone, pregnenolone, 17-hydroxypregnenolone (17Pregn), cortisol, and cortisone. Chromosomal sex was determined by karyotype and gestational age by biometric measurements. Results: Analysis of control samples showed a statistically significant difference for D4 and testosterone levels according to fetal sex. Cortisol, corticosterone, and DOC had lower concentrations before 20 GW than after 20 GW, whereas 17Pregn and pregnenolone had higher concentrations before 20 GW. This allowed us to establish age- and sex-dependent reference values. We observed higher 21OH, 17Pregn, D4, and testosterone levels in females with 21OHD than female controls. The ratios 17OHP/17Pregn, D4/DHEA, and 11OH/17OHP appeared discriminant for the diagnosis of 21OHD. Conclusion: Our study provides information on fetal steroidogenesis and suggests reference values for 12 steroids during pregnancy. This allows a prenatal diagnosis of 21OHD within 24 hours and might be useful in the diagnosis of other variations of sex development. [ABSTRACT FROM AUTHOR]
