Background: Individuals with type 2 diabetes mellitus are at increased risk for heart failure (HF), particularly those with coexisting atherosclerotic cardiovascular disease and/or kidney disease. Some but not all dipeptidyl peptidase-4 inhibitors have been associated with increased HF risk. We performed secondary analyses of HF and related outcomes with the dipeptidyl peptidase-4 inhibitor linagliptin versus placebo in CARMELINA (The Cardiovascular and Renal Microvascular Outcome Study With Linagliptin), a cardiovascular outcomes trial that enrolled participants with type 2 diabetes mellitus and atherosclerotic cardiovascular disease and/or kidney disease. Methods: Participants in 27 countries with type 2 diabetes mellitus and concomitant atherosclerotic cardiovascular disease and/or kidney disease were randomized 1:1 to receive once daily oral linagliptin 5 mg or placebo, on top of standard of care. All hospitalization for HF (hHF), cardiovascular outcomes, and deaths were prospectively captured and centrally adjudicated. In prespecified and post hoc analyses of HF and related events, Cox proportional hazards models adjusting for region and baseline history of HF were used. Recurrent hHF events were analyzed using a negative binomial model. In a subset of participants with left ventricular ejection fraction captured within the year before randomization, HF-related outcomes were assessed in subgroups stratified by left ventricular ejection fraction > or 50%. Results: CARMELINA enrolled 6979 participants (mean age, 65.9 years; estimated glomerular filtration rate, mL/min per 1.73m(2); hemoglobin A1c, 8.0%; 62.9% men; diabetes mellitus duration, 14.8 years), including 1873 (26.8%) with a history of HF at baseline. Median follow-up was 2.2 years. Linagliptin versus placebo did not affect the incidence of hHF (209/3494 [6.0%] versus 226/3485 [6.5%], respectively; hazard ratio [HR], 0.90; 95% CI, 0.74-1.08), the composite of cardiovascular death/hHF (HR, 0.94; 95% CI, 0.82-1.08), or risk for recurrent hHF events (326 versus 359 events, respectively; rate ratio, 0.94; 95% CI, 0.75-1.20). There was no heterogeneity of linagliptin effects on hHF by history of HF at baseline, baseline estimated glomerular filtration rate or urine albumin-creatinine ratio, or prerandomization left ventricular ejection fraction. Conclusions: In a large, international cardiovascular outcome trial in participants with type 2 diabetes mellitus and concomitant atherosclerotic cardiovascular disease and/or kidney disease, linagliptin did not affect the risk of hHF or other selected HF-related outcomes, including among participants with and without a history of HF, across the spectrum of kidney disease, and independent of previous left ventricular ejection fraction. RESUMEN Antecedentes: España por su situación geopolítica no está exenta de amenazas y desafíos asociados a enfermedades infecciosas víricas como las «fiebres hemorrágicas víricas». Implementar acciones para disminuir estos riesgos frente a agentes víricos con el desarrollo de un tratamiento de urgencia por vía intravenosa resulta de gran interés. Objetivos: Desarrollar dos formulaciones de Ribavirina 100 mg/ml con distintos excipientes y condiciones de fabricación para buscar la formulación más estable. Material y métodos: Se ha utilizado ribavirina, distintos tampones y agua para inyectables. Se ha evaluado alteraciones físicas, riqueza y pH siguiendo especificaciones de la monografía de la Farmacopea Europea. Resultados: Se exponen resultados de pH y riqueza de ambas formulaciones con y sin esterilización terminal. Conclusiones: Los resultados obtenidos cumplen especificaciones para ambas formulaciones, demostrando una estabilidad química tras su conservación en condiciones de estudios a largo plazo durante 12 meses y acelerados durante 6 meses. El desarrollo de la ribavirina 100 mg/ml (10% w/v) inyectable satisface una necesidad terapéutica relacionada con el tratamiento de primera elección en enfermedades víricas altamente contagiosas que carecen de tratamientos eficaces.

- Abstract
IMPORTANCE Type 2 diabetes is associated with increased cardiovascular (CV) risk. Prior trials have demonstrated CV safety of 3 dipeptidyl peptidase 4 (DPP-4) inhibitors but have included limited numbers of patients with high CV risk and chronic kidney disease. OBJECTIVE To evaluate the effect of linagliptin, a selective DPP-4 inhibitor, on CV outcomes and kidney outcomes in patients with type 2 diabetes at high risk of CV and kidney events. DESIGN, SETTING, AND PARTICIPANTS Randomized, placebo-controlled, multicenter noninferiority trial conducted from August 2013 to August 2016 at 605 clinic sites in 27 countries among adults with type 2 diabetes, hemoglobin A(1c) of 6.5% to 10.0%, high CV risk (history of vascular disease and urine-albumin creatinine ratio [UACR] > 200mg/g), and high renal risk (reduced eGFR and micro-or macroalbuminuria). Participants with end-stage renal disease (ESRD) were excluded. Final follow-up occurred on January 18, 2018. INTERVENTIONS Patients were randomized to receive linagliptin, 5 mg once daily (n = 3494), or placebo once daily (n = 3485) added to usual care. Other glucose-lowering medications or insulin could be added based on clinical need and local clinical guidelines. MAIN OUTCOMES AND MEASURES Primary outcomewas time to first occurrence of the composite of CV death, nonfatalmyocardial infarction, or nonfatal stroke. Criteria for noninferiority of linagliptin vs placebo was defined by the upper limit of the 2-sided 95% CI for the hazard ratio (HR) of linagliptin relative to placebo being less than 1.3. Secondary outcome was time to first occurrence of adjudicated death due to renal failure, ESRD, or sustained 40% or higher decrease in eGFR from baseline. RESULTS Of 6991 enrollees, 6979 (mean age, 65.9 years; eGFR, 54.6 mL/min/1.73m2; 80.1% with UACR > 30mg/g) received at least 1 dose of study medication and 98.7% completed the study. During a median follow-up of 2.2 years, the primary outcome occurred in 434 of 3494 (12.4%) and 420 of 3485 (12.1%) in the linagliptin and placebo groups, respectively, (absolute incidence rate difference, 0.13 [95% CI,-0.63 to 0.90] per 100 person-years) (HR, 1.02; 95% CI, 0.89-1.17; P

Contains fulltext : 208089.pdf (publisher's version ) (Open Access), BACKGROUND: the European Union of Medical Specialists (UEMS-GMS) recommendations for training in Geriatric Medicine were published in 1993. The practice of Geriatric Medicine has developed considerably since then and it has therefore become necessary to update these recommendations. METHODS: under the auspices of the UEMS-GMS, the European Geriatric Medicine Society (EuGMS) and the European Academy of Medicine of Ageing (EAMA), a group of experts, representing all member states of the respective bodies developed a new framework for education and training of specialists in Geriatric Medicine using a modified Delphi technique. Thirty-two expert panel members from 30 different countries participated in the process comprising three Delphi rounds for consensus. The process was led by five facilitators. RESULTS: the final recommendations include four different domains: 'General Considerations' on the structure and aim of the syllabus as well as quality indicators for training (6 sub-items), 'Knowledge in patient care' (36 sub-items), 'Additional Skills and Attitude required for a Geriatrician' (9 sub-items) and a domain on 'Assessment of postgraduate education: which items are important for the transnational comparison process' (1 item). CONCLUSION: the current publication describes the development of the new recommendations endorsed by UEMS-GMS, EuGMS and EAMA as minimum training requirements to become a geriatrician at specialist level in EU member states.

INTRODUCCIÓN: el propósito del presente estudio (primera fase) fue evaluar clínica y radiográficamente la oseointegración de implantes con regeneración ósea por medio de plasma rico en plaquetas (PRP) y hueso liofilizado, colocados en pacientes con pérdida dentoalveolar por trauma en el sector anterior, durante las primeras etapas de cicatrización de tejidos blandos; atendidos en el Hospital Universitario San Vicente de Paúl (HUSVP) de la ciudad de Medellín, Colombia. MÉTODOS: se colocaron quince implantes en seis pacientes que sufrieron pérdida dental por trauma en la zona anterior superior e inferior. La colocación de los implantes se realizó entre los ocho a veinte días posteriores al trauma, los cuales se dejaron sumergidos cuatro meses en la mandíbula y seis meses en el maxilar superior. En la segunda fase quirúrgica se realizó una evaluación clínica y radiográfica para determinar la oseointegración. RESULTADOS: el porcentaje de oseointegración fue 93,3% (catorce implantes). Un implante falló (6,7%) y presentó movilidad sin signos de infección o radiolucidez en la radiografía periapical, lo que sugiere que la falla pudo ocurrir por poca cantidad ósea o trauma quirúrgico. CONCLUSIÓN: el presente estudio mostró que el porcentaje de oseointegración en pacientes con trauma dentoalveolar fue bueno con la utilización del PRP según el protocolo utilizado.INTRODUCTION: the aim of the present study was to evaluate clinically and radiographically the osseointegration of implants with bone regeneration by platelet rich plasma (PRP) and freeze-dried bone, placed in patients with dentoalveolar loss by trauma in the anterior region, during the firsts stages of soft tissue healing, treated at the Hospital Universitario San Vicente de Paúl (Medellín-Colombia). METHODS: 15 implants were placed in six patients who suffered tooth loss due to trauma in the anterior maxillary and mandible area. The placement of the implants was performed between eight to twenty days after trauma, and they were submerged four months in the mandible and six months in the maxilla. A clinical and radiographic evaluation was done at the second-stage surgery to determine osseointegration. RESULTS: osseointegration percentage was 93.3% (14 implants). An implant failed (6.7%) and showed mobility without any infection signs or radiolucency, which suggests that failure, could be the result of surgical trauma. CONCLUSIONS: the present study showed that osseointegration percentage in patients with dentoalveolar trauma was good with the utilization of PRP according to the present protocol.

Introducción: La brucelosis es un enfermedad zoonotica relacionada al contacto con perros y con ganado vacuno, porcino y caprino, el agente etiológico pertenece al género Brucella con diez especies de importancia para el ser humano; su capacidad para evadir el sistema inmune y sobrevivir en el fago-lisosoma genera un cuadro clínico crónico en el ser humano caracterizado por fiebre ondulante y afectación osteoarticular. Su amplia variedad de expresión clínica genera un reto al momento del diagnóstico y tratamiento; por tal razón es relevante el reconocimiento de las pruebas diagnósticas que permitan establecer el posible contacto con el microorganismo. Objetivo: Establecer la seroprevalencia a Brucella spp en una población de estudiantes de Medicina Veterinaria con posible exposición ocupacional. Materiales y Métodos: Estudio descriptivo observacional donde se estudiaron 272 sueros obtenidos de estudiantes de medicina veterinaria mediante la técnica de rosa de bengala. Resultados: El 18,4% (50/272) de los sujetos presento positividad por rosa de bengala; 28 hombres (56%) y 22 mujeres (44%). Conclusiones: La serología mediante rosa de bengala en esta población de riesgo evidenció un 18,5% de sujetos seropositivos. Se sugiere el empleo de barreras efectivas de protección durante el contacto con animales portadores del microorganismo durante el entrenamiento como estudiantes de Medicina veterinaria. Introduction: Brucellosis is a zoonotic disease related to contact with dogs and cattle, pigs and goats, the etiologic agent belongs to the genus Brucella with ten species of importance to humans, its ability to evade the immune system and survive in the phage-lysosome generate a chronic clinical picture in humans characterized by undulant fever and osteoarticular involvement. Its wide range of clinical expression generates a challenge at the time of diagnosis and treatment, for this reason, it is important the recognition of the diagnostic tests for establishing the possible contact with the microorganism. Objective: To establish the seroprevalence to Brucella spp in a sample of Veterinary Medicine students with possible occupational exposure. Materials and Methods: Observational descriptive study by means of rose bengal technique 272 sera obtained from veterinary students were processed. Results: 18.4% (50/272) of subjects were positive by rose bengal test; 28 men (56%) and 22 women (44%). Conclusion: Serology by rose bengal test in this risk population showed 18.5% of seropositive subjects. It suggests the use of effective protective barriers during contact with animals carrying the organism during training as veterinary medical students.

Sago flour obtained using the Argao process (indigenous Philippine flour extraction process) was prepared into different mesh sizes (60, 100 and 200 mesh). The sieved flour was divided into fine (F) and course (C) fractions and were tested for percent process recovery, proximate analysis, total starch, total phenolic content, and antioxidant activity. Percent process recovery ranged from 77-89%. Crude fat content was less than 1% (w/w) for all samples while crude fiber content were significantly higher in the coarse fractions. Starch content(db) of the fine fractions were 88.31±1.78% (F60), 92.87±1.49% (F100), and 96.62±1.03% (F200) while coarse fractions had a total starch content(db) of 74.65±1.20% (C60), 68.66±0.96% (C100), and 64.67±1.14% (C200). Total polyphenol content of fine flour fractions (2.83-6.17mg GAE/g sample) was significantly lower than the course fractions (31.50-42.76mg GAE/g sample). Moreover, sago was used as substrate for lactic acid production. Agar, alginate and ?-carrageenan were tested as immobilization matrices for E. faecium DMF78. Alginate (3% w/v) extruded drop wise on 0.2M CaCl2, and cured for 1hr was found to be the most efficient immobilizing matrix. The use of immobilized and free cells led to the production of 3.011g/L and 4.80g/L lactic acid after 24hrs, representing lactic acid productivity of 0.125 g.L
-1 h-1 and 0.200 g.L-1 h-1 and lactic acid yield (conversion) of 25.42% and 39.76%, respectively. [ABSTRACT FROM AUTHOR]- Published

33 páginas, 18 figuras, 4 tablas., Se provee una síntesis del conocimiento actual sobre la evolución de la relación río Amazonas- sistema lagunar de Yahuarcaca a través de las características limnológicas y de la ecología del fitoplancton durante 16 años de estudio. La síntesis de la información fue realizada mediante la revisión de los datos existentes sobre el sistema, correspondiente al tiempo indicado y el análisis mediante estadística descriptiva y correlación lineal de las variables encontradas. Asimismo, se presenta una síntesis de los aspectos más relevantes del primer y único trabajo que en Colombia ha evaluado la influencia del pulso de inundación sobre el fitoplancton en una escala temporal diaria. Su patrón térmico corresponde al de un lago polimíctico cálido continuo según la clasificación de Lewis (1983). Las variables físicas, químicas y biológicas analizadas en este sistema de várzea varían de acuerdo al período hidrológico. La conductividad, transparencia y riqueza del fitoplancton presentan sus mayores valores en temporada de aguas altas, mientras que la concentración de nutrientes, densidad, productividad y biomasa fitoplanctónica aumentan en aguas bajas. El nitrato constituye el nutriente limitante en aguas bajas y el fosfato en aguas altas. Los cambios que se han dado en la conectividad del río con el sistema a través de los años se reflejan en la dinámica de la estructura y composición del fitoplancton durante el tiempo de estudio., El programa Bicentenario - Amazonia patrocinó con el financiamiento de una beca a la primera autora para desarrollar este escrito.

From the n-butanol extract of the aerial parts of Exellodendron coriaceum (Benth.) Prance the flavonoids quercetin-3-O-beta-D-galactopyranoside (1), quercetin-3-O-alpha-L-arabinopyranoside (2), quercetin-3-O-alpha-L-rhamnopyranosyl-(1--2)-alpha-L-rhamnopyranoside (3), and quercetin-3-O-alpha-L-rhamnopyranosyl-(1--6)-beta-D-galactopyranoside (4) were isolated. Additionally from this extract three flavonoids were isolated and partially characterized as quercetin glycosides. All these compounds were tested for their hypoglycemic activity using the glucose-6-phosphatase microsomal hepatic system. The flavonoids inhibited the activity of the enzyme when intact microsomes were used, the highest percentage of inhibition being 65%. To the best of our knowledge, this is the first report of chemical and biological activity of E. coriaceum.

This paper presents an analysis of the performance of TerraSAR-X for subsidence monitoring in urban areas. The city of Murcia has been selected as a test-site due to its high deformation rate and the set of extensometers deployed along the city that provide validation data. The obtained results have been compared with those obtained from ERS/ENVISAT data belonging to the same period and validated with the in-situ measurements. - Abstract
Grey Platelet disease is a rare disease characterized by morphologic changes in platelets alpha-granules. These proteins are essential for the homeostasis, so the patients with this blood defect present hemorrhagic disturbs. The blood discrasia is usually mild, however some patients could present more serious manifestations, usually after a severe trauma. The authors present the following clinical report about a patient that was admitted in a Medical Department with a severe bleeding and mild thrombocytopenia. The patient was submitted to an extensive study to determine the etiology (autoimmunity serology, myelogram, coagulation study) that were all normal. The diagnosis of Idiopathic Thrombocytopenic Purpura was considered. Six months after a worsening of the hemorrhagic discrasia and thrombocytopenia the patient was assisted. Corticotherapy was initiated without improvement. The morphology of the platelets was revised and the blood smear with Wright coloration revealed the presence of large, pale and grey platelets. The electronic microscopy confirmed the diagnosis of Grey Platelet disease. The family of the patient was studied and we found that two direct relatives were affected with the same disease. In these family these syndrome probably has autossomic dominant inherence. publishersversion published

La osteotomía Le Fort I es un procedimiento rutinario en cirugía ortognática. Los autores reportan la ocurrencia de complicaciones intra- y posquirúrgicas. La osteotomía Le Fort I está relacionada con gran variedad de complicaciones. Un conocimiento preciso de las complicaciones debería ayudar al cirujano, al ortodoncista y al paciente para estimar los beneficios de una cirugía electiva versus los riesgos. El conocer las complicaciones también debería ayudar a prevenir su ocurrencia y facilitar el tratamiento. El propósito de esta revisión fue reportar las complicaciones quirúrgicas y posquirúrgicas que están relacionadas con la osteotomía Le Fort I. Se presentan las complicaciones en el siguiente orden: oftalmológicas, vasculares, del seno cavernoso, necrosis avascular, edema, infecciosas, neurológicas, resultados estéticos desfavorables, cambios pulpares y otras complicaciones de rara ocurrencia. Le Fort I osteotomy has become a routine procedure in elective orthognathic surgery. The authors report the occurrence of intra and post operative complications in Le Fort I osteotomy. The Le Fort I osteotomy is related to a variety of complications. A precise assessment of complications might be helpful for the patient, the orthodontist, and the surgeon in order to estimate the benefit of an elective operation versus its immanent risks. Knowledge of complications also might help to prevent their occurrence and facilitate their management. The aim of this review was to report the types and frequencies of intra- and post operative complications related to Le Fort I osteotomy. This review presents surgical complications in the following order: Ophthalmic complications, Vascular complications, Cavernous sinus fistula, Ischemic complications including aseptic necrosis, swelling, significant infections such as abscesses or maxillary sinusitis, Nerve injures, unfavorable esthetic results, unfavorable pulpal reactions and other unusual complications.

Objetivos: Establecer la incidencia y evolución de malnutrición en una cohorte de pacientes ingresados en un servicio de traumatología. Ámbito: Es un estudio observacional y prospectivo, realizado en un hospital terciario que dispone de Unidad de Nutrición Clínica. Pacientes: Se estudian 107 pacientes (56,1% mujeres, 43,9% hombres, edad 62,1 ± 20,4 años) al ingreso en el Servicio de Traumatología y la evolución en la primera consulta externa de los pacientes intervenidos para colocar una prótesis de cadera o rodilla. Intervenciones: Se realizó una historia clínica, recogida de datos antropométricos y tres tests de cribado nutricional: Mini-Nutritional Assessment (MNA), Malnutrition Universal Screening Tool (MUST) y Valoración Global Subjetiva (VGS). Con ellos se calcula la prevalencia de malnutrición al ingreso, comparando las diferencias entre los métodos y la posible existencia de asociación entre la malnutrición y otros factores, tales como úlceras por presión o dependencia para las actividades de la vida diaria. Resultados: Según el MNA el 22% tiene riesgo de malnutrición, en la VGS es un 24% (un 4% están malnutridos) y en el MUST un 80% presenta alto riesgo de malnutrición. Un 1,7% de los pacientes tienen un Índice de Masa Corporal (IMC) menor que 18,5. Existe una relación significativa entre la malnutrición y estado neuropsicológico (p = 0,001), dependencia para las actividades de la vida diaria (p = 0,002) y entre el MUST de alto y medio riesgo con la aparición de úlceras por presión (p = 0,003). Hay empeoramiento del estado nutricional postquirúrgico con 2 tests: MNA y VGS (p = 0,000). Conclusión: En las condiciones del estudio la prevalencia de malnutrición al ingreso en el Servicio de Traumatología es elevada, los pacientes con cirugía protésica de rodilla y cadera muestran al mes un claro empeoramiento de su estado nutricional, así como una mayor incidencia de úlceras por presión. Objective: To assess the nutritional status of orthopaedic patients. Design: Prospective observation study. Setting: Tertiary hospital, Clinical Nutrition Unit. Patients: 107 patients (56,1% women, 43,9% men, 62,1± 20,4 years) are studied at admission to the orthopaedic unit and it is observed the evolution of orthopaedic surgery patients with hip or knee prosthesis. Material and methods: Medical history, anthropometric data and three nutritional screening tools were considered: Mini-Nutritional Assessment (MNA), Malnutrition Universal Screening Tool (MUST) y Valoración Global Subjetiva (VGS). It was assessed the prevalence of malnutrition in patients on admission to hospital, using different methods and determining if a correlation exists between malnutrition and other factors, such as the development of pressure sores of the dependence in activities of daily living. Results: The risk of malnutrition is 22 and 24/100 patients admitted at hospital, if MNA and SGA are used, respectively (SGA caregorized 4% of the group as "malnourished"). 80% patients are considered to be at high risk of malnutrition using MUST. 1,7% patients presented a Body Mass Index (BMI) lower than 18,5. There is a strong agreement between malnutrition and neropsychological status (p = 0,001), and the dependence in activities of daily living (p = 0,002), and between medium and high risk of malnutrition and the development of pressure sores (p = 0,003). If MNA and SGA (p = 0,000) are used, post-surgical patients presents a deteriorating nutritional status. Conclusions: The prevalence of malnutrition in patients on admission to hospital is elevated. Orthopaedic surgery patients with hip and knee prosthesis presented a deteriorating nutritional status, and a higher incidence of pressure sores in a month.

Objetivos: Establecer la incidencia y evolución de malnutrición en una cohorte de pacientes ingresados en un servicio de traumatología. Ámbito: Es un estudio observacional y prospectivo, realizado en un hospital terciario que dispone de Unidad de Nutrición Clínica. Pacientes: Se estudian 107 pacientes (56,1% mujeres, 43,9% hombres, edad 62,1 ± 20,4 años) al ingreso en el Servicio de Traumatología y la evolución en la primera consulta externa de los pacientes intervenidos para colocar una prótesis de cadera o rodilla. Intervenciones: Se realizó una historia clínica, recogida de datos antropométricos y tres tests de cribado nutricional: Mini-Nutritional Assessment (MNA), Malnutrition Universal Screening Tool (MUST) y Valoración Global Subjetiva (VGS). Con ellos se calcula la prevalencia de malnutrición al ingreso, comparando las diferencias entre los métodos y la posible existencia de asociación entre la malnutrición y otros factores, tales como úlceras por presión o dependencia para las actividades de la vida diaria. Resultados: Según el MNA el 22% tiene riesgo de malnutrición, en la VGS es un 24% (un 4% están malnutridos) y en el MUST un 80% presenta alto riesgo de malnutrición. Un 1,7% de los pacientes tienen un Índice de Masa Corporal (IMC) menor que 18,5. Existe una relación significativa entre la malnutrición y estado neuropsicológico (p = 0,001), dependencia para las actividades de la vida diaria (p = 0,002) y entre el MUST de alto y medio riesgo con la aparición de úlceras por presión (p = 0,003). Hay empeoramiento del estado nutricional postquirúrgico con 2 tests: MNA y VGS (p = 0,000). Conclusión: En las condiciones del estudio la prevalencia de malnutrición al ingreso en el Servicio de Traumatología es elevada, los pacientes con cirugía protésica de rodilla y cadera muestran al mes un claro empeoramiento de su estado nutricional, así como una mayor incidencia de úlceras por presión.

O presente estudo teve como objetivo analisar a percepção do adolescente em conflito com a lei da medida sócio-educativa de liberdade assistida, tendo em vista os valores que ele destaca como importantes no processo de integração social. Os valores deveriam ser propiciadores da mediação para a construção de uma cultura da não-violência. Foram realizadas entrevistas semi-estruturadas com 15 (quinze) adolescentes que participaram do Programa de Liberdade Assistida da Prefeitura Municipal de Colatina/ES durante o ano de 2005. Foi remetido questionário para a equipe técnica do programa, além de grupo focal com os familiares para corroborar com as informações fornecidas. A abordagem foi quanti-qualitativa e atécnica de análise dos dados utilizada foi a aálise de conteúdo. Segundo a percepção dos adolescentes, o programa conseguiu destacar os valores de família e cultura. O programa foi considerado relevante em relação às qualificações profissionais, mas deficitário na inserção no mercado de trabalho. A cidadania e a educação como questão política é ainda insuficiente, até porque a redução do Estado conduz as soluções individuais para satisfação das necessidades sociais. O programa de LA aparece como uma tentativa de articular as alianças sociais em torno da problemática que envolve o adolescente em conflito com lei. A experiência da prefeitura Municipal de Colatina/ES demosntra ser reaplicável em outras realidades, pois trabalha com elementos universalizantes sem esquecer as especificidades da realidade local. Introduction: Brucellosis is a zoonotic disease related to contact with dogs and cattle, pigs and goats, the etiologic agent belongs to the genus Brucella with ten species of importance to humans, its ability to evade the immune system and survive in the phage-lysosome generate a chronic clinical picture in humans characterized by undulant fever and osteoarticular involvement. Its wide range of clinical expression generates a challenge at the time of diagnosis and treatment, for this reason, it is important the recognition of the diagnostic tests for establishing the possible contact with the microorganism. Objective: To establish the seroprevalence to Brucella spp in a sample of Veterinary Medicine students with possible occupational exposure. Materials and Methods: Observational descriptive study by means of rose bengal technique 272 sera obtained from veterinary students were processed. Results: 18.4% (50/272) of subjects were positive by rose bengal test; 28 men (56%) and 22 women (44%). Conclusion: Serology by rose bengal test in this risk population showed 18.5% of seropositive subjects. It suggests the use of effective protective barriers during contact with animals carrying the organism during training as veterinary medical students., Introducción: La brucelosis es un enfermedad zoonotica relacionada al contacto con perros y con ganado vacuno, porcino y caprino, el agente etiológico pertenece al género Brucella con diez especies de importancia para el ser humano; su capacidad para evadir el sistema inmune y sobrevivir en el fago-lisosoma genera un cuadro clínico crónico en el ser humano caracterizado por fiebre ondulante y afectación osteoarticular. Su amplia variedad de expresión clínica genera un reto al momento del diagnóstico y tratamiento; por tal razón es relevante el reconocimiento de las pruebas diagnósticas que permitan establecer el posible contacto con el microorganismo. Objetivo: Establecer la seroprevalencia a Brucella spp en una población de estudiantes de Medicina Veterinaria con posible exposición ocupacional. Materiales y Métodos: Estudio descriptivo observacional donde se estudiaron 272 sueros obtenidos de estudiantes de medicina veterinaria mediante la técnica de rosa de bengala. Resultados: El 18,4% (50/272) de los sujetos presento positividad por rosa de bengala; 28 hombres (56%) y 22 mujeres (44%). Conclusiones: La serología mediante rosa de bengala en esta población de riesgo evidenció un 18,5% de sujetos seropositivos. Se sugiere el empleo de barreras efectivas de protección durante el contacto con animales portadores del microorganismo durante el entrenamiento como estudiantes de Medicina veterinaria.

Introduction: In recent years, Colombia recognized orphan diseases as a problem of public health interest and ordered its mandatory notification. Objective: Describe the information on orphan-rare diseases obtained in Cali through SIVIGILA in the first 2 years of registration. Materials and methods: Analytical cross-sectional observational study. Absolute and relative frequen-cies were calculated. Normality analysis of Shapiro Wilk was performed. Prevalence was calculated. The relationship of different sociodemographic and clini-cal variables and mortality risk was evaluated, using Generalized Linear Models, the Poisson distribution family, Logarithmic link function and robust variance models. Results: 635 cases were notified, 78 in 2016 for a pre-valence of 3,25 / 100.000 and 557 in 2017 for a pre-valence of 23,01 / 100.000. Most cases belong to the tax system. The communes with the highest number of cases and the highest prevalence were 17 and 22. Among the first most common orphan-rare diseases, sickle cell disease was the most reported in Cali with 25 cases in 2016 (prevalence 1,04/100.000) and 77 ca-ses in 2017 (prevalence 3,1/100.000). The estimated crude mortality rate for the study period was 0,83 / 100.000, and the diseases with the highest mortality were sickle cell disease in women (0,12 / 100.000) and polyneuropathy in men (0,13 / 100.000). Discussion: It is necessary to carry out and publish in the future, deeper analyzes through the detailed review of medical records and the incorporation of other available sources such as the Individual Registry of Provision of Services (RIPS) and the Unique Regis-try of Affiliates (RUAF), with in order to reduce the sub-registry and provide the whole community with more precise and detailed information.
