The accuracy of two home blood glucose monitors using forearm blood samples was studied. Blood samples were obtained from adults by venipuncture and analyzed for glucose concentration. Within five minutes after venipuncture, peripheral blood glucose measurements were taken in duplicate with the FreeStyle and One Touch Ultra monitors by using the same forearm as used for venipuncture. Accuracy was assessed by comparing the results for each first peripheral stick obtained by each monitor with the laboratory reference value. Monitor precision was evaluated by evaluating the mean difference in glucose values between the first and second peripheral sticks. Blood samples were obtained from 250 subjects, 170 (68%) of whom had diabetes mellitus. A total of 98.8% and 98.4% of the blood glucose readings obtained with FreeStyle and One Touch Ultra, respectively, were clinically acceptable, and 65.2% and 59.2% of the measurements, respectively, were within 10% of the laboratory reference values. The average difference between the first and second glucose measurements was 9.7 mg/dL for FreeStyle and 5.2 mg/dL for One Touch Ultra. One peripheral stick was needed to obtain an adequate blood sample in 95.6% of subjects using FreeStyle, compared with 83.2% of subjects using One Touch Ultra. Although in-school substance abuse prevention efforts have improved over the past decade, youth with disabilities have frequently been neglected by those efforts, despite the fact that they use alcohol, tobacco, and other drugs (ATOD) as must as or more than their peers. In the current pilot study special education teachers were exposed to experiences designed to enhance their skills in adapting substance abuse prevention activities and materials, and presenting them to their students. Although pilot students noted an increase in their teachers' emphasis on substance abuse prevention in their classes and their criterion-related attitudes/behaviors were somewhat higher than those observed for a group of control students, the differences between that two groups' criterion scores did not differ significantly. Cardiovascular diseases (CVDs) are a leading cause of morbidity and mortality, with disparities observed between males and females. Psychosocial work exposures (PWE), including workload, job control, reward and long working hours, are associated with CVD development. Despite higher prevalence among females, the association with CVD is consistently observed in males, with limited explanations for these differences. This study aimed to examine the consideration of sex and gender in prospective studies within systematic reviews on PWE - specifically, the demand-control model, the effort-reward imbalance model, and long working hours - and CVD. Conducting a systematic review, we assessed sex and gender considerations using criteria from the Sex and Gender Equity in Research (SAGER) guidelines. While most studies recognized potential sex and gender differences in the associations between PWE and CVD, only about half of the 28 studies that included both sexes (15 studies) analyzed females and males separately. Moreover, few studies included criteria for sex- and gender-based analyses. Less than half of the studies (23 studies) incorporated a sex and/or gender perspective to discuss observed differences and similarities between men and women. Objective: The choice of optimal analgesia following an adenotonsillectomy is a clinical issue because of the risk of respiratory depression and bleeding. The objective of this study was to assess the effect of celecoxib on opioid use and pain scores in children hospitalized after adenotonsillectomy and to document its adverse effects., Methods: This retrospective study was conducted in a tertiary care pediatric hospital. We compared a group of subjects aged 1 to 17 years who were prescribed celecoxib and opioids between January 2017 and June 2020 following an adenotonsillectomy during a 3-day or less hospitalization to a group of matched controls for sex, age, and length of stay who were prescribed opioids., Results: A total of 228 patients were identified (76 in the celecoxib + opioids group, 152 in the control group). Opioid use, in oral morphine equivalent daily dose, was lower in the celecoxib + opioids group at 0 to 24 hours of hospitalization (0.15 vs 0.20 mg/kg/day, p = 0.05). Initiating celecoxib within 24 hours of surgery (n = 60) significantly reduced opioid requirement for up to 48 hours compared with controls (0-24 hours: 0.12 vs 0.20 mg/kg/day, p = 0.002; 25-48 hours: 0.02 vs 0.09 mg/kg/day, p = 0.001). A shorter length of stay was observed for patients receiving celecoxib + opioids during the first 24-hour post--operative period (27 vs 32 hours, p = 0.01). With celecoxib use, no significant change in pain scores and occurrence of adverse effects including bleeding was found., Conclusions: Using celecoxib early after an adenotonsillectomy has reduced both opioid use and duration of hospital stay without increasing adverse effects or bleeding., Competing Interests: Disclosure. The severe shortfall in testing supplies during the initial COVID-19 outbreak and ensuing struggle to manage the pandemic have affirmed the critical importance of optimal supply-constrained resource allocation strategies for controlling novel disease epidemics. To address the challenge of constrained resource optimization for managing diseases with complications like pre- and asymptomatic transmission, we develop an integro partial differential equation compartmental disease model which incorporates realistic latent, incubation, and infectious period distributions along with limited testing supplies for identifying and quarantining infected individuals. Our model overcomes the limitations of typical ordinary differential equation compartmental models by decoupling symptom status from model compartments to allow a more realistic representation of symptom onset and presymptomatic transmission. To analyze the influence of these realistic features on disease controllability, we find optimal strategies for reducing total infection sizes that allocate limited testing resources between 'clinical' testing, which targets symptomatic individuals, and 'non-clinical' testing, which targets non-symptomatic individuals. We apply our model not only to the original, delta, and omicron COVID-19 variants, but also to generically parameterized disease systems with varying mismatches between latent and incubation period distributions, which permit varying degrees of presymptomatic transmission or symptom onset before infectiousness. We find that factors that decrease controllability generally call for reduced levels of non-clinical testing in optimal strategies, while the relationship between incubation-latent mismatch, controllability, and optimal strategies is complicated. In particular, though greater degrees of presymptomatic transmission reduce disease controllability, they may increase or decrease the role of non-clinical testing in optimal strategies depending on other disease factors like transmissibility and latent period length. Importantly, our model allows a spectrum of diseases to be compared within a consistent framework such that lessons learned from COVID-19 can be transferred to resource constrained scenarios in future emerging epidemics and analyzed for optimality., (© 2023 The Authors.)
