Abstract

Trigonella foenum-graecum L. (fenugreek) is widely used as a leafy vegetable and spice in China and North Africa. Recent studies have reported that fenugreek can reduce fatigue; however, its antifatigue mechanism remains unclear. Therefore, this study aimed to investigate the potential antifatigue effects of fenugreek extract (FE) on mitophagy and the underlying mechanisms.We evaluated the potential effects of FE tablet on an exhaustive exercise-induced fatigue (EEF) rat model. Oxidative stress indicators and fatigue biomarkers in the serum and skeletal muscle were detected. Mitophagy and mitochondrial morphology were observed using transmission electron microscopy. The expression levels of mitochondrial autophagy-related proteins were detected using western blot and immunofluorescence.Compared with the model group, FE enhanced the activities of the antioxidant enzymes superoxide dismutase and glutathione peroxidase as well as total antioxidant capacity; however, it decreased the level of malondialdehyde in the serum and skeletal muscle after a 7-day treatment. Moreover, certain indicators of mitochondrial function, such as reactive oxygen species levels, ATP levels, cellular and mitochondrial Ca2+ levels, and ATPase activity, were significantly improved in the FE group compared with the model group. Finally, we found that mitophagy was induced by exhaustive exercise and inhibited by FE. Regarding mitochondrial autophagy-related proteins, the expression levels of LC3B, FUNDC1, PGAM5, PARKIN, and PINK1 in the skeletal muscle tissue were increased in the EEF group compared with the control group. After administration of FE and a positive control drug, a significant reversal in the expression of the above-mentioned proteins was noted.Our findings demonstrate that FE exerted antifatigue effects in the EEF rat model by regulating the mitophagy-related FUNDC1/LC3B signaling pathway rather than the PINK1/PARKIN signaling pathway.

52. Comparison of community and clinic-based blood pressure measurements: A cross-sectional study from Haiti

Abstract

Hypertension (HTN) is the leading modifiable cardiovascular disease (CVD) risk factor in low and middle-income countries, and accurate and accessible blood pressure (BP) measurement is essential for identifying persons at risk. Given the convenience and increased use of community BP screening programs in low-income settings, we compared community and clinic BP measurements for participants in the Haiti CVD Cohort Study to determine the concordance of these two measurements. Participants were recruited using multistage random sampling from March 2019 to August 2021. HTN was defined as systolic BP (SBP) ≥ 140mmHg, diastolic BP (DBP) ≥ 90mmHg or taking antihypertensives according to WHO guidelines. Factors associated with concordance versus discordance of community and clinic BP measurements were assessed with multivariable Poisson regressions. Among 2,123 participants, median age was 41 years and 62% were female. Pearson correlation coefficients for clinic versus community SBP and DBP were 0.78 and 0.77, respectively. Using community BP measurements, 36% of participants screened positive for HTN compared with 30% using clinic BPs. The majority of participants had concordant measurements of normotension (59%) or HTN (26%) across both settings, with 4% having isolated elevated clinic BP (≥140/90 in clinic with normal community BP) and 10% with isolated elevated community BP (≥140/90 in community with normal clinic BP). These results underscore community BP measurements as a feasible and accurate way to increase HTN screening and estimate HTN prevalence for vulnerable populations with barriers to clinic access.

Abstract

Haiti is one of the most food-insecure (FIS) nations in the world, with increasing rates of overweight and obesity. This study aimed to characterize FIS among households in urban Haiti and assess the relationship between FIS and body mass index (BMI) using enrollment data from the Haiti Cardiovascular Disease Cohort Study. FIS was characterized as no/low, moderate/high, and extreme based on the Household Food Security Scale. Multinomial logistic generalized estimating equations were used to evaluate the association between FIS categories and BMI, with obesity defined as BMI ≥ 30 kg/m2. Among 2972 participants, the prevalence of moderate/high FIS was 40.1% and extreme FIS was 43.7%. Those with extreme FIS had higher median age (41 vs. 38 years) and were less educated (secondary education: 11.6% vs. 20.3%) compared to those with no/low FIS. Although all FIS categories had high obesity prevalence, those with extreme FIS compared to no/low FIS (15.3% vs. 21.6%) had the lowest prevalence. Multivariable models showed an inverse relationship between FIS and obesity: moderate/high FIS (OR: 0.77, 95% CI: 0.56, 1.08) and extreme FIS (OR: 0.58, 95% CI: 0.42, 0.81) versus no/low FIS were associated with lower adjusted odds of obesity. We found high prevalence of extreme FIS in urban Haiti in a transitioning nutrition setting. The inverse relationship between extreme FIS and obesity needs to be further studied to reduce both FIS and obesity in this population.

Abstract

Despite the increasing role of chronic kidney disease (CKD) as a major cause of global morbidity and mortality, there is limited understanding of its prevalence and risk factors in low-income countries, including Haiti. We hypothesized the prevalence of CKD is high in urban Haiti due to high rates of uncontrolled risk factors, primarily hypertension (HTN). We evaluated the prevalence and risk factors for CKD in a population-based cohort from Haiti. Among 2426 participants ≥18 years, serum creatinine and urinary albumin and creatinine were measured at time of study enrollment. Estimated glomerular filtration rate (eGFR) was dichotomized at 2 . Albuminuria was defined as an albumin to creatinine ratio (ACR) ≥30 mg/g. CKD was defined as reduced eGFR and/or albuminuria. HTN was defined as a systolic blood pressure (BP) ≥140 mmHg, diastolic BP ≥90 mmHg, or use of antihypertensive medication. Factors associated with CKD were assessed with logistic regression. Abstract

Ultra-wideband (UWB) positioning technology stands out from many indoor positioning technologies with its advantages of high precision. However, non-line-of-sight (NLOS) propagate leads to heavy range error and reduces position accuracy, this paper proposes a NLOS identification method based on channel impulse response (CIR), which includes three stages. Firstly, CIR based feature selection is carried out, which includes correlation analysis of calculated features. Secondly, fuzzy comprehensive evaluation model is introduced to NLOS identification. Finally, time of arrival (TOA) based location estimation is realized after NLOS ranging error mitigation. Simulation results show that the average identification rate of NLOS can exceed 86%, and the average positioning error can be reduced by about 0.5m.

Abstract

Abstract The decays of the χb1(1P), χb2(1P), χb1(2P) and χb2(2P) mesons into the Υ(1S)μ+μ − final state are observed with a high significance using proton-proton collision data collected with the LHCb detector and corresponding to an integrated luminosity of 9 fb −1. The newly observed decays together with the Υ(2S) → Υ(1S)π+π − and Υ(3S) → Υ(2S)π+π − decay modes are used for precision measurements of the mass and mass splittings for the hidden-beauty states.

Abstract

Abstract A comprehensive study of the local and nonlocal amplitudes contributing to the decay B 0 → K *0(→ K + π − )μ + μ − is performed by analysing the phase-space distribution of the decay products. The analysis is based on pp collision data corresponding to an integrated luminosity of 8.4 fb −1 collected by the LHCb experiment. This measurement employs for the first time a model of both one-particle and two-particle nonlocal amplitudes, and utilises the complete dimuon mass spectrum without any veto regions around the narrow charmonium resonances. In this way it is possible to explicitly isolate the local and nonlocal contributions and capture the interference between them. The results show that interference with nonlocal contributions, although larger than predicted, only has a minor impact on the Wilson Coefficients determined from the fit to the data. For the local contributions, the Wilson Coefficient C 9 $$ {\mathcal{C}}_9 $$ , responsible for vector dimuon currents, exhibits a 2.1σ deviation from the Standard Model expectation. The Wilson Coefficients C 10 $$ {\mathcal{C}}_{10} $$ , C 9 ′ $$ {\mathcal{C}}_9^{\prime } $$ and C 10 ′ $$ {\mathcal{C}}_{10}^{\prime } $$ are all in better agreement than C 9 $$ {\mathcal{C}}_9 $$ with the Standard Model and the global significance is at the level of 1.5σ. The model used also accounts for nonlocal contributions from B 0 → K *0[τ + τ − → μ + μ − ] rescattering, resulting in the first direct measurement of the bsττ vector effective-coupling C 9 τ $$ {\mathcal{C}}_{9\tau } $$ .

Abstract

Abstract A measurement of the transverse polarization of the Λ and Λ ¯ $$ \overline{\varLambda} $$ hyperons in pNe fixed-target collisions at s NN $$ \sqrt{{\textrm{s}}_{\textrm{NN}}} $$ = 68.4 GeV is presented using data collected by the LHCb detector. The polarization is studied using the decay Λ → pπ − together with its charge conjugated process, the integrated values measured are P Λ = 0.029 ± 0.019 stat ± 0.012 syst , P Λ ¯ = 0.003 ± 0.023 stat ± 0.014 syst . $$ {\displaystyle \begin{array}{c}{P}_{\varLambda }=0.029\pm 0.019\left(\textrm{stat}\right)\pm 0.012\left(\textrm{syst}\right),\\ {}{P}_{\overline{\varLambda}}=0.003\pm 0.023\left(\textrm{stat}\right)\pm 0.014\left(\textrm{syst}\right).\end{array}} $$ Furthermore, the results are shown as a function of the Feynman x variable, transverse momentum, pseudorapidity and rapidity of the hyperons, and are compared with previous measurements.

Abstract

Abstract Decays of Ξ b − $$ {\Xi}_b^{-} $$ and Ω b − $$ {\Omega}_b^{-} $$ baryons to Λ c + h − h ′ − $$ {\Lambda}_c^{+}{h}^{-}{h}^{\prime -} $$ final states, with h − h ′− being π − π − , K − π − and K − K − meson pairs, are searched for using data collected with the LHCb detector. The data sample studied corresponds to an integrated luminosity of 8.7 fb −1 of pp collisions collected at centre-of-mass energies s $$ \sqrt{s} $$ = 7, 8 and 13 TeV. The products of the relative branching fractions and fragmentation fractions for each signal mode, relative to the B − → Λ c + p ¯ π − $$ {B}^{-}\to {\Lambda}_c^{+}\overline{p}{\pi}^{-} $$ mode, are measured, with Ξ b − → Λ c + K − π − $$ {\Xi}_b^{-}\to {\Lambda}_c^{+}{K}^{-}{\pi}^{-} $$ , Ξ b − → Λ c + K − K − $$ {\Xi}_b^{-}\to {\Lambda}_c^{+}{K}^{-}{K}^{-} $$ and Ω b − → Λ c + K − K − $$ {\varOmega}_b^{-}\to {\Lambda}_c^{+}{K}^{-}{K}^{-} $$ decays being observed at over 5 σ significance. The Ξ b − → Λ c + K − π − $$ {\Xi}_b^{-}\to {\Lambda}_c^{+}{K}^{-}{\pi}^{-} $$ mode is also used to measure the Ξ b − $$ {\Xi}_b^{-} $$ production asymmetry, which is found to be consistent with zero. In addition, the B − → Λ c + p ¯ K − $$ {B}^{-}\to {\Lambda}_c^{+}\overline{p}{K}^{-} $$ decay is observed for the first time, and its branching fraction is measured relative to that of the B − → Λ c + p ¯ π − $$ {B}^{-}\to {\Lambda}_c^{+}\overline{p}{\pi}^{-} $$ mode.

Abstract

Abstract A search for radiative decay of B s 0 $$ {B}_s^0 $$ mesons to orbitally excited K + K − states is performed using proton proton collisions recorded by the LHCb experiment, corresponding to an integrated luminosity of 9 fb −1. The dikaon spectrum in the mass range m KK < 2400 MeV/c 2 is dominated by the ϕ(1020) resonance that accounts for almost 70% of the decay rate. Considering the possible contributions of f 2(1270), f 2 ′ $$ {f}_2^{\prime } $$ (1525) and f 2(2010) meson states, the overall tensor contribution to the amplitude is measured to be F f 2 = 16.8 ± 0.5 stat . ± 0.7 syst . % , $$ {\mathcal{F}}_{\left\{{f}_2\right\}}=16.8\pm 0.5\left(\textrm{stat}.\right)\pm 0.7\left(\textrm{syst}.\right)\%, $$ mostly dominated by the f 2 ′ $$ {f}_2^{\prime } $$ (1525) state. Several statistically equivalent solutions are obtained for the detailed resonant structure depending on whether the smaller amplitudes interfere destructively or constructively with the dominant amplitude. The preferred solution that corresponds to the lowest values of the fit fractions along with constructive interference leads to the relative branching ratio measurement B B s 0 → f 2 ′ γ B B s 0 → ϕγ = 19.4 − 0.8 + 0.9 stat . − 0.5 + 1.4 syst . ± 0.5 B % , $$ \frac{\mathcal{B}\left({B}_s^0\to {f}_2^{\prime}\gamma \right)}{\mathcal{B}\left({B}_s^0\to \phi \gamma \right)}={19.4}_{-0.8}^{+0.9}{\left(\textrm{stat}.\right)}_{-0.5}^{+1.4}\left(\textrm{syst}.\right)\pm 0.5\left(\mathcal{B}\right)\%, $$ where the last uncertainty is due to the ratio of measured branching fractions to the K + K − final state. This result represents the first observation of the radiative B s 0 → f 2 ′ 1525 γ $$ {B}_s^0\to {f}_2^{\prime }(1525)\gamma $$ decay, which is the second radiative transition observed in the B s 0 $$ {B}_s^0 $$ sector.

Abstract

Abstract The decays of the B + meson to the final state D ∗ − D s + π + $$ {D}^{\ast -}{D}_s^{+}{\pi}^{+} $$ are studied in proton-proton collision data collected with the LHCb detector at centre-of-mass energies of 7, 8, and 13 TeV, corresponding to a total integrated luminosity of 9 fb −1. The ratio of branching fractions of the B + → D ∗ − D s + π + $$ {B}^{+}\to {D}^{\ast -}{D}_s^{+}{\pi}^{+} $$ and B 0 → D ∗ − D s + $$ {B}^0\to {D}^{\ast -}{D}_s^{+} $$ decays is measured to be 0.173 ± 0.006 ± 0.010, where the first uncertainty is statistical and the second is systematic. Using partially reconstructed D s ∗ + → D s + γ $$ {D}_s^{\ast +}\to {D}_s^{+}\gamma $$ and D s + π 0 $$ {D}_s^{+}{\pi}^0 $$ decays, the ratio of branching fractions between the B + → D ∗ − D s ∗ + π + $$ {B}^{+}\to {D}^{\ast -}{D}_s^{\ast +}{\pi}^{+} $$ and B + → D ∗ − D s + π + $$ {B}^{+}\to {D}^{\ast -}{D}_s^{+}{\pi}^{+} $$ decays is determined as 1.31 ± 0.07 ± 0.14. An amplitude analysis of the B + → D ∗ − D s + π + $$ {B}^{+}\to {D}^{\ast -}{D}_s^{+}{\pi}^{+} $$ decay is performed for the first time, revealing dominant contributions from known excited charm resonances decaying to the D *− π + final state. Abstract

Arc radiation-induced polymer nozzle ablation plays a crucial role in the operation of self-blast circuit breakers. Up to now, there is no study specifically on the spectral distribution of radiated power from a switching arc towards the nozzle surface and its implication to nozzle ablation. Three-dimensional radiation transfer calculation has been performed in this study for arcs burning in the mixture of C 4 F 7 N + C O 2 + PTFE , a gaseous environment that is under significant focus in the development of SF6 free switching technology to aim net-zero by the middle of this century. Results show that photons from infrared to extreme ultraviolet (UV, up to 5 × 1015 Hz) are emitted from the core of high current arcs. However, the high-frequency photons are mostly absorbed within the arc column, especially at the arc edge, and only photons up to 1.7 × 10 15 Hz (far UV, 7 eV) can penetrate the arc edge and cold gas and reach the nozzle surface. The spectral distribution of the radiative energy at different instantaneous currents (15 kA, 28 kA and 60 kA peak) and different locations of the nozzle is presented and compared in detail. The location of the radiation absorption zone at the arc edge and the radiative power reaching the nozzle wall as a function of the instantaneous current is, in particular, purposely considered.

Bisher werden diese nur an wenigen, hochspezialisierten Zentren durchgefuhrt. Standardisierte Ausbildung und kontinuierliches Training sind zur fehlerfreien Bedienung des Roboters erforderlich. Der Patientenselektion und dem perioperativen Monitoring kommt besondere Bedeutung zu. Neben dem an der Roboterkonsole tatigen Chirurgen ist ein weiterer geschulter Chirurg, der patientenseitig positioniert ist, erforderlich. Die klinischen Ergebnisse bei selektionierten Patienten sind hervorragend. Bisher ist ein Robotersystem klinisch verfugbar. Zukunftig werden weitere, innovative Modifizierungen der Robotersysteme die Akzeptanz der Roboterchirurgie im Rahmen der Herzchirurgie beeinflussen.

Abstract

BACKGROUND: Minimally invasive aortic valve replacement (MiAVR) and transcatheter aortic valve implantation (TAVI) provide aortic valve replacement (AVR) by less invasive methods than conventional surgical AVR, by avoiding complete sternotomy. This study directly compares and analyses the available evidence for early outcomes between these two AVR methods. METHODS: Electronic databases were searched from inception until August 2019 for studies comparing MiAVR to TAVI, according to predefined search criteria. Propensity-matched studies with sufficient data were included in a meta-analysis. RESULTS: Eight studies with 9,744 patients were included in the quantitative analysis. Analysis of risk-matched patients showed no difference in early mortality (RR 0.76, 95% CI, 0.37–1.54, P=0.44). MiAVR had a signal towards lower rate of postoperative stroke, although this did not reach statistical significance (OR 0.42, 95% CI, 0.13–1.29, P=0.13). Abstract

Background Aortic valve surgery (AVS) is the gold standard treatment for symptomatic aortic valve (AV) disease patients. We report the temporal trends in the incidence of patients requiring isolated AVS in an unselected statewide population and their mortality outcomes over 17-years. Methods Patients were identified from the New South Wales, Australia, Admitted-Patient-Data-Collection registry between 1-July-2001 and 31-December-2018. Annual case-volumes and survival outcomes, adjusted for age, sex, referral source, endocarditis, concomitant coronary-artery-bypass-grafting, comorbidities including atrial fibrillation, hypertension and Charlson comorbidity index, were compared across calendar years. Results The study cohort comprised 16436 patients who underwent isolated AVS (mean age: 72.2 ± 11.3y; 67.5% males). Annual case-volume increased from 768 to 1048 cases between 2002 and 2017 (r2 = 0.82; p Conclusion The volume of AVS has increased progressively over time and has been associated with increased use of bioprosthetic valves and markedly improved 30-day and 1-year survival.

BACKGROUND: One of the most frequent complications of coronary artery bypass grafting (CABG) is pleural effusion. Limited previous studies have found post-CABG pleural effusion to be associated with increased length-of-stay and greater morbidity post-CABG. Despite this the associations of this common complication are poorly described. This study sought to identify modifiable risk factors for effusion post-CABG. METHODS: A retrospective cohort study of prospectively collected data assessed patients who underwent CABG over two-years. Data was collected for risk factors and sequelae related to pleural effusion requiring drainage. RESULTS: A total of 409 patients were included. Average age was 64.9±10.2 years, 330 (80.7%) were male. 59 (14.4%) patients underwent drainage of pleural effusion post-CABG. Effusions were drained on average 9.9±8.4 days post-CABG. BackgroundMultidrug therapy is a World Health Organization “best buy” for the prevention and control of noncommunicable diseases. CVD polypills, including ≥2 blood pressure medications, and a statin with or without aspirin, are an effective, scalable strategy to close the treatment gap that exists in many low- and middle-income countries, including Haiti. We estimated the number of Haitian adults eligible for an atherosclerotic CVD (ASCVD) polypill, and the number of potentially preventable CVD events if polypills were implemented nationally.MethodsWe used cross-sectional data from the Haiti CVD Cohort, a population-based cohort of 3,005 adults ≥18 years in Port-au-Prince, to compare two polypill implementation strategies: high-risk primary prevention and secondary prevention. High-risk primary prevention included three scenarios: (a) age ≥40 years, (b) hypertension, or (c) predicted 10-year ASCVD risk ≥7.5%. Secondary prevention eligibility included history of stroke or myocardial infarction. We then used the 2019 Global Burden of Disease database and published polypill trials to estimate preventable CVD events, defined as nonfatal MI, nonfatal stroke, and cardiovascular death over a 5-year timeline.ResultsAmong 2,880 participants, the proportion of eligible adults for primary prevention were: 51.6% for age, 32.5% for hypertension, 19.3% for high ASCVD risk, and 5.8% for secondary prevention. Based on current trends, an estimated 462,509 CVD events (95% CI: 369,089–578,475) would occur among adults ≥40 years in Haiti from 2019–2024. Compared with no polypill therapy, we found 32% or 148,003 CVD events (95% CI: 70,126–248,744) could be prevented by a combined primary and secondary prevention approach in Haiti if polypills were fully implemented over 5 years.ConclusionThese modeling estimates underscore the potential magnitude of preventable CVD events in low-income settings like Haiti. Robotic-assisted mitral valve surgery (RMVS) is becoming an increasingly performed procedure in cardiac surgery, however, its true safety and efficacy compared to the gold standard conventional sternotomy approach [conventional sternotomy mitral valve surgery (CSMVS)] remains debated. The aim of this meta-analysis was to provide a comprehensive analysis of all available literature comparing RMVS to CSMVS.An electronic search of five databases was performed to identify all relevant studies comparing RMVS to CSMVS. Pre-defined primary outcomes of interest included all-cause mortality, cerebrovascular accidents (CVA) and re-operation for bleeding. Secondary outcomes of interest included cross clamp time, cardiopulmonary bypass (CPB) time, intensive care unit (ICU) and hospital length of stay (LOS), post-operative atrial fibrillation (POAF) and red blood cell (RBC) transfusion.The search strategy identified fourteen studies qualifying for inclusion in this meta-analysis comparing RMVS to CSMVS. The outcomes of 6,341 patients (2,804 RMVS and 3,537 CSMVS) were included. RMVS had significantly lower mortality when compared to CSMVS group in both the unmatched [odds ratio (OR) 0.33; 95% confidence interval (CI): 0.19-0.57; P0.001] and matched cohorts (OR 0.35; 95% CI: 0.15-0.80; P=0.01). There was no significant difference in rates of CVA or re-operation for bleeding between the two groups in either the entire included cohort or matched patients. CSMVS had significantly shorter cross clamp time by 28 minutes (95% CI: 19.30-37.32; P0.001) and CPB time by 49 minutes (95% CI: 36.16-61.01; P0.001) which remained significantly shorter in the matched cohorts. RMVS had shorter ICU [mean difference (MD) 26 hours; 95% CI: -34.31 to -18.52; P0.001] and hospital LOS (MD 2 days; 95% CI: -2.66 to -1.37; P0.001), which were again both significantly shorter in the matched cohort. RMVS group also had fewer RBC transfusions (OR 0.44; 95% CI: 0.28-0.70; P0.001).Current evidence on comparative outcomes of RMVS and CSMVS is limited with only low-quality studies currently available. This present meta-analysis suggests that RMVS may have lower mortality and shorter ICU and hospital LOS, however CSMVS may be associated with significantly shorter cross clamp and CPB times. Further analysis of high-quality studies with randomized data is required to verify these results.

Haiti is a low-income country whose population lives under repeated and chronic stress from multiple natural disasters, civil unrest, and extreme poverty. Stress has been associated with cardiovascular (CVD) risk factors including hypertension, and the impact of stress on blood pressure may be moderated by support. The distribution of stress, support, and their association with blood pressure has not been well described in low-income countries. We measured stress and support using validated instruments on cross-sectional enrollment data of a population-based cohort of 2,817 adults living in Port-au-Prince, Haiti between March 2019 and April 2021. Stress was measured using the Perceived Stress Scale, while support was measured using the Multidimensional Scale of Perceived Social Support. Continuous scores were categorized into three groups for stress (low (1–5), moderate (6–10), high (11–16), and five groups for support (low (7–21), low-moderate (22–35), moderate (36–49), moderate-high (50–64), high (65–77)). Linear regression models were used to quantify the associations between: 1) support and stress adjusting for age and sex, and 2) stress and blood pressure adjusting for age and sex. Abstract

Aims: Modeling estimates indicate that heart failure (HF) prevalence may be lowest in low-income (LICs). Conversely, HF has been shown to be the leading cause of hospital admission in hospitals in many LICs. This disconnect between estimates and hospital-based observations may be explained by the underlying data. Modeling studies may have selection bias reporting data on individuals presenting with symptomatic disease and may introduce ascertainment bias based on accessibility of care and other structural factors in these communities. Population-based studies are needed to identify the true clinical epidemiology of HF in order to improve HF-related health outcomes in these under-resourced settings where four billion persons live. Haiti is the poorest, most under-resourced country in the Western Hemisphere. We describe HF prevalence, survival, subtypes, risk factors, and one-year mortality in the population-based Haiti Cardiovascular Disease Study. Methods and Results: Multistage cluster-area random sampling identified 2,981 adults, who completed standardized history and exam, laboratory measures, and cardiac imaging. Kaplan-Meier and Cox proportional hazard regression assessed survival; logistic regression identified associated factors. Median follow-up was 15.4 months (IQR 9-22). Age-standardized HF prevalence was 3.2% (93/2,981 [CI:2.6-3.9]). One-year HF mortality was 6.6% versus 0.8% (HR: 7.7 [CI 2.9-20.6], p Conclusion: The age-standardized prevalence of HF in this low-income setting was alarmingly high at 3.2%--50% higher than rates in high-income countries and 5-fold higher than modeling estimates for LMICs. Adults with HF were two decades younger as compared to high-income settings and 7.7 times more likely to die at one year. These data serve as a warning sign and may have implications for policy makers and future HF interventions in LMICs.

Abstract

This study examined the moderating effects of parent-child, same-sex, opposite-sex and teacher-student relationships between sibling status and loneliness during COVID-19. A total of 1,591 adolescents in China completed the study questionnaires which assessed the aforementioned variables. The results showed that same-sex and teacher-student relationships moderated the association between sibling status and loneliness. The results indicated that among participants with good same-sex relationships during COVID-19, compared to non-only-children participants, the only-children participants were more likely to experience a lower level of loneliness. However, participants with poor same-sex relationships experienced similar level of loneliness regardless of whether they had siblings. These findings would be helpful for parents, teachers, and other stakeholders to take effective measures to reduce loneliness feelings among Chinese adolescents.

Abstract

With the growing demand for oilfield development, the use of inclined and directional wells has gradually increased. However, due to limitations in the shaft structure of inclined wells, the efficiency of rod pumping is significantly compromised. To investigate the delayed closure of fixed valves, a numerical model was developed using computational fluid dynamics technology, by combining the standard k–ω turbulence model with the volume-of-fluid multiphase flow model. The SIMPLE algorithm was employed to calculate the valve closing process in inclined well pumps. The delayed closure of the valve ball, caused by changes in the well inclination angle and swabbing parameters (such as sinking pressure, pump diameter, and stroke speed), was investigated. The simulation results showed that inclined well pumps are most effective at inclination angles below the critical angle (which, in this study, was 59.036°). In addition, lower sinking pressures, larger pump diameters, longer strokes, or fewer stroke times can reduce the time taken for valve closure and minimize leakage. As the well inclination angle approaches the critical value, pump valve leakage increases sharply. By quantitatively analyzing the numerical calculation results, we established a fitting relationship for valve ball fall time. This improved the proposed calculation model for pump efficiency in inclined well pumps, offering valuable insights for their practical application.

Abstract

Background Although Guyana has made significant progress toward malaria control, limited access to malaria testing and treatment services threatens those gains. Mining activities create breeding environments for mosquitoes, and the migrant and mobile mining populations are hard to reach with information and services. The Ministry of Public Health (MoPH) has trained volunteers to test and treat malaria cases in remote regions. However, it remains unclear how miners perceive these testers, the services they provide, or what their malaria care-seeking behaviour is in general. To better address these challenges, Breakthrough ACTION Guyana and MoPH conducted qualitative research from October to November 2018 in Regions 7 and 8 in Guyana. Methods A total of 109 individuals, 70 miners, 17 other mining camp staff, and 22 other key stakeholders (e.g. community health workers, pharmacists, and regional leadership), participated in semi-structured interviews and focus group discussions. Results were derived using a framework analysis, with an adjusted doer and non-doer analysis, and organized using the integrated behaviour framework. Results Miners sought MoPH-approved services because of close geographic proximity to testing services, a preference for public service treatment, and a desire to correctly diagnose and cure malaria rather than just treat its symptoms. Those who chose to initiate self-treatment—using unregulated medications from the private and informal sector—did so out of convenience and the belief that self-treatment had worked before. Miners who completed the full MoPH-approved treatment understood the need to complete the treatment, while those who prematurely stopped treatment did so because of medication side effects and a desire to feel better as soon as possible. Conclusion Reasons why miners do and do not pursue malaria testing and treatment services are diverse. These results can inform better MoPH programming and new solutions to improve malaria outcomes in Guyana.

Abstract

OBJECTIVES Current evidence on sutureless and rapid deployment aortic valve replacement (SURD-AVR) is limited and does not allow for the assessment of the clinical impact and the evolution of procedural and clinical outcomes of this new valve technology. The Sutureless and Rapid Deployment International Registry (SURD-IR) represents a unique opportunity to evaluate the current trends and outcomes of SURD-AVR interventions. METHODS Data from 3682 patients enrolled between 2007 and 2018 were analysed. Patients were divided according to the date of surgery into 6 equal groups and by the type of intervention: isolated SURD-AVR (n = 2472) and combined SURD-AVR (n = 1086). RESULTS Across the 11-year study period, significant changes occurred in patient characteristics including a decrease in age and in estimated surgical risk. Less invasive approaches for isolated SURD-AVR increased considerably from 49.4% to 85.5%. The overall in-hospital mortality rate was 1.6% and 3.9% in isolated and combined procedures, respectively, with no change over time. The rate of perioperative stroke decreased significantly (from 4% to 0.5%), as did the rates of postoperative pacemaker implantation (from 12.8% to 5.9%) and aortic regurgitation (from 17.8% to 2.7%). CONCLUSIONS The present study provides a comprehensive analysis of the current trends and results of SURD-AVR interventions. The most notable changes over time were the increasing implantation of SURD valves in a younger population, with more frequent utilization of less invasive techniques. SURD-AVR demonstrated remarkable improvements in clinical outcomes with a significant reduction in the rates of stroke, pacemaker implantation and postoperative aortic regurgitation.

Abstract

Background Cardiovascular diseases (CVD) are rapidly increasing in low-middle income countries (LMICs). Accurate risk assessment is essential to reduce premature CVD by targeting primary prevention and risk factor treatment among high-risk groups. Available CVD risk prediction models are built on predominantly Caucasian risk profiles from high-income country populations, and have not been evaluated in LMIC populations. We aimed to compare six existing models for predicted 10-year risk of CVD and identify high-risk groups for targeted prevention and treatment in Haiti. Methods We used cross-sectional data within the Haiti CVD Cohort Study, including 1345 adults ≥ 40 years without known history of CVD and with complete data. Six CVD risk prediction models were compared: pooled cohort equations (PCE), adjusted PCE with updated cohorts, Framingham CVD Lipids, Framingham CVD Body Mass Index (BMI), WHO Lipids, and WHO BMI. Risk factors were measured during clinical exams. Primary outcome was continuous and categorical predicted 10-year CVD risk. Secondary outcome was statin eligibility. Results Sixty percent were female, 66.8% lived on a daily income of ≤ 1 USD, 52.9% had hypertension, 14.9% had hypercholesterolemia, 7.8% had diabetes mellitus, 4.0% were current smokers, and 2.5% had HIV. Predicted 10-year CVD risk ranged from 3.6% in adjusted PCE (IQR 1.7–8.2) to 9.6% in Framingham-BMI (IQR 4.9–18.0), and Spearman rank correlation coefficients ranged from 0.86 to 0.98. The percent of the cohort categorized as high risk using model specific thresholds ranged from 1.8% using the WHO-BMI model to 41.4% in the PCE model (χ2 = 1416, p value Conclusions In the Haiti CVD Cohort, there was substantial variation in the proportion identified as high-risk and statin eligible using existing models, leading to very different treatment recommendations and public health implications depending on which prediction model is chosen. There is a need to design and validate CVD risk prediction tools for low-middle income countries that include locally relevant risk factors. Trial registration clinicaltrials.gov NCT03892265.

Abstract

Introduction: Cardiovascular disease is the leading cause of death in Haiti and many low-middle income countries. Environmental lead exposure is associated with higher blood pressure (BP) and cardiovascular disease mortality in high income countries but has not been systematically evaluated in lower income countries where 6.5 billion people reside. Hypothesis: We hypothesized higher lead levels would be associated with higher BP in Haiti. Methods: Blood lead levels (BLLs) were measured among 2,257 participants ≥18 years enrolled in a population-based longitudinal cohort study in Port-au-Prince (Haiti Cardiovascular Disease Cohort Study). BLL screening was conducted using LeadCare II (detection limit ≥3.3 μg/dL). BLLs below detection were imputed. Associations between BLLs (quartiles) and systolic BP (SBP) and diastolic BP (DBP) were assessed, adjusting for age, sex, obesity, smoking, physical activity, and income. BLLs were also categorized as ≥5 versus Results: The median age was 40 years; 62.6% were female. The geometric mean BLL was 4.60 μg/dL, 67.3% had detectable BLLs >3.3 μg/dL, and 40.9% had elevated BLLs ≥5 μg/dL. BLLs in quartile four (≥6.5 μg/dL) in both unadjusted and adjusted analyses were associated with higher SBP (adjusted 3.02 mmHg; 95%CI 0.80, 5.25) and DBP (adjusted 2.19 mmHg; 95%CI 0.70, 3.67). BLL ≥5 μg/dL were associated with a higher SBP (2.21 mmHg; 95%CI 0.15, 4.26) and DBP (1.50 mmHg; 95%CI 0.20, 2.81) in unadjusted analysis. After adjustment there was no difference in SBP (1.41; 95%CI -0.25, 3.09) and 1.15 mmHg (95%CI 0.01, 2.30) higher DBP. Conclusions: Widespread environmental lead exposure is evident in urban Haiti, with higher BLLs associated with higher SBP and DBP. Lead is a modifiable pollutant in low-income countries that warrants urgent public health remediation.

Results: Seventy percent (460/653) were female, 65.5% (428/653) lived on a daily income of ≤1 USD, 57.0% (372/653) had hypertension, 14.5% (95/653) had hypercholesterolemia, 9.3% (61/653) had diabetes mellitus, and 5.5% (36/653) were current smokers. Predicted 10-year CVD risk ranged from 3.9% in adjusted PCE (IQR 1.7-8.4) to 9.8% in Framingham-BMI (IQR 5.0-17.8), and Spearman correlation coefficients ranged from 0.87 to 0.98. The percent of the cohort categorized as high risk using the uniform threshold of 10-year CVD risk ≥ 7.5% ranged from 28.8% in the adjusted PCE model to 62.0% in the Framingham-BMI model (χ 2 = 331, p value < 0.001). Statin eligibility also varied widely. Conclusions: In the Haiti CVD Cohort, there was substantial variation in the proportion identified as high-risk and statin eligible using existing models, leading to very different treatment recommendations and public health implications depending on model choice. There is a need to design and validate CVD risk prediction tools for low-middle income countries.

Abstract

Introduction: Neighborhood social context including violence and cohesion may impact cardiovascular disease risk factors in countries like Haiti with high civil unrest. We describe these factors using validated instruments (Neighborhood Collective Efficacy, City Stress Inventory), and hypothesized higher cohesion and lower violence were associated with lower stress, depression, blood pressure (BP), and hypertension. Methods: We collected data from a population-representative cohort of adults in Port-au-Prince, Haiti between March 2019 to April 2021, including stress (Perceived Stress Scale), depression (PHQ-9), blood pressure, and hypertension (SBP ≥ 140 mmHg, DBP ≥ 90 mmHg, or on medications). Adjusting for age and sex, multivariable linear and Poisson regressions assessed the relationship between exposures (neighborhood violence, neighborhood cohesion) and outcomes (stress, depression, BP, hypertension). Results: Among 2,799 adults, 59.7% were female and median age was 41 years (IQR:28-55). Participants reported high cohesion (median 15/25, IQR:14-17) and moderate violence (9/20, IQR:7-11). Stress was moderate (8/16), 12.8% had at least moderate depression, and 30.4% had hypertension. In regressions, higher violence at all levels was associated with higher depressive symptoms (Q4 vs Q1: +2.94 score, 95%CI 2.51, 3.38). Violence was also weakly associated with slightly higher stress (Q4 vs Q1: +0.32 score, 95%CI 0.04, 0.59), and lower hypertension prevalence (Q2 vs Q1: PR 0.86, 95%CI 0.75, 0.99). Cohesion was weakly associated with lower stress (T3 vs T1: -0.43 score, 95%CI -0.71, -0.16), depression (T2 vs T1: -0.63 score, 95%CI -1.02, -0.24), and higher DBP (T2 vs T1: 1.23 mmHg, 95%CI 0.065, 2.39). Conclusions: In conclusion, we found high perceived cohesion and moderate violence. The strongest association was between higher violence and higher depression. Paradoxically, higher violence and lower cohesion were associated with lower hypertension and DBP, possibly due to unmeasured confounding.

Abstract

Introduction: Stress is associated with multiple adverse health outcomes, including hypertension. The impact of stress on health may be moderated by social support. The distribution of stress, support, and their association with hypertension have not been well described in low-income countries that face severe poverty. Over the past decade, Haiti has suffered massive natural disasters including the 2010 earthquake, cholera outbreak, COVID-19 pandemic, and recurrent civil instability—all of which may act as prolonged stressors. Hypothesis: We assessed the hypotheses that 1) there are high levels of both stress and support in Haiti, and 2) high support would moderate the relationship between high stress and hypertension. Methods: We measured stress and social support using validated instruments in a population-representative cohort of adults living in urban Port-au-Prince, Haiti between March 2019 and April 2021. Stress was measured using the Perceived Stress Scale, while social support was measured using the Multidimensional Scale of Perceived Social Support. For visualization, continuous scores were categorized using equal-width groups (stress: low (1-5), moderate (6-10), high (11-16); support: low (7-21), low-moderate (22-35), moderate (36-49), moderate-high (50-64), high (65-77)). Linear regression models were used to quantify the associations between: 1) stress and support adjusting for age and sex, 2) blood pressure and stress adjusting for age and sex. A formal moderation analysis was conducted to assess if support moderated the relationship between stress and blood pressure. Results: Among 2,817 adults, 59.7% female and the median age was 40 years (IQR 28-55). The majority had an income of less than 1 US dollar a day (69.7%). The median stress score was 8 out of 16 points, and median support score was 61 out of 77 points. Stress was higher with older ages (60+ years versus 18-29 years: +0.79 points, 95% CI 0.51 to 1.08) and in females (+0.85 points, 95% CI +0.65 to +1.06). Support was higher in males (+3.29 points, 95% CI 2.19 to 4.39). Support was inversely associated with stress, adjusting for age and sex (-0.04 points, 95% CI -0.04 to -0.03). Stress was not associated with systolic or diastolic blood pressure after adjustment for age and sex. Support did not moderate the association between stress and blood pressure. Conclusion: In this urban cohort of Haitian adults living with chronic civil instability, stress was moderate and support was high. While support was associated with lower stress, it did not moderate the relationship between stress and blood pressure. Despite the high levels of instability in Haiti, participants displayed resiliency through high levels of support, which may be an underutilized resource in reducing stress and long-term negative health outcomes.

100. Qualitative assessment of family caregiver-centered neonatal education program in Karnataka, India

Abstract

Globally 2.5 million newborns die every year before they reach the age of one month; the majority of these deaths occur in low- and middle-income countries. Among other factors, inadequate knowledge and skills to take care of newborns contribute to these deaths. To fill this gap, training patients and family members on the behaviors needed to improve essential newborn care practices at home is a promising opportunity. One program that aims to do this is the Care Companion Program (CCP) which provides in-hospital, skills-based training on care of mothers and newborns to families. This study uses semi-structured interviews to understand how and why knowledge and behaviors of maternal and newborn care behaviors change (or don’t change) as a result of CCP sessions and participants’ perception of the impact of CCP on change. Interviews focused on knowledge and behaviors around key neonatal and newborn topics and health seeking behaviors for health complications. Forty-two in-depth interviews were conducted among families with recently-delivered babies at their homes from four districts in Karnataka, India. Respondents have a positive perception about CCP, found training useful and appreciated other family members presence during the training. CCP increased knowledge and awareness and provided critical details to key behaviors like breastfeeding. Respondents were more likely to be receptive toward details on already known topics, like hand washing before touching the baby. Awareness increased on newly learned behaviors, like skin-to-skin care, which don’t conflict with cultural norms. The CCP did not influence nonrestrictive maternal diet as much, which cultural norms heavily influence. In-hospital family caregiver education programs, like CCP, can positively influence key neonatal behaviors by imparting knowledge and key skills. However, the effect is not universal across health behaviors.
