Abstract

Introduction ICTs (Information and Communication Technologies) play a crucial role in improving efficiency and productivity in various economic sectors. The research focuses on measuring the impact of ICTs on the economic growth of Latin American nations during the period 2000-2020. Methods A fixed effects, panel data model covering 17 countries in the region has been applied using ordinary least squares methodology. The variables selected for the empirical exercise are: the number of individuals using the Internet, fixed and mobile telephone subscriptions, trade balance, domestic credit, foreign direct investment and gross domestic product per capita. Results The study’s findings show that ICTs have a positive impact on economic growth. That is, for every percentage point increase in the percentage of the population using the Internet, GDP per capita increases by US$16.82. Conclusion This highlights the unique influence exerted by digital connectivity in shaping the regional economic landscape. At the same time, mobile and fixed telephone subscriptions are not significant variables in the model.

Abstract

Summary: Evolvability is an emergent hallmark of cancer that depends on intra-tumor heterogeneity and genetic variation. Mutations generated by APOBEC3 contribute to genetic variation and tumor evolvability. However, the influence of APOBEC3 on the evolvability of the genome and its differential impact on cancer genes versus non-cancer genes remains unclear. Analyzing over 40,000 human protein-coding transcripts, we identified distinct distribution patterns of APOBEC3A/B TC motifs between cancer and non-cancer genes, suggesting unique associations with cancer. Studying a bat species with numerous APOBEC3 genes, we found distinct motif patterns in orthologs of cancer genes compared to non-cancer genes, as in humans, suggesting APOBEC3 evolution to reduce impacts on the genome rather than the converse. Simulations confirmed that APOBEC3-induced heterogeneity enhances cancer evolution through bimodal patterns of mutations in certain classes of genes. Our results suggest the bimodal distribution of APOBEC-induced mutations can significantly increase cancer heterogeneity.

Abstract

All of the world's productive sectors are being impacted by the phenomenon known as digitalization, but different economies are experiencing this phenomenon in different ways due to a variety of internal and external factors at play in the businesses. According to official statistics, they make up 97% of the nation's commercial fabric and generate a sizable amount of employment (about 60%) and gross domestic product (GDP) for the nation. By categorizing Paraguayan MSMEs by economic sector, size, and age, the goal of this paper is to examine the influence of digitalization drivers in this context. In order to achieve this purpose, data was collected through a survey during the months of April to September 2022. The findings show that MSMEs' owners value the key forces influencing digitization. On the other hand, they point out to the fact that varied behaviors are displayed depending on the business sector, size, and age of the company, which calls for specialized tactics and attention to improve digitalization in MSMEs.

Abstract

Double-strand breaks (DSBs) are the most dangerous injuries for a genome. When unrepaired, death quickly ensues. In most bacterial systems, DSBs are repaired through homologous recombination. Nearly one-quarter of bacterial species harbor a second system, allowing direct ligation of broken ends, known as Non-Homologous End Joining (NHEJ). The relative role of both systems in DSBs repair in bacteria has been explored only in a few cases. To evaluate this in the bacterium Rhizobium etli, we used a modified version of the symbiotic plasmid (264 kb), containing a single copy of the nifH gene. In this plasmid, we inserted an integrative plasmid harboring a modified nifH gene fragment containing an I-SceI site. DSBs were easily inflicted in vivo by conjugating a small, replicative plasmid that expresses the I-SceI nuclease into the appropriate strains. Repair of a DSB may be achieved through homologous recombination (either between adjacent or distant repeats) or NHEJ. Characterization of the derivatives that repaired DSB in different configurations, revealed that in most cases (74%), homologous recombination was the prevalent mechanism responsible for repair, with a relatively minor contribution of NHEJ (23%). Inactivation of the I-SceI gene was detected in 3% of the cases. Sequence analysis of repaired derivatives showed the operation of NHEJ. To enhance the number of derivatives repaired through NHEJ, we repeated these experiments in a recA mutant background. Derivatives showing NHEJ were readily obtained when the DSB occurred on a small, artificial plasmid in a recA mutant. However, attempts to deliver a DSB on the symbiotic plasmid in a recA background failed, due to the accumulation of mutations that inactivated the I-SceI gene. This result, coupled with the absence of derivatives that lost the nonessential symbiotic plasmid, may be due to an unusual stability of the symbiotic plasmid, possibly caused by the presence of multiple toxin-antitoxin modules.

Abstract

Probiotics are live microorganisms that promote host health through microbiota balance and immune modulation. We assessed an acidogenic bacterial consortium (ABC) with promising probiotic properties, focusing on its resilience during transit through the digestive tract in ruminants and determining its optimal in vitro dosage. The ABC exhibited antibiotic resistance, thrived at pH levels between 5 and 7 for 24 and 48 h, and showed a 77% survival rate in artificial gastric juice. Moreover, it not only endured bile salt exposure but also multiplied. The ABC exhibited 10.74% of coaggregation capabilities against E. coli. Optimal dosage determination revealed that 4 × 108 was the ideal concentration, as higher doses did not yield significant differences in dry matter digestion. In the in vivo trial with Limousin Heifers, the ABC led to enhanced total volatile fatty acid (VFA) production, increased daily weight gains, and improved feed conversion rates compared to the control group. These findings underscore the potential of the ABC as a probiotic to boost animal productivity and overall health.

Abstract

Bandwidth fragmentation is a critical problem for Elastic Optical Networks (EON), and spectrum defragmentation is the most important strategy to mitigate this phenomenon. In this work we propose a Machine Learning (ML) based method for estimating the Blocking Rate, which, when exceeding a threshold, triggers a defragmentation process. This is done in order to achieve better results in terms of the number of blocking demands and the number of re-routed connections. The performance of the proposed method was compared with two other known strategies: fixed-time (FT) defragmentation, and triggering based on one fragmentation metric (BFR). Simulation results were evaluated using two multi-objective metrics. Experimental results show that the proposed method is more efficient than the other two, being the best method in 85.7% of comparisons using the Pareto Coverage metric, and obtaining 47.4% of non-dominated solutions in the Pareto Front.

Este estudio se propuso analizar los discursos usados por los principales actores implicados en el sistema nacional de ciencia y tecnología de Paraguay. El hecho de indagar sobre el sistema nacional de innovación a partir de la consulta directa a los skateholders permitió profundizar por sobre la información que aportan los policy makers. Y es aquí donde radica el carácter innovador de este material, en el contexto de investigaciones que abordan la política de innovación y que se centran en el análisis de documentos o declaraciones de los policy makers, mientras que aquí se ha trabajado con una multivocalidad que puso en relieve discursos opuestos, otros que se complementan y aquellos dominantes. Así pues, desde una perspectiva basada en el constructivismo social -de acuerdo con la cual el lenguaje construye la realidad social-, se analizó la forma en que el discurso de determinados grupos incide sobre cómo se constituye el sistema nacional de ciencia y tecnología paraguayo.

Abstract

Abstract Background Intravascular leiomyomatosis (IVL) is a histologically benign smooth muscle tumor arising from the uterus that can spread through the pelvic veins and, on rare occasions, extend as far as the heart via the inferior vena cava. Despite its benign characteristics, it can behave like a malignant tumor leading to significant morbidity and even mortality if left untreated. Case presentation The patient is a 42-year-old woman with a past medical history of uterine leiomyomas. She presented with heavy bleeding and frequent spotting; therefore, she went to her gynecologist. After further evaluation, a mass within the uterus that expanded into the pelvic veins, inferior vena cava, and right atrium was discovered. After the complete removal of the mass, the patient underwent full recovery. IVL with cardiac extension was the final diagnosis. Conclusion Although IVL is rare, it must be considered in women who underwent previous hysterectomies or myomectomies and present with symptoms of right heart failure. The ideal therapy will need the aid of a multidisciplinary team and will depend on the patient’s symptoms, previous operative history, the tumor’s extension, and resectability.

77. Device-associated infections in neonatal care units in a middle-income country, 2016–2018

Abstract

Objective: Describe the device-associated infections in the NICUs in Cali – Colombia, a middle-income country, between August 2016 to December 2018. Methods: Observational cross-sectional study evaluating reports of device-associated infections in 10 NICUs in Cali, Colombia, between August 2016 and December 2018. Socio-demographic and microbiological data were obtained from the National Public Health surveillance system, through a specialized notification sheet. The relationship of device-associated infections with several outcomes including birth weight, microorganisms, and mortality was evaluated using OR CI95%, using the logistic regression model. Data processing was performed using the statistical program STATA 16. Results: 226 device-associated infections were reported. The rate of infection with central line-associated bloodstream infections was 2.62 per 1000 days of device use and 2.32 per 1000 days for ventilator-associated pneumonia. This was higher in neonates under 1000 g; 4.59 and 4.10, respectively. 43.4% of the infections were due to gram-negative bacteria and 42.3% were due to gram-positive bacteria. Time from hospitalization to diagnosis of all device-associated infections had a median of 14 days. When compared by weight, infants with a weight lower than 1000 g had a greater chance of death (OR 3.61; 95% CI 1.53–8.49, p = 0.03). Infection by gram-negative bacteria was associated with a greater chance of dying (OR 3.06 CI 95 1.33–7.06, p = 0.008). Conclusions: These results highlight the need to maintain epidemiological surveillance processes in neonatal intensive care units, especially when medical devices are used.

Este documento presenta los resultados obtenidos en una investigación orientada por la medición del nivel de satisfacción de los turistas por los destinos visitados en las comunidades locales receptoras del Parque Nacional Cotacachi-Cayapas, Imbabura-Ecuador. Dicho estudio reporta un análisis multivariado sostenido en una relación causal donde se indica la incidencia de la valoración de los turistas por el destino visitado y la cualidad del destino a visitar, respecto al nivel de satisfacción del destino visitado, teniendo al factor sociodemográfico como variable moderadora, conformada por la edad de los turistas, nivel de estudios y género, utilizando una muestra de 408 turistas que respondieron a una entrevista administrada al momento de su salida del destino. Previo a una regresión lineal múltiple, se hizo un análisis factorial exploratorio seguido de uno combinatorio, con apoyo del programa estadístico IBM-SPSS y el SPSS-AMOS para finalmente hacer un análisis de moderación ambientado en PROCESS, lo cual permitió, en primera instancia, explorar y confirmar los factores latentes puestos en escena y explicar que el nivel de satisfacción de los visitantes dependió, significativamente, de la valoración del destino manifiesta por los visitantes, ligado al proceso, logrando moderar la relación ya sostenida entre, al menos una de sus dimensiones.

Abstract

Abstract Background Ventilator-Associated pneumonia (VAP) is one of the leading causes of morbidity and mortality in critically ill COVID-19 patients in lower-and-middle-income settings, where timely access to emergency care and accurate diagnostic testing is not widely available. Therefore, rapid microbiological diagnosis is essential to improve effective therapy delivery to affected individuals, preventing adverse outcomes and reducing antimicrobial resistance. Methods We conducted a cross-sectional study of patients with suspected VAP and COVID-19, evaluating the diagnostic performance of the BioFire® FilmArray® Pneumonia Panel (FA-PP). Respiratory secretion samples underwent standard microbiological culture and FA-PP assays, and the results were compared. Results We included 252 samples. The traditional culture method detected 141 microorganisms, and FA-PP detected 277, resulting in a sensitivity of 95% and specificity of 60%, with a positive predictive value of 68% and negative predictive value of 93%. In samples with high levels of genetic material (> 10^5 copies/mL), the panel had a sensitivity of 94% and specificity of 86%. In addition, 40% of the culture-negative samples had positive FA-PP® results, of which 35% had > 10^5 copies/mL of genetic material. The most prevalent bacteria were Gram-negative bacilli, followed by Gram-positive cocci. The panel identified 98 genes associated with antimicrobial resistance, predominantly extended-spectrum beta-lactamases (28%). Conclusion The FA-PP is a sensitive assay for identifying bacteria causing VAP in patients with COVID-19, with a greater capacity to detect bacteria than the conventional method. The timely microbiological recognition offered by this panel could lead to optimized decision-making processes, earlier tailored treatment initiation, and improved antibiotic stewardship practices.

Abstract

El cáncer de mama triple negativo es una presentación clínica poco frecuente, pero de manejo difícil del cáncer de mama, en el que la metástasis a partes blandas como primera manifestación clínica es rara. En los estados unidos 1 de cada 8 mujeres desarrollara cáncer de mama invasivo en el transcurso de su vida. El objetivo del trabajo es demostrar el comportamiento inusual de la manifestación del cáncer de mama, por medio de una presentación de un caso clínico. Se obtuvo el diagnóstico del origen primario gracias a la realización de la inmunohistoquímica de la resección del tumor metastásico en piel. El caso aquí reportado integra tres puntos de interés: cáncer de mama triple negativo, afectación de una mujer de 58 años y tumor interescapular metastásico a tejido blando, lo que torna un comportamiento inusual del cáncer de mama debido a que las metástasis cutáneas ocurren entre el 0,6% y 10,4% en ese tipo de cáncer de mama.

Abstract

Relationships among laurasiatherian clades represent one of the most highly disputed topics in mammalian phylogeny. In this study, we attempt to disentangle laurasiatherian interordinal relationships using two independent genome-level approaches: (1) quantifying retrotransposon presence/absence patterns, and (2) comparisons of exon datasets at the levels of nucleotides and amino acids. The two approaches revealed contradictory phylogenetic signals, possibly due to a high level of ancestral incomplete lineage sorting. The positions of Eulipotyphla and Chiroptera as the first and second earliest divergences were consistent across the approaches. However, the phylogenetic relationships of Perissodactyla, Cetartiodactyla, and Ferae, were contradictory. While retrotransposon insertion analyses suggest a clade with Cetartiodactyla and Ferae, the exon dataset favoured Cetartiodactyla and Perissodactyla. Future analyses of hitherto unsampled laurasiatherian lineages and synergistic analyses of retrotransposon insertions, exon and conserved intron/intergenic sequences might unravel the conflicting patterns of relationships in this major mammalian clade.

88. How has labour market power evolved? Comparing labour market monopsony in Peru and the United States

Abstract

We document the evolution of labour market power by employers on the US and Peruvian labour markets during the 2010s. Making use of a structural estimation model of labour market dynamics, we estimate differences in market power that workers face depending on their sector of activity, their age, sex, location and educational level. In particular, we show that differences in cross-sectional market power are significant and higher than variations over the ten-year time span of our data. In contrast to findings of labour market power in developed countries such as the US, we document significant market power of employers in Peru vis-\`a-vis the tertiary educated workforce, regardless of age and sector. In contrast, for the primary educated workforce, market power seems to be high in (private) services and manufacturing. For secondary educated workers, only the mining sector stands out as moderately more monopsonistic than the rest of the labour market. We also show that at least for the 2010s, labour market power declined in Peru. We contrast these findings with similar estimates obtained for the United States where we are able to show that increases in labour market power are particularly acute in certain sectors, such as agriculture and entertainment and recreational services, as well as in specific geographic areas where these sectors are dominant. Moreover, we show that for the US, the labour market power has gradually increased over the past ten years, in line with the general rise in inequality. Importantly, we show that the pervasive gender pay gap cannot be linked to differential market power as men face higher labour market power than women. We also discuss possible reasons for these findings, including for the differences of labour market power across skill levels. Especially, we discuss the reasons for polarization of market power that are specific to sectors and locations.

Abstract

The main objective of this study was to analyze the relationship between Geriatric Syndromes (GSs) and in-hospital mortality in adults aged 65 and older admitted to the Emergency Department (ED). The study included 202 Older Adults (OAs) who met the inclusion criteria. We conducted a Comprehensive Geriatric Assessment and collected clinical and demographic data. A univariate analysis was carried out for each of the GSs analyzed. Those variables with p < 0.05 were entered into a multiple logistic regression using the backward stepwise entry method to analyze the independent predictor variables. The average number of GSs per individual was 4.65 (±2.76). Frailty syndrome was the most prevalent (70.2% of patients). Our study found an association between mortality and some GSs, such as frailty (p = 0.042), risk of falls (p = 0.010), delirium, cognitive impairment, dependence, and risk of ulcers (p < 0.001). We found that cognitive impairment (adjusted OR, 6.88; 95% CI, 1.41–33.5; p = 0.017) and dependence (adjusted OR, 7.52; 95% CI, 1.95–29.98; p = 0.003) were independent predictors associated with mortality in our population. It is necessary to develop new care strategies in the ED that respond to the needs of aging societies, including the use of new technologies and personnel with experience in gerontology.

Abstract

Using the quantum map formalism, we provide a framework to construct fuzzy and coarse grained quantum states of many-body systems that account for limitations in the resolution of real measurement devices probing them. The first set of maps handles particle-indexing errors, while the second deals with the effects of detectors that can only resolve a fraction of the system constituents. We apply these maps to a spin-$1/2$ XX-chain obtaining a blurred picture of the entanglement generation and propagation in the system. By construction, both maps are simply related via a partial trace, which allow us to concentrate on the properties of the former. We fully characterize the fuzzy map, identifying its symmetries and invariants spaces. We show that the volume of the tomographically accessible states decreases at a double exponential rate in the number of particles, imposing severe bounds to the ability to read and use information of a many-body quantum system., Comment: 8 pages, 4 figures

Abstract

Background and purposeBenefit of early endovascular treatment (EVT) for ischemic stroke varies considerably among patients. The MR PREDICTS decision tool, derived from MR CLEAN (Multicenter Randomized Clinical Trial of Endovascular Treatment for Acute Ischemic Stroke in the Netherlands), predicts outcome and treatment benefit based on baseline characteristics. Our aim was to externally validate and update MR PREDICTS with data from international trials and daily clinical practice.MethodsWe used individual patient data from 6 randomized controlled trials within the HERMES (Highly Effective Reperfusion Evaluated in Multiple Endovascular Stroke Trials) collaboration to validate the original model. Then, we updated the model and performed a second validation with data from the observational MR CLEAN Registry. Primary outcome was functional independence (defined as modified Rankin Scale score 0–2) 3 months after stroke. Treatment benefit was defined as the difference between the probability of functional independence with and without EVT. Discriminative performance was evaluated using a concordance (C) statistic.ResultsWe included 1242 patients from HERMES (633 assigned to EVT, 609 assigned to control) and 3156 patients from the MR CLEAN Registry (all of whom underwent EVT within 6.5 hours). The C-statistic for functional independence was 0.74 (95% CI, 0.72–0.77) in HERMES and, after model updating, 0.80 (0.78–0.82) in the Registry.
