Objective: To assess the responsiveness of the EuroQol (EQ-5D) by comparing it with the Functional Living Index-Cancer (FLIC) and a self-reported rating of health-related quality of life (HRQL).Methods: HRQL was measured four times during the course of high dose chemotherapy (HDC) and bone marrow transplantation in 40 patients with stage II and III breast cancer. Measurements were at baseline (T1), pre-HDC (T2), 3 weeks post-HDC (T3), and 3 months post-HDC (T4).Results: Effect size ranged from 1.16 (T1-T3) to 0.66 (T3-T4) for the EQ-5D and 0.85-0.91 respectively for the FLIC. No significant differences in effect sizes between the EQ-5D and the FLIC were found. Repeated measures ANOVA yielded a significant change for all measures, with HRQL decreasing post-HDC and returning to baseline levels by 3 m post-HDC. EQ-5D dimensions changed significantly over time for mobility, self-care, and usual activities. EQ-5D index scores at T3 had a bimodal distribution. Interpretation of psychological changes was facilitated by an analysis of FLIC items.Conclusions: The EQ-5D is responsive to the clinically large changes associated with HDC in breast cancer patients. Resolution of presenting problems at three-month follow-up was studied in 228 parasuicides. 'Non-resolvers' presented with less severe problems, but prodromes were longer, prior episodes more frequent, and they experienced more powerlessness and internally directed hostility than the resolvers. At follow-up resolvers had improved more on measures of depression, externally and internally directed hostility, locus of control, powerlessness, self-esteem, sensitivity to criticism, and social adjustment. Baseline problem severity, powerlessness (negatively), and 'normlessness' 'predicted' degree of problem resolution, while baseline problem severity, the number of previous parasuicide episodes, and baseline internal hostility (the latter two variables negatively) were associated with problem resolution as a dichotomous variable. Resolvers improved more in social functioning during the follow-up than non-resolvers, but a similar proportion (16%) in each group reported repeat episodes of parasuicide at follow-up. The survival of Gram positive anaerobic cocci on plain cotton wool and albumin coated swabs held in various transport media was investigated. Results suggested that in most cases Amies', Stuart's and VMGII media do not offer any more protection to the bacteria than storing swabs dry in their containers. A technique was developed for the isolation and identification of Gram positive anaerobic cocci from the mouth and vagina, incorporating bicozamycin in the medium as a selective agent. Few strains were recovered from the oral cavity, but larger numbers were isolated from the vagina. Using a minimum number of antibiotic sensitivity and biochemical tests, including analysis of end products by gas-liquid chromatography, most isolates were identified to species level. The recovery of upper respiratory tract bacteria on laboratory media from a variety of swabs and swab-transport media kits was examined. Organisms studied included strains of Streptococcus pyogenes, Streptococcus pneumoniae, Staphylococcus aureus, Neisseria meningitidis and Haemophilus influenzae. Although a wide range of results was obtained with regard to variables such as the type of swab or swab-transport media kit used, the time of plating, the temperature of storage of swab-transport media kits, and the nature of the suspension of the organisms, it was generally noted that recovery of organisms was better from swabs held in their plastic containers prior to plating than from swabs held in transport medium. The survival of group B streptococci on a variety of swabs, held either in their containers or in transport media for periods of up to 48 hours, at room temperature and at 4 degrees C, has been assessed. Results indicated that holding swabs in transport media did not favour prolonged survival of the streptococci and that the yield of organisms was much greater from swabs held in their ordinary plastic tubes. A holding temperature of 4 degrees C rather than room temperature is recommended if any delay in plating out swabs is anticipated. [ABSTRACT FROM PUBLISHER]

Langerhans cells (LC) serve as antigen presenting cells and provide immune surveillance within epithelia. Since depression of LC number and/or function may allow tolerance to antigens, we evaluated LC in median rhomboid glossitis (MRG). a condition linked to persistent candidal infection of lingual mucosa. Material included a total of 36 cases of MRG (7 of which did not show PAS + gungi) and 6 controls. LC were identified by their expression of S-100 and HLA-DR antigens and quantified using image analysis. Equal number of LC were identified using S-100 + and HLA-DR + markers. The density of LC (cells/mm of basement membrane, mean ± SD) in both PAS + MRG (2.6 ± 1.3) and PAS - MRG (3.0 ± 1.7) was markedly depressed compared with controls (17.2 ± 6.4), (P ± 0.001). These findings indicate that the LC network is perturbed in MRG, and are consistent with the view that of localized defect in immune surveillance may contribute to persistent fungal infection of the oral mucosa. The Streptosec test, which embodies the coagglutination principle for grouping beta-haemolytic streptococci, was used against 72 streptococci previously grouped by precipitin methods. Only two of the 72 strains failed to react. The test is easy to manipulate, represents a considerable saving in time and effort, and produces results with an acceptable degree of accuracy.
