Background Context\ud Classification criteria are recommended for diseases that lack specific biomarkers to improve homogeneity in clinical research studies. Because imaging evidence of lumbar disc herniations (LDHs) may not be associated with symptoms, clinical classification criteria based on patient symptoms and physical examination findings are required.\ud \ud Purpose\ud This study aimed to produce clinical classification criteria to identify patients with radicular pain caused by LDH.\ud \ud Study Design\ud The study design was a two-stage process. Phase 1 included a Delphi process and Phase 2 included a cohort study.\ud \ud Patient Sample\ud The patient sample included outpatients recruited from spine clinics in five countries.\ud \ud Outcome Measures\ud The outcome measures were items from history and physical examination.\ud \ud Materials and Methods\ud In Phase 1, 17 spine experts participated in a Delphi process to select symptoms and signs suggesting radicular pain caused by LDH. In Phase 2, 19 different clinical experts identified patients they confidently classified as presenting with (1) radicular pain caused by LDH, (2) neurogenic claudication (NC) caused by lumbar spinal stenosis, or (3) non-specific low back pain (NSLBP) with referred leg pain. Patients completed survey items and specialists documented examination signs. A score to predict radicular pain caused by LDH was developed based on the coefficients of the multivariate model. An unrestricted grant of less than US$15,000 was received from MSD: It was used to support the conception of the Delphi, data management, and statistical analysis. No fees were allocated to participating spine specialists.\ud \ud Results\ud Phase 1 generated a final list of 74 potential symptoms and signs. - Abstract
Background Incident reporting is a widely recommended method to measure undesirable events in anaesthesia. Under-utilization is a major weakness of voluntary incident reporting systems. Little is known about factors influencing reporting practices, particularly the clinical environment, anaesthesia team composition, severity of the incident, and perceived risk of litigation. The purpose of this study was to assess each of these, using an existing anaesthesia database. Methods We performed a retrospective cohort study and analysed 46 207 surgical patients. We used multivariate analysis to identify factors associated with the non-utilization of the reporting system. Results We found that in 7022 (15.1%) of the procedures performed, the incident reporting system was not used. Factors associated with the non-use of the system were regional anaesthesia/local anaesthesia, odds ratio (OR) 1.64 [95% confidence interval (CI) 1.03-2.62], emergency procedures OR 1.15 (95% CI: 1.05-1.27), and a consultant anaesthetist working without a trainee, OR 1.71 (95% CI: 1.03-2.82). In contrast, factors such as longer duration of surgery, OR 0.85 (95% CI: 0.76-0.94), the presence of a senior anaesthesia trainee, OR 0.86 (95% CI: 0.81-0.92), and the occurrence of severe complications with a high risk of litigation (i.e. death, nerve injuries) were less associated with a non-use of the reporting system, OR 0.65 (95% CI: 0.44-0.97). Team composition and time of day had no measurable impact on reporting practices. Conclusions Clinical factors play a significant role in the utilization of an anaesthesia incident reporting system and more particularly, severity of complications and higher liability risks which appear more as incentives than barriers to incident reporting

Objectives: Grounded in Basic Psychological Needs Theory (Ryan & Deci, 2002), this study examined the temporal relationships between perception of coaches’ autonomy support and different facets of controlling behaviors, the satisfaction-frustration of athletes’ basic needs, and subjective vitality, self-esteem and burnout in elite sportsmen. Methods: Participants (N = 110 males) from three elite youth soccer academies in northwest France completed a questionnaire on three occasions during the last three months of the competitive season. Results: Linear mixed models revealed that perceptions of coach-autonomy support and only two facets of controlling coach behaviors (excessive personal control and negative conditional regard) were related to basic need satisfaction-frustration, which in turn were related to the indices of well- and ill-being. In most cases, the relationships were observed both at the within- and between-person levels, but some were observed only at one level. Conclusions: The findings highlight the importance of considering the different facets of controlling coach behaviors separately and disaggregating the between-person and within-person effects. © 2016 Elsevier Ltd

Background:Patients with rheumatoid arthritis (RA) with an inadequate response to TNF antagonists (aTNFs) may switch to an alternative aTNF or start treatment from a different class of drugs, such as rituximab (RTX). It remains unclear in which clinical settings these therapeutic strategies offer most benefit.Objective:To analyse the effectiveness of RTX versus alternative aTNFs on RA disease activity in different subgroups of patients.Methods:A prospective cohort study of patients with RA who discontinued at least one aTNF and subsequently received either RTX or an alternative aTNF, nested within the Swiss RA registry (SCQM-RA) was carried out. The primary outcome, longitudinal improvement in 28-joint count Disease Activity Score (DAS28), was analysed using multivariate regression models for longitudinal data and adjusted for potential confounders.Results:Of the 318 patients with RA included; 155 received RTX and 163 received an alternative aTNF. The relative benefit of RTX varied with the type of prior aTNF failure: when the motive for switching was ineffectiveness to previous aTNFs, the longitudinal improvement in DAS28 was significantly better with RTX than with an alternative aTNF (p = 0.03; at 6 months, −1.34 (95% CI −1.54 to −1.15) vs −0.93 (95% CI −1.28 to −0.59), respectively). When the motive for switching was other causes, the longitudinal improvement in DAS28 was similar for RTX and alternative aTNFs (p = 0.40). These results were not significantly modified by the number of previous aTNF failures, the type of aTNF switches, or the presence of co-treatment with a disease-modifying antirheumatic drug.Conclusion:This observational study suggests that in patients with RA who have stopped a previous aTNF treatment because of ineffectiveness changing to RTX is more effective than switching to an alternative aTNF.

To investigate the role of rheumatoid factor (RF) status and anti-citrullinated peptide antibody (ACPA) status as predictors of abatacept (ABA) effectiveness in patients with rheumatoid arthritis (RA).We conducted a pooled analysis of data from 9 observational RA registries in Europe (ARTIS [Sweden], ATTRA [Czech Republic], BIOBADASER [Spain], DANBIO [Denmark], GISEA [Italy], NOR-DMARD [Norway], ORA [France], Reuma.pt [Portugal], and SCQM-RA [Switzerland]). Inclusion criteria were a diagnosis of RA, initiation of ABA treatment, and available information on RF and/or ACPA status. The primary end point was continuation of ABA treatment. Secondary end points were ABA discontinuation for ineffectiveness or adverse events and response rates at 1 year (good or moderate response according to the European League Against Rheumatism criteria with LUNDEX adjustment for treatment continuation). Hazard ratios (HRs) and 95% confidence intervals (95% CIs) for the study end points in relation to RF and ACPA status were calculated.We identified 2,942 patients with available data on RA-associated autoantibodies; data on RF status were available for 2,787 patients (77.0% of whom were RF positive), and data on ACPA status were available for 1,903 patients (71.3% of whom were ACPA positive). Even after adjustment for sociodemographic and disease- and treatment-related confounders, RF and ACPA positivity were each associated with a lower risk of ABA discontinuation for any reason (HR 0.79 [95% CI 0.69-0.90], P0.001 and HR 0.78 [95% CI 0.68-0.90], P0.001, respectively), compared to RF-negative and ACPA-negative patients. Similar associations with RF and ACPA were observed for discontinuation of ABA treatment due to ineffectiveness, with HRs of 0.72 (95% CI 0.61-0.84) and 0.74 (95% CI 0.62-0.88), respectively (both P0.001).Our results strongly suggest that positivity for RF or ACPA is associated with better effectiveness of ABA therapy.

A search has been made for a very light Higgs boson in the processes e+e- → e+e-H and e+e- →μ+μ-H using data collected by ALEPH at the LEP e+e- collider at centre of mass energies close to the Z peak. The mass range between 0 and 57 MeV is unambigously excluded at the 95% confidence level. If we combine this with our previously published analysis, the complete range from 0 to 24 GeV is excluded at 95% CL. The search is extended to light Higgs bosons of the minimal supersymmetric standard model, with the result that all possibilities of coupling are excluded for Higgs masses below 3 GeV.

Objective: To investigate the presence of eosinophils in oral intraepithelial neoplasia (OIN) and oral squamous cell carcinoma (OSCC) lesions and its relation to invasion. Subjects and methods: Ninety‐nine oral biopsies were selected and subdivided into the following: OIN‐1(16 cases), OIN‐2 (18 cases), OIN‐3 (17 cases), microinvasive OSCC (10 cases), non‐metastatic OSCC (22 cases) and metastatic OSCC (16 cases). The tissue eosinophilia was evaluated histologically in slices stained with haematoxylin and eosin. Results: Eosinophil distribution was associated with diagnosis severity (
p < .01). A significant difference was found between OIN‐3 or microinvasive OSCC and non‐metastatic or metastatic OSCC. Stromal invasion threshold was 7 eos/10 high power field (hpf) (96.1% specificity and 62.5% sensitivity). Eosinophils were absent in OIN‐1; in OIN‐2, two cases were positive. In OIN‐3, five cases showed tissue eosinophilia, four of which had ≥3 eos/hpf or ≥7 eos/10 hpf. Three cases were suspected of invasion; two had a previous history of OSCC with elevated eosinophil infiltrate. In microinvasive OSCC, the four positive cases presented ≥3 eos/hpf and ≥7 eos/10 hpf. Although not significantly different, non‐metastatic invasive OSCC had a higher number of cases (68.2%) with ≥22 eos/10 hpf contrasting with 50% in metastatic OSCC. Conclusion: Our results suggest that eosinophils can be considered an indicator of invasion in OIN, helping in cases of difficult diagnosis. [ABSTRACT FROM AUTHOR]- Published

Using 106 000 hadronic events obtained with the ALEPH detector at LEP at energies close to the Z resonance peak, the strong coupling constant αs is measured by an analysis of energy-energy correlations (EEC) and the global event shape variables thrust, C-parameter and oblateness. It is shown that the theoretical uncertainties can be significantly reduced if the final state particles are first combined in clusters using a minimum scaled invariant mass cut, Ycut, before these variables are computed. The combined result from all shape variables of pre-clustered events is αs(MZ2 = 0.117±0.005 for a renormalization scale μ= 1 2 M Z . For μ values between MZ and the b-quark mass, the result changes by −0.009+0.006.

The properties of the Z resonance are measured on the basis of 190000 Z decays into fermion pairs collected with the ALEPH detector at LEP. Assuming lepton universality, M(Z) = (91.182 +/- 0.009exp +/- 0.020L;P) GeV, GAMMA(Z) = (2484 +/- 17) MeV, sigma(had)0 = (41.44 +/- 0.36) nb, and GAMMA(had)/GAMMA(ll) = 21.00 +/- 0.20. The corresponding number of light neutrino species is 2.97 +/- 0.07. The forward-backward asymmetry in leptonic decays is used to determine the ratio of vector to axial-vector coupling constants of leptons: g(V)2(M(Z)2)/g(A)2(M(Z)2) = 0.0072 +/- 0.0027. Combining these results with ALEPH results on quark charge and bbBAR asymmetries, and tau-polarization, sin2 theta(W)(M(Z)2) = 0.2312 +/- 0.0018. In the context of the Minimal Standard Model, limits are placed on the top-quark mass.

Limits on Z decay branching ratios into neutralinos are reported. They were obtained from searches for monojets, acoplanar jets, acoplanar lepton pairs, single photons and acoplanar photon pairs as signaturs for the reactions e+e- → χχ′ and e+e- → χχ′, where χ is the lightest neutralino and χ′ any heavier one. The data sample used for these searches corresponds to about 23 000 events of Z decay into multihadrons, collected at LEP by the ALEPH detector for centre of mass energies at and near the Z peak. The results obtained are used to restrict the parameter space of the minimal supersymmetric standard model.

The utilitarian representation of sport in Rumania, a brake in the development of leisure practice. Since the countries of central Europe entered the liberal sphere influence, the consumptions of possessions and services develop there. We observe a movement of tutellaire and ideological emancipation of the sports organizations, but the practice of popular physical activities delays developing. If, on the ground, we notice the appearance of an structured professionally offer, contrary to all expectations, it does not pull a real increase of consumption. This situation which rises in inferred paradox a cultural interrogation on the previous history of these brakes. Historically, the sport during the influence of communist sphere was enpreint of elitist and nationalist ideologies. An inquiry, made with the professionals of the forming in physical and sports education, moves closer to this weak development of the popular sports with the cultural inheritance of the country. The frame of sports of leisure in the representation that builds itself the professional actors, as the consumers, always revet a strongly utilitarian dimension which hinders its development. To develop the sports of leisure implies to integrate this peculiarity into the professionals of sports forming programme.

A search for Z0 decays into pairs of possible new heavy quarks (t and b′), new heavy charged leptons (L±), stable heavy neutral leptons (νL) and unstable heavy neutral leptons (L0) is performed on data collected by the ALEPH detector corresponding to 11 550 events of Z0→hadrons. The limits on the masses of the heavy quarks are Mt > 45.8 GeV and Mb′ > 46.0 GeV, allowing for both charged-current and flavor-changing neutral-current decays of the b′. If an L± decays into a stable νL, then for MνL MνL. Finally, while the mass of the stable νL is excluded up to 42.7 GeV, the mass of the unstable L0 is excluded up to 45.7 GeV with the mixing parameters |UlL0|2 down to 10−13 at this mass. For 25.0 GeV < ML0 < 42.7 GeV, all values of |UlL0|2 are excluded. All limits are given at 95% CL.

From an analysis of inclusive leptons in data collected by the ALEPH detector at LEP, we measure the fractions of b b and c c events in hadronic Z decays. The b b fraction times semileptonic branching ratio is measured to be Br ( b → e )·Γ b b /Γ had = 0.0224 ± 0.0016 ± 0.0010 . Assuming a b semileptonic branching ratio of 0.102 ± 0.010 gives Γ b b /Γ had = 0.220 ± 0.016 ± 0.024 , in good agreement with the standard model prediction of 0.217. The c c fraction times semileptonic branching ratio is measured to be Br ( c → e )·Γ c c /Γ had = 0.0133 ± 0.0040 −0.0031 +0.0038 . Assuming a c semileptonic branching ratio of 0.090 ± 0.013 gives Γ c c /Γ had = 0.148 ± 0.044 −0.038 +0.045 , in agreement with the standard model prediction of 0.171.

It has recently been shown that perseverance specifically relates to resisting proactive interference [Gay, P., Rochat, L., Billieux, J., d'Acremont, M., & Van der Linden, M. (2008). Heterogeneous inhibition processes involved in different facets of self-reported impulsivity: Evidence from a community sample. Acta Psychologica, 129, 332-339]. The aim of this study was to replicate and extend this finding by investigating the relationships between unintentional control of interference (in a recent-negatives task), intentional control of interference (in a directed-forgetting task), and the four facets of impulsivity. The performance of 71 volunteers indicated that the relevant variables of the two tasks shared very little or no variance. In particular, regression analyses showed that lower perseverance (i.e., higher impulsivity on this facet) predicted more interference-related errors in both tasks and less time dedicated to resolving proactive interference; however, lower perseverance did not predict directed-forgetting cost. Higher urgency predicted higher interference time due to response-conflict. © 2009 Elsevier Inc.

Pooling the hazard ratios is not always feasible in meta-analyses of two-arm survival studies, because the measure of the intervention effect is not systematically reported. An alternative approach proposed by Moodie et al. is to use the survival probabilities of the included studies, all collected at a single point in time: the intervention effect is then summarised as the pooled ratio of the logarithm of survival probabilities (which is an estimator of the hazard ratios when hazards are proportional). In this article, we propose a generalization of this method. By using survival probabilities at several points in time, this generalization allows a flexible modeling of the intervention over time. The method is applicable to partially proportional hazards models, with the advantage of not requiring the specification of the baseline survival. As in Moodie et al.'s method, the study-level factors modifying the survival functions can be ignored as long as they do not modify the intervention effect. The procedures of estimation are presented for fixed and random effects models. Two illustrative examples are presented. [ABSTRACT FROM AUTHOR]
