Self-inflicted dermatoses (SID) are not uncommon in dermatological practice. They can be classified into those associated with hidden or denied underlying behavior and those without hidden or denied underlying behavior. Malingering and factitious disorders are the most common of the first subgroup. Skin picking and related syndromes of compulsive and non-compulsive behavior belong to the second subgroup. Treatment is interdisciplinary, but sometimes frustrating. Conclusion: This method is a safe and efficient alternative to more invasive procedures by minimal to no social downtime and mild to no side effects. [ABSTRACT FROM AUTHOR]

Introduction: An ingrown toenail is a serious medical problem that cannot be overlooked, and the decision of choosing between conservative versus surgical treatment may be difficult in daily practice.

Hyperkeratotic lesions result from continuous mechanical action on the skin forming a callus or a corn. The accumulation of horny layers will increase pressure, creating a vicious cycle. We present a new approach based on relieving pressure or friction, strictly based on the results of pedography (pedobarography).

Background: Daclizumab is a humanized monoclonal antibody against the D-subunit (CD25) of the high-affinity interleukin (IL)–2 receptor, used for the treatment of relapsing-remitting multiple sclerosis with a large spectrum of cutaneous adverse reactions. Case presentation: We present the case of a middle-aged man treated with daclizumab for multiple sclerosis, who developed skin reactions difficult to evaluate. A 4 mm punch-biopsy was taken from the plantar area. Histological examination of the biopsy revealed hyperkeratosis and acanthosis but no parakeratosis, while a discrete inflammatory infiltrate was noticed around vessels in the dermis. Treatment with fluconazole 50 mg/day for 10 days, moisturizers, and grade I topical steroids was followed by slight improvement of the clinical picture. Treatment with daclizumab was not discontinued. Conclusion: The clinical efficacy and side effects of daclizumab have to be reported and confirmed in clinical practice in the following years. Any clinical report can contribute to validate the efficacy and risk of the drug’s administration. Any type of adverse skin reaction must be reported for clarifying the diagnosis.

Kerion Celsi is an inflammatory, deep fungal infection of the scalp. It is rare in neonates but gets more common in children about 3 years and older. It represents with swelling, boggy lesions, pain, alopecia and purulent secretions. Secondary bacterial infection is not unusual after maceration. Extracutaneous manifestations include regional lymphadenopathy, fever and very rare fungemia. Id-reactions can occur. Diagnosis is based on clinical suspicion, clinical examination and medical history. Diagnosis should be confirmed by microscopy, fungal culture and molecular procedures. The most common isolated fungal species are anthropophilic Trichophyton (T.) tonsurans and zoophilic Microsporum (M.) canis, while geophilic species and moulds rarely cause Kerion Celsi. Treatment is medical with systemic and topical antifungals supplemented by systemic antibiotics when necessary, while surgery needs to be avoided. Early and sufficient treatment prevents scarring alopecia. The most important differential diagnosis is bacterial skin and soft tissue infections. [ABSTRACT FROM AUTHOR]

A 67 year-old man was diagnosed 10 years ago with severe plaque psoriasis and was hospitalized for re-evaluation of therapy. He was treated previously with topical corticosteroids, methotrexate, and phototherapy. As part of the assessment protocol for initiation of biological treatment, a tuberculin skin test was done, which was positive without signs of active tuberculosis. Isoniazid therapy was started with marked improvement of psoriatic lesions before biologic treatment could be initiated.

French manicure, a simple and elegant manicure style, caught the public’s attention by the 19th century in French nail salons. Now this style has spread all over the world and is the first choice of 80% of women in the USA and Europe. It consists of a natural, pink, beige, or nude base tipped with pure white at the distal end.Onycholysis is a disease of the nails characterized by separation of the nail plate from the nail bed, with variable causes. We present 2 cases of onycholysis that resemble the beautiful and elegant French manicure.

This article presents a case study of a 59-year-old woman who developed systemic allergic dermatitis after receiving an iodinated contrast media injection for a computed tomography scan. The patient experienced erythematous and vesicular lesions, with skin manifestations lasting for about 3 months despite treatment with corticosteroids. Intradermal tests confirmed a reaction to the contrast media, leading to a major flare-up of symptoms. The study highlights the challenges and complexities of diagnosing and managing delayed hypersensitivity reactions to contrast media, emphasizing the importance of thorough testing and patient follow-up. Angiotensin-converting enzyme inhibitors (ACEI) are widely used drugs nowadays in treating patients diagnosed with cardiovascular disorders. We present two consecutive cases of acquired angioedema caused by the administration of enalapril and lisinopril in patients with indication for ACE-inhibitors therapy. Rigorous follow-up of side effects of ACEI is required, due to these possible life-threatening adverse reactions.

Cystic fibrosis (CF) is an autosomal recessive affliction triggered by genetic mutations in the cystic fibrosis transmembrane conductance regulator. The lung and pancreas are the most frequently affected organs in cystic fibrosis, cutaneous involvement is undervalued and underdiag-nosed. Skin lesions observed in patients diagnosed with cystic fibrosis are not well known and can create confusions with other dermatological diseases. The diagnosis of cutaneous lesions as signs of cystic fibrosis by pediatricians or dermatologists, despite their overlapping with different nutritional deficiencies, would allow earlier diagnosis and proper treatment and could improve quality of life and outcomes.

Background: Also known as papulopustular dermatitis (chronic form), rosacea-like dermatitis, periorificial dermatitis, or airhostess’ dermatitis, perioral dermatitis is a commonly encountered dermatological disease, especially in adult women and less frequently in children. Its diagnosis and treatment are a challenge especially in small children.

Congenital skin aplasia, known more as aplasia cutis congenita (ACC), is a rare congenital disease, characterized by absence of the skin, observed since birth, more often affecting the scalp and rarely the trunk or limbs. We report here for the first time a non-syndromic localized ACC, characterized by a small solitary area of skin atrophy on the cervical area in a healthy 3-day-old female infant.

We present a case of “gloves and socks” syndrome associated with Mycoplasma pneumoniae infection in a 6-year-old child hospitalized for febrile syndrome associated with monomorphic purpuric papular eruption localized on the distal part of extremities, in a “gloves and socks” pattern. Clinical diagnosis was confirmed by positivity of specific IgM against Mycoplasma pneumoniae. Favorable outcome was obtained by administration of oral clarithromycin.

Granuloma annulare (GA) is a granulomatous skin condition that can present with a diversity of clinical manifestations and locations, with an unknown etiology and diagnosed on clinical-pathological grounds/correlations. Although many trigger factors have been described and several pathogenic mechanisms proposed, the etiology of GA remains unknown. We report a case of work-related, isolated, unilateral GA localized on the right palmar area of a young worker, possibly induced by work-related direct trauma.

Congenital malalignment of the toenail is characterized by the lateral (rarely medial) deviation of nail plates that affects mostly the great toes from one foot or both, but has also been described on other toes, even on the hands. This nail disease is still considered a rare entity, although it is not a rare clinical observation in daily practice. We present a few cases in children and adults, highlighting the diagnosis made by clinical observation, regardless of the different grades of severity of the nail disease. Conclusion: It is of great importance to clinically recognize this entity in young children and to make the correct recommendations.

Introduction: The aim of this presentation is to highlight the usefulness of high-frequency ultrasound (18 MHz) in localized morphea for: identification of the lesion, guided skin biopsy, quantification of skin thickness, evaluating the severity by measuring total echogenicity.

Introduction: Pilomatrixoma or pilomatricoma is a benign appendageal growth, originating from hair cortex cells.

Knuckle pads are thickening of the skin over the extensor surface of the proximal interphalangeal joints. Clinical picture, ultrasound imaging, and histopathological examination of the skin biopsy ascertain the diagnosis. In routine practice, two main differential diagnoses are important: knuckle pads vs. pseudo-knuckle pads and idiopathic vs. non-idiopathic forms of knuckle pads.

Lipodermatosclerosis and diabetic dermopathy are low-risk skin lesions with many similar clinical features, except for venous abnormalities such as chronic venous insufficiency, but are rarely a reason for referring the patient to vascular ultrasound examination. We present 3 serial cases in which the compression ultrasound examination (CUS) of the venous circulation of the affected limbs was of utmost importance in the severity stratification. Asymptomatic deep venous thrombosis (DVT) was found in the first two cases, while in the third case the CUS excluded any type of vascular involvement, leading to a definite diagnosis of diabetic dermopathy. Lipodermatosclerosis may be associated with asymptomatic DVT due to chronic venous insufficiency, and early referral to CUS positively impacts further patient management.

Newborns are more likely to develop bruises due to mechanical trauma during birth. Establishing the correct diagnosis in newborns presenting with different skin lesions is not an easy task, and besides the well-known pathology, one must not forget simple posttraumatic injuries. We present three cases that raised questions before establishing that the lesions had been induced by simple mechanical trauma during birth. Trauma-induced skin lesions in newborns may represent an overlooked problem. The three cases presented here are meant to draw attention to the possibility of trauma-induced lesions in newborns, which require only close follow-up and surveillance instead of exhaustive clinical and laboratory investigations, which are inevitably accompanied by anxiety.

The present paper highlights the usefulness of 70% trichloroacetic acid in treating sebaceous hyperplasia in elderly patients. Esthetics are an important issue, and different therapeutic modalities can be used, such as systemic isotretinoin, surgical excision, electrocautery, cryosurgery, topical photodynamic therapy and laser, but all these methods are expansive and invasive procedures that may result in scars, which are more extensive than the original lesions.
