Objective To help facilitate economic evaluations of oncology treatments, we mapped responses on cancer-specific instrument to generic preference-based measures. Methods Cancer patients (n = 367) completed one cancer-specific instrument, the FACT-G, and two preference-based measures, the EQ-5D and SF-6D. Responses were randomly divided to form development (n = 184) and cross-validation (n = 183) samples. Relationships between the instruments were estimated using ordinary least squares (OLS), generalized linear models (GLM), and censored least absolute deviations (CLAD) regression approaches. The performance of each model was assessed in terms of how well the responses to the cancerspecific instrument predicted EQ-5D and SF-6D utilities using mean absolute error (MAE) and root mean squared error (RMSE). Results Physical, functional, and emotional well-being domain scores of the FACT-G best explained the EQ-5D and SF-6D. In terms of accuracy of prediction as measured in RMSE, the CLAD model performed best for the EQ-5D (RMSE = 0.095) whereas the GLM model performed best for the SF-6D (RMSE = 0.061). The GLM predicted SF-6D scores matched the observed values more closely than the CLAD and OLS. Conclusion Our results demonstrate that the estimation of both EQ-5D and SF-6D utility indices using the FACT-G responses can be achieved. The CLAD model for the EQ-5D and the GLM model for the SF-6D are recommended. Thus, it is possible to estimate quality-adjusted life years for economic evaluation from studies where only cancer-specific instrument have been administered. [ABSTRACT FROM AUTHOR]

Abstract: Purpose: To examine the acute cardiotoxicity of internal mammary chain (IMC) irradiation with concurrent trastuzumab. Materials and Methods: Clinical and cardiac function data were collected on 59 patients with early breast cancer who were treated with adjuvant trastuzumab and chemotherapy with or without radiotherapy (often including IMC) at BC Cancer Agency in 2005. Results: Forty-four of fifty-nine patients received adjuvant radiotherapy (RT). Thirteen had left-sided IMC RT. For left-sided RT, IMC inclusion increased the mean percentage dose to 5% of the heart, but the mean doses to 50% and 90% of the heart were similar. Median baseline left ventricular ejection fraction (LVEF) was 62% and similar in all groups. Median absolute decrease in LVEF after RT was 4%, which was not significantly different according to side or inclusion of IMCs. Trastuzumab was stopped in 11 of 59 patients (18.6%) due to decrease in LVEF. After median follow up of 15 months, three patients developed clinical congestive heart failure, none of whom received left-sided IMC RT. Conclusions: There was no excess acute cardiotoxicity observed with the combination of left-sided IMC irradiation and concurrent trastuzumab. [Copyright &y& Elsevier]

The article discusses research being done on the risk of early recurrence among postmenopausal women with estrogen-positive early breast cancer treated with adjuvant tamoxifen. It references a study by Hagen Kennecke et al, published in the 2008 issue of "Cancer." It was found that subgroups of women with early estrogen-receptor (ER)-positive breast cancer may be identified who are at increased risk of recurrence within 2.5 years of diagnosis in spite of tamoxifen.

Abstract: Objectives. : Amplification of the 11q13 locus is commonly observed in a number of human cancers including both breast and ovarian cancer. Cyclin D1 and EMS1 have been implicated as candidate oncogenes involved in the emergence of amplification at this locus. Detailed analysis of the 11q13 amplicon in breast cancer led to the discovery of four regions of amplification suggesting the involvement of other genes. Here, we investigate the role of EMSY, a recently described BRCA2 interacting protein, as a key element of the 11q13 amplicon in ovarian cancer. EMSY maps to 11q13.5 and is amplified in 13% of breast and 17% of ovarian carcinomas. Methods. : EMSY amplification was assessed by fluorescent in-situ hybridization (FISH) in 674 ovarian cancers in a tissue microarray and correlated with histopathological subtype and tumor grade. A detailed map of the 11q13 amplicon in 51 cases of ovarian cancer was obtained using cDNA-array-based comparative genomic hybridization (aCGH). To further characterize the role of EMSY within this amplicon, we evaluated both the amplification profiles and RNA expression levels of EMSY and two other genes from the 11q13 amplicon in an additional series of 22 ovarian carcinomas. Results. : EMSY amplification was seen in 52/285 (18%) high grade papillary serous carcinomas, 4/27 (15%) high grade endometrioid carcinomas, 3/38 (8%) clear cell carcinomas, and 3/10 (30%) undifferentiated carcinomas. aCGH mapping of 11q13 in ovarian cancer showed that EMSY localized to the region with the highest frequency of copy number gain. Cyclin D1 and EMS1 showed a lower frequency of copy number gain. A highly significant correlation between EMSY gene amplification and RNA expression was also observed (P = 0.0001). This was a stronger correlation than for other genes at 11q13 including Cyclin D1 and PAK1. Conclusions. : These findings support the role of EMSY as a key oncogene within the 11q13 amplicon in ovarian cancer. [Copyright &y& Elsevier]

Rearrangements of the neuregulin (NRG1) gene have been implicated in breast carcinoma oncogenesis. To determine the frequency and clinical significance of NRG1 aberrations in clinical breast tumors, a breast cancer tissue microarray was screened for NRG1 aberrations by fluorescent in situ hybridization (FISH) using a two-color split-apart probe combination flanking the NRG1 gene. Rearrangements of NRG1 were identified in 17/382 cases by FISH, and bacterial artificial chromosome array comparative genomic hybridization was applied to five of these cases to further map the chromosome 8p abnormalities. In all five cases, there was a novel amplicon centromeric to NRG1 with a minimum common region of amplification encompassing two genes, SPFH2 and FLJ14299. Subsequent FISH analysis for the novel amplicon revealed that it was present in 63/262 cases. Abnormalities of NRG1 did not correlate with patient outcome, but the novel amplicon was associated with poor prognosis in univariate analysis, and in multivariate analysis was of prognostic significance independent of nodal status, tumor grade, estrogen receptor status, and human epidermal growth factor receptor (HER)2 overexpression. Of the two genes in the novel amplicon, expression of SPFH2 correlated most significantly with amplification. The BRCA2 gene is mutated in familial breast and ovarian cancer, and its product is implicated in DNA repair and transcriptional regulation. Here we identify a protein, EMSY, which binds BRCA2 within a region (exon 3) deleted in cancer. EMSY is capable of silencing the activation potential of BRCA2 exon 3, associates with chromatin regulators HP1β and BS69, and localizes to sites of repair following DNA damage. EMSY maps to chromosome 11q13.5, a region known to be involved in breast and ovarian cancer. We show that the EMSY gene is amplified almost exclusively in sporadic breast cancer (13%) and higher-grade ovarian cancer (17%). In addition, EMSY amplification is associated with worse survival, particularly in node-negative breast cancer, suggesting that it may be of prognostic value. The remarkable clinical overlap between sporadic EMSY amplification and familial BRCA2 deletion implicates a BRCA2 pathway in sporadic breast and ovarian cancer. [Copyright &y& Elsevier]

• NiO nanoparticles supported TiO 2 catalysts are prepared via sol-gel method. • Octahedrally coordinated NiO nanoparticles are well dispersed on the support. • Acetylation of glycerol is performed under non-microwave instant heating condition. • 90% conversion and 66% selectivity of triacetyl glyceride is achieved. • No significant catalyst deterioration after four successive runs. NiO-supported TiO 2 (n NiO/TiO 2) as highly active and selective catalysts for triacetyl glyceride (TAG) biofuel additive production via acetylation of glycerol is reported. The catalysts are prepared by incorporating various amounts of Ni2+ (n = 10, 30 and 50 wt% Ni added during catalysts preparation) onto the TiO 2 support using simple sol-gel approach followed by calcination. The spectroscopy and microscopy analyses reveal that the NiO nanoparticles (ca. 6 nm) are uniformly distributed on the surface of TiO 2 support where octahedrally coordinated NiO as active species is formed upon calcination. Among the catalysts prepared, 30NiO/TiO 2 with strongest acidity and highest accessible surface area is found to be the most active in the acetylation of glycerol with acetic acid, achieving 90.2% glycerol conversion with 65.9% selective to TAG under non-microwave instant heating conditions (170 °C, 30 min). In addition, 30NiO/TiO 2 also shows better catalytic performance than previous reported catalysts. No significant deterioration in catalytic activity is shown by 30NiO/TiO 2 after eight successive runs, thus offering a green pathway for synthesizing TAG vehicle fuel additives with high selectivity. In this study, ultra-small MCM-41 mesoporous solids (<30 nm) were synthesized using colloidal silica and cesium hydroxide as cheap silica source and mineralizer, respectively. Four samples with a SiO 2 /Al 2 O 3 ratio of 5, 20, 30 and ∞ (designated as CsM-5, CsM-20, CsM-30 and CsM-∞, respectively) have been prepared. The effects of SiO 2 /Al 2 O 3 ratio on the formation, physico-chemical and catalytic properties of CsMCM-41 nanoparticles were studied. The results show that incorporation of high content of Al atoms into the framework reduces the ordering of the mesostructure, and hence the porosity of the solids decreases. By using CsOH as mineralizer, the morphology and the size of CsMCM-41 were also tuned from 3-D nanospheres (CsM-5: 29 nm and CsM-20: 27 nm) to hollow nanospheres with very thin walls (CsM-30: 25 nm, CsM-∞: 20 nm) when Al content was decreased. The basicity of CsMCM-41 nanoparticles was also affected by the Al and Cs contents. Hence, their catalytic behavior in the base-catalyzed reactions such as aldol condensation of heptanal with benzaldehyde, and Perkin condensation of benzaldehyde with acetic anhydride was evaluated under microwave heating conditions. CsM-20 displays the best catalytic activities among the samples studied, achieving 87.2% conversion and 78.4% selectivity to jasminaldehyde under optimum reaction conditions, while it also shows comparable catalytic performance as NaOH homogeneous catalyst in the Perkin condensation reaction under non-microwave instant heating conditions. Tumor-infiltrating lymphocytes (TILs) are predominantly present in breast cancer patients with estrogen receptor negative tumors, among whom increasing levels correlate with favorable outcomes. Nevertheless, currently available immune checkpoint inhibitors appear to benefit only a small number of women with breast cancer. Upregulation of additional immune checkpoint markers is one mechanism of resistance to current inhibitors that might be amenable to targeting with newer agents. T-cell Immunoglobulin and Mucin domain-containing molecule 3 (TIM-3) is an immune checkpoint receptor that is an emerging target for cancer immunotherapy. We investigated TIM-3 immunohistochemical expression in 3,992 breast cancer specimens assembled into tissue microarrays, linked to detailed outcome, clinico-pathological parameters and biomarkers including CD8, PD-1, PD-L1 and LAG-3. We scored and reported absolute counts for TIM-3+ intra-epithelial and stromal TILs (iTILs and sTILs), and find that breast cancer patients with TIM-3+ iTILs (≥ 1) represent a minority of cases (11%), with a predilection for basal-like breast cancers (among which 28% had TIM-3+ iTILs). TIM-3+ sTILs (≥ 2) represented 20% of cases and included more non-basal cases. The presence of TIM-3+ iTILs highly correlates with hematoxylin and eosin-stained stromal TILs and with other immune checkpoint markers (PD-1+ iTILs, LAG-3+ iTILs and PD-L1+ tumors). In prognostic analyses, early breast cancer patients with TIM-3+ iTILs have significantly improved breast cancer-specific survival whereas TIM-3+ sTILs did not reach statistical significance. In multivariate analyses, the presence of TIM-3+ iTILs is an independent favorable prognostic factor in the whole cohort as well as among ER negative patients.
