Background: Esophageal Atresia (EA) with or without associated tracheo-esophageal fistula (TEF) is one of the common congenital anomaly that can be life threatening if left unattended. In low and middle income countries like Pakistan, the management and outcome of such type of cases depends upon many factors related to resource limitation. Objective: To prospectively evaluate the management and outcome of esophageal atresia at Children Hospital, Pakistan Institute of Medical Sciences (PIMS), Islamabad, Pakistan. Study design, settings and duration: An Observational-descriptive study was conducted at the Department of Pediatric Surgery, The Children Hospital, PIMS, Islamabad from October 2017 to August 2018. Methodology: Consecutive patients diagnosed with esophageal atresia were included in the study. Demographic data, investigations, procedure performed and outcome were collected on a pre designed proforma and results were analysed. Results: Total 140 consecutive patients of esophageal atresia were enrolled in study. Out of 140 patients, 79 (56.4%) were male and 61 (43.6%) were female. Mean age at presentation of esophageal atresia was 5.5 days (ranged from 1-30 days). Mean weight was 2.43 kg. Regarding type of esophageal atresia, 10 (7.1%) patients had type A, 97 (69.3%) had type C, 1 (0.7%) had type E and 1 (0.7%) had type F esophageal atresia. Nine patients with type A underwent cervical esophagostomy along with feeding gastrostomy. Right thoracotomy was performed in 98 cases. End to end esophageal anastomosis was possible in 76 patients. Twenty one patients had long gap EA for which cervical esophagostomy and feeding gastrostomy was done. Sepsis was the main complication post operatively (29.3%) followed by pneumonia (14.3%), Anastomotic leak (7.9%) and surgical site-infection (2.1%). Overall mortality was 57.9% (81/140) with pre operative mortality of 21.4% (30/140) and post operative mortality of 36.5% (51/140). Low birth weight and post operative sepsis, anastomotic leak and pneumonia had statistically significant relationship with mortality using SPSS version 21. Conclusion: With the improvement of medical facilities, better survival rates of patients with esophageal atresia can be achieved. [ABSTRACT FROM AUTHOR]

Background: Congenital epulis is a rare benign, smooth, sessile, or pedunculated, solitary mass of varying sizes commonly occurring at the anterior alveolar ridge of the maxilla. It can occur as a solitary as well as a multifocal lesion. Case Presentation: Case 1: A two-day-old female newborn presented with a single round, smooth, pedunculated mass measuring 3x2x2cm on the anterior alveolar ridge of the maxilla. Complete excision of the mass was done under general anesthesia. Histopathology confirms congenital epulis. No recurrence was found till one year of follow-up. Case 2: A one-day-old female newborn presented with a 3x4x3cm soft to firm mass arising from the right upper alveolar margin attached with a stalk and extending into the mouth without any respiratory difficulty. Complete excision of the mass was done under general anesthesia. Follow-up for one year showed no recurrence. Conclusion: Congenital epulis is a rare benign lesion most commonly arising from the anterior alveolar ridge of the maxilla without the involvement of bone or teeth. Complete surgical excision is the treatment of choice without any reported recurrence. [ABSTRACT FROM AUTHOR]

Influenza is a serious and common viral infection of the respiratory system that carries a major global healthcare burden. It can result in several complications, including bacteria pneumonia and even death, especially among those who at highest risk (eg, elderly individuals and those with chronic comorbidities). Despite advances in vaccination and antiviral treatment, outbreaks of novel strains remain a public health dilemma. This article provides an overview of the potentially lethal viral illness, including an understanding of the virology, epidemiology, and diagnosis. Background: Epinephrine is currently the only recommended cardio-resuscitative medication for use in neonatal cardiopulmonary resuscitation (CPR), as per consensus of science and treatment recommendations. An alternative medication, vasopressin, may be beneficial, however there is limited data regarding its effect on cardiac and brain tissue following recovery from neonatal CPR., Aim: To compare the effects of vasopressin and epinephrine during resuscitation of asphyxiated post-transitional piglets on cardiac and brain tissue injury., Methods: Newborn piglets (n = 10/group) were anesthetized, tracheotomized and intubated, instrumented, and exposed to hypoxia-asphyxia and cardiac arrest. Piglets were randomly allocated to receive intravenous vasopressin (Vaso, 0.4 U/kg) or epinephrine (Epi, 0.02 mg/kg) during CPR until return of spontaneous circulation (ROSC). Left ventricle cardiac tissue, and frontoparietal cerebral cortex and thalamus samples from brain tissue were collected from piglets that survived four hours after ROSC. The concentrations of the pro-inflammatory cytokines interleukin (IL)-1β, IL-6, IL-8, and tumour necrosis factor (TNF)-α, cardiac troponin-1, lactate, and levels of oxidized and total glutathione were quantified in tissue homogenates., Main Results: The median time (IQR) to ROSC was 127 (98-162)sec with Vaso and 197 (117-480)sec with Epi (p = 0.07). ROSC rate was 10/10 (100 %) with Vaso and 7/10 (70 %) with Epi (p = 0.21); survival to four hours after ROSC was 10/10 (100 %) with Vaso and 5/7 (71 %) with Epi (p = 0.15). Kaplan-Meier survival curves were significantly different between groups (p = 0.011). Cardiac tissue IL-8 concentration was significantly lower with Vaso than Epi (16.9 (2.94)pg/mg vs. 33.0 (6.75)pg/mg, p = 0.026). All other markers of cardiac and brain tissue injury were similar between Vaso and Epi groups., Conclusions: Vasopressin is effective in the resuscitation of asphyxiated newborn piglets and is associated with reduced inflammation of the myocardium compared to epinephrine, and there was no evidence of increased tissue injury in the frontoparietal cortex and thalamus regions of the brain.
