Abstract

The LANSCE linear accelerator at Los Alamos National Laboratory provides H⁻ and H⁺ beams to several user facilities that support Isotope Production, NNSA Stockpile Stewardship, and Basic Energy Science programs. These beams are initially accelerated to 750 keV using Cockcroft-Walton (CW) based injectors that have been in operation for over 37 years. To reduce long-term operational risks and to realize future beam performance goals for LANSCE we are completing fabrication of a 4-rod Radio-Frequency Quadrupole (RFQ) and design of an associated beam transport line that together will eventually become the modern injector replacement for the existing obsolete H⁺ injector system. Abstract

The Matter-Radiation Interactions in Extremes (MaRIE) experimental facility will be used to discover and design the advanced materials needed to meet 21st century national security and energy security challenges. This new facility will provide the new tools scientists need to develop next-generation materials that will perform predictably and on-demand for currently unattainable lifetimes in extreme environments. The MaRIE facility is based on upgrades to the existing LANSCE 800-MeV proton linac and a new 12-GeV electron linac and associated X-ray FEL to provide simultaneous multiple probe beams, and new experimental areas. In addition to the high-energy photon probe beam, both electron and proton radiography capabilities will be available at the MaRIE facility. Recently, detailed radiography system studies have been performed to develop conceptual layouts of high-magnification electron and proton radiography systems that can meet the experimental requirements for the expected first experiments to be performed at the facility. This paper examines the opportunities and challenges of using Shakespeare as a teaching resource for English language learners, and reflects on an open distance learning programme aimed at engaging EFL students across diverse world contexts. The paper reviews the design, production, and dissemination of these teaching materials and in doing so discusses how points of language, issues of historical culture, and insights into modern usage can all be addressed in ways that are both entertaining and instructional by reference to Shakespeare's life and works. Additionally, it considers how the harnessing of the transmedia affordances of digital communication can create a broad, interactive experience for students, thus mixing the traditional with the cutting-edge in English language pedagogy. Abstract

The 6-item Kessler Psychological Distress Scale (K6; Kessler et al., 2002) is a screener for psychological distress that has robust psychometric properties among adults. Given that a significant proportion of adolescents experience mental illness, there is a need for measures that accurately and reliably screen for mental disorders in this age group. This study examined the psychometric properties of the K6 in a large general population sample of adolescents (N = 4,434; mean age = 13.5 years; 44.6% male). Factor analyses were conducted to examine the dimensionality of the K6 in adolescents and to investigate sex-based measurement invariance. This study also evaluated the K6 as a predictor of scores on the Strengths and Difficulties Questionnaire (SDQ; Goodman, 1997). The K6 demonstrated high levels of internal consistency, with the 6 items loading primarily on 1 factor. Consistent with previous research, females reported higher mean levels of psychological distress when compared with males. The identification of sex-based measurement noninvariance in the item thresholds indicated that these mean differences most likely represented reporting bias in the K6 items rather than true differences in the underlying psychological distress construct. The K6 was a fair to good predictor of abnormal scores on the SDQ, but predictive utility was relatively low among males. Abstract

Introduction and Aims: Learning is most effective when it is active, enjoyable and incorporates feedback. Past research demonstrates that serious games are prime candidates to utilise these principles, however the potential benefits of this approach for delivering drug education are yet to be examined in Australia, a country where drug education in schools is mandatory.Design and Methods: The serious game 'Pure Rush' was developed across three stages. First, formative consultation was conducted with 115 students (67% male, aged 15-17 years), followed by feasibility and acceptability testing of a prototype of the game (n = 25, 68% male). In the final stage, 281 students (62% female, aged 13-16 years) were randomly allocated to receive a lesson involving Pure Rush or an active control lesson. The lessons were compared in terms of learning outcomes, lesson engagement and future intentions to use illicit drugs.Results: Students enjoyed playing Pure Rush, found the game age-appropriate and the information useful to them. Both the Pure Rush and the active control were associated with significant knowledge increase from pre to post-test. Among females, multi-level mixed-effects regression showed knowledge gain was greater in the Pure Rush condition compared to control (β = 2.36, 95% confidence interval 0.36-4.38). There was no evidence of between condition differences in lesson engagement or future intentions to use illicit drugs.Discussion and Conclusions: Pure Rush is an innovative online drug education game that is well received by students and feasible to implement in schools. Abstract

The report describes the feasibility of the addition of multiple viable HLA-mismatched unrelated cord blood units, to a low cell number matched unrelated cord, to assist clinical engraftment. An ablative stem cell transplant was performed in an adult with relapsed acute lymphoblastic leukaemia (ALL), using a single HLA-matched cord blood unit (mononuclear cell dose 0 . 8 × 1 0 7 ), supported by six mismatched cord blood units (one unit per 10 kg recipient weight). No adverse reaction occurred following the infusion of mismatched units and engraftment of the suboptimal-dose matched unit occurred rapidly, with no molecular evidence of engraftment of mismatched cords. Early molecular remission of ALL was demonstrated using a novel PCR for a mitochondrial DNA mutation in the leukaemic clone. The cell dose of the matched cord was well below that recommended to engraft a 70 kg recipient. We suggest that a factor or factors in the mismatched cords enhanced/supported engraftment of the matched cord.

Abstract

Background Anxiety, depressive and substance use disorders account for three quarters of the disability attributed to mental disorders and frequently co-occur. While programs for the prevention and reduction of symptoms associated with (i) substance use and (ii) mental health disorders exist, research is yet to determine if a combined approach is more effective. This paper describes the study protocol of a cluster randomised controlled trial to evaluate the effectiveness of the CLIMATE Schools Combined intervention, a universal approach to preventing substance use and mental health problems among adolescents. Methods/design Participants will consist of approximately 8400 students aged 13 to 14-years-old from 84 secondary schools in New South Wales, Western Australia and Queensland, Australia. The schools will be cluster randomised to one of four groups; (i) CLIMATE Schools Combined intervention; (ii) CLIMATE Schools - Substance Use; (iii) CLIMATE Schools - Mental Health, or (iv) Control (Health and Physical Education as usual). The primary outcomes of the trial will be the uptake and harmful use of alcohol and other drugs, mental health symptomatology and anxiety, depression and substance use knowledge. Secondary outcomes include substance use related harms, self-efficacy to resist peer pressure, general disability, and truancy. The link between personality and substance use will also be examined. Discussion Compared to students who receive the universal CLIMATE Schools - Substance Use, or CLIMATE Schools - Mental Health or the Control condition (who received usual Health and Physical Education), we expect students who receive the CLIMATE Schools Combined intervention to show greater delays to the initiation of substance use, reductions in substance use and mental health symptoms, and increased substance use and mental health knowledge

Abstract

Plateau phase has been achieved in 64% of all newly diagnosed patients with multiple myeloma treated with the ABCM (adriamycin, BiCNU, cyclophosphamide and melphalan) regimen in the Medical Research Council (MRC) trials; this stable clinical stage of the disease is associated with no more than minimal symptoms. Several studies have found that α-interferon (α-IFN) maintenance therapy increases the duration of plateau phase, but it is less clear if this translates into prolonged survival. We report the effect of α-IFN on the duration of plateau phase and overall survival in a trial with 284 patients who were randomized to receive α2b-IFN (Intron-A) or no maintenance therapy during first plateau phase. The minimum follow-up after randomization was 21 months. There was no significant difference in the overall survival between the two treatment groups (χ2 = 0.32, P = 0.57). There was a trend towards longer relapse-free survival in the patients allocated α-IFN, but this trend to longer plateau phase was not statistically significant (χ2 = 1.62, P = 0.2). Disease progression at relapse on α-IFN appears to be more severe with greater elevations from plateau levels of serum paraprotein (P = 0.06) and β2-microglobulin (P = 0.03) levels. Physicians tended to start chemotherapy sooner after diagnosis of relapse when patients had received α-IFN (P = 0.16). Although, in common with most other studies, there is a trend for patients treated with α-IFN to have a longer plateau phase, this is counteracted by morbidity attributable to the treatment and a somewhat shortened survival post relapse. Meta-analysis of interferon trials is required to assess whether the minor trend for longer survival in patients maintained on α-IFN found in some studies is significant and, if so, the extent of this advantage. 87. Monozygotic Twins With Seizures: Shared Characteristics

Abstract

• Eleven sets of monozygotic twins with idiopathic seizure disorders seen at Children's Hospital, Boston, Mass, were reviewed. Nine twin pairs were concordant for seizures. A marked correlation in the precise timing of onset of seizures was found; three sets of twins had their first clinical seizures within a 24-hour period of one another. The clinical features of the seizures and the electroencephalographic patterns, including the location of focal discharges, were strikingly similar in the twin pairs. The significance of these findings in suggesting aberrant processes that may contribute to epileptic syndromes is discussed.

• Motor proficiency, in terms of speed, rhythm and absence of overflow, has previously been shown to distinguish non—learning disabled hyperactive boys from matched controls. Accepted screening methods for selecting children with dyslexia do not include assessments of hyperactivity or other attentional deficits. Dyslexic children selected in this customary manner were compared with an otherwise matched group that had been screened for attentional disorders, on a series of repetitive and alternating movements of the fingers, hands, and feet. The screened dyslexic group performed more rapidly on five of six movements and had fewer qualitative signs of dysrhythmia or overflow.
