Abstract

Digital signal processing (DSP) is nowadays a key enabling technology in coherent optical transmission systems, triggering an increasing research effort on the development and optimization of DSP subsystems. The first DSP development step is often based on the use of simulation tools and offline processing of experimental data. Complementarily, real-time implementation is a critical implementation step to assess the performance and feasibility of advanced DSP subsystems. In this work, we address both the offline and real-time stages of development, supported by an experimental optical testbed. Real-time demonstration is enabled by an FPGA processing platform and 1.25 Gsample/s analog-to-digital conversion.

Abstract

Background: The kidney-brain interaction has been a topic of growing interest. Past studies of the effect of kidney function on intracerebral hemorrhage (ICH) outcomes have yielded inconsistent findings. Although the second, main phase of the Intensive Blood Pressure Reduction in Acute Cerebral Hemorrhage Trial (INTERACT2) suggests the effectiveness of early intensive blood pressure (BP) lowering in improving functional recovery after ICH, the balance of potential benefits and harms of this treatment in those with decreased kidney function remains uncertain. Study Design: Secondary analysis of INTERACT2, which randomly assigned patients with ICH with elevated systolic BP (SBP) to intensive (target SBP < 140 mm Hg) or contemporaneous guideline-based (target SBP < 180 mm Hg) BP management. Setting & Participants: 2,823 patients from 144 clinical hospitals in 21 countries. Predictors Admission estimated glomerular filtration rates (eGFRs) of patients were categorized into 3 groups based on the CKD-EPI (Chronic Kidney Disease Epidemiology Collaboration) creatinine equation: normal or high, mildly decreased, and moderately to severely decreased (>90, 60-90, and <60 mL/min/1.73 m2, respectively). Outcome(s): The effect of admission eGFR on the primary outcome of death or major disability at 90 days (defined as modified Rankin Scale scores of 3-6) was analyzed using a multivariable logistic regression model. Potential effect modification of intensive BP lowering treatment by admission eGFR was assessed by interaction terms. Result(s): Of 2,623 included participants, 912 (35%) and 280 (11%) had mildly and moderately/severely decreased eGFRs, respectively. Patients with moderately/severely decreased eGFRs had the greatest risk for death or major disability at 90 days (adjusted OR, 1.82; 95% CI, 1.28-2.61). Abstract

Background Diabetes increases the risk of cardiovascular and microvascular disease, particularly when not properly treated. Despite advances in diabetes management, inadequate glycemic control has been observed in 60% to 80% of patients. The objective of this study was to identify characteristics correlated with poor glycemic control in a large multicenter survey of Brazilian patients with type 1 diabetes.

Abstract

BackgroundIn April 2016 the Central IntraVenous Additive Service (CIVAS) began to compound non-toxic injectable therapies with a robotic system.1The production started with antiemetic therapies, followed by antibiotic therapies ready to infuse, as cefazoline 1 g in syringes and piperacillin-tazobactam 4.5 g in bags.PurposeThe purpose of this study is to present the new drugs involved that led to production increasing and to describe how CIVAS guarantees the non-hazardous preparations’ supply to hospital departments.Material and methodsAn analysis concerning the actual hospital antibiotic treatment needs has been carried out through the electronic medical record (EMR). Drugs in ready-to-use injectable form were immediately excluded as well as those with a daily consumption under 20 units. Among the remaining amount of medications, only those with a sufficient physical and chemical stability,2that allow batch production and at least 7 days of storage, were considered. For every new molecule, a testing production with five batchs of 10 preparations each were compounded through a robotic system, in order to evaluate the dosage accuracy.ResultsThe new validated drugs included in the automated production result were: oxacillin 1 g, cefotaxime 2 g, azithromycin 500 mg, vancomycin 500 mg and ceftriaxone 1 g, all in 100 ml NaCl bags. The average dosage accuracy ranges from 93.43% of azithromycin to 99.64% of cefotaxime, always compliant with the 10% error limit set by the Official Pharmacopoeia. The annual workload, including piperacillin-tazobactam 4.5 g and cefazoline 1 g already compounded inside the automated system, is estimated as 80 000 bags.ConclusionWithin 2017, the robotic system will cover the dispensing of antibiotic treatments of 50% of hospital departments, reaching 100% by the end of 2018.References and/or Acknowledgements1. Abstract

Domestication of horses fundamentally transformed long-range mobility and warfare 1 . However, modern domesticated breeds do not descend from the earliest domestic horse lineage associated with archaeological evidence of bridling, milking and corralling 2-4 at Botai, Central Asia around 3500 BC 3 . Other longstanding candidate regions for horse domestication, such as Iberia 5 and Anatolia 6 , have also recently been challenged. Thus, the genetic, geographic and temporal origins of modern domestic horses have remained unknown. Here we pinpoint the Western Eurasian steppes, especially the lower Volga-Don region, as the homeland of modern domestic horses. Furthermore, we map the population changes accompanying domestication from 273 ancient horse genomes. This reveals that modern domestic horses ultimately replaced almost all other local populations as they expanded rapidly across Eurasia from about 2000 BC, synchronously with equestrian material culture, including Sintashta spoke-wheeled chariots. We find that equestrianism involved strong selection for critical locomotor and behavioural adaptations at the GSDMC and ZFPM1 genes. Our results reject the commonly held association 7 between horseback riding and the massive expansion of Yamnaya steppe pastoralists into Europe around 3000 BC 8,9 driving the spread of Indo-European languages 10 . Abstract

Aim: Dentistry in Italy is mainly private and therefore perceived as forcedly expensive. Dental tourism is becoming popular in order to get cheap dental care. The aim of this study was to carry out a survey aimed at investigating the perception of dental tourism by Italian patients who have recently received such dental care abroad., Methods: A questionnaire was posted on the web site of an Italian consumers association (Altroconsumo) and members were invited to fill in the questionnaire in case they had experienced dental treatment abroad., Results: Eighty-one complete forms were included in the study. The main motivation for turning to dental tourism was saving money. Another frequent reason for choosing dental tourism was that full rehabilitation was offered in a short time span, usually one week. Patients have often underlined that they appreciated different aspects of the foreign dentists, like speed, kindness, humanity, interest in the course of the treatment, the feeling of ease conveyed by the environment and the personnel. Past negative experiences with Italian dentists were in some cases advocated as reasons for searching dental care abroad. Such negative experiences resulted in a lack of trust in Italian dentists. Some patients reported that while the overall treatment abroad was carried out in a short time, single prosthetic sessions were very long, up to 6 hours., Conclusions: Most patients were satisfied with dental tourism.

Abstract

Objectives: To compare the histological features of bone filled with Bio-Oss, Ostim-Paste or PerioGlas placed in defects in the rabbit tibiae by evaluating bone tissue composition and the integration of titanium implants placed in the grafted bone., Material and Methods: Two cylindrical bone defects, about 4 mm in diameter and 6 mm in depth, were created in the tibiae of 10 rabbits. The defects were filled with either Bio-Oss, PerioGlas, Ostim-Paste or left untreated, and covered with a collagen membrane. Six weeks later, one titanium sandblasted and acid-etched (SLA) implant was inserted at the centre of each previously created defect. The animals were sacrificed after 6 weeks of healing., Results: Implants placed in bone previously grafted with Bio-Oss, PerioGlas or Ostim-Paste obtained a larger extent of osseointegration, although not statistically significant, than implants placed in non-grafted bone. The three grafting materials seemed to perform in a similar way concerning their contribution towards implant osseointegration. All grafting materials appeared to be osteoconductive, thus leading to the formation of bridges of mineralized bone extending from the cortical plate towards the implants surface through the graft scaffold., Conclusions: Grafting with the above-mentioned biomaterials did not add any advantage to the osseointegration of titanium SLA implants in a self-contained defect.

Abstract

Objectives: We tested the hypothesis that patients on bisphosphonates with ONJ might have a poorer dental and periodontal history than non-ONJ patients. Our aim was to retrospectively compare medical and oral history of patients on bisphosphonates with and without ONJ., Study Design: A total of 39 oncologic patients on bisphosphonates were interviewed, examined, and had a panoramic radiograph taken. DMFT values and residual bone levels were calculated and compared for ONJ and non-ONJ patients., Results: Twenty of 39 patients had ONJ. No statistical differences in caries or residual bone level as well as dental extractions were detected, although twice as many patients with ONJ underwent dental extractions. ONJ patients had received statistically more administrations of bisphosphonates compared to non-ONJ patients., Conclusion: A long history of bisphosphonate administration might represent a risk for ONJ onset.
