Abstract

Surgery digitalization is the process of creating a virtual replica of real-world surgery, also referred to as a surgical digital twin (SDT). It has significant applications in various fields such as education and training, surgical planning, and automation of surgical tasks. In addition, SDTs are an ideal foundation for machine learning methods, enabling the automatic generation of training data. In this paper, we present a proof of concept (PoC) for surgery digitalization that is applied to an ex-vivo spinal surgery. The proposed digitalization focuses on the acquisition and modelling of the geometry and appearance of the entire surgical scene. We employ five RGB-D cameras for dynamic 3D reconstruction of the surgeon, a high-end camera for 3D reconstruction of the anatomy, an infrared stereo camera for surgical instrument tracking, and a laser scanner for 3D reconstruction of the operating room and data fusion. We justify the proposed methodology, discuss the challenges faced and further extensions of our prototype. While our PoC partially relies on manual data curation, its high quality and great potential motivate the development of automated methods for the creation of SDTs., Comment: Accepted for the DCA in MI Workshop @ CVPR 2024. Abstract

Financial exchanges are migrating to the cloud, but the best-effort nature of the public cloud is at odds with the stringent latency requirements of exchanges. We present Jasper, a system for meeting the networking requirements of financial exchanges on the public cloud. Jasper uses an overlay tree to scalably multicast market data from an exchange to ~1000 participants with low latency (250 microseconds) and a 1-microsecond difference in data reception time between any two participants. Jasper reuses the same tree for scalable inbound communication (participants to exchange), augmenting it with order pacing and a new priority queue, Limit Order Queue (LOQ), to efficiently handle bursts of market orders. Jasper achieves better scalability and 50% lower latency than the AWS multicast service. During bursty market activity, LOQ nearly doubles the order processing rate.

Abstract

Characterization of noise in current near-term quantum devices is of paramount importance to fully use their computational power. However, direct quantum process tomography becomes unfeasible for systems composed of tens of qubits. A promising alternative method based on tensor networks was recently proposed [Nat. Commun. 14, 2858 (2023)]. In this paper, we adapt it for the characterization of noise channels on near-term quantum computers and investigate its performance thoroughly. In particular, we show how experimentally feasible tomographic samples are sufficient to accurately characterize realistic correlated noise models affecting individual layers of quantum circuits, and study its performance on systems composed of up to 20 qubits. Furthermore, we combine this noise characterization method with a recently proposed noise-aware tensor network error mitigation protocol for correcting outcomes in noisy circuits, resulting accurate estimations even on deep circuit instances. In recent years, informationally complete measurements have attracted considerable attention, especially in the context of classical shadows. In the particular case of informationally over-complete measurements, for which the number of possible outcomes exceeds the dimension of the space of linear operators in Hilbert space, the dual POVM operators used to interpret the measurement outcomes are not uniquely defined. In this work, we propose a method to optimize the dual operators after the measurements have been carried out in order to produce sharper, unbiased estimations of observables of interest. We discuss how this procedure can produce zero-variance estimations in cases where the classical shadows formalism, which relies on so-called canonical duals, incurs exponentially large measurement overheads. We also analyze the algorithm in the context of quantum simulation with randomized Pauli measurements, and show that it can significantly reduce statistical errors with respect to canonical duals on multiple observable estimations.

Abstract

This study tackles key obstacles in adopting surgical navigation in orthopedic surgeries, including time, cost, radiation, and workflow integration challenges. Recently, our work X23D showed an approach for generating 3D anatomical models of the spine from only a few intraoperative fluoroscopic images. This negates the need for conventional registration-based surgical navigation by creating a direct intraoperative 3D reconstruction of the anatomy. Despite these strides, the practical application of X23D has been limited by a domain gap between synthetic training data and real intraoperative images. In response, we devised a novel data collection protocol for a paired dataset consisting of synthetic and real fluoroscopic images from the same perspectives. Utilizing this dataset, we refined our deep learning model via transfer learning, effectively bridging the domain gap between synthetic and real X-ray data. A novel style transfer mechanism also allows us to convert real X-rays to mirror the synthetic domain, enabling our in-silico-trained X23D model to achieve high accuracy in real-world settings. Our results demonstrated that the refined model can rapidly generate accurate 3D reconstructions of the entire lumbar spine from as few as three intraoperative fluoroscopic shots. It achieved an 84% F1 score, matching the accuracy of our previous synthetic data-based research. Additionally, with a computational time of only 81.1 ms, our approach provides real-time capabilities essential for surgery integration. Through examining ideal imaging setups and view angle dependencies, we've further confirmed our system's practicality and dependability in clinical settings. Our research marks a significant step forward in intraoperative 3D reconstruction, offering enhancements to surgical planning, navigation, and robotics.

Abstract

As software systems increasingly interact with humans in application domains such as transportation and healthcare, they raise concerns related to the social, legal, ethical, empathetic, and cultural (SLEEC) norms and values of their stakeholders. Normative non-functional requirements (N-NFRs) are used to capture these concerns by setting SLEEC-relevant boundaries for system behavior. Since N-NFRs need to be specified by multiple stakeholders with widely different, non-technical expertise (ethicists, lawyers, regulators, end users, etc.), N-NFR elicitation is very challenging. To address this challenge, we introduce N-Check, a novel tool-supported formal approach to N-NFR analysis and debugging. N-Check employs satisfiability checking to identify a broad spectrum of N-NFR well-formedness issues (WFI), such as conflicts, redundancy, restrictiveness, insufficiency, yielding diagnostics which pinpoint their causes in a user-friendly way that enables non-technical stakeholders to understand and fix them. We show the effectiveness and usability of our approach through nine case studies in which teams of ethicists, lawyers, philosophers, psychologists, safety analysts, and engineers used N-Check to analyse and debug 233 N-NFRs comprising 62 issues for the software underpinning the operation of systems ranging from assistive-care robots and tree-disease detection drones to manufacturing collaborative robots.

Abstract

BACKGROUND: Although immunomodulators have established benefit against the new coronavirus disease (COVID-19) in general, it is uncertain whether such agents improve outcomes without increasing the risk of secondary infections in the specific subgroup of previously immunocompromised patients. We assessed the effect of immunomodulators on outcomes of immunocompromised patients hospitalized for COVID-19. METHODS: The protocol was prospectively registered with PROSPERO (CRD42022335397). MEDLINE, Cochrane Central Register of Controlled Trials and references of relevant articles were searched up to 01-06-2022. Authors of potentially eligible randomized controlled trials were contacted to provide data on immunocompromised patients randomized to immunomodulators vs control (i.e., placebo or standard-of-care). FINDINGS: Eleven randomized controlled trials involving 397 immunocompromised patients hospitalized for COVID-19 were included. Ten trials had low risk of bias. There was no difference between immunocompromised patients randomized to immunomodulators vs control regarding mortality [30/182 (16.5%) vs 41/215 (19.1%); RR 0.93, 95% CI 0.61-1.41; p = 0.74], secondary infections (RR 1.00, 95% CI 0.64-1.58; p = 0.99) and change in World Health Organization ordinal scale from baseline to day 15 (weighed mean difference 0.27, 95% CI -0.09-0.63; p = 0.15). In subgroup analyses including only patients with hematologic malignancy, only trials with low risk of bias, only trials administering IL-6 inhibitors, or only trials administering immunosuppressants, there was no difference between comparators regarding mortality. INTERPRETATION: Immunomodulators, compared to control, were not associated with harmful or beneficial outcomes, including mortality, secondary infections, and change in ordinal scale, when administered to immunocompromised patients hospitalized for COVID-19. Abstract

BACKGROUND: Halofuginone (PJS-539) is an oral prolyl-tRNA synthetase inhibitor that has a potent in vitro activity against SARS-CoV-2 virus. The safety and efficacy of halofuginone in Covid-19 patients has not been studied. METHODS: We conducted a phase II, randomized, double-blind, placebo-controlled, dose ranging, safety and tolerability trial of halofuginone in symptomatic (≤ 7 days), mostly vaccinated, non-hospitalized adults with mild to moderate Covid-19. Patients were randomized in a 1:1:1 ratio to receive halofuginone 0.5mg, 1mg or placebo orally once daily for 10 days. The primary outcome was the decay rate of the SARS-CoV-2 viral load logarithmic curve within 10 days after randomization. RESULTS: From September 25, 2021, to February 3, 2022, 153 patients were randomized. The mean decay rate in SARS-CoV-2 viral load log10 within 10 days was -3.75 (95% CI, -4.11; -3.19) in the placebo group, -3.83 (95% CI, -4.40; -2.27) in the halofuginone 0.5mg group and -4.13 (95% CI, -4.69; -3.57) in the halofuginone 1mg group, with no statistically significant difference in between placebo vs. halofuginone 0.5mg (mean difference -0.08; 95% CI -0.82 to 0.66, p = 0.96) and between placebo vs. halofuginone 1mg (mean difference -0.38; 95% CI, -1.11; 0.36, p = 0.41). There was no difference on bleeding episodes or serious adverse events at 28 days. CONCLUSIONS: Among non-hospitalized adults with mild to moderate Covid-19 halofuginone treatment was safe and well tolerated but did not decrease SARS-CoV-2 viral load decay rate within 10 days.

Abstract

Coccidioidomycosis, listed as a priority mycosis by the WHO, is endemic in the United States but often overlooked in Central and South America. Employing a multi-institutional approach, we investigate how disease characteristics, pathogen genetic variation, and environmental factors impact coccidioidomycosis epidemiology and outcomes in South America. We identified 292 cases (1978-2021) and 42 outbreaks in Piauí and Maranhão states, Brazil, the largest series outside the US/Mexico epidemic zone. The male-to-female ratio was 57.4:1 and the most common activity was armadillo hunting (91.1%) 4 to 30 days before symptom onset. Most patients (92.8%) exhibited typical acute pulmonary disease, with cough (93%), fever (90%), and chest pain (77%) as predominant symptoms. The case fatality rate was 8%. Our negative binomial regression model indicates that reduced precipitation levels in the current (p = 0.015) and preceding year (p = 0.001) predict heightened incidence. Unlike other hotspots, acidic soil characterizes this region. Brazilian strains differ genomically from other C. posadasii lineages. Northeastern Brazil presents a distinctive coccidioidomycosis profile, with armadillo hunters facing elevated risks. Low annual rainfall emerges as a key factor in increasing cases. A unique C. posadasii lineage in Brazil suggests potential differences in environmental, virulence, and/or pathogenesis traits compared to other Coccidioides genotypes.
