International audience; Xq28 duplications encompassing MECP2 have been described in male patients with a severe neurodevelopmental disorder associated with hypotonia and spasticity, severe learning disability, stereotyped movements, and recurrent pulmonary infections. We report on standardized brain magnetic resonance imaging (MRI) data of 30 affected patients carrying an Xq28 duplication involving MECP2 of various sizes (228 kb to 11.7 Mb). The aim of this study was to seek recurrent malformations and attempt to determine whether variations in imaging features could be explained by differences in the size of the duplications. We showed that 93% of patients had brain MRI abnormalities such as corpus callosum abnormalities (n = 20), reduced volume of the white matter (WM) (n = 12), ventricular dilatation (n = 9), abnormal increased hyperintensities on T2-weighted images involving posterior periventricular WM (n = 6), and vermis hypoplasia (n = 5). The occipitofrontal circumference varied considerably between \textgreater+2SD in five patients and \textless-2SD in four patients. Among the nine patients with dilatation of the lateral ventricles, six had a duplication involving L1CAM. The only patient harboring bilateral posterior subependymal nodular heterotopia also carried an FLNA gene duplication. We could not demonstrate a correlation between periventricular WM hyperintensities/delayed myelination and duplication of the IKBKG gene. We thus conclude that patients with an Xq28 duplication involving MECP2 share some similar but non-specific brain abnormalities. These imaging features, therefore, could not constitute a diagnostic clue. The genotype-phenotype correlation failed to demonstrate a relationship between the presence of nodular heterotopia, ventricular dilatation, WM abnormalities, and the presence of FLNA, L1CAM, or IKBKG, respectively, in the duplicated segment. © 2015 Wiley Periodicals, Inc

The SRY-BOX17 gene (SOX17) encodes a transcription factor playing a key role in different developmental processes including endoderm formation, cardiac myogenesis, kidney/urinary development and differentiation of oligodendrocytes, the brain myelinating cells. In a candidate gene approach, we analyzed the SOX17 gene in hypomyelinating leukodystrophies (HL) characterized by a permanent deficit in the amount of central nervous system myelin. Five genes are involved in the aetiology of HL but 40% of HL remains without known genetic origin (UHL). New sequence variations in SOX17 were identified but all correspond to nonpathogenic variants, suggesting that SOX17 is not involved in UHL phenotype. In one patient, we identified the c.775TA (p.Tyr259Asn) variation already reported as causative of congenital kidney and urinary tract abnormalities (CAKUT). Nevertheless, since our patient did not present such a phenotype, we propose that this variant may alternatively represent an "at-risk" allele for CAKUT rather than a causative allele. This observation strengthens the idea that caution must be taken when linking genetic variation to disease, especially in discrete phenotypes such as CAKUT.

Background Influenza-related neurological complications (INC) have been reported during seasonal flu in children. Objectives To investigate the types, outcomes and incidence of INC occurring during the 2009 A(H1N1) pandemic, a retrospective analyze was conducted in the single French pediatric hospital of Lyon from October 2009 to February 2010. Study design All children presenting with fever, influenza-like illness, respiratory distress or neurological symptoms were tested for influenza A(H1N1)pdm09 infection from respiratory specimens using real time RT-PCR. Results INC occurred in 14 A(H1N1)pdm09 positive children (7.7% of A(H1N1)pdm09 positive children admitted to hospital) with a median age of 5.1 years. Admission to the intensive care unit (ICU) was required for nine children (64.3%). Half of the children with INC had comorbidity and three had coinfection, both characteristics mainly found in children requiring the ICU. All children received oral oseltamivir treatment. Febrile seizures were observed in eight children, half of them having a chronic comorbidity (2 epilepsy, 1 nonketotic hyperglycinemia, 1 anoxic encephalopathy). Other INC, less commonly reported, included 2 cases of encephalitis, 1 encephalopathy, 1 basilar artery thrombosis, 1 myasthenic crisis and 1 coma. Eleven of the 14 children (78.6%) recovered, one had a minor disability, one child developed a locked-in syndrome and one died from complications of an acute necrotizing encephalopathy. Discussion INC can be observed even in children with no underlying disorder. It may lead to dramatic issue in a significant number of cases.

Sjogren-Larsson syndrome (SLS) is a rare autosomal recessive disorder characterized by ichthyosis, spastic di- or tetraplegia and mental retardation due a defect of the fatty aldehyde dehydrogenase (FALDH), related to mutations in the ALDH3A2 gene. In this study, we screened a French cohort of patients with Sjogren-Larsson syndrome (SLS) for mutations in the ALDH3A2 gene. The five unrelated patients with typical SLS all present mutations in this gene. Three novel mutations were identified whereas three other ones were previously described. We also realized functional analyses at the mRNA level for two splice site mutations to study their deleterious consequences. Two of the previously described mutations had already been identified in the same region of Europe, suggesting a putative founder effect. We suggest that, (1) when clinical and MR features are present, direct sequencing of the ALDH3A2 gene in SLS is of particular interest without necessity of a skin biopsy for enzymatic assay in order to propose genetic counsel and (2) identification of mutations already described in the same population with putative founder effects may simplify genetic analysis in this context. Moyamoya is a cerebrovascular angiopathy characterized by a progressive stenosis of the terminal part of the intracranial carotid arteries and the compensatory development of abnormal and fragile collateral vessels, also called moyamoya vessels, leading to ischemic and hemorrhagic stroke. Moyamoya angiopathy can either be the sole manifestation of the disease (moyamoya disease) or be associated with various conditions, including neurofibromatosis, Down syndrome, TAAD (autosomal-dominant thoracic aortic aneurysm), and radiotherapy of head tumors (moyamoya syndromes). Its prevalence is ten times higher in Japan than in Europe, and an estimated 6%–12% of moyamoya disease is familial in Japan. The pathophysiological mechanisms of this condition remain obscure. Here, we report on three unrelated families affected with an X-linked moyamoya syndrome characterized by the association of a moyamoya angiopathy, short stature, and a stereotyped facial dysmorphism. Other symptoms include an hypergonadotropic hypogonadism, hypertension, dilated cardiomyopathy, premature coronary heart disease, premature hair graying, and early bilateral acquired cataract. We show that this syndromic moyamoya is caused by Xq28 deletions removing MTCP1/MTCP1NB and BRCC3. We also show that brcc3 morphant zebrafish display angiogenesis defects that are rescued by endothelium-specific expression of brcc3. Altogether, these data strongly suggest that BRCC3, a deubiquitinating enzyme that is part of the cellular BRCA1 and BRISC complexes, is an important player in angiogenesis and that BRCC3 loss-of-function mutations are associated with moyamoya angiopathy.

Background: In order to identify biomarkers useful for the diagnosis of genetic white matter disorders we compared the metabolic profile of patients with leukodystrophies with a hypomyelinating or a non-hypomyelinating MRI pattern. Methods: We used a non-a priori method of in vitro 1 H-NMR spectroscopy on CSF samples of 74 patients with leukodystrophies. Results: We found an elevation of CSF N-acetylaspartylglutamate (NAAG) in patients with Pelizaeus–Merzbacher disease (PMD)— PLP1 gene, Pelizaeus–Merzbacher-like disease— GJC2 gene and Canavan disease— ASPA gene. In the PMD group, NAAG was significantly elevated in the CSF of all patients with PLP1 duplication (19/19) but was strictly normal in 6 out of 7 patients with PLP1 point mutations. Additionally, we previously reported increased CSF NAAG in patients with SLC17A5 mutations. Conclusions: Elevated CSF NAAG is a biomarker that suggests specific molecular diagnostic abnormalities in patients with white matter diseases. Our findings also point to unique pathological functions of the overexpressed PLP in PMD patients with duplication of this gene.

The human myelin proteolipid protein 1 gene (hPLP1), which encodes the major structural myelin proteins of the central nervous system (CNS), is classically described as expressed in the oligodendrocytes, the CNS myelinating cells. We identified two new exons in the intron 1 of the hPLP1 gene that lead to the expression of additional mRNA and protein isoforms mainly expressed in neurons instead of oligodendrocytes. Those novel neuronal PLP isoforms are detected as soon as human fetal development and their concomitant expression is specific of the human species. As classical PLP proteins, the novel protein isoforms seem to be addressed to the plasma membrane. These results suggest for the first time that PLP may have functions in humans not only in oligodendrocytes but also in neurons and could be implicated in axono-glial communication. Moreover, this neuronal expression of the hPLP1 gene might explain the neuronal dysfunctions in patients carrying hPLP1 gene mutations.

International audience; Pelizaeus-Merzbacher Disease is an X-linked hypomyelinatiing leukodystrophy. We report mutations in the thyroid hormone transporter gene MCT8 in 11% of 53 families affected by hypomyelinating leukodystrophies of unknown aetiology. The 12 MCT8 mutated patients express initially a Pelizaeus-Merzbacher-Like disease phenotype with a latter unusual improvement of magnetic resonance imaging white matter signal despite absence of clinical progression. This observation underlines the interest of determining both free T3 and free T4 serum concentrations to screen for MCT8 mutations in young patients (

Objective: This study aims to determine the natural history of genetic hypomyelinating disorders Pelizaeus–Merzbacher disease (PMD) and Spastic Paraplegia type 2 (SPG2) on magnetic resonance imaging (MRI) and to correlate MRI results with clinical severity. Methods: A prospective study in 35 patients with variable forms of PMD or SPG2 was performed on a total of 66 cerebral MRI. A myelination score and a global atrophy score were determined for each MRI. We also performed image segmentation to calculate corpus callosum area, cerebellum, white matter and gray matter volumes. Results: Multivariate analysis with adjustment to the age revealed a significant difference for global atrophy score and corpus callosum area in different subgroups of severity at the first MRI acquisition. Evolution of MRI over time showed an increasing score of myelination until the age of 12 years, followed by stabilization. On the contrary, global atrophy, atrophy of corpus callosum and cerebellum worsened in the majority of patients with aging. No change in gray matter and white matter proportions was noticed over time. Discussion: This study points out a correlation between cerebral atrophy and clinical severity in patients with PMD and SPG2 whereas degree of myelination was not clearly discriminating. It also highlighted, for the first time in a large cohort of patients, a clear inter- and intra-individual cerebral atrophy over time involving white and gray matter.

Nous avons effectue 2 travaux successifs sur la proteine S100s pour analyser son interet eventuel comme marqueur de gravite dans les traumatismes crâniens. A l’appui de ces deux travaux nous proposons de generaliser ce dosage dans les services d’urgences pediatriques. Dans la premiere etude nous avons determine les valeurs normales de cette proteine a partir d’une serotheque d’enfants de 0 a 16 ans. Nous avons pu determiner que le taux de cette proteine diminuait de la naissance a 3 ans. Dans la deuxieme etude nous avons dose cette proteine chez 446 enfants presentant un traumatisme crânien modere. Il existe une correlation significative entre le taux de cette proteine est la gravite clinique apprecie par le score de Glasgow (0,21, 0,31, et 0,44 μg /L selon qu’il appartenait a un Master 1,2 ou 3, p Le principe de la generalisation du dosage de cette proteine dans les services d’urgences au cours des traumatismes crâniens moderes merite de faire l’objet d’une discussion collegiale.

The human hypothalamus is a small deeply located region placed at the crossroad of neurovegetative, neuroendocrine, limbic, and optic systems. Although deep brain stimulation techniques have proven that it could be feasible to modulate these systems, targeting the hypothalamus and in particular specific nuclei and white bundles, is still challenging. Our goal was to make a synthesis of relevant topographical data of the human hypothalamus, under the form of magnetic resonance imaging maps useful for mastering its elaborated structure as well as its neighborhood. As from 1.5 Tesla, Inversion-Recovery sequence allows locating the hypothalamus and most of its components. Spotting hypothalamic compartments is possible according to specific landmarks: the anterior commissure, the mammillary bodies, the preoptic recess, the infundibular recess, the crest between the preoptic and the infundibular recesses, the optical tract, the fornix, and the mammillo-thalamic bundle. The identification of hypothalamus and most of its components could be useful to allow the quantification of local pathological processes and to target specific circuitry to alleviate severe symptoms, using physical or biological agents.

Genetic studies of intellectual disability and identification of monogenic causes of obesity in humans have made immense contribution toward the understanding of the brain and control of body mass. The leptin > melanocortin > SIM1 pathway is dysregulated in multiple monogenic human obesity syndromes but its downstream targets are still unknown. In ten individuals from six families, with overlapping 6q16.1 deletions, we describe a disorder of variable developmental delay, intellectual disability, and susceptibility to obesity and hyperphagia. The 6q16.1 deletions segregated with the phenotype in multiplex families and were shown to be de novo in four families, and there was dramatic phenotypic overlap among affected individuals who were independently ascertained without bias from clinical features. Analysis of the deletions revealed a ∼350 kb critical region on chromosome 6q16.1 that encompasses a gene for proneuronal transcription factor POU3F2, which is important for hypothalamic development and function. Using morpholino and mutant zebrafish models, we show that POU3F2 lies downstream of SIM1 and controls oxytocin expression in the hypothalamic neuroendocrine preoptic area. We show that this finding is consistent with the expression patterns of POU3F2 and related genes in the human brain. Our work helps to further delineate the neuro-endocrine control of energy balance/body mass and demonstrates that this molecular pathway is conserved across multiple species.

International audience; Six clinical studies of chronic electrical modulation of deep brain circuits published between 1968 and2010 have reported effects in 55 vegetative or minimally conscious patients. The rationale stimulation wasto activate the cortex through the reticular-thalamic complex, comprising the tegmental ascending reticularactivating system and its thalamic targets. The most frequent intended target was the central intralaminarzone and adjacent nuclei. Hassler et al. also proposed to modulate the pallidum as part of the arousal andwakefulness system. Stimulation frequency varied from 8 Hz to 250 Hz. Most patients improved, althoughin a limited way. Schiff et al. found correlations between central thalamus stimulation and arousal andconscious behaviours. Other treatments that have offered some clinical benefit include drugs, repetitivemagnetic transcranial stimulation, median nerve stimulation, stimulation of dorsal column of the uppercervical spinal cord, and stimulation of the fronto-parietal cortex. No one treatment has emerged as a goldstandard for practice, which is why clinical trials are still on-going. Further clinical studies are needed todecipher the altered dynamics of neuronal network circuits in patients suffering from severe disorders ofconsciousness as a step towards novel therapeutic strategies.; Six études cliniques de modulation électrique chronique de circuits cérébraux profonds, publiées entre1968 et 2010, ont rapporté des effets chez 55 patients, végétatifs ou en état de conscience minimale. D’autres traitements ont pu améliorer cliniquement le patient : médicaments, stimulationmagnétique transcrânienne répétée, stimulation du nerf médian, stimulation des cordons postérieurs de lamoelle cervicale et stimulation du cortex frontopariétal. Aucun traitement n’a été validé pour la cliniquecourante, d’où la réalisation d’études cliniques, enregistrées, en cours. De nouvelles études sontnécessaires pour décrypter les altérations de la dynamique des circuits neuronaux chez des patientssouffrant de troubles de la conscience sévères, permettant ainsi de proposer de nouvelles stratégiesthérapeutiques.

International audience; Aims: We performed quantitative diffusion tensor imaging and brain tractography to distinguish clinical severity in a series of 35 patients with hypomyelinating PLP1-related disorders classified using the Motor Developmental Score according to the best motor function acquired before the age of 5 years and the gross motor function measure (GMFM) at the time of magnetic resonance imaging acquisition. Methods: We calculated fractional anisotropy and diffusivity values in 26 regions of interest and the numbers of fibers and volumes of hemisphere tractograms. Fiber bundles on tractograms were characterized according to 3 criteria: size, direction of main-stream fibers, and connectivity of bundles (extratelencephalic projections, commissural fibers, and intrahemispheric connections). Results: Age-adjusted multivariate analysis in 3 severity groups revealed increased isotropic diffusion in the superior cerebellar peduncle and grey matter in the most severe group, and larger tractogram volumes and increased numbers of fibers in the least severely affected group. Tractogram patterns showed preserved extratelencephalic projections and a main anterior-posterior aspect of intrahemispheric fibers in most patients, whereas interhemispheric connectivity was variable. The most severely affected and intermediate patients had less intrahemispheric connectivity, with a frequent predominant anterior-posterior direction of main-stream fibers. Interpretation: Diffusion tensor imaging and tractographic parameters can operate as biomarkers to distinguish clinical severity in PLP1-related disorders and could improve our understanding of hypomyelinating leukodystrophies.

INTRODUCTION Les traumatismes crâniens sont un problème majeur de santé publique. Ils représentent la première cause de mortalité et d’incapacité chez les enfants de plus d’un an. Un traumatisme crânien léger est défini par un score de Glasgow compris entre 13 et 15. Dans ces traumatismes légers, la grande majorité des patients ne présentent pas de symptômes au décours. Cependant, certains patients décrivent la persistance de symptômes invalidants à moyen ou long terme. L’objectif de cette étude est d’évaluer la prévalence des symptômes à J8 d’un traumatisme crânien léger chez l’enfant afin d’évaluer l’intérêt de mettre en place un suivi régulier pour ces enfants.MATÉRIELS ET MÉTHODESNous avons réalisé une étude multicentrique, rétrospective, sur une période de 4 mois (entre septembre 2021 et fin décembre 2021) incluant des patients de 1 à 16 ans, vus pour traumatisme crânien léger au CHU de Clermont-Ferrand (63) et aux CH de Montluçon (03), de Moulins (03) et d’Aurillac (15). Une non-opposition des familles à l’utilisation des données était requise. Les participants répondaient à un questionnaire de soins courants composé de 20 symptômes, sur la persistance de symptômes à J8 du traumatisme par e-mail. Ces données étaient analysées pour identifier leur prévalence, leur nature, par classe d’âge et par sexe.RÉSULTATSSur les 78 patients, à qui le questionnaire a été transmis en contexte post-commotionnel aux urgences sur la période donnée, nous avons récupéré 65 réponses. Parmi ces patients, 35 avaient entre 1 et 5 ans, 5 entre 6 et 11 ans et 5 avaient entre 12 et 16 ans. Le sexe ratio était de 2,25. 55,4 % des patients présentaient des symptômes à J8, les plus fréquents étant la céphalée (23 patients) l’asthénie (15 patients) et les difficultés d’endormissement (10 patients). Ces symptômes étaient présents dans toutes les classes d’âge, y compris chez les plus jeunes patients.CONCLUSIONDans notre étude, une prévalence importante des symptômes à J8, supérieure à 50 % des patients inclus, est retrouvée après un traumatisme crânien léger. Il apparaît indispensable de suivre à distance ces patients pour proposer une prise en charge ré-éducative précoce si nécessaire et éviter la survenue d’un syndrome post-commotionnel.

CONTEXTE : l’épilepsie est la maladie neurologique chronique la plus fréquente chez l’enfant. Les formes d’épilepsie sont variées avec des présentations cliniques, traitements, conséquences neuropsychologiques, et pronostics pouvant être différents. Néanmoins, le quotidien des familles est souvent marqué par la crainte d’une crise imprévisible pouvant occasionner des répercussions psychosociales et familiales. L’éducation thérapeutique et le suivi rapproché sont des clés de l’efficacité et de l’observance thérapeutique, ainsi que de l’obtention d’une meilleure qualité de vie.OBJECTIFS : recueillir les besoins et les attentes des familles en termes de suivi et de prise en charge d’une épilepsie en pédiatrie au CHU de Clermont-Ferrand. Évaluer les répercussions sur le quotidien. Faire un état des lieux des connaissances sur l’épilepsie, la gestion des crises, les examens et les traitements avec évaluation de l’observance thérapeutique. Évaluer la satisfaction du suivi. Proposer des voies d’amélioration de coordination et de communication adaptées aux attentes communes.MÉTHODES : étude qualitative, observationnelle et descriptive, prospective, et monocentrique, de perceptions recueillies par des questionnaires, auprès de parents d’enfants suivis pour une épilepsie au CHU de Clermont-Ferrand avec participation proposée à l’enfant.RÉSULTATS : 72 questionnaires ont été remplis : 52 par un ou les deux parents ; 6 par les enfants âgés de 12 à 17 ans inclus ; et 14 par les enfants âgés de 6 à 11 ans inclus. 71 % parents ont déclaré au moins un trouble cognitif lié à l’épilepsie. Le niveau de changement dans le quotidien est majeur pour les parents et les adolescents (médiane 80.0 et 87.5), et moins ressenti par les enfants âgés de 6 à 11 ans (21 % beaucoup), avec des restrictions pour 46 % (arrêt d’activités sportives pour 24 %, difficultés d’intégration, …), dispositifs de surveillance pour 25 % et aménagements du domicile pour 13 %. Les parents ont déclaré avoir un niveau de connaissance sur l’épilepsie correct (médiane 61.0) notamment sur la gestion des crises (médiane 73.5) les examens complémentaires (médiane 69.5), l’intérêt des traitements(médiane 67.0) et un peu moins sur leurs effets indésirables (médiane 50.0). L’observance thérapeutique déclarée est bonne avec 62 % des parents qui affirment ne l’avoir jamais oublié. 79 % des parents sont satisfaits de la fréquence du suivi par le neuropédiatre. Cependant, 69 % seraient intéressé par un suivi plus rapproché par un IPA. Pour les parents, l’élaboration d’un programme d’ETP serait très utile (médiane 81.5). Les thèmes suscitant le plus vif intérêt auprès des parents sont la conduite à tenir en cas de crise, et ses facteurs déclenchants possibles. L’intérêt porté à un programme d’ETP par les enfants âgés de 12 à 17 ans est plus mitigé (médiane 58.0). Ils sont plus intéressés par l’adaptation de la vie quotidienne et de la scolarité et le fonctionnement du cerveau pendant une crise. 86 % des enfants âgés de 6 à 11 ans souhaiteraient avoir des informations sur leur épilepsie.CONCLUSION : cette étude a permis d’objectiver la nécessité d’éducation thérapeutique et d’identifier les principaux besoins pédagogiques des parents et des enfants. Elle pourra servir de support à l’élaboration d’un Programme d’Éducation Thérapeutique du Patient, et à la demande d’un Infirmier de Pratique Avancée en épileptologie pédiatrique.

Rationnel : La paralysie cérébrale (PC) est la déficience motrice la plus courante chez l’enfant entrainant le plus fréquemment une diplégie ou une hémiplégie spastique. La marche est une priorité pour les familles en terme d’indépendance dans la vie quotidienne, ainsi l’amélioration des paramètres de marche est l’objectif principal des systèmes de réadaptation. Le système G-EO est un robot de ré-entrainement à la marche de dernière génération avec lequel le mouvement de marche est généré selon des données de capture de mouvements, avec des réglages de cadence, de longueur de pas et d'angles articulaires. L’objectif principal de cette étude est de montrer l’efficacité d’une rééducation robotisée intensive à l’aide du système GE-O® sur la vitesse de marche chez des enfants porteurs d’une PC hémiplégique. Les objectifs secondaires sont de montrer l’efficacité de cette rééducation intensive sur l’endurance, les paramètres osseux et musculo-adipeux du membre inférieur lésé et d’évaluer s’il existe une modification de l’activation des régions corticales d’intérêt au repos et de leur connectivité fonctionnelle et structurelle.Matériel et méthode : Étude pilote incluant des enfants avec hémiplégie infantile appariés par âge et score moteur dans un groupe Robot (n=19) ou un groupe Kiné (n=19). Pendant 2 semaines, le groupe Robot a bénéficié de 20 minutes de rééducation par le système G-EO, 5 jours par semaine et le groupe Kiné a bénéficié de 30 minutes de kinésithérapie classique, 3 jours par semaine. Nous avons réalisé 2 semaines avant (T0) et 1 mois après (T1) la rééducation une évaluation de la vitesse de la marche, une IRM cérébrale fonctionnelle au repos et une IRM par tenseur de diffusion (DTI) ainsi qu’une mesure des paramètres de micro-architecture osseuse et musculo-adipeuse du membre inférieur pathologique par Tomodensitométrie quantitative périphérique (pQCT).Résultats : Nous avons inclus 38 enfants âgés de 4 à 18 ans sur une période d’un an au CHU de Clermont-Ferrand. Dans le groupe Robot, une amélioration significative de la vitesse de la marche après rééducation était identifiée alors que le groupe Kiné voit sa vitesse de marche diminuer mais sans que cela ne soit significatif. De plus, l’analyse des différences d’évolution entre les deux groupes est apparue significative en faveur d’une amélioration dans le groupe Robot. Par ailleurs, la rééducation robotisée intensive à l’aide du système G-EO semble entraîner une augmentation de connectivité fonctionnelle au repos dans les hémisphères sain et lésé comparativement à la rééducation conventionnelle en kinésithérapie alors qu’il n’a pas été retrouvé d’amélioration des paramètres de DTI ni de pQCT. Enfin, une plus nette amélioration de la vitesse de marche a été constatée chez les patients âgés de 8 ans et plus.Conclusion : Une rééducation intensive avec le système G-EO® permet aux enfants porteurs de PC hémiplégique d'améliorer leur vitesse de marche. On constate également une augmentation de la connectivité fonctionnelle au repos dans les hémisphères sain et lésé comparativement à la rééducation conventionnelle en kinésithérapie. Les rééducations robotisées intensives dans la paralysie cérébrale, ont déjà démontré leur intérêt dans quelques études, mais les mécanismes de neuroplasticité sous-jacents restent largement méconnus. Cette étude innovante dans un large groupe de patients, couplant évaluation clinique de la rééducation intensive robotisée et imagerie cérébrale permet de mieux préciser les mécanismes physiologiques mis en jeu dans les programmes rééducatifs intensifs et argumenter leur intérêt thérapeutique dans la prise en charge globale des patients.
