Fundamento: La demencia tiene una alta prevalencia tanto en países desarrollados como en aquellos en vía de desarrollo, y constituye una de las principales causas de estadio terminal por enfermedad no oncológica. Objetivo: Determinar el comportamiento de algunas variables biológicas en pacientes con demencia en estadio terminal. Métodos: Estudio descriptivo en 16 pacientes con diagnóstico de demencia en estadio terminal, ingresados en el Hospicio de Seychelles entre febrero de 2010 y febrero de 2011. Se analizó el comportamiento de la demencia en relación con las variables: edad, tipo de demencia, respuesta al mini examen cognitivo de Folstein y a los Índices de Charlson y Barthel, presencia de enfermedades crónicas no transmisibles y escenario de salud responsable de la remisión. Resultados: El 31,3 % de los pacientes ingresados en el Hospicio presentaban una demencia. El grupo de edad con mayor número de casos fue el de 75 a 84 años. La demencia vascular y Alzheimer tuvieron igual número de casos con un 37,5 % cada una. El 56 % de los pacientes fallecieron durante los primeros 15 días del ingreso y solo el 12,5 % después de los 6 meses. La hipertensión arterial y la enfermedad cerebrovascular fueron las enfermedades crónicas no transmisibles más frecuentes. El 56 % de los casos fue remitido desde el escenario hospitalario. Conclusiones: La demencia en estadio terminal muestra un comportamiento que permite anticipar una adecuada estrategia de cuidados paliativos a nivel del Hospicio.

Purpose – The purpose of this paper is to test an augmented technology acceptance model (TAM) in the online financial trading context. This research aims to investigate how e‐investors are influenced by perceived trust, security, and privacy jointly with traditional TAM constructs.Design/methodology/approach – The research examines e‐investors' behavioral intention to use online dealers' and stockbrokers' services. The model suggests that perceived trust jointly with perceived usefulness and perceived ease of use are important antecedents of intentions; the hypotheses are statistically tested using structural modeling.Findings – The results from this study suggest that perceived trust, usefulness and ease of use are important issues in online trading systems. Based on self-determination theory (SDT), this study proposed an extended Technology Acceptance Model (TAM) in the context of e-learning service. In the proposed model perceived usefulness, perceived playfulness and perceived ease of use are predicted to be influenced by perceived autonomy support, perceived competence and perceived relatedness. Although TAM has received fairly extensive attention in prior research, this study is one of the first to examine the effects of motivational factors affecting TAM constructs. The results show that applying SDT to e-learning in a work setting can be useful for predicting continuance intention.

Based on the expectancy disconfirmation theory, this study proposes a decomposed technology acceptance model in the context of an e-learning service. In the proposed model, the perceived performance component is decomposed into perceived quality and perceived usability. A sample of 172 respondents took part in this study. The results suggest that users' continuance intention is determined by satisfaction, which in turn is jointly determined by perceived usefulness, information quality, confirmation, service quality, system quality, perceived ease of use and cognitive absorption.

In this paper we will analyze telework in the companies which belong to the information technology sector. To this purpose we have made an empirical research of 107 Spanish companies. First of all we have analyzed the tasks carried out through telework, the form is which they are carried out, both the individual tasks and the shared ones, the methods used for exchanging results, the changes brought about by telework in the realization of those tasks and the coordination and assignment problems involved in telework. On the other hand we analyzed the most extended resources used, that is to say hardware and software as well as the importance of this activity for the development of these companies in an economy with an ever more global character.

Purpose To evaluate safety and preliminary efficacy of the humanized anti-epidermal growth factor receptor monoclonal antibody h-R3 in combination with radiotherapy (RT) in unresectable head and neck cancer patients. Secondary end points were the measurement of h-R3 serum levels and the assessment of the potential mechanisms of antitumor effect on patient biopsies. Anti-idiotypic response to h-R3 was assessed. To predict pharmacologic effect, a mathematical model for antibodies recognizing antigens expressed in tumors and normal tissues was built. Patients and Methods Twenty-four patients with advanced carcinomas of the head and neck received six once-weekly infusions of h-R3 at four dose levels in combination with RT. Pretreatment tumor biopsies were obtained to evaluate epidermal growth factor receptor expression as an enrollment criterion. Second biopsies were taken to evaluate the proliferative activity and angiogenesis in comparison with the pretreatment samples. Patient serum samples were collected to measure h-R3 levels and anti-idiotypic response. Results The combination of h-R3 and RT was well tolerated. Antibody-related adverse events consisted in infusion reactions. No skin or allergic toxicity appeared. Overall survival significantly increased after the use of the higher antibody doses. Immunohistochemistry studies of tumor specimens before and after treatment revealed that antitumor response correlated with antiproliferative and antiangiogenic effect. One patient developed antibodies to h-R3. The mathematical model predicted that the maximum difference between the area under the curve in tumors and normal tissues is reached when the antibody has intermediate affinity. Conclusion h-R3 is a well-tolerated drug that may enhance radiocurability of unresectable head and neck neoplasms.

En materia de salud pública, se analizan los contenidos de nicotina, alquitrán y monóxido de carbono del humo, y se comparan con los de los cigarrillos convencionales. En materia de consumo, se examina el etiquetado. Resultados: Los contenidos de nicotina, alquitrán y monóxido de carbono alcanzan valores de hasta el 70%, el 85% y el 84%, respectivamente, más de lo permitido para los cigarrillos convencionales. El 67% de las muestras no indican los contenidos de nicotina y alquitrán, y el 100% el monóxido de carbono. A pesar de la existencia de etiquetado, éste no garantiza una información suficiente al consumidor. Conclusiones: El tabaco de liar supone un problema tanto en materia de salud pública como en consumo. Introduction. Dementia is one of the diseases with the greatest impact on the individual and family health, constituting a strong predictor of institutionalization. Objective. To identify cognitive impairment and dementia in residents of a nursing home and to describe these biological variables. Methods. A descriptive, correlational and cross-sectional quantitative approach in 61 elderly male residents was conducted to determine the presence of cognitive impairment (CI), dementia, comorbidities and functional status. Information was obtained from medical records. Results. 35 residents had CI. Mean age of patients with CI was 79.4 (± 9.81) years vs. 78.6 (± 9.79) in patients without CI. 25 residents were diagnosed with Alzheimer's disease, 4 had vascular dementia, 3 had frontotemporal dementia and 1 resident had Lewy bodies dementia. 2 patients with cognitive impairment did not meet any dementia criteria. Significant association between the degree of cognitive impairment and functional status of patients was found (x2 = 25.76, P = 0.0001), with a mean negative correlation between the response to MMSE and FAQ (Spearman = -0.726, P = 0.000). Hypertension was the most commonly found non communicable disease in residents and patients with CI had a Charlson Comorbidity Index greater than those without CI (z = -2.434, P = 0.015). Conclusions. Mean age, functional impairment, and comorbidities were greater in residents with CI as compared to residents without CI. Among older adults with cognitive impairment, Alzheimer’s was the predominant dementia.

In this paper, we examine the success of Online Trading Systems and focus on User Satisfaction in the context of an investor’s investment decision. In our study, we have extended the Technology Acceptance Model to the Model of the Success of an Information System based on the Web as a precursor, and then investigated the role of perceived credibility in user acceptance of an Online Trading Website. The results indicate the importance of service quality, perceived usefulness and information quality in mediating the relationship between decision support satisfaction and general satisfaction.

Not available, Desde 1753, en que fue creado por el insigne marino Jorge Juany Santacilia, el Real Instituto y Observatorio de la Armada (ROA) ha sido el núcleo de la investigación científica de la Armada, manteniendo hoy día un reconocido prestigio tanto a nivel nacional como internacional. El autor de este artículo, director del ROA, a través de una síntesis descriptiva de la estructura orgánica del centro en cuatro Secciones (Efemérides, Astronomía, Geofísica, y Hora), nos introduce en la evolución histórica de esta institución y en la amplia gama de actividades que desarrolla en la actualidad, participando en numerosos proyectos y foros internacionales, y manteniendo relaciones de colaboración permanente con organismos científicos similares de diversos países. Sus publicaciones de efemérides astronómicas, sus trabajos en geomagnetismo, sismología, seguimiento de satélites, astrofísica, metrología, etc. etc., así como el hecho de ser el depositario del Patrón Nacional de la unidad de tiempo, son el exponente de que el ROA continúa siendo un centro científico en plena actividad y pujanza, y en no pocas ocasiones la voz de España en el concierto internacional de estas ramas del conocimiento y de la investigación. Finaliza el artículo con una breve referencia a la Escuela de Estudios Superiores, vinculada al ROA desde su creación y que continúa con sus cursos de «estudios mayores» manteniendo vivo el espíritu científico de la Armada, mediante la formación de un selecto núcleo de oficiales en las áreas de ciencias Físico-Matemáticas, Astronomía y Geofísica, y en la preparación para los cursos de Ingeniería.

Motivated by the growing interest in electronic brokerage firms, we focus our research questions on examining the factors that contribute to acceptance of online trading systems. The research model, based on a Technology Acceptance Model (TAM) and added perceived security, perceived privacy and personal innovativeness, was tested by means of a Partial Least Square (PLS) modelling approach. The findings indicate that perceived security, Perceived Usefulness (PU) and personal innovativeness are positively related to behavioural intentions.

This paper presents an analysis of the use of telework in firms belonging to the Information and Communications Technology (ICT) sector. Using data gathered from 280 Spanish firms, our empirical study included analysing the general characteristics of this new type of work practice in ICT firms. The view is to reveal the most widespread mode of telework in use, the number of days it occurs, the number of hierarchical levels and other relevant aspects determining its operation.

In the period 2002–2003 we carried out a broad study of Information Systems (IS) in financial firms in Spain. Specifically, we surveyed one in five (44) banks, savings banks, rural credit cooperatives and credit cooperative banks. In general we can affirm that the IS of financial firms are in a more developed stage of their evolution than those of other firms, since they are more automated in terms of equipment and information. There is still, however, room for improvement, above all with regard to data transmission and computing tools among the highest levels of management.

Background: Paracetamol's solubility is achieved by adding to the excipient sodium salts, either as bicarbonate, carbonate or citrate. As the relationship between salt and hypertension is well known, due to the sodium content it has raised a hypothesis that may interfere with the control of that risk factor. Therefore, the objective of this study is to evaluate the effect on blood pressure of effervescent paracetamol compared to non-effervescent, in hypertensive patients. Methods/Design: This is the protocol of a phase IV multicenter clinical trial, randomized, controlled, crossover, open, which will compare the effect of two different formulations of paracetamol (effervescent or non-effervescent) in the blood pressure of hypertensive patients, with a seven weeks follow up. 49 controlled hypertensive patients will be included (clinical BP lower than 150 and 95 mmHg, and lower than 135 mmHg and 85 mmHg in patients with diabetes or a history of cardiovascular event, and daytime ambulatory measurements lower than 140 and 90 mmHg) and mild to moderate pain (Visual Analog Scale between 1 and 4). The study was approved by the ethics committee of the Fundacio Jordi Gol i Gurina and following standards of good clinical practice. The primary endpoint will be the variations in systolic BP in 24 h Ambulatory Blood Pressure Monitoring, considering significant differences 2 or more mmHg among those treated with non-effervescent and effervescent formulations. Intention-to-treat and per-protocol analysis will be held. Discussion: Despite the broad recommendation not to use effervescent drugs in patients with hypertension, there are relatively little studies that show exactly this pressor effect due to sodium in salt that gives the effervescence of the product. This is the first clinical trial designed to study the effect of effervescence compared to the non-effervescent, in well-controlled hypertensive patients with mild to moderate pain, performed in routine clinical practice

La Sierra Minera de Cartagena-La Unión presenta gran acumulación de residuos mineros que no han recibido ninguna estrategia de rehabilitación, suponiendo un riesgo de transferencia de metales y metaloides tóxicos a las zonas adyacentes, con peligro para la población y los ecosistemas. Para estabilizar dichos residuos y reducir la dispersión de contaminantes, se va a realizar un experimento de campo con la construcción de parcelas experimentales sobre un depósito de residuos mineros donde se van evaluar el efecto de la adición de biocarbón en la reducción de la disponibilidad de metales y metaloides y en el desarrollo de dos especies que se van a plantar con potencial fitoestabilizador. En las parcelas se plantarán dos especies vegetales donde en unas se añadirá biocarbón y en otras estiércol como fuente de carbono. Se realizará un monitoreo de la riqueza, biodiversidad y cobertura vegetal. Al cabo de tres meses se realizará un muestreo de suelo y planta para evaluar la dinámica de metales y metaloides y la creación de un suelo con estructura.
