Abstract

The sharing of human neuroimaging data has great potential to accelerate the development of imaging biomarkers in neurological and psychiatric disorders; however, major obstacles remain in terms of how and why to share data in the Open Science context. In this Health Policy by the European Cluster for Imaging Biomarkers, we outline the current main opportunities and challenges based on the results of an online survey disseminated among senior scientists in the field. Although the scientific community fully recognises the importance of data sharing, technical, legal, and motivational aspects often prevent active adoption. Therefore, we provide practical advice on how to overcome the technical barriers. We also call for a harmonised application of the General Data Protection Regulation across EU countries. Finally, we suggest the development of a system that makes data count by recognising the generation and sharing of data as a highly valuable contribution to the community.

Abstract

Introduction: Standardized cognitive assessment would enhance diagnostic reliability across memory clinics. An expert consensus adapted the Uniform Dataset (UDS)-3 for European centers, the clinician's UDS (cUDS). This study assessed its implementation acceptability and feasibility. Methods: We developed a survey investigating barriers, facilitators, and willingness to implement the cUDS. With a mixed-methods design, we analyzed data from academic memory clinics. Results: Seventy-eight percent of responding clinicians were experienced neuropsychologists/psychologists and 22% were medical specialists coming from 18 European countries. Sixty-five percent clinicians were willing to implement cUDS. General barriers related to implementation (43%) and clinical-methodological domains (21%). Favorable clinicians reported finances (15%) and digitalization (9%) as facilitating, but unavailability of local norms (23%) as hindering. Unfavorable clinicians reported logistical (23%) and time issues (18%). Discussion: Despite challenges, data showed moderate clinicians' acceptability and requirements to improve feasibility. Nonetheless, these results come from academic clinicians. The next steps will require feasibility evaluation in non-academic contexts.

Abstract

Objective: Syntax involves complex neurobiological mechanisms, which are difficult to disentangle for multiple reasons. Approach: Using a protocol able to separate syntactic information from sound information we investigated the neural causal connections evoked by the processing of homophonous phrases, i.e. with the same acoustic information but with different syntactic content. These could be either verb phrases (VP) or noun phrases (NP). We used event-related causality (ERC) from stereo-electroencephalographic (SEEG) recordings in 10 epileptic patients in multiple cortical and subcortical areas, including language areas and their homologous in the non-dominant hemisphere. The recordings were made while the subjects were listening to the homophonous phrases. Main results: We identified the different networks involved in the processing of these syntactic operations (faster in the dominant hemisphere) showing that VPs engage a wider cortical and subcortical network. We also present a proof-of-concept for the decoding of the syntactic category of a perceived phrase based on causality measures. Significance: Our findings help unravel the neural correlates of syntactic elaboration and show how a decoding based on multiple cortical and subcortical areas could contribute to the development of speech prostheses for speech impairment mitigation. .

79. FDG‐PET markers of heterogeneity and different risk of progression in amnestic MCI.

Abstract

INTRODUCTION: Amnestic mild cognitive impairment (aMCI) is emerging as a heterogeneous condition. METHODS: We looked at a cohort of N = 207 aMCI subjects, with baseline fluorodeoxyglucose positron emission tomography (FDG‐PET), T1 magnetic resonance imaging, cerebrospinal fluid (CSF), apolipoprotein E (APOE), and neuropsychological assessment. An algorithm based on FDG‐PET hypometabolism classified each subject into subtypes, then compared biomarker measures and clinical progression. RESULTS: Three subtypes emerged: hippocampal sparing–cortical hypometabolism, associated with younger age and the highest level of Alzheimer's disease (AD)‐CSF pathology; hippocampal/cortical hypometabolism, associated with a high percentage of APOE ε3/ε4 or ε4/ε4 carriers; medial–temporal hypometabolism, characterized by older age, the lowest AD‐CSF pathology, the most severe hippocampal atrophy, and a benign course. Within the whole cohort, the severity of temporo‐parietal hypometabolism, correlated with AD‐CSF pathology and marked the rate of progression of cognitive decline. DISCUSSION: FDG‐PET can distinguish clinically comparable aMCI at single‐subject level with different risk of progression to AD dementia or stability. The obtained results can be useful for the optimization of pharmacological trials and automated‐classification models. Abstract

Background and ObjectiveThis was a multicenter study aimed at investigating the characteristics of cognitive decline, neuropsychiatric symptoms, and brain imaging in individuals with subjective cognitive decline (SCD) and subtle cognitive decline (pre–mild cognitive impairment [pre–MCI]).MethodsData were obtained from the Network-AD project (NET-2011-02346784). The included participants underwent baseline cognitive and neurobehavioral evaluation, FDG-PET, and amyloid PET. We used principal component analysis (PCA) to identify independent neuropsychological and neuropsychiatric dimensions and their association with brain metabolism.ResultsA total of 105 participants (SCD = 49, pre–MCI = 56) were included. FDG-PET was normal in 45% of participants and revealed brain hypometabolism in 55%, with a frontal-like pattern as the most frequent finding (28%). Neuropsychiatric symptoms emerging from the Neuropsychiatric Inventory and the Starkstein Apathy Scale were highly prevalent in the whole sample (78%). An abnormal amyloid load was detected in the 18% of the participants who underwent amyloid PET (n = 60). PCA resulted in 3 neuropsychological factors: (1) executive/visuomotor, correlating with hypometabolism in frontal and occipital cortices and basal ganglia; (2) memory, correlating with hypometabolism in temporoparietal regions; and (3) visuospatial/constructional, correlating with hypometabolism in frontoparietal cortices. Two factors emerged from the neuropsychiatric PCA: (1) affective, correlating with hypometabolism in orbitofrontal and cingulate cortex and insula; (2) hyperactive/psychotic, correlating with hypometabolism in frontal, temporal, and parietal regions.DiscussionFDG-PET evidence suggests either normal brain function or different patterns of brain hypometabolism in SCD and pre–MCI. These results indicate that SCD and pre–MCI represent heterogeneous populations. Different neuropsychological and neuropsychiatric profiles emerged, which correlated with neuronal dysfunction in specific brain regions. Long-term follow-up studies are needed to assess the risk of progression to dementia in these conditions.

92. Metacognition as a tool to differentiate cognitively unimpaired from impaired subjects: judgments of learning in prospective memory task.

Abstract

Background: Metacognition may give useful insight in early stages of Alzheimer's disease (AD) continuum. Recent studies suggest differences in the ability of judging their own cognitive performance along clinical stages. For instance, subjects with mild cognitive impairment (MCI) overestimate, whereas people with subjective cognitive decline (SCD) tend to underestimate their cognitive performance. One cognitive function compromised in prodromal AD is prospective memory (PM), which refers to the ability to remember something that has to be done. However, the metacognitive abilities in PM tasks are still not clear when comparing individuals with varying degrees of cognitive impairment. Method: Our study included 94 subjects from the Geneva Memory Center which are cognitively unimpaired (CU; n = 53), SCD (n = 26), and MCI (n = 15). All subjects performed a PM task with judgements of learning (JOL) to evaluate their metacognition. For that, participants were instructed to memorize a list of pairs of words and to press a specific key if the first word of the pair appeared in a posterior lexical decision task. A one‐way ANOVA tested the differences in PM performance and corresponding JOLs among the three clinical groups. Lastly, the association of PM metacognition with subjective complaints and anxiety levels was tested using Pearson correlations. Result: The PM ability was significantly different among clinical groups (F = 10.5, p<0.001). Post‐hoc analysis revealed that MCI showed significantly lower PM in comparison with CU (p<0.001) and SCD (p = 0.003). Moreover, the JOL of PM also differed among the three groups (F = 4.5, p = 0.02), namely the MCI group showed lower JOLs in comparison with CU (p = 0.04). Likewise, correlations suggested a significant association between PM‐JOLs and subjective complaints (self‐other; R = 0.33, p = 0.006), whereas the association with anxiety was not statistically significant (p>0.05). Conclusion: Our study suggested differences in the PM and its JOL between MCI and CU/SCD. However, our study was not able to differentiate between the CU and SCD groups at the PM and the corresponding JOL, even though their difference was significantly correlated with subjective functioning.
