Purpose: The first clinical biology-guided radiation therapy (BgRT) system-RefleXionTM X1-was installed and commissioned for clinical use at our institution. This study aimed at evaluating the treatment plan quality and delivery efficiency for IMRT/SBRT cases without PET guidance. Methods: A total of 42 patient plans across 6 cancer sites (conventionally fractionated lung, head, and neck, anus, prostate, brain, and lung SBRT) planned with the EclipseTM treatment planning system (TPS) and treated with either a TrueBeam® or Trilogy® were selected for this retrospective study. For each Eclipse VMAT plan, 2 corresponding plans were generated on the X1 TPS with 10 mm jaws (X1-10mm) and 20 mm jaws (X1-20mm) using our institutional planning constraints. All clinically relevant metrics in this study, including PTV D95%, PTV D2%, Conformity Index (CI), R50, organs-at-risk (OAR) constraints, and beam-on time were analyzed and compared between 126 VMAT and RefleXion plans using paired t-tests. Results: All but 3 planning metrics were either equivalent or superior for the X1-10mm plans as compared to the Eclipse VMAT plans across all planning sites investigated. The Eclipse VMAT and X1-10mm plans generally achieved superior plan quality and sharper dose fall-off superior/inferior to targets as compared to the X1-20mm plans, however, the X1-20mm plans were still considered acceptable for treatment. On average, the required beam-on time increased by a factor of 1.6 across all sites for X1-10mm compared to X1-20mm plans. Conclusions: Clinically acceptable IMRT/SBRT treatment plans were generated with the X1 TPS for both the 10 mm and 20 mm jaw settings.

Deep learning affords enormous opportunities to augment the armamentarium of biomedical imaging, albeit its design and implementation have potential flaws. Fundamentally, most deep learning models are driven entirely by data without consideration of any prior knowledge, which dramatically increases the complexity of neural networks and limits the application scope and model generalizability. Here we establish a geometry-informed deep learning framework for ultra-sparse 3D tomographic image reconstruction. We introduce a novel mechanism for integrating geometric priors of the imaging system. We demonstrate that the seamless inclusion of known priors is essential to enhance the performance of 3D volumetric computed tomography imaging with ultra-sparse sampling. The study opens new avenues for data-driven biomedical imaging and promises to provide substantially improved imaging tools for various clinical imaging and image-guided interventions.

Extra virgin olive oil (EVOO) is a cornerstone of the Mediterranean diet. Many studies have highlighted its crucial preventive role against cardiovascular disease, neurodegenerative disorders, metabolic syndrome and cancer, with these effects being due to the synergistic anti-inflammatory and antioxidant activities of minor components, such as polyphenols and tocols. The aim of the present study is to implement new technologies for olive oil mills and develop an efficient large-sized industrial process for the continuous extraction of healthier EVOOs that are enriched with these bioactive compounds. Non-thermal technologies, namely ultrasound (US) and pulsed electric field (PEF), have been tested, separately and in combination, to eliminate the need for traditional malaxation. There is extensive literature to support the efficacy of ultrasound-assisted extraction (UAE) and PEF treatments in EVOO production. A newly designed US device and a PEF industrial chamber have been combined into a single, integrated continuous-flow setup, the performance of which in the extraction of EVOO from green Coratina olives has been evaluated herein. Extraction yields, physico-chemical and organoleptic characteristics, and polyphenol and tocol contents were monitored throughout the trials, and the last three were measured at accelerated aging times (AAT) of 15 and 30 days. The US and combined US-PEF processes not only increased daily oil production (ton/day, by nearly 45%), but also eliminated the need for kneading during malaxation, resulting in significant energy savings (approximately 35%). In addition, these innovations enriched the resulting EVOO with nutritionally relevant minor components (8–12% polyphenols, 3–5% tocols), thereby elevating its quality and market value, as well as overall stability. The introduction of continuous-flow US and PEF technologies is a remarkable innovation for the EVOO industry, as they offer benefits to both producers and consumers. The EVOO resulting from non-thermal continuous-flow production meets the growing demand for healthier, nutrient-enriched products.

Abstract: Central Argentina from the Pampean flat‐slab segment to northern Patagonia (27°–41°S) represents a classic example of a broken retroarc basin with strong tectonic and climatic control on fluvial sediment transport. Combined with previous research focused on coastal sediments, this actualistic provenance study uses framework petrography and heavy‐mineral data to trace multistep dispersal of volcaniclastic detritus first eastwards across central Argentina for up to ca. 1,500 km and next northwards for another 760 km along the Atlantic coast. Although detritus generated in the Andes is largely derived from mesosilicic volcanic rocks of the cordillera, its compositional signatures reflect different tectono‐stratigraphic levels of the orogen uplifted along strike in response to varying subduction geometry as well as different character and crystallization condition of arc magmas through time and space. River sand, thus, changes from feldspatho‐litho‐quartzose or litho‐feldspatho‐quartzose in the north, where sedimentary detritus is more common, to mostly quartzo‐feldspatho‐lithic in the centre and to feldspatho‐lithic in the south, where volcanic detritus is dominant. The transparent‐heavy‐mineral suite changes markedly from amphibole ≫ clinopyroxene > orthopyroxene in the north, to amphibole ≈ clinopyroxene ≈ orthopyroxene in the centre and to orthopyroxene ≥ clinopyroxene ≫ amphibole in the south. In the presently dry climate, fluvial discharge is drastically reduced to the point that even the Desaguadero trunk river has become endorheic and orogenic detritus is dumped in the retroarc basin, reworked by winds and temporarily accumulated in dune fields. During the Quaternary, instead, much larger amounts of water were released by melting of the Cordilleran ice sheet or during pluvial events. The sediment‐laden waters of the Desaguadero and Colorado rivers then rushed from the tract of the Andes with greatest topographic and structural elevation, fostering alluvial fans inland and flowing in much larger valleys than today towards the Atlantic Ocean. Sand and gravel supply to the coast was high enough not only to promote rapid progradation of large deltaic lobes but also to feed a cell of littoral sediment transport extending as far north as the Río de la Plata estuary.

We conducted a preregistered multilaboratory project (k = 36; N = 3,531) to assess the size and robustness of ego-depletion effects using a novel replication method, termed the paradigmatic replication approach. Each laboratory implemented one of two procedures that was intended to manipulate self-control and tested performance on a subsequent measure of self-control. Confirmatory tests found a nonsignificant result (d = 0.06). Confirmatory Bayesian meta-analyses using an informed-prior hypothesis (δ = 0.30, SD = 0.15) found that the data were 4 times more likely under the null than the alternative hypothesis. Hence, preregistered analyses did not find evidence for a depletion effect. Exploratory analyses on the full sample (i.e., ignoring exclusion criteria) found a statistically significant effect (d = 0.08); Bayesian analyses showed that the data were about equally likely under the null and informed-prior hypotheses. Exploratory moderator tests suggested that the depletion effect was larger for participants who reported more fatigue but was not moderated by trait self-control, willpower beliefs, or action orientation.

68. Brain Remain: Shifting the Paradigm of Pride

Volumes of research studies, surveys, and census data document the "brain drain," the phenomenon of highly educated and highly skilled workers migrating from their hometown to an urban or metropolitan area that promises a better life. Early indications of brain drain begin with high school graduates determining where to attend college. There is a pervasive belief that it is a measure of success and part of a process of upward mobility to go away to college, and therefore an implied failure if one remains in their hometown or region while earning a degree. This mentality and encouraged brain drain behavior is reinforced by a K-12 education system that sorts students early in their academic careers and invests in the best and brightest, while paying little attention to the majority of students (Harmon, 2010). This is a skewed approach and leaves many individuals, and their hometowns, with few opportunities and stifled upward social and economic mobility. The needs of the workforce are rapidly changing. Some form of post-secondary education is required for the majority of entry-level jobs in the United States (Bureau of Labor Statistics, 2017). Yet, higher education is being faced with increasing competition and growing skepticism regarding the value of a college degree. By collaborating and partnering with the region in which an institution exists, urban, suburban, or rural, colleges and universities can cut through higher education's competitive noise and growing skepticism, while addressing the changing workforce needs and redefining students' pride about where they earn a degree, and begin their careers, close to home. We call this paradigm shift the "Brain Remain," and it has the potential to significantly alter the way higher education, K-12 districts, businesses, and community leaders work, operate, and collaborate in the new economy. This, in turn, can create bold new opportunities for students where they least expect them: right in their own backyards.

Many students want to use cheat sheets, or crib notes, on exams. Whether or not those aids actually help them has not been carefully studied. This paper measures 16 students' notes by scoring the writing density as well as the number of definitions, examples, and mistakes. To consider the effectiveness of the notes, they are matched against exam solutions. This closer look at the content and application of the online students' notes revealed that they were neither well made nor well used.

Abstract Mitochondrial dysfunction has been suggested to contribute to Parkinson’s disease pathogenesis, though an understanding of the extent or exact mechanism of this contribution remains elusive. This has been complicated by challenging nature of pathway-based analysis and an inability simultaneously study multiple related proteins within human brain tissue. We used imaging mass cytometry (IMC) to overcome these challenges, measuring multiple protein targets, whilst retaining the spatial relationship between targets in post-mortem midbrain sections. We used IMC to simultaneously interrogate subunits of the mitochondrial oxidative phosphorylation complexes, and several key signalling pathways important for mitochondrial homoeostasis, in a large cohort of PD patient and control cases. We revealed a generalised and synergistic reduction in mitochondrial quality control proteins in dopaminergic neurons from Parkinson’s patients. Further, protein-protein abundance relationships appeared significantly different between PD and disease control tissue. Our data showed a significant reduction in the abundance of PINK1, Parkin and phosphorylated ubiquitinSer65, integral to the mitophagy machinery; two mitochondrial chaperones, HSP60 and PHB1; and regulators of mitochondrial protein synthesis and the unfolded protein response, SIRT3 and TFAM. Further, SIRT3 and PINK1 did not show an adaptive response to an ATP synthase defect in the Parkinson’s neurons. We also observed intraneuronal aggregates of phosphorylated ubiquitinSer65, alongside increased abundance of mitochondrial proteases, LONP1 and HTRA2, within the Parkinson’s neurons with Lewy body pathology, compared to those without. Taken together, these findings suggest an inability to turnover mitochondria and maintain mitochondrial proteostasis in Parkinson’s neurons. This may exacerbate the impact of oxidative phosphorylation defects and ageing related oxidative stress, leading to neuronal degeneration. Our data also suggest that that Lewy pathology may affect mitochondrial quality control regulation through the disturbance of mitophagy and intramitochondrial proteostasis.

In recent years, inexpensive and easy to use robotics platforms have been incorporated into middle school, high school, and college educational curricula and competitions all over the world. Students have access to advanced microprocessors and sensor systems that engage, educate, and encourage their creativity. In this study, the capabilities of the widely available VEX Robotics System are extended using the wireless ESP-NOW protocol to allow for real-time data logging and to extend the computational capabilities of the system. Specifically, this study presents an open source system that interfaces a VEX V5 microprocessor, an OpenMV camera, and a computer. Images from OpenMV are sent to a computer where object detection algorithms can be run and instructions sent to the VEX V5 microprocessor while system data and sensor readings are sent from the VEX V5 microprocessor to the computer. System performance was evaluated as a function of distance between transmitter and receiver, data packet round trip timing, and object detection using YoloV8. Three sample applications are detailed including the evaluation of a vision-based object sorting machine, a drivetrain trajectory analysis, and a proportional-integral-derivative (PID) control algorithm tuning experiment. It was concluded that the system is well suited for real time object detection tasks and could play an important role in improving robotics education.

BackgroundThere is a gross shortage of sleep specialist providers within the military health system. Telehealth and mobile health represent promising approaches to increase access to high quality, cost-effective care in the U.S military.ObjectivesThis paper reports findings from a mixed-methods clinical implementation study of a novel sleep telehealth platform at two military treatment facilities in the National Capitol Region. The platform includes a mobile app and integrated wearable sensors (i.e., a commercial off-the-shelf sleep tracker [Fitbit]). The primary purpose was to evaluate the implementation of a 10-day remote monitoring assessment and provision of evidence-based sleep treatment recommendations to patients and providers. In addition, we sought to observe, in an exploratory manner, subsequent engagement with the app during 5 days of personalized sleep education and training.MethodsPatients with sleep problems completed an intensive 10-day remote monitoring assessment that included a baseline intake questionnaire, daily sleep diaries, twice daily symptom surveys, and Fitbit. Based on this assessment, patients received personalized assessment results. Concurrently, a provider report was generated that included provisional diagnoses and evidence-based treatment recommendations. Next, participants gained access to personalized sleep education and trainings within the mobile app. Within an established implementation science framework, outcomes were assessed via behavioral adherence (engagement with the app) and separate questionnaires for patients and providers. Last, we conducted four focus groups with patients and 12 key informant interviews with primary care managers (PCMs) and economic stakeholders to seek feedback and recommendations for future directions.ResultsTwo hundred and seventy patients participated in the study. Using validated research questionnaires, participants reported high-risk for obstructive sleep apnea (65.6%), moderate to severe insomnia (38.2%), and moderate to severe daytime sleepiness (38.5%), and moderate to severe anxiety (14.1%) and depressive (20.4%) symptoms. Total sleep time was 6.6 (SD = 1.8) h based on sleep diaries and 6.1 (SD = 1.8) h based on Fitbit. Regarding implementation, reach, effectiveness, adoption, implementation, and maintenance were all notably high, based on quantitative and qualitative data from participants and PCMs.ConclusionsSleep telehealth and mobile health represent promising approaches to increase access to cost-effective, evidence-based care for sleep disorders in the U.S. military.

We propose an algorithm to select parameter subset combinations that can be estimated using an ordinary least-squares (OLS) inverse problem formulation with a given data set. First, the algorithm selects the parameter combinations that correspond to sensitivity matrices with full rank. Second, the algorithm involves uncertainty quantification by using the inverse of the Fisher Information Matrix. Nominal values of parameters are used to construct synthetic data sets, and explore the effects of removing certain parameters from those to be estimated using OLS procedures. We quantify these effects in a score for a vector parameter defined using the norm of the vector of standard errors for components of estimates divided by the estimates. In some cases the method leads to reduction of the standard error for a parameter to less than 1\% of the estimate.

Abstract: The spatial and temporal distribution of arc magmatism and associated isotopic variations provide insights into the Phanerozoic history of the western margin of South America during major shifts in Andean and pre-Andean plate interactions. We integrated detrital zircon U-Th-Pb and Hf isotopic results across continental magmatic arc systems of Chile and western Argentina (28°S–33°S) with igneous bedrock geochronologic and zircon Hf isotope results to define isotopic signatures linked to changes in continental margin processes. Key tectonic phases included: Paleozoic terrane accretion and Carboniferous subduction initiation during Gondwanide orogenesis, Permian–Triassic extensional collapse, Jurassic–Paleogene continental arc magmatism, and Neogene flat slab subduction during Andean shortening. The ~550 m.y. record of magmatic activity records spatial trends in magma composition associated with terrane boundaries. East of 69°W, radiogenic isotopic signatures indicate reworked continental lithosphere with enriched (evolved) εHf values and low (0.7) zircon Th/U values consistent with increased asthenospheric contributions during lithospheric thinning. Spatial constraints on Mesozoic to Cenozoic arc width provide a rough approximation of relative subduction angle, such that an increase in arc width reflects shallower slab dip. Comparisons among slab dip calculations with time-averaged εHf and Th/U zircon results exhibit a clear trend of decreasing (enriched) magma compositions with increasing arc width and decreasing slab dip. Collectively, these data sets demonstrate the influence of subduction angle on the position of upper-plate magmatism (including inboard arc advance and outboard arc retreat), changes in isotopic signatures, and overall composition of crustal and mantle material along the western edge of South America.

IntroductionA growing number of Canadian studies have examined the link between sleep and mental health. This research builds upon that work by investigating associations of sleep duration and quality with positive mental health (PMH) and mental illness and suicidal ideation (MI/SI) outcomes among youth and adults from three Canadian provinces (i.e. Ontario, Manitoba and Saskatchewan). Methods Using cross-sectional data from respondents 12 years and older (n = 18 683) who were asked questions on their sleep in the 2015 Canadian Community Health Survey – Annual Component, we conducted unadjusted and adjusted logistic regressions with self-reported measures of sleep duration and sleep quality as independent variables and a range of PMH (e.g. high self-rated mental health) and MI/SI indicators (e.g. mood disorder diagnosis) as dependent variables. Analyses were conducted of all complete cases and also stratified by sex and age group. Results Good sleep quality was associated with higher odds of PMH indicators (adjusted odds ratio [aOR]: 1.52–4.24) and lower odds of MI/SI indicators (aOR: 0.23– 0.47); associations remained significant when analyses were stratified. Meeting sleep duration recommendations was positively associated with PMH indicators (aOR: 1.27– 1.56) and negatively associated with MI/SI indicators (aOR: 0.41–0.80), but some associations did not remain significant when stratified. Conclusion This study provides support for associations between sleep duration and quality and indicators of PMH and MI/SI. Findings can inform future research and surveillance efforts that monitor sleep behaviours and indicators of PMH and MI/SI.

MethodsÀ l’aide de données transversales recueillies auprès de répondants de 12 ans et plus (n = 18 683) ayant répondu à des questions au sujet de leur sommeil dans l’Enquête sur la santé dans les collectivités canadiennes de 2015 – Composante annuelle, nous avons réalisé des analyses de régression logistique non ajustées et des analyses de régression logistique ajustées en fonction des mesures autodéclarées sur la durée et la qualité du sommeil sous forme de variables indépendantes et d’un éventail d’indicateurs d’une santé mentale positive (par ex. une bonne santé mentale autoévaluée) et de maladie mentale ou d’idées suicidaires (par ex. un diagnostic de trouble de l’humeur) sous forme de variables dépendantes. Les analyses ont été réalisées au moyen des données des répondants ayant répondu à toutes les questions et elles ont été stratifiées selon le sexe et le groupe d’âge. ResultsUne bonne qualité de sommeil était associée à une probabilité plus élevée d’indicateurs d’une santé mentale positive (rapport de cotes ajusté [RCa] : 1,52 à 4,24) et à une probabilité plus faible d’indicateurs de maladie mentale et d’idées suicidaires (RCa : 0,23 à 0,47), et les associations sont demeurées significatives lorsque les analyses ont été stratifiées. Le respect des recommandations sur la durée du sommeil était associé positivement aux indicateurs d’une santé mentale positive (RCa : 1,27 à 1,56) et était associé négativement aux indicateurs de maladie mentale et d’idées suicidaires (RCa : 0,41 à 0,80), mais certaines associations ne sont pas demeurées significatives après la stratification. However, less is known about how these experiences are independently associated with positive mental health (PMH) during the COVID-19 pandemic. MethodsWe analyzed data from the 2020 and 2021 cycles of the Survey on COVID-19 and Mental Health to provide estimates of social isolation (i.e. living alone), loneliness and PMH outcomes (i.e. high self-rated mental health, high community belonging, mean life satisfaction) in the overall older adult population (i.e. 65+ years) and across sociodemographic groups. We also conducted logistic and linear regressions to separately and simultaneously examine how social isolation and loneliness are associated with PMH. ResultsNearly 3 in 10 older adults reported living alone, and over a third reported feelings of loneliness due to the pandemic. When examined separately, living alone and loneliness were each associated with lower PMH. When assessed simultaneously, loneliness remained a significant independent factor associated with all three PMH outcomes (overall and across all sociodemographic groups), but living alone was only a significant factor for high community belonging in the overall population, for males and for those aged 65 to 74 years. ConclusionOverall, social isolation and loneliness were associated with poorer wellbeing among older adults in Canada during the pandemic. Loneliness remained a significant factor related to all PMH outcomes after adjusting for social isolation, but not vice versa. The findings highlight the need to appropriately identify and support lonely older adults during (and beyond) the pandemic.

MéthodologieNous avons analysé les données des cycles de 2020 et de 2021 de l’Enquête sur la COVID-19 et la santé mentale pour être en mesure d’estimer le degré d’isolement social (le fait de vivre seul), la solitude et divers indicateurs d’une santé mentale positive (bonne santé mentale autoévaluée, fort sentiment d’appartenance à la communauté locale et satisfaction moyenne à l’égard de la vie) chez les aînés en général (65 ans ou plus) et en fonction de divers groupes sociodémographiques. Nous avons également effectué des régressions logistiques et linéaires pour analyser séparément et conjointement la façon dont l’isolement social et la solitude sont associés à une santé mentale positive. RésultatsPrès de 3 aînés sur 10 ont déclaré vivre seuls, et plus du tiers ont déclaré éprouver un sentiment de solitude en raison de la pandémie. Analysés séparément, le fait de vivre seul et le sentiment de solitude ont été associés à une santé mentale positive plus faible. Dans l’analyse conjointe, le sentiment de solitude est demeuré un important facteur indépendant associé aux trois indicateurs de santé mentale positive (globalement et dans tous les groupes sociodémographiques), mais le fait de vivre seul ne s’est révélé un facteur important que dans le cas d’un fort sentiment d’appartenance à la communauté locale dans la population globale, pour les hommes et pour les 65 à 74 ans. 86. Diagnosed or prescribed only? A national analysis of initial evaluation and management of insomnia among older adult Medicare beneficiaries.

STUDY OBJECTIVES: To describe initial insomnia-related encounters among a national sample of Medicare beneficiaries, and to identify older adults at risk for potentially inappropriate prescription insomnia medication usage. METHODS: Our data source was a random 5% sample of Medicare administrative claims data (2006-2013). Insomnia was operationalized as International Classification of Disease, Ninth Revision, Clinical Modification diagnostic codes. Insomnia medications included FDA-approved insomnia-related medication classes and drugs. Logistic regression was employed to identify predictors of being prescribed only (i.e., being prescribed an insomnia medication without a corresponding insomnia diagnosis). RESULTS: A total of N = 60 362 beneficiaries received either an insomnia diagnosis or a prescription for an insomnia medication as their first sleep-related encounter during the study period. Of these, 55.1% (n = 33 245) were prescribed only, whereas 44.9% (n = 27 117) received a concurrent insomnia diagnosis. In a fully adjusted regression model, younger age (odds ratio (OR) 0.98; 95% confidence interval (CI) 0.98, 0.99), male sex (OR 1.15; 95% CI 1.11, 1.20), and several comorbid conditions (i.e., dementia [OR 1.21; 95% CI 1.15, 1.27] and anemia [OR 1.17; 95% CI 1.13, 1.22]) were positively associated with being prescribed only. Conversely, black individuals (OR 0.83; 95% CI 0.78, 0.89) and those of other race (OR 0.89; 95% CI 0.84, 0.94) were less likely to be prescribed only. Individuals who received care from a board-certified sleep medicine provider (BCSMP) were less likely to be prescribed only (OR 0.27; 95% CI 0.16, 0.46). CONCLUSIONS: Fewer than half of Medicare beneficiaries prescribed insomnia medications ever received a formal sleep-related diagnosis.

- Abstract
Student retention is important to any university, especially keeping commuter students who are traditionally less anchored in campus life. Even more at risk, given the leaky STEM pipeline, are STEM commuter students. In 2016, Valparaiso University launched the Establishing Practices Integrating Commuter Students (EPIC) program, centered around engaging students in undergraduate research. Students participate in a research laboratory for their four academic years, and take part in one summer of funded research. This program has achieved its goal of providing scholarships, research opportunities, and cohort support to over 30 commuter and residential students while preparing them for research-oriented careers. This article shares successes and lessons learned, along with data demonstrating the program's impact on broadening participation in STEM and increasing retention.

BackgroundSleep problems are common and costly in the US military. Yet, within the military health system, there is a gross shortage of trained specialist providers to address sleep problems. As a result, demand for sleep medicine care far exceeds the available supply. Telehealth including telemedicine, mobile health, and wearables represents promising approaches to increase access to high-quality and cost-effective care. ObjectiveThe purpose of this study was to evaluate patient engagement and provider perceived effectiveness of a novel sleep telehealth platform and remote monitoring assessment in the US military. The platform includes a desktop web portal, native mobile app, and integrated wearable sensors (ie, a commercial off-the-shelf sleep tracker [Fitbit]). The goal of the remote monitoring assessment was to provide evidence-based sleep treatment recommendations to patients and providers. MethodsPatients with sleep problems were recruited from the Internal Medicine clinic at Walter Reed National Military Medical Center. Patients completed intensive remote monitoring assessments over 10 days (including a baseline intake questionnaire, daily sleep diaries, and 2 daily symptom surveys), and wore a Fitbit sleep tracker. Following the remote monitoring period, patients received assessment results and personalized sleep education in the mobile app. In parallel, providers received a provisional patient assessment report in an editable electronic document format. Patient engagement was assessed via behavioral adherence metrics that were determined a priori. Patients also completed a brief survey regarding ease of completion. Provider effectiveness was assessed via an anonymous survey. ResultsIn total, 35 patients with sleep problems participated in the study. There were no dropouts. Results indicated a high level of engagement with the sleep telehealth platform, with all participants having completed the baseline remote assessment, reviewed their personalized sleep assessment report, and completed the satisfaction survey. Patients completed 95.1% of sleep diaries and 95.3% of symptom surveys over 10 days. Patients reported high levels of satisfaction with most aspects of the remote monitoring assessment. In total, 24 primary care providers also participated and completed the anonymous survey. The results indicate high levels of perceived effectiveness and identified important potential benefits from adopting a sleep telehealth approach throughout the US military health care system. ConclusionsMilitary patients with sleep problems and military primary care providers demonstrated high levels of engagement and satisfaction with a novel sleep telehealth platform and remote monitoring assessment. Sleep telehealth approaches represent a potential pathway to increase access to evidence-based sleep medicine care in the US military. Further evaluation is warranted.

The dual pathway hypothesis of risk for substance use was tested by examining risk from symptoms of conduct problems and depressive symptoms in adolescence (from ages 10-11 to 17-18 years) to substance use—including tobacco, alcohol, cannabis, and other illicit drugs—in both early adulthood (approximately from ages 20 to 29 years) and middle adulthood (approximately from ages 29 to 38 years). Hypotheses were tested on a sample of boys who were at risk for conduct problems by virtue of the neighborhoods where they lived in childhood (the Oregon Youth Study; N = 206 at Wave 1). Dual-trajectory modeling (Latent Class Analysis) resulted in a 3-group solution of high, moderate, and low co-occurring symptoms. The latent class of boys with co-occurring symptoms in adolescence showed higher levels of substance use in adulthood; namely, higher levels of cannabis and illicit substance use during early adulthood compared to either of the moderate or low symptom classes, and higher use of cannabis in midadulthood than the low symptom class. Those with co-occurring symptoms also showed, overall, higher vulnerability to use of tobacco in these 2 periods, but not to higher use of alcohol. Regression analyses indicated that the higher substance use of the co-occur group of men was related to their adolescent conduct problems, but was not related to their adolescent depressive symptoms; however, these associations were nonsignificant when adolescent use of the respective substances were included in the models. Thus, the dual-trajectory hypothesis was not supported. However, the findings indicated that, as assessed in the present study, the psychopathology symptoms of boys with conduct problems in adolescence who show risk for later substance use may be complex, involving depressive symptoms.

Purpose: Segmentation of organs-at-risk (OARs) is a bottleneck in current radiation oncology pipelines and is often time consuming and labor intensive. In this paper, we propose an atlas-based semi-supervised registration algorithm to generate accurate segmentations of OARs for which there are ground truth contours and rough segmentations of all other OARs in the atlas. To the best of our knowledge, this is the first study to use learning-based registration methods for the segmentation of head and neck patients and demonstrate its utility in clinical applications. Methods: Our algorithm cascades rigid and deformable deformation blocks, and takes on an atlas image (M), set of atlas-space segmentations (S_A), and a patient image (F) as inputs, while outputting patient-space segmentations of all OARs defined on the atlas. We train our model on 475 CT images taken from public archives and Stanford RadOnc Clinic (SROC), validate on 5 CT images from SROC, and test our model on 20 CT images from SROC. Results: Our method outperforms current state of the art learning-based registration algorithms and achieves an overall dice score of 0.789 on our test set. Moreover, our method yields a performance comparable to manual segmentation and supervised segmentation, while solving a much more complex registration problem. Whereas supervised segmentation methods only automate the segmentation process for a select few number of OARs, we demonstrate that our methods can achieve similar performance for OARs of interest, while also providing segmentations for every other OAR on the provided atlas. Conclusions: Our proposed algorithm has significant clinical applications and could help reduce the bottleneck for segmentation of head and neck OARs. Further, our results demonstrate that semi-supervised diffeomorphic registration can be accurately applied to both registration and segmentation problems.

PurposeRadiation dose delivered to targets located near the upper abdomen or thorax are significantly affected by respiratory motion, necessitating large margins, limiting dose escalation. Surrogate motion management devices, such as the Real-time Position Management (RPM™) system (Varian Medical Systems, Palo Alto, CA), are commonly used to improve normal tissue sparing. Alternative to current solutions, we have developed and evaluated the feasibility of a real-time position management system that leverages the motion data from the onboard hardware of Apple iOS devices to provide patients with visual coaching with the potential to improve the reproducibility of breathing as well as improve patient compliance and reduce treatment delivery time.Methods and materialsThe iOS application, coined the Instant Respiratory Feedback (IRF) system, was developed in Swift (Apple Inc., Cupertino, CA) using the Core-Motion library and implemented on an Apple iPhone® devices. Operation requires an iPhone®, a three-dimensional printed arm, and a radiolucent projector screen system for feedback. Direct comparison between IRF, which leverages sensor fusion data from the iPhone®, and RPM™, an optical-based system, was performed on multiple respiratory motion phantoms and volunteers. The IRF system and RPM™ camera tracking marker were placed on the same location allowing for simultaneous data acquisition. The IRF surrogate measurement of displacement was compared to the signal trace acquired using RPM™ with univariate linear regressions and Bland-Altman analysis.ResultsPeriodic motion shows excellent agreement between both systems, and subject motion shows good agreement during regular and irregular breathing motion. For sparse sampling that accelerates magnetic resonance (MR) image acquisition, non-linear reconstruction algorithms have been developed, which incorporated patient specific a prior information. More generic a prior information could be acquired via deep learning and utilized for image reconstruction. In this study, we developed a volumetric hierarchical deep residual convolutional neural network, referred to as T-Net, to provide a data-driven end-to-end mapping from sparsely sampled MR images to fully sampled MR images, where cartilage MR images were acquired using an Ultra-short TE sequence and retrospectively undersampled using pseudo-random Cartesian and radial acquisition schemes. The network had a hierarchical architecture that promoted the sparsity of feature maps and increased the receptive field, which were valuable for signal synthesis and artifact suppression. Relatively dense local connections and global shortcuts were established to facilitate residual learning and compensate for details lost in hierarchical processing. Additionally, volumetric processing was adopted to fully exploit spatial continuity in three-dimensional space. Data consistency was further enforced. The network was trained with 336 three-dimensional images (each consisting of 32 slices) and tested by 24 images. The incorporation of a priori information acquired via deep learning facilitated high acceleration factors (as high as 8) while maintaining high image fidelity (quantitatively evaluated using the structural similarity index measurement). The proposed T-Net had an improved performance as compared to several state-of-the-art networks.

The impact of traumatic brain injury (TBI) severity and loss of consciousness (LOC) on the development of neuropsychiatric symptoms was studied in injured service members (SMs; n?=?1278) evacuated from combat settings between 2003 and 2012. TBI diagnoses of mild TBI (mTBI) or moderate-to-severe TBI (MS-TBI) along with LOC status were identified using International Classification of Diseases, Ninth Revision (ICD-9) codes and the Defense and Veterans Brain Injury Center Standard Surveillance Case Definition for TBI. Self-reported psychiatric symptoms were evaluated for post-traumatic stress disorder (PTSD) with the PTSD Checklist, Civilian Version for PTSD, the Patient Health Questionnaire-9 for major depressive disorder (MDD), and the Patient Health Questionnaire-15 for somatic symptom disorder (SSD) in two time periods post-injury: Assessment Period 1 (AP1, 0.0?2.5 months) and Assessment Period 2 (AP2, 3?12 months). mTBI, but not MS-TBI, was associated with increased neuropsychiatric symptoms: PTSD in AP1 and AP2; MDD in AP1; and SSD in AP2. A subgroup analysis of mTBI with and without LOC revealed that mTBI with LOC, but not mTBI without LOC, was associated with increased symptoms as compared to non-TBI: PTSD in AP1 and AP2; MDD in AP1; and SSD in AP1 and AP2. Moreover, mTBI with LOC was associated with increased MDD symptoms in AP2, and SSD symptoms in AP1 and AP2, compared to mTBI without LOC. These findings reinforce the need for the accurate characterization of TBI severity and a multi-disciplinary approach to address the devastating impacts of TBI in injured SMs.

The EU has planned the phase-out of new vehicles based on internal combustion engines in favor of high-efficiency battery electric vehicles (BEV) by 2035 (Fit for 55 package). However, many doubts remain about the effectiveness of this choice for each country of the Union in terms of CO2 emissions reduction, as each State is characterized by a different carbon intensity related to the production of electricity needed to manufacture and recharge vehicles. This study seeks to explore the Italian case. To this aim, carbon intensities related to electricity production were calculated considering both the Italian electricity mix production in 2022 and those envisaged in 2035, considering two energy scenarios based on different introductions of renewable energy sources (RES). Afterward, the values obtained were adopted for determining the CO2 emissions related to the whole production process of battery systems in Italy (emissions from mining and refining, scrap materials, and final assembly included) by comparing some of the most up-to-date Life-Cycle Assessment (LCA) analyses related to the manufacturing cycle of the batteries. Finally, the results were adopted to calculate the starting carbon debit for A, B, C, and M car segments for Mild Hybrid, Full Hybrid, and Full Electric powertrains. At the same time, statistical road fuel/electricity consumption data were collected and overall CO2 emissions were calculated for the same vehicles adopting a dynamic approach and plotted for a defined distance, so as to determine break-even points with respect to the cumulative (i.e., from battery and road) carbon emissions. The results showed that advantages related to electric vehicles are significant only if a low carbon intensity related to electricity production is reached by means of a very high introduction of RES, thus keeping the door open for innovative hybrid powertrain technologies, if fed with low carbon fuels.
