A systematic review and meta-analysis of cross-sectional and prospective cohort studies was conducted to assess the associations between total dairy consumption and its different subtypes with the prevalence and incidence of overweight, obesity, and overweight/obesity in children and adolescents. A literature search was conducted in Medline through PUBMED and Cochrane Library databases until October 18, 2021. Articles reporting the risk estimates as odd ratios (OR), risk ratios (RR), or hazard ratios and their corresponding 95% confidence interval (CI) for the association between dairy product consumption and the risk of overweight and/or obesity were included. In the meta-analysis from cross-sectional studies, results showed an inverse association between total dairy consumption and obesity prevalence (OR (95% CI): 0.66 (0.48-0.91). No significant associations were found between milk or yogurt and obesity prevalence risk. Regarding prospective studies, total milk consumption was positively associated with overweight prevalence (OR (95% CI): 1.13 (1.01-1.26)) and incidence (RR (95%CI): 1.17 (1.01-1.35)) risk. Evidence from pooled analysis of cross-sectional studies suggested an inverse association between total dairy consumption and obesity. However, there is limited and no conclusive evidence to confirm an inverse relationship from pooled analysis of prospective studies in children and adolescents.

Item does not contain fulltext, INTRODUCTION: Comprehensive updated information on cervical cancer surgical treatment in Europe is scarce. OBJECTIVE: To evaluate baseline characteristics of women with early cervical cancer and to analyze the outcomes of the ESGO quality indicators after radical hysterectomy in the SUCCOR database. METHODS: The SUCCOR database consisted of 1272 patients who underwent radical hysterectomy for stage IB1 cervical cancer (FIGO 2009) between January 2013 and December 2014. After exclusion criteria, the final sample included 1156 patients. This study first described the clinical, surgical, pathological, and follow-up variables of this population and then analyzed the outcomes (disease-free survival and overall survival) after radical hysterectomy. Surgical-related ESGO quality indicators were assessed and the accomplishment of the stated recommendations was verified. RESULTS: The mean age of the patients was 47.1 years (SD 10.8), with a mean body mass index of 25.4 kg/m(2) (SD 4.9). A total of 423 (36.6%) patients had a previous cone biopsy. Tumor size (clinical examination) <2 cm was observed in 667 (57.7%) patients. The most frequent histology type was squamous carcinoma (794 (68.7%) patients), and positive lymph nodes were found in 143 (12.4%) patients. A total of 633 (54.8%) patients were operated by open abdominal surgery. Intra-operative complications occurred in 108 (9.3%) patients, and post-operative complications during the first month occurred in 249 (21.5%) patients, with bladder dysfunction as the most frequent event (119 (10.3%) patients). Clavien-Dindo grade III or higher complication occurred in 56 (4.8%) patients. A total of 510 (44.1%) patients received adjuvant therapy. After a median follow-up of 58 months (range 0-84), the 5-year disease-free survival was 88.3%, and the overall survival was 94.9%. In our population, 10 of the 11 surgical-related quality indicators currently recommended by ESGO were fully fulfilled 5 years before its implementation. CO

Global sustainability targets demand transformative changes. Nature-based solutions (NbS) are gaining traction in science and policy, but their potential for transformative change remains unexplored. We provide a framework to evaluate how NbS contribute to transformative change and apply it to 93 NbS from mountain social-ecological systems (SES). The framework serves to assess what elements may catalyze transformative change, how transformative change occurs, and what its outcomes are. Our results show that NbS are as much people based as nature based. Most NbS are based on four elements with transformation potential: nature's values, knowledge types, community engagement, and nature management practices. Our results confirm the potential of NbS for transformative change, observed through changes in non-sustainable trajectories of SES. We illustrate the components of our framework through a novel classification of NbS. The framework provides key components for assessing the effectiveness of NbS and allows tracking long-term transformative change processes. © 2021 Elsevier Inc.

Governance is a fundamental aspect and precondition for conservation strategies, such as protected areas or OECMs. A methodology for Site-level Assessment of Governance and Equity (SAGE) has been proposed by IUCN to explore different aspects of governance, based on ten principles. This article describes the application of SAGE to a candidate OECM for what we believe is the first time: the Andakí Municipal Natural Park in the Amazon region of Colombia. The application of SAGE generated useful insights into different aspects of governance, including the importance of good communication and recognising and respecting the rights and responsibilities of different actors. Based on this experience, we believe that the SAGE methodology could be a useful tool not only to identify priorities for improving governance, but also to assess governance and equity in order to evaluate if an area meets the criteria for an OECM that it is governed and managed to achieve sustained and effective contributions to in situ conservation of biodiversity, associated ecosystem functions and services, and cultural, spiritual, socio-economic and other locally relevant values. © 2021, IUCN - International Union for the Conservation of Nature. Debate about what proportion of the Earth to protect often overshadows the question of how nature should be conserved and by whom. We present a systematic review and narrative synthesis of 169 publications investigating how different forms of governance influence conservation outcomes, paying particular attention to the role played by Indigenous peoples and local communities. We find a stark contrast between the outcomes produced by externally controlled conservation, and those produced by locally controlled efforts. Crucially, most studies presenting positive outcomes for both well-being and conservation come from cases where Indigenous peoples and local communities play a central role, such as when they have substantial influence over decision making or when local institutions regulating tenure form a recognized part of governance. In contrast, when interventions are controlled by external organizations and involve strategies to change local practices and supersede customary institutions, they tend to result in relatively ineffective conservation at the same time as producing negative social outcomes. Our findings suggest that equitable conservation, which empowers and supports the environmental stewardship of Indigenous peoples and local communities represents the primary pathway to effective long-term conservation of biodiversity, particularly when upheld in wider law and policy. Whether for protected areas in biodiversity hotspots or restoration of highly modified ecosystems, whether involving highly traditional or diverse and dynamic local communities, conservation can become more effective through an increased focus on governance type and quality, and fostering solutions that reinforce the role, capacity, and rights of Indigenous peoples and local communities. Social robots can be used to motivate children to engage in learning activities in education. In such contexts, they might need to persuade children to achieve specific learning goals. We conducted an exploratory study with 42 children in a museum setting. Children were asked to play an interactive storytelling game on a touchscreen. A Furhat robot guided them through the steps of creating the character of a story in two conditions. In one condition, the robot tried to influence children's choices using high-controlling language. In the other, the robot left children free to choose and used a low-controlling language. Participants in the persuasive condition generally followed the indications of the robot. Interestingly, the use of high-controlling language did not affect children's perceived trust towards the robot. During a workshop held in Oaxaca, Mexico, a shared vision, mission and strategies to foster a more plural valuation of nature were developed. The participants represent a wide range of backgrounds and are active in science, policy and practitioner networks and activities. Their common ground is the recognition of the need to change the prevailing culture of how nature is valued and subsequently managed as an essential step towards a more just and sustainable world. After an open plenary session in which the goal of the workshop was determined and the diverse perspectives and backgrounds of the participants were heard, breakout groups developed the components of a shared vision, mission and strategies for plural valuation of nature. Consequently, these components were discussed back in plenary and consolidated into a consensus text, which was further debated and its main building blocks agreed upon. The compilation of our shared views converged into a normative call and perspective to share with our peers. The information generated throughout the workshop was collaboratively synthesized, amended, reviewed and validated by all workshop participants/co-authors. Our message aims to contribute to advancing plural valuation approaches as a science-policy field, as well as to raise personal awareness among researchers and practitioners on implicit inequality and power issues.

Formalised knowledge systems, including universities and research institutes, are important for contemporary societies. They are, however, also arguably failing humanity when their impact is measured against the level of progress being made in stimulating the societal changes needed to address challenges like climate change. In this research we used a novel futures-oriented and participatory approach that asked what future envisioned knowledge systems might need to look like and how we might get there. Findings suggest that envisioned future systems will need to be much more collaborative, open, diverse, egalitarian, and able to work with values and systemic issues. They will also need to go beyond producing knowledge about our world to generating wisdom about how to act within it. To get to envisioned systems we will need to rapidly scale methodological innovations, connect innovators, and creatively accelerate learning about working with intractable challenges. We will also need to create new funding schemes, a global knowledge commons, and challenge deeply held assumptions. To genuinely be a creative force in supporting longevity of human and non-human life on our planet, the shift in knowledge systems will probably need to be at the scale of the enlightenment and speed of the scientific and technological revolution accompanying the second World War. This will require bold and strategic action from governments, scientists, civic society and sustained transformational intent.

The primary objective of the SUCCOR study, a European, multicenter, retrospective, observational cohort study was to evaluate disease-free survival in patients with stage IB1 (FIGO 2009) cervical cancer undergoing open vs minimally invasive radical hysterectomy. As a secondary objective, we aimed to investigate the association between protective surgical maneuvers and the risk of relapse. METHODS: We obtained data from 1272 patients that underwent a radical hysterectomy by open or minimally invasive surgery for stage IB1 cervical cancer (FIGO 2009) from January 2013 to December 2014. After applying all the inclusion-exclusion criteria, we used an inverse probability weighting to construct a weighted cohort of 693 patients to compare outcomes (minimally invasive surgery vs open). The first endpoint compared disease-free survival at 4.5 years in both groups. Secondary endpoints compared overall survival among groups and the impact of the use of a uterine manipulator and protective closure of the colpotomy over the tumor in the minimally invasive surgery group. RESULTS: Mean age was 48.3 years (range; 23-83) while the mean BMI was 25.7 kg/m(2) (range; 15-49). The risk of recurrence for patients who underwent minimally invasive surgery was twice as high as that in the open surgery group (HR, 2.07; 95% CI, 1.35 to 3.15; P=0.001). Similarly, the risk of death was 2.42-times higher than in the open surgery group (HR, 2.45; 95% CI, 1.30 to 4.60, P=0.005). Patients that underwent minimally invasive surgery using a uterine manipulator had a 2.76-times higher hazard of relapse (HR, 2.76; 95% CI, 1.75 to 4.33; P<0.001) and those without the use of a uterine manipulator had similar disease-free-survival to the open surgery group (HR, 1.58; 95% CI, 0.79 to 3.15; P=0.20).
