This study examines three L3 transfer proposals, namely the L1 Factor (Hermas, 2010, 2014a, 2014b), the CEM (Flynn et al., 2004) and the TPM (Rothman, 2010, 2011, 2013, 2015) as well as investigates the role of the language of dominance in L3 acquisition of English attributive adjectives. Three groups of bilinguals took part in this study: L1 Mazandarani/L2 Persian, with Mazandarani as the dominant language of communication, L1 Mazandarani/L2 Persian, with Persian as the dominant language of communication and L1 Persian/L2 Mazandarani, with Persian as the dominant language of communication. The results of a grammaticality judgment task and an element rearrangement task show that the predictions of the above-mentioned L3 transfer proposals were not realized. Instead, the dominant language of communication turns out to be the main source of syntactic crosslinguistic influence at the initial stages of L3 acquisition, irrespective of its status as an L1 or L2. Objectives The “angiosome” concept as a model for decision making in revascularisation of patients with critical limb ischaemia (CLI) has been subject to lively discussion in recent years. The aim of this prospective pilot study was to use intra-operative fluorescence angiography to provide further data on the angiosome concept on the level of microcirculation after tibial bypass surgery. Design, materials, and methods This was a prospective analysis of 40 patients presenting with CLI Rutherford stage IV to VI before and after tibial bypass surgery. The macrocirculation was measured by the ankle brachial index. Skin microcirculation was assessed by intra-operative fluorescence angiography. The alteration of microcirculation was compared in direct and indirect revascularised angiosomes. Clinical follow-up investigations were performed and the wound healing rate was compared between the different revascularisation methods. Results Cumulated microcirculation parameters showed a significant improvement after surgery (ingress, ingress rate p <.001). Likewise, general microcirculatory improvement was observed in each foot angiosome after revascularisation, regardless of the tibial artery revascularised. Furthermore, a comparison of the direct (DR) and the indirect revascularised (IR) angiosomes did not show a significant difference concerning the improvement of microcirculation (difference DR-IR, ingress: 1.69, 95% CI 71.73–75.11; ingress rate: 0.08, 95% CI −12.91 to 13.07). The wound healing rate was similar in both groups, although the time to wound healing was faster by on average 2.5 months in the DR group ( p =.083). Conclusion Microcirculatory improvement was seen over the whole foot after tibial bypass. Purpose: To identify possible risk factors in predicting clinical outcome in critical limb ischemia (CLI) patients undergoing percutaneous transluminal angioplasty (PTA).Materials and Methods: PubMed and EMBASE were searched for studies analyzing CLI and clinical outcome after PTA from January 2006 to April 2017. Outcome measures were ulcer healing, amputation free survival (AFS)/limb salvage and overall survival. Data on predictive factors for ulcer healing, AFS/limb salvage and survival were extracted.Results: Ten articles with a total of 2448 patients were included, all cohorts and based on prospective-designed databases. For ulcers, it seems that complete healing can be achieved in most of the patients within 1 year. No significant predictive factors were found. AFS/limb salvage: AFS rates for 1, 2 and 3 years ranged from 49.5 to 75.2%, 37 to 58% and 22 to 59%, respectively. Limb salvage rates for 1, 2 and 3 years ranged from 71 to 95%, 54 to 93.3% and 32 to 92.7%, respectively. All studies had different univariate and multivariate outcomes for predictive factors; however, age and diabetes were significant predictors in at least three studies. Survival: Survival rates for 1, 2 and 3 years ranged from 65.4 to 91.5%, 45.7 to 76% and 37.3 to 83.1%, respectively. Different predictive factors were found; however, age was found in 2 out of 5 studies reporting on predictive factors.Conclusions: In several studies two factors, age and diabetes, were found as predictive factors for AFS/limb salvage and survival in patients with CLI undergoing PTA. The slimming product Mysimba®, a combination of naltrexon and bupropion, was recently included in the Dutch drug reimbursement system (Geneesmiddelenvergoedingssysteem, GVS). The drug is fully reimbursed under the national basic health insurance package, though with specific preconditions, including its use together with the Dutch programme known as combined lifestyle intervention (CLI). Its inclusion in the GVS was reported in the Dutch TV news on Tuesday 24 May 2022. Despite what this media attention suggests, results achieved with naltrexon/bupropion in terms of weight loss have been disappointing, and it is to be expected that over a third of users will discontinue the treatment for this reason. A study using cardiovascular endpoints yielded no conclusions, as it was prematurely ended. There is therefore no reason to recommend this drug to obese patients in practice. [ABSTRACT FROM AUTHOR]

Human mesenchymal/stromal stem cells (hMSC) are the most promising cell source for adult cell therapies in regenerative medicine. Many clinical trials have reported the use of autologous transplantation of hMSCs in several disorders, but with limited results. To exert their potential, hMSCs could exhibit efficient homing and migration toward lesion sites among other effects, but the underlying process is not clear enough. To further increase the knowledge, we studied the co-regulation between hypoxia-regulated genes and miRNAs. To this end, we investigated the miRNA expression profile of healthy hMSCs in low oxygen/nutrient conditions to mimic ischemia and compared with cells of patients suffering from critical limb ischemia (CLI). miRNAs are small, highly conserved, non-coding RNAs, skilled in the control of the target’s expression level in a fine-tuned way. After analyzing the miRNOme in CLI-derived hMSC cells and healthy controls, and intersecting the results with the mRNA expression dataset under hypoxic conditions, we identified two miRNAs potentially relevant to the disease: miR-29b as a pathological marker of the disease and miR-638 as a therapeutic target. This study yielded a deeper understanding of stem cell biology and ischemic disorders, opening new potential treatments in the future.

This paper presents a fully computerized method to backup the router configuration file. The method consists of a friendly graphical interface programmed by Java programming language The proposed method is compared with the two existing methods, namely: TFTP server method and Copy/Paste method. The comparison reveals that the proposed method has many advantages over the existing ones. The proposed method has been implemented on Cisco routers (series 2500, 2600 and 2800)

This Special Issue of Higher Learning Research Communications (HLRC) is dedicated to research in content language integrated learning (CLIL) and English-medium instruction (EMI). A we make our way in the 21st century, higher education institutions are faced with multiple challenges as they try to prepare future professionals. Issues such as globalization and internationalization, technology, access to information, interculturality, and the like pose challenges for future generations. And, higher education institutions play a fundamental role in shaping how that future will look like. In the face of multiculturalism and the communication barriers that are bound to happen, many countries and educational systems are incorporating English, not only as a means to facilitate contact among cultures, but as a means of instruction in itself. The groundwork laid down during the past decades by several researchers, such as David Marsh, has paved the way to promote multilingualism, not just by learning a language but also by using the second or foreign language as a content delivery method. Most research in this area has focused on primary and secondary education; thus, the research regarding CLIL and EMI in tertiary education is limited at the moment. However, in recent years there has been a boom in English-taught programs in non-English speaking countries. This means the need for research is this area is bound to grow exponentially as faculty, students, and institutions, and even governments and the labor market, take on the challenges of English-mediated instruction. It is out hope that with this Special Issue, which features research from Japan and Spain, we can contribute meaningful research to the field.

This volume of HLRC is dedicated to the emergent and rapidly growing interest in English Medium Instruction as a major driver in the internationalization of higher education. It focuses on sharing knowledge and experience in CLIL (Content and Language Integrated Learning) and EMI (English Medium Instruction) from different universities in Eastern and European countries as a means to reflect and improve upon teaching and learning. As editors, we would like to kick this volume off with a review of one of the most comprehensive and relevant publications on the topic in recent times, The Higher Education Language Landscape: Ensuring Quality in English Language Degree Programmes, (2013). Following the book review is a short question and answer section with its authors and renowned experts in the field, David Marsh, Victor Pavón Vázquez, and María Jesús Frigols Martín. We would like to extend our gratitude to all three for providing us with their insights, which are, no doubt, of great use to higher education institutions considering embarking upon or improving English-taught degree programs.

OBJECTIVE: Fibroblast growth factor 23 (FGF23) was demonstrated to be involved in the occurrence and development of cardiovascular disease (CVD). The aim of this study was to investigate the potential role of FGF23 on presence and severity of peripheral arterial disease (PAD) in type 2 diabetic patients. PATIENTS AND METHODS: In this study, we analyzed FGF23 serum levels in 413 type 2 diabetic patients with PAD and in 598 diabetic controls without lower limbs atherosclerosis. RESULTS: We found that FGF23 median serum levels were significantly higher in patients than in diabetic controls (69.3 (58.8-75.1) pg/mL in PAD and 42.98 (37.1-49.8) pg/mL in subjects without PAD (p < 0.001) and were significantly and independently associated with critical limb ischemia (CLI) [OR, 7.69 (2.64-16.31); p = 0.001]. CONCLUSIONS: We have found, for the first time, that FGF23 could be associated with presence and severity of PAD in Italian patients with type 2 diabetes. [ABSTRACT FROM AUTHOR]

Abrasiveness of rocks plays an important role in the wear of TBM tunnelling. Cerchar Abrasivity Index (CAI) test is the most commonly used method to estimate rock abrasiveness due to its simple, fast test procedure and economic benefits. This study investigated the correlation between CAI and geomechanical properties of igneous and metamorphic rocks. Single and multiple regression analysis were performed and predictive models for the CAI from geomechanical properties were developed. It is found that single parameter alone is not suitable to predict the value of CAI. The result of the multiple regression analysis shows that quartz content is less influencing factor to predict the value of CAI. Also, the correlation between CAI and NTNU’s Cutter Life Index (CLI) was examined and a predictive model for the CLI from CAI and geomechanical properties was suggested. Finally, the influence of CAI on disc cutter life prediction models was investigated and it was found that the variation of CAI has the maximum effect on the predicted disc cutter life in Gehring model. Although L1 has been treated as a rigid system which is more likely to act as a sender than a receiver of CLI in bilinguals and multilinguals, recent studies have provided some evidence of the influence of both L2 and L3 on L1. The study is aimed at shedding further light on how Lns can influence the native language and how these changes can be explained by means of the Speech Learning Model. The first and second formant of L1 Polish vowels of three groups of multilinguals were compared. Evidence of a systemic influence of L2 on L1 was observed in the raising and backing of L1 Polish vowels due to L2 English and lowering and backing or fronting of L1 Polish vowels due to L2 German. No systemic influence of L3 on L1 was observed. The predictions derived from equivalence classification of SLM were tested for the Polish vowel /ɛ/ and the closest vowels from Lns. Objective/Background Critical limb ischemia (CLI) is the most advanced stage of peripheral artery disease (PAD), and many patients with CLI are not eligible for conventional revascularization. In the last decade, cell based therapies have been explored as an alternative treatment option for CLI. A meta-analysis was conducted of randomized placebo controlled trials investigating bone marrow (BM) derived cell therapy in patients with CLI. Methods The MEDLINE, Embase, and the Cochrane Controlled Trials Register databases were systematically searched, and all included studies were critically appraised by two independent reviewers. The meta-analysis was performed using a random effects model. Results Ten studies, totaling 499 patients, were included in this meta-analysis. No significant differences were observed in major amputation rates (relative risk [RR] 0.91; 95% confidence interval [CI] 0.65–1.27), survival (RR 1.00; 95% CI 0.95–1.06), and amputation free survival (RR 1.03; 95% CI 0.86–1.23) between the cell treated and placebo treated patients. The ankle brachial index (mean difference 0.11; 95% CI 0.07–0.16), transcutaneous oxygen measurements (mean difference 11.88; 95% CI 2.73–21.02), and pain score (mean difference –0.72; 95% CI –1.37 to –0.07) were significantly better in the treatment group than in the placebo group. Conclusions This meta-analysis of placebo controlled trials showed no advantage of stem cell therapy on the primary outcome measures of amputation, survival, and amputation free survival in patients with CLI. Peripheral arterial disease is a condition associated with high rates of cardiovascular morbidity and mortality, increased risk of adverse limb events, including development of critical limb ischemia and amputation, and also with decreased quality of life. This manuscript provides an overview of the medical management of patients with peripheral arterial disease. We discuss contemporary therapies that decrease major adverse cardiovascular and limb events among patients with peripheral arterial disease, and also therapies that improve the patient's ability to walk and quality of life in general. Debido a que Cladocera es el grupo de microfiltradores más abundante del zooplancton límnico y cuyas poblaciones se encuentran usualmente limitadas por alimento, la respuesta de los individuos frente a un gradiente de concentración de recursos tendría significativas implicancias sobre los patrones poblacionales y comunitarios. Se comparó la conducta alimentaria de Moina micrura, Ceriodaphnia dubia y Daphnia ambigua (Cladocera) frente a las microalgas Chlorella sp. y Oocystis sp. (Chlorophyceae) mediante los siguientes parámetros: Tasa de Ingesta Máxima (Imax), Concentración Limitante Incipiente (CLI) y Eficiencia de Consumo (b), los cuales fueron obtenidos a través de un modelo de respuesta funcional. Los resultados mostraron que D. ambigua presentó las mayores Imax y CLI sobre ambos recursos, sin embargo, esta especie presentó la menor b. Mientras que D. ambigua no presentó diferencias en b sobre Chlorella sp. y Oocystis sp., M. micrura presentó una mayor b sobre Chlorella sp. y C. dubia sobre Oocystis sp. Results indicated that D. ambigua showed the highest Imax and ILC on both resources, however, this specie presented the lowest b. Whereas, D. ambigua did not show differences in b on Chlorella sp. or Oocystis sp., M. micrura showed a higher b on Chlorella sp. and C. dubia on Oocystis sp. These results reveal significant interspecific differences in the feeding response of Cladocera, which showed a complex behavior exposed to variations in the availability of food resources

Critical limb ischemia (CLI) is associated with high rates of morbidity and mortality. Many patients with CLI are poor surgical candidates. Endovascular therapy has been shown to be an effective technique to improve arterial perfusion for patients with CLI. In patients with isolated infrapopliteal occlusive disease, endovascular therapy may be more effective than conventional bypass surgery. When antegrade endovascular revascularization fails, an understanding of tibiopedal access and retrograde crossing techniques is essential to re-establish flow to aid in tissue healing, provide symptomatic relief, and avoid amputation. Critical limb ischemia (CLI) is exhibited in patients with symptoms of severe claudication (rest pain) and ischemic tissue loss and gangrene. Magnetic resonance angiography and computed tomography angiography have risen to the forefront of vascular imaging over the last 2 decades. Both modalities have been shown to compare favorably with digital subtraction angiography in guiding the clinical management of patients with CLI. Understanding the advantages and limitations of both modalities allows for the proper selection of the best examination for a particular patient with CLI. Ultimately, the enhanced understanding of the vascular anatomy by obtaining noninvasive imaging should make subsequent revascularization safer and more effective. [ABSTRACT FROM AUTHOR]

Retrograde access techniques involve the addition of a retrograde access into the distal target vessel to aid in recanalization of chronic total occlusions. Many patients with critical limb ischemia are also poor surgical candidates because of comorbidities or lack of suitable landing zone for bypass procedures. This approach may be helpful in the setting of chronic occlusions that cannot be crossed via conventional antegrade true-lumen approaches. Subintimal arterial flossing with antegrade-retrograde intervention technique can be used when the occlusion was crossed in the subintimal plane and antegrade re-entry techniques failed. It may also be useful for flush superficial femoral artery occlusions or those lesions that extend into the trifurcation vessels. The unique anatomical location and particular biomechanical factors affecting the popliteal artery provide a challenge to determine the proper endovascular, surgical, or combined intervention for patients with critical limb ischemia who often require prompt management in the presence of severe lifestyle-limiting symptoms or of the risk of limb loss or both. Background aims. Non-revascularizable critical limb ischemia (CLI) is the most severe stage of peripheral arterial disease, with no therapeutic option. Extensive preclinical studies have demonstrated that adipose-derived stroma cell (ASC) transplantation strongly improves revascularization and tissue perfusion in ischemic limbs. This study, named ACellDREAM, is the first phase I trial to evaluate the feasibility and safety of intramuscular injections of autologous ASC in non-revascularizable CLI patients. Methods. Seven patients were consecutively enrolled, on the basis of the following criteria: (i) lower-limb rest pain or ulcer; (ii) ankle systolic oxygen pressure <50 or 70 mm Hg for non-diabetic and diabetic patients, respectively, or first-toe systolic oxygen pressure <30 mm Hg or 50 mm Hg for non-diabetic and diabetic patients, respectively; (iii) not suitable for revascularization. ASCs from abdominal fat were grown for 2 weeks and were then characterized. Results. More than 200 million cells were obtained, with almost total homogeneity and no karyotype abnormality. The expressions of sternness markers Oct4 and Nanog were very low, whereas expression of telomerase was undetectable in human ASCs compared with human embryonic stem cells. ASCs (108) were then intramuscularly injected into the ischemic leg of patients, with no complication, as judged by an independent committee. Trans-cutaneous oxygen pressure tended to increase in most patients. Ulcer evolution and wound healing showed improvement. Conclusions. These data demonstrate the feasibility and safety of autologous ASC transplantation in patients with objectively proven CLI not suitable for revascularization. Since 2013, the Japanese Society for Vascular Surgery has started the project of nationwide registration and tracking database for patients with critical limb ischemia (CLI) who are treated by vascular surgeons. The objective of this project is to elucidate the current status of the medical practice for CLI patients to contribute to the improvement of the quality of medical care. This database, called JAPAN Critical Limb Ischemia Database (JCLIMB), is created on the National Clinical Database (NCD) and collects data of patients' background, therapeutic measures, early results, and long-term prognosis as long as 5 years after the initial treatment. The limbs managed conservatively are also registered in JCLIMB, together with those treated with surgery and/or endovascular treatment (EVT). In 2019, 1070 CLI limbs (male: 725 limbs, 68%) were registered by 83 facilities. Arteriosclerosis obliterans (ASO) accounted for 98% of the pathogenesis of these limbs. In this manuscript, the background data and the early prognosis of the registered limbs are reported. Although the registration format for the simultaneous surgery of bilateral limbs in NCD was changed to one patient and two limbs, JCLIMB still counted two patients and two limbs to eliminate discrepancy with the past annual reports. Introduction: Patients with critical limb ischemia (CLI), who are unsuitable for intervention, face the consequence of primary amputation. Sequential compression biomechanical device (SCBD) therapy provides a limb salvage option for these patients. Objectives: To assess the outcome of SCBD in patients with severe CLI who are unsuitable for revascularization. Primary end points were limb salvage and 30-day mortality. Methods: From 2005 to 2012, 189 patients with severe CLI were not suitable for revascularization. In all, 171 joined the SCBD program. We match controlled 75 primary amputations. Results: All patients were Rutherford category 4 or higher. Sustained clinical improvement was 68% at 1 year. Mean toe pressure increased from 19.9 to 35.42 mm Hg, P < .0001. Mean popliteal flow increased from 35.44 to 55.91 cm/sec, P < .0001. The 30-day mortality was 0.6%. Limb salvage was 94% at 5 years. Freedom from major adverse clinical events was 62.5%. All-cause survival was 69%. Median cost of managing a primary amputation patient is €29 815 compared to €3985 for SCBD. We treated 171 patients with artassist at a cost of €681 965. However, primary amputation for 75 patients cost €2 236 125. Conclusion: The SCBD therapy is a cost-effective and clinically effective solution in patients with CLI having no option of revascularization. Background: Peripheral vascular disease of the lower extremities comprises a clinical spectrum that extends from no symptoms to presentation with critical limb ischemia (CLI). Bone marrow derived Mesenchymal Stem Cells (BM- MSCs) may ameliorate the consequences of CLI due to their combinatorial potential for inducing angiogenesis and immunomodulatory environment in situ. The primary objective was to determine the safety of BM- MSCs in patients with CLI. Methods: Prospective, double blind randomized placebo controlled multi-center study was conducted in patients with established CLI as per Rutherford classification in category II-4, III-5, or III-6 with infra-inguinal arterial occlusive disease and were not suitable for or had failed revascularization treatment. The primary end point was incidence of treatment - related adverse events (AE). Exploratory efficacy end points were improvement in rest pain, increase in Ankle Brachial Pressure Index (ABPI), ankle pressure, healing of ulcers, and amputation rates. Twenty patients (BM-MSC: Placebo = 1:1) were administered with allogeneic BM-MSCs at a dose of 2 million cells/kg or placebo (PlasmaLyte A) at the gastrocnemius muscle of the ischemic limb. Results: Improvement was observed in the rest pain scores in both the arms. Significant increase in ABPI and ankle pressure was seen in BM-MSC arm compared to the placebo group. Incidence of AEs in the BM-MSC arm was 13 vs. 45 in the placebo arm where as serious adverse events (SAE) were similar in both the arms (5 in BM-MSC and 4 in the placebo group). SAEs resulted in death, infected gangrene, amputations in these patients. It was observed that the SAEs were related to disease progression and not related to stem cells. Conclusion: BM-MSCs are safe when injected IM at a dose of 2 million cells/kg body weight. Few efficacy parameters such as ABPI and ankle pressure showed positive trend warranting further studies. There is a large body of preclinical research demonstrating the efficacy of gene and cellular therapy for the potential treatment of severe (limb-threatening) peripheral arterial disease (PAD), including evidence for growth and transcription factors, monocytes, and mesenchymal stem cells. While preclinical research has advanced into early phase clinical trials in patients, few late-phase clinical trials have been conducted. The reasons for the slow progression of these therapies from bench to bedside are as complicated as the fields of gene and cellular therapies. The variety of tissue sources of stem cells (embryonic, adult bone marrow, umbilical cord, placenta, adipose tissue, etc.); autologous versus allogeneic donation; types of cells (hematopoietic, mesenchymal stromal, progenitor, and mixed populations); confusion and stigmatism by the public and patients regarding gene, protein, and stem cell therapy; scaling of manufacturing; and the changing regulatory environment all contribute to the small number of late phase (Phase 3) clinical trials and the lack of Food and Drug Administration (FDA) approvals. This review article provides an overview of the progression of research from gene therapy to the cellular therapy field as it applies to peripheral arterial disease, as well as the position of Aastrom's cellular therapy, ixmyelocel-T, within this field. Drillability indices, i.e., the Drilling Rate Index™ (DRI), Bit Wear Index™ (BWI), Cutter Life Index™ (CLI), and Vickers Hardness Number Rock (VHNR), are indirect measures of rock drillability. These indices are recognized as providing practical characterization of rock properties used in the Norwegian University of Science and Technology (NTNU) time and cost prediction models available for hard rock tunneling and surface excavation. The tests form the foundation of various hard rock equipment capacity and performance prediction methods. 70 years old patient treated for type 2 diabetes mellitus (oral hypoglycemic agents) was admitted to the surgical department of our hospital because of rest pain of his left foot and necrosis of its third toe with underlying purulent inflammation of antetarsum. Computed tomographic angiography revealed typical for diabetes distal pattern of peripheral artery disease with a multilevel stenoses of both tibial and peroneal arteries with only distinct findings in pedal arteries. There was patent femoral and popliteal artery with typical calcification. ABI measured bilaterally was of 0.62 and in laboratory assignments there was a rise of CRP level, that was 145 mg/l. Because of aferomentioned findings patient was temporally discharged of revascularization and his treatment was diminished to amputation of third toe of his left foot only. His pain resolved and it was obvious it was caused previously mainly by purulent inflammation. However after eight weeks of treatment his wound stayed still unhealed and anergic and then patient was treated surgically. We performed a distal pedal bypass originated in popliteal artery below the knee using inverted saphenous vein harvested in thigh. A runoff artery was posterior tibial artery just below the ankle. After next ten weeks the healing of postoperative wound was almost completed. Its bed was filled up with a granulation tissue and there was prompt epithelization. [ABSTRACT FROM AUTHOR]

BUI: Browser User Interface. BUI is a technology which unifies the desktop environment inside the browser. Implementing such an interface will radically alter the desktop environment. First and foremost, the concept of a separate browser running on a desktop is eliminated. In fact, the browser will be the desktop. The need for having browser as a separate application running on the desktop is eliminated and the browser is made the desktop itself. Such a technology will provide a better access to the internet and better management of work on and off-line. The Browser User Interface will be easier to implement and provides easy use to all its users. This technology basks on the growth of the browsers. The BUI based operating system is perfect for organizations which involves cloud computing for storage. BUI eradicates the explicit connection to the internet and automatically connects to the net while the OS program is loaded.
