51. Strengthening of Vaccine-Preventable Disease (VPD) Surveillance to Enhance National Health Capacity and Security: Perspective from India

Abstract

The Government of India, in collaboration with the World Health Organization (WHO), established the National Polio Surveillance Project (NPSP) in 1997 and initiated acute flaccid paralysis (AFP) surveillance to achieve the goal of polio eradication. The WHO South-East Asia Region, comprising of 11 countries, including India, was certified as polio-free in March 2014. India was also validated to have eliminated maternal and neonatal tetanus in May 2015. Over the years, the surveillance of other vaccine-preventable diseases (VPDs) was integrated with AFP surveillance in the country. Outbreak-based measles–rubella (MR) surveillance was initiated in 2005 using AFP surveillance as a platform, case-based fever–rash (FR) surveillance started in 2021 as one of the strategies to achieve measles and rubella elimination in the country. The surveillance of diphtheria, pertussis, and neonatal tetanus was integrated with AFP surveillance in a phased manner during 2015–2022. The surveillance system for VPDs in India, supported by a laboratory network of 10 polio laboratories, 28 measles–rubella laboratories, and 20 diphtheria–pertussis laboratories, has enhanced the national health capacity and security. The setting up and expansion of the surveillance system in the country involved the important component of capacity building of personnel on various components of surveillance, including case identification, case investigation, sample collection and shipment, data analysis and public health response. These capacities have been used effectively during other emergencies, such as the recent COVID-19 pandemic, as well as during outbreaks of other diseases and natural calamities.

Abstract

Most data sets comprise of measurements on continuous and categorical variables. In regression and classification Statistics literature, modeling high-dimensional mixed predictors has received limited attention. In this paper we study the general regression problem of inferring on a variable of interest based on high dimensional mixed continuous and binary predictors. The aim is to find a lower dimensional function of the mixed predictor vector that contains all the modeling information in the mixed predictors for the response, which can be either continuous or categorical. The approach we propose identifies sufficient reductions by reversing the regression and modeling the mixed predictors conditional on the response. We derive the maximum likelihood estimator of the sufficient reductions, asymptotic tests for dimension, and a regularized estimator, which simultaneously achieves variable (feature) selection and dimension reduction (feature extraction). We study the performance of the proposed method and compare it with other approaches through simulations and real data examples.

Abstract

Farming practices reliant solely on synthetic agrochemicals face unreliability in the current era marked by unpredictable climate changes and rapid soil health deterioration. Consequently, a shift towards sustainable approaches is imperative to ensure both food security and environmental quality. Molasses and vinasse, abundant organic liquid by-products from sugar processing and distillery industries respectively, have historically served as soil conditioners and biofertilizers. Despite their potential, their effectiveness as organic amendments remain relatively unknown globally. In response, we conducted a systematic literature review to unveil the benefits of molasses and vinasse as organic amendments. Our findings reveal that these by-products consist of both inorganic and organic compounds that enhance soil and aquatic ecosystem performance. These compounds include essential plant nutrients as mineral elements and organic matter, contributing to improved soil physico-chemical and biological properties. Notably, the application of molasses and vinasse in crop production has demonstrated superiority over chemical fertilizers, particularly when combined with other inorganic amendments. Molasses and vinasse have been reported to significantly increase yield in several crops including sugarcane (Saccharum officinarum), tomatoes (Solanum lycopersicum), soybean (Glycine max), maize (Zea mays) and rice (Oryza sativa). Strategic utilization of vinasse has the potential to enhance environmental quality by reducing soil heavy metal loads and mitigating negative impacts associated with synthetic fertilizers. However, it is crucial to note that irregular disposal or misuse of these by-products can result in detrimental effects on the environment and human health. To encourage sustainable utilization on a global scale, it is essential to establish appropriate dosages, raise awareness among farmers and stakeholders regarding judicious use, and develop effective methods for handling and application of molasses and vinasse. This approach ensures cost-effective and environmentally friendly organic amendments, fostering a harmonious balance between agricutural productivity and ecological well-being.

Abstract

Many chemotherapeutics drugs have low water solubility, which potentially can decrease their anticancer potential. The use of drug delivery systems has proven to be highly effective in addressing the challenges associated with delivering hydrophobic chemotherapy drugs to tumor tissues. However, two major issues that arise in the clinical nanoparticle-based treatment of cancer are hepatotoxicity and suppression of the hematopoietic system, which can limit their medical applicability. As previously established, thiazole derivative N-(5-benzyl-1,3-thiazol-2-yl)-3,5-dimethyl-1-benzofuran-2-carboxamide in complex with polymeric nanocarriers (nanomicelles) based on polyethylene glycol exhibited a greater level of cytotoxicity towards specific tumor cell lines melanoma, glioblastoma, hepatocarcinoma, leukemia, etc. This compound and its complexes with polymeric nanomicelle significantly changed the activity of antioxidant enzymes in lymphoma cells. Therefore, the purpose of this study was to examine the impact of a thiazole derivative with polymeric nanomicelles based on polyethylene glycol on the hepatocytes (liver cells) of mice that had been implanted with Nemet-Kelner lymphoma. The investigated compounds thiazole derivative, polymeric nanomicelle, and combination of thiazole derivative with nanomicelle at a final concentration of 10 μM were added to the liver samples and incubated for 10 min. The activity of antioxidant defense system enzymes such as superoxiddismutase, catalase, glutathionperoxidase was determined in liver homogenate under the action of studied compounds in vitro. It was reported that neither thiazole derivative, nanomicelle, nor their complex changed the activity of antioxidant enzymes in hepatocytes from mice with lymphoma. Thiazole derivative and it complex with nanomicelle had limited negative side effects in the mice with lymphoma. The investigated compounds were not hepatotoxic toward murine liver cells.

Abstract

Abstract High grade serous ovarian carcinoma (HGSOC) is a highly heterogeneous disease that typically presents at an advanced, metastatic state. The multi-scale complexity of HGSOC is a major obstacle to predicting response to neoadjuvant chemotherapy (NACT) and understanding critical determinants of response. Here we present a framework to predict the response of HGSOC patients to NACT integrating baseline clinical, blood-based, and radiomic biomarkers extracted from all primary and metastatic lesions. We use an ensemble machine learning model trained to predict the change in total disease volume using data obtained at diagnosis (n = 72). The model is validated in an internal hold-out cohort (n = 20) and an independent external patient cohort (n = 42). In the external cohort the integrated radiomics model reduces the prediction error by 8% with respect to the clinical model, achieving an AUC of 0.78 for RECIST 1.1 classification compared to 0.47 for the clinical model. Our results emphasize the value of including radiomics data in integrative models of treatment response and provide methods for developing new biomarker-based clinical trials of NACT in HGSOC.

Abstract

Abstract The crystallite size of the materials considerably influences the material properties, including their compressibility and resistance to external forces and the stability of the crystalline structure; a corresponding study for which, so far, has been limited for the important class of nanocrystalline Rare Earth Sesquioxides (REOs). In the present study, we report the crystallographic structural transitions in nanocrystalline Rare Earth Oxides (REOs) under the influence of pressure, investigated via high-energy X-Ray Diffraction (XRD) measurements. The study has been carried out on three of the REOs, namely Lutetium oxide (Lu2O3), Thulium oxide (Tm2O3) and Europium oxide (Eu2O3) up to the pressures of 33, 22 and 11 GPa, respectively. The diffraction data of Lu2O3 and Tm2O3 suggests the occurrence of irreversible structural transitions from cubic to monoclinic phase, while Eu2O3 showed a transition from the cubic to hexagonal phase. The transitions were found to be accompanied by a collapse in the volume and the resulting Pressure–Volume (P–V) graphs are fitted with the 3rd order Birch-Murnaghan (BM) equation of state (EOS) to estimate the bulk moduli and their pressure derivatives. Our study establishes a qualitative relationship between the crystallite size and various material properties such as the lattice parameters, transition pressure, bulk modulus etc., and strengthens the knowledge regarding the behaviour of this technologically important class of materials.

In the development of the prototype flying vehicle, especially the development of the avionics and flight control system, the vehicle will go through sequential simulation steps from Software in the Loop Simulation (SILS), Hardware in the Loop Simulation (HILS), and Ready-to-Fly System (RTFS). In this paper, the simulation steps will be described with the intention of facilitating integration and testing of each sub-system being developed, testing the control strategy applied or eliminating bugs if something goes wrong. In the end, with a series of flying vehicle simulations, it can be developed quickly and cost-effectively, including saving human resources.

In contrast to the majority of moment based sufficient dimension reduction methods, Conditional Variance Estimation is fully data driven, does not require the restrictive linearity and constant variance conditions, and is not based on inverse regression. Abstract

Traditional houses and villages are a cultural asset of the West Sulawesi region and should be passed down to future generations to preserve the community’s identity. The Orobua settlement, located in Mamasa Regency, West Sulawesi Province, is a settlement that had maintained its traditional landscape until the 1990s. However, since the mid-2000s, most traditional houses have either been modified or replaced with modern ones. This paper examines the cultural heritage of the Orobua settlement and suggests strategies to preserve its traditional landscape. Source data were obtained through a literature review, field survey, and interviews with residents.

The Influence of the German Language on Czech and Upper Sorbian Phraseology: Czech-Upper Sorbian Parallels The article is an attempt to assess the influence of the German language on Czech and Upper Sorbian phraseology. Research on this topic is motivated by the fact of the very long contact of Czech and the constant contact of Upper Sorbian with German, as well as Upper Sorbian-Czech contacts. The study analyses idioms of German origin in contemporary Upper Sorbian and Czech languages – primarily calques, but also semi-calques and idioms containing a German component. A separate place is devoted to the so-called winged words of German origin belonging to the phraseological resources of these two languages. The collected material shows that all the mentioned types of idioms significantly enrich these resources. Wpływ języka niemieckiego na czeską i górnołużycką frazeologię: czesko-górnołużyckie paralele Artykuł jest próbą oceny wpływu języka niemieckiego na czeską i górnołużycką frazeologię. Do podjęcia tego tematu skłania bardzo długi kontakt czeszczyzny i stały kontakt górnołużycczyzny z językiem niemieckim, a także kontakty górnołużycko-czeskie. Przedmiotem analizy są frazeologizmy pochodzenia niemieckiego we współczesnych językach górnołużyckim i czeskim – przede wszystkim kalki, ale także półkalki i frazeologizmy zawierające komponent niemiecki. Osobne miejsce poświęca się tzw. skrzydlatym słowom pochodzenia niemieckiego należącym do zasobów frazeologicznych tych dwóch języków. Zgromadzony materiał pokazuje, że wszystkie wymienione typy frazeologizmów istotnie te zasoby wzbogacają.

Abstract

Many physical systems have underlying safety considerations that require that the policy employed ensures the satisfaction of a set of constraints. The analytical formulation usually takes the form of a Constrained Markov Decision Process (CMDP). We focus on the case where the CMDP is unknown, and RL algorithms obtain samples to discover the model and compute an optimal constrained policy. Our goal is to characterize the relationship between safety constraints and the number of samples needed to ensure a desired level of accuracy -- both objective maximization and constraint satisfaction -- in a PAC sense. We explore two classes of RL algorithms, namely, (i) a generative model based approach, wherein samples are taken initially to estimate a model, and (ii) an online approach, wherein the model is updated as samples are obtained. Our main finding is that compared to the best known bounds of the unconstrained regime, the sample complexity of constrained RL algorithms are increased by a factor that is logarithmic in the number of constraints, which suggests that the approach may be easily utilized in real systems.

Abstract

Crucial performance metrics of a caching algorithm include its ability to quickly and accurately learn a popularity distribution of requests. However, a majority of work on analytical performance analysis focuses on hit probability after an asymptotically large time has elapsed. We consider an online learning viewpoint, and characterize the "regret" in terms of the finite time difference between the hits achieved by a candidate caching algorithm with respect to a genie-aided scheme that places the most popular items in the cache. We first consider the Full Observation regime wherein all requests are seen by the cache. We show that the Least Frequently Used (LFU) algorithm is able to achieve order optimal regret, which is matched by an efficient counting algorithm design that we call LFU-Lite. We then consider the Partial Observation regime wherein only requests for items currently cached are seen by the cache, making it similar to an online learning problem related to the multi-armed bandit problem. We show how approaching this "caching bandit" using traditional approaches yields either high complexity or regret, but a simple algorithm design that exploits the structure of the distribution can ensure order optimal regret. We conclude by illustrating our insights using numerical simulations.

Abstract

Introduction: Little is known about the clinical characteristics of women at increased risk for abnormal uterine bleeding (UB) during anticoagulation for venous thromboembolism (VTE). Methods: We used the RIETE registry to identify the baseline characteristics of women developing abnormal UB during anticoagulation. We used logistic regression analysis to identify independent predictors for abnormal UB. Then, we built a prognostic score to identify at-risk women. Results: From March 2001 through October 2022, there were 54,372 women with VTE. During anticoagulation (median, 181 days), 318 (0.6%) developed abnormal UB (major bleeding = 88, clinically relevant non-major (CRNM) = 230). On multivariable analysis, women aged 70 kg, with uterine cancer, recent UB, anemia, estrogen-related VTE, or receiving rivaroxaban or apixaban were at increased risk for abnormal UB. Using the prognostic score, 42,273 women (78%) were at low-risk, 8,828 (16%) intermediate-, and 3,271 (6.1%) at high-risk for abnormal UB. Their rates of abnormal UB were: 0.28 (95%CI: 0.23–0.35), 1.32 (95%CI: 1.07–1.61) and 7.12 (95%CI: 5.98–8.41) bleeds per 100 patient-years, respectively. The c-statistic was 0.80 (95%CI: 0.77–0.83). The rates of major UB were: 0.06 (95%CI: 0.04–0.09), 0.43 (95%CI: 0.30–0.60) and 1.85 (95%CI: 1.31–2.53) per 100 patient-years, respectively (c-statistic: 0.84; 95%CI: 0.80–0.89). The rates of CRNM uterine bleeding were: 0.21 (95%CI: 0.17–0.26), 0.85 (95%CI: 0.65–1.08), and 5.02 (95%CI: 4.09–6.10) bleeds per 100 patient-years, respectively (c-statistic: 0.78; 95%CI: 0.75–0.82). Conclusions: Using 7 variables easily available at admission, we built a prognostic score that reliably identified women with VTE at increased risk for abnormal UB during anticoagulation.

In living organisms the positron-electron annihilation (occurring during the PET imaging) proceeds in about 30% via creation of a metastable ortho-positronium atom. In this article we assess the feasibility for reconstruction of the mean ortho-positronium lifetime image based on annihilations into two photons. The main objectives of this work include: (i) estimation of the sensitivity of the total-body PET scanners for the ortho-positronium mean lifetime imaging using $2\gamma$ annihilations, and (ii) estimation of the spatial and time resolution of the ortho-positronium image as a function of the coincidence resolving time (CRT) of the scanner. Simulations are conducted assuming that radiopharmaceutical is labelled with $^{44}Sc$ isotope emitting one positron and one prompt gamma. The image is reconstructed on the basis of triple coincidence events. The ortho-positronium lifetime spectrum is determined for each voxel of the image. Calculations were performed for cases of total-body detectors build of (i) LYSO scintillators as used in the EXPLORER PET, and (ii) plastic scintillators as anticipated for the cost-effective total-body J-PET scanner. To assess the spatial and time resolution the three cases were considered assuming that CRT is equal to 140ps, 50ps and 10ps. The estimated total-body PET sensitivity for the registration and selection of image forming triple coincidences is larger by a factor of 12.2 (for LYSO PET) and by factor of 4.7 (for plastic PET) with respect to the sensitivity for the standard $2\gamma$ imaging by LYSO PET scanners with AFOV=20cm.
