Disasters frequently demand exceptional skills from medical responders. Providers work most efficiently and effectively, however, within the roles and hierarchical structures with which they are familiar. The goal of disaster medical response planners is to assign personnel to roles that are as familiar as possible and to simultaneously enhance flexibility of response to extraordinary circumstances. We have outlined the most common disaster medical response roles and the personnel types that fit most directly as a primary provider within each role. Medics excel in field operations and field care of patients, whereas the training of nurses and physicians makes them the most flexible all-around providers, if specially trained in field emergency care, and the sole providers of definitive care. None of the providers, by virtue of their basic training, is well equipped to manage the public health consequences of disasters, but nurses and physicians should be able to easily move into the role, given appropriate special training. Some of the special courses needed to make medics, nurses, and physicians capable of serving flexible roles already exist; others need to be developed or enhanced.

Objectives: To determine the incidence of alcohol use in subcritically injured patients presenting to the ED, by using a saliva alcohol test (SAT) at ED triage during the ED initial assessment; to compare the incidence of alcohol use revealed by the SAT with documentation of alcohol use by ED nurses and emergency physicians (EPs) blinded to the SAT results; and to describe the demographics of the SAT-positive, subcritically injured population. Methods: A blinded, prospective, observational evaluation of ED patients presenting with subcritical injuries was performed. The patients were tested for alcohol use with an SAT, and a subsequent record review was conducted for extraction of demographic data and evidence of documentation of alcohol use by ED nurses and EPs blinded to the SAT results. Results: During the study. 791 subcritically injured patients had SATs performed. Twenty-one percent of these patients were found to be alcohol-positive by SAT. Either the ED nurse or the EP documented a clinical impression of alcohol use for 52% of the SAT-positive patients. There were higher SAT-positive rates among men (24%), victims of assault (47%), and patients arriving at night (41%). Conclusions: While the SAT identified 21% of the subcritically injured patient population as alcohol-positive, ED nurse and EP documentation did not identify half of these alcohol-positive patients. Many of these patients may be at risk for additional injuries related to their drinking behavior.

Study objective Brief interventions (BI) for alcohol misuse and recently for marijuana use for emergency department patients have demonstrated effectiveness. We report a 12-month outcome data of a randomized controlled trial of emergency department (ED) patients using a novel model of BI that addresses both alcohol and marijuana use. Methods ED research assistants recruited adult patients who admitted alcohol use in the last month, and marijuana use in the last year. In the ED, patients received an assessment of alcohol and marijuana use and were randomized to treatment (n = 249) or standard care (n = 266). Treatment consisted of two sessions of BI. At 3 and 12 months, both groups had an assessment of alcohol and marijuana use and negative consequences of use. Results 515 patients were randomized. We completed a 12-month follow-up assessments on 83% of those randomized. Measures of binge drinking and conjoint marijuana and alcohol use significantly decreased for the treatment group compared to the standard care group. At 12-month binge alcohol use days per month in the treatment group were (M = 0.72:95% CI = 0.36–1.12) compared to standard care group (M = 1.77:95% CI = 1.19–1.57) Conjoint use days in the treatment group (M = 1.25.1:95% CI = 0.81–1.54) compared to standard care group (M = 2.16:95% CI = 1.56–2.86). No differences in negative consequences or injuries were seen between the treatment and standard care groups. Conclusions BI for alcohol and marijuana decreased binge drinking and conjoint use in our treatment group. BI appears to offer a mechanism to reduce risky alcohol and marijuana use among ED patients but expected reductions in consequences of use such as injury were not found 12 months after the ED visit.

Emergency departments and hospitals are being urged to implement onsite interventions to promote smoking cessation, yet little is known about the theoretical underpinnings of behavior change after a healthcare visit. This observational pilot study evaluated three factors that may predict smoking cessation after an acute health emergency: perceived illness severity, event-related emotions, and causal attribution. Fifty smokers who presented to a hospital because of suspected cardiac symptoms were interviewed, either in the emergency department (ED) or, for those who were admitted, on the cardiac inpatient units. Their data were analyzed using both qualitative and quantitative methodologies to capture the individual, first-hand experience and to evaluate trends over the illness chronology. Reported perceptions of the event during semistructured interview varied widely and related to the individual’s intentions regarding smoking cessation. No significant differences were found between those interviewed in the ED versus the inpatient unit. Although the typical profile was characterized by a peak in perceived illness severity and negative emotions at the time the patient presented in the ED, considerable pattern variation occurred. Our results suggest that future studies of event-related perceptions and emotional reactions should consider using multi-item and multidimensional assessment methods rated serially over the event chronology.

Peripheral intravenous catheter (PIV) placement is a common, painful, and frequently difficult procedure in children. The VeinViewer is a device that delineates subcutaneous veins using near-infrared light and video technology. To the best of our knowledge, the benefit of this device for PIV placement in children in the emergency department (ED) has not been studied.The authors enrolled a prospective, randomized sample of children aged 0 to 17 years who required a nonemergent PIV in a tertiary care pediatric ED. Participants were randomized to standard PIV cannulation (SC) or PIV cannulation with the VeinViewer (VV). The primary outcome measure was time to PIV placement. Secondary outcome measures included number of PIV attempts and pain scores as reported by the child, parent or guardian, and nurse using a 100-mm visual analog scale (VAS).A total of 323 patients completed the study: 174 boys and 149 girls. Age, sex, and body mass index (BMI) were not different between groups. There were no differences in time to PIV placement, number of PIV attempts, or pain scores for the overall study group. However, a planned subgroup analysis of children age 0 to 2 years (n = 107) did yield significant results for the geometric mean time to place the PIV (121 seconds [VV] vs. 167 seconds [SC], p = 0.047) and for nurses' perception of pain (median VAS 34 [VV] vs. 46 [SC], p = 0.01).While no results were significant for the overall study group, subgroup analysis of children age 0 to 2 years suggests that the VeinViewer may decrease the time to PIV placement.

Background Tobacco smoking remains the leading preventable cause of death among American women. Exercise has shown promise as an aid to smoking cessation because it reduces weight gain and weight concerns, improves affect, and reduces nicotine withdrawal symptoms and cigarette craving. Studies have shown that the practice of yoga improves weight control, and reduces perceived stress and negative affect. Yoga practice also includes regulation of breathing and focused attention, both of which may enhance stress reduction and improve mood and well-being and may improve cessation outcomes. Methods/Design This pilot efficacy study is designed to examine the rates of cessation among women randomized to either a novel, 8-week Yoga plus Cognitive Behavioral Therapy (CBT) smoking cessation intervention versus a Wellness program plus the same CBT smoking cessation intervention. Outcome measures include 7-day point prevalence abstinence at end of treatment, 3 and 6 months follow up and potential mediating variables (e.g., confidence in quitting smoking, self-efficacy). Other assessments include measures of mindfulness, spirituality, depressive symptoms, anxiety and perceived health (SF-36). Discussion Innovative treatments are needed that address barriers to successful smoking cessation among men and women. The design chosen for this study will allow us to explore potential mediators of intervention efficacy so that we may better understand the mechanism(s) by which yoga may act as an effective complementary treatment for smoking cessation. If shown to be effective, yoga can offer an alternative to traditional exercise for reducing negative symptoms that often accompany smoking cessation and predict relapse to smoking among recent quitters. Public health research requires sound design and thoughtful consideration of potential biases that may influence the validity of results. It also requires careful implementation of protocols and procedures that are likely to translate from the research environment to actual clinical practice. This article is the product of a breakout session from the 2009 Academic Emergency Medicine consensus conference entitled "Public Health in the ED: Screening, Surveillance, and Intervention" and serves to describe in detail aspects of performing emergency department (ED)-based public health research, while serving as a resource for current and future researchers. In doing so, the authors describe methodologic features of study design, participant selection and retention, and measurements and analyses pertinent to public health research. In addition, a number of recommendations related to research methods and future investigations related to public health work in the ED are provided. Public health investigators are poised to make substantial contributions to this important area of research, but this will only be accomplished by employing sound research methodology in the context of rigorous program evaluation.

The aim of this investigation was to assess emergency department (ED) patients' perceptions and preferences about an opt-in, universal, rapid HIV screening program and identify patient groups who expressed stronger beliefs about components of the testing program. From July 2005 to July 2006, ED patients in the opt-in, universal, rapid HIV screening program were interviewed in person. Multivariable regression models were used to compare participants on their beliefs about the program components. Of the 561 participants, 62.0% had previously been tested for HIV. The majority of participants (58.8%) believed the rapid and standard/conventional HIV tests to be equally accurate, 27.7% believed the rapid test to be less or much less accurate, and 8.7% believed the rapid test to be more or much more accurate. Almost two-thirds (65.1%) favored having a rapid instead of a standard/conventional HIV test, 94.6% wanted the test results within one hour, and 61.3% would be likely or very likely to undergo testing in the ED if it prolonged their ED visit. Almost all (92.5%) believed that their medical care was "not at all" delayed because of being tested, 94.1% believed that testing did "not at all" divert attention from the reason for their ED visit, and 80.9% thought that testing in the ED was "not at all" stressful. In multivariable logistic regression models, males and those with more than 12 years of formal education showed greater concerns about the rapid HIV test's accuracy. Hispanic/Latinos, participants with governmental insurance, and those previously HIV tested were more apt to be screened for HIV even if testing delayed their ED departure. Overall, participants were highly accepting of the components of this opt-in rapid HIV screening program. However, concerns regarding the accuracy of the rapid HIV test might limit test acceptance and should be addressed during pre-test information procedures.

Objectives. We assessed emergency department (ED) patient acceptance of opt-in, rapid human immunodeficiency virus (HIV) screening and identified demographic characteristics and HIV testing-history factors associated with acceptance of screening. Methods. A random sample of 18- to 55-year-old ED patients was offered rapid HIV screening. Patient acceptance or decline of screening and the reasons for acceptance or decline were analyzed with multivariable regression models. Odds ratios (ORs) with 95% confidence intervals (CIs) were estimated for the logistic regression models. Results. Of the 2,099 participants, 39.3% accepted HIV screening. In a multinomial regression model, participants who were never married/not partnered, did not have private health insurance, and had 12 or fewer years of education were more likely to be screened due to concern about a possible HIV exposure. In a multivariable logistic regression model, the odds of accepting screening were greater among those who were younger than 40 years old (OR=1.61, 95% CI 1.32, 2.00), nonwhite (OR=1.28, 95% CI 1.04, 1.58), not married (OR=1.82, 95% CI 1.44, 2.28), lacking private health insurance (OR=1.40, 95% CI 1.13, 1.74), and who had 12 or fewer years of education (OR=1.43, 95% CI 1.16, 1.75). Despite use of a standardized protocol, patient acceptance of screening varied by which research assistant asked them to be screened. Patients not previously tested for HIV who were white, married, and 45 years or older and who had private health insurance were more likely to decline HIV screening. Conclusions. In an opt-in, universal, ED HIV screening program, patient acceptance of screening varied by demography, which indicates that the impact of such screening programs will not be universal. Future research will need to determine methods of increasing uptake of ED HIV screening that transcend patient demographic characteristics, HIV testing history, and motivation for testing.

We sought to determine the extent to which adult female emergency department participants viewed two women’s cancer screening and two contraceptive measures negatively. The study also explored the relationship between having a negative opinion about these measures and participant demography, lack of knowledge, and lack of usage of these measures. Few women expressed negative opinions about these measures. Lack of knowledge about and lack of use of these measures were associated with having negative opinions on these cancer screening and contraceptive measures. Having any negative opinion about one cancer screening or contraceptive measure was associated with a higher risk of having any negative opinion on another measure. The results suggest that influencing opinion and knowledge about these measures might impact the success of emergency department-based cancer screening and contraceptive health programs. Editors’ Strategic Implications: Emergency departments (and primary care settings) provide key opportunities for prevention. Replication is needed, but the authors present important data on knowledge, attitudes, and characteristics that might influence women’s receptivity to consent to and engage in behaviors consistent with prevention, screening, and health promotion.

This study examines the efficacy of a smoking cessation intervention on abstinence rates and motivation to quit smoking. Participants were adult smokers (N = 543) who presented to the emergency department with chest pain and who were admitted to an observation unit for 24-hour observation to rule out myocardial infarction. Participants were randomly assigned to either usual care or a tailored intervention employing motivational interviewing and telephone follow-up. All individuals choosing to quit were offered nicotine patch therapy. Follow-up assessments were conducted at 1, 3 and 6 months. Abstinence (7-day point prevalence) rates were significantly greater among participants receiving the tailored intervention compared with those given usual care (OR = 1.62, 95% CI [1.05-2.50]). The largest difference occurred at 1 month: 16.8% of usual care and 27.3% of the tailored intervention group were abstinent, with differences decreasing over time. One-third of participants who were quit at month 6 were late quitters whose initial abstinence began after the 1-month follow up. In addition to treatment assignment, psychosocial variables including motivation to quit, confidence, reduced temptation to smoke in response to negative affect, and the perception that their chest pain was related to their smoking, were significant predictors of cessation. Tailored interventions are effective in promoting initial quit attempts for emergency chest pain patients admitted to an observation unit. Additional intervention may be needed to assist late quitters and to prevent relapse.

The objectives of this study were to estimate the incidence and identify the temporal patterns of visits to Rhode Island emergency departments (EDs) by adults who were sexually assaulted. Visits to all Rhode Island EDs from January 1995–June 2001 by adults who were sexually assaulted were identified using International Classification of Disease, Ninth Revision, Clinical Modification (ICD-9) codes. Medical records of these visits were reviewed. Frequency distributions of the assault characteristics and patient demographics were generated. Incidence rates (IRs) of ED visits after sexual assault were estimated using 2000 US Census data. Analyses of the temporal patterns of the ED visits after sexual assault were conducted. Of the 823 ED visits, 796 (96.7%) were by females and 27 (3.3%) were by males. The median age for females was 25 years (range, 18–96 years) and was 28 years (range, 18–87 years) for males. Among the female patients, 76.6% sustained a vaginal/anal assault. Among the male patients, 59.3% sustained an anal assault. The average annual IR of ED visits after sexual assault was 30.3/100,000/year for females and 1.2/100,000/year for males, which is a 25-fold greater incidence of these visits for females than males. ED visits after adult sexual assault were more frequent during warmer months and around 5 p.m. There was a gradual 43% increase in the IRs of ED visits after sexual assault over the 6.5-year period. These findings should help direct EDs to maximize supportive services when they are needed most often.

Background Police and correctional officers face the occupational hazard of blood and body fluid exposures, which carry the risk of infection with HIV. Aims To estimate the incidence rate (IR) of emergency department (ED) visits for blood or body fluid exposures sustained by police and corrections officers in an entire state and to quantify the utilization of HIV post-exposure prophylaxis (PEP) in response to these exposures. Methods A retrospective study of police and corrections officers presenting to EDs in Rhode Island between 1995 and 2001. The investigators estimated IRs of ED visits for these exposures with 95% confidence intervals and determined factors associated with HIV PEP using bivariate Pearson's χ2 analyses. Results The average annual incidence of ED visits for blood or body fluid exposures over the study period was IR 4.41 (2.31–6.51) exposures per 1000 police and corrections personnel. Only 15% of officers sustained percutaneous injuries or blood-to-mucous membrane exposures. Sixteen officers were offered HIV PEP and 10 accepted it. Offering of HIV PEP was 3.3-fold greater for officers sustaining percutaneous and blood-to-mucous membrane exposures instead of other body fluid exposures. Conclusion The incidence of ED visits for blood or body fluid exposures by police and corrections officers was low and most exposures did not have the potential for HIV transmission. HIV PEP was infrequently used for these exposures.

To determine how often sexually assaulted adult female emergency department (ED) patients are being offered testing and prophylaxis for sexually transmitted diseases (STDs) and pregnancy and identify factors associated with the offering of tests and prophylaxis.This is a retrospective study of ED visits for adult female sexual assault in all Rhode Island EDs from January 1995 through June 2001. The percentage of patients offered testing and prophylaxis was calculated. Multivariable logistic regression was used to identify factors related to testing and prophylaxis use for women sustaining an anal/vaginal assault.Of the 780 patients, 78.2% sustained anal/vaginal penetration, 5.0% genital touching only, and 3.7% oral sex only, and 13.1% did not know what happened to them. Of those women anal/vaginally assaulted, 83.8% were offered chlamydia/gonorrhea testing, 69.4% syphilis testing, 82.9% pregnancy testing, 77.0% chlamydia/gonorrhea prophylaxis, 47.6% emergency contraception, and 19.2% HIV prophylaxis. In multivariable logistic regression models, patients evaluated at the state's women's hospital instead of academic or community hospitals and those who sustained anal/vaginal assault instead of other assaults had a greater odds of being offered testing and prophylaxis. In some cases, older women were offered testing and prophylaxis less often than younger women.Testing and prophylaxis for chlamydia/gonorrhea were used much more often than prophylaxis for emergency contraception and HIV, even for patients who were anal/vaginally assaulted. Disparities in testing and prophylaxis exist by type of hospital and, in some cases, by age. Educational campaigns should be instituted to ensure that all women receive adequate testing and prophylaxis commensurate with the exposure they sustained from a sexual assault.

Depression is the most common psychiatric disorder among adolescents and is more prevalent among those seeking care in the emergency department (ED). However, adolescents are rarely screened for depressive symptoms in the pediatric emergency department (PED).To evaluate the sensitivity and specificity of one- and two-item screens for depressive symptoms compared to the 20-question Center for Epidemiologic Studies Depression Scale (CESD) among adolescents seeking care in a PED.This was a cross-sectional study of a convenience sample of adolescents 12-17 years old presenting to an urban PED with subcritical illness or injury. Participants completed three screening instruments: the two-question screen, the single-question screen, and the CESD.A total of 321 patients were approached to enter the study, of whom 212 (66%) agreed to participate. Seventy-eight (37%) of the study participants screened positive for depression on the CESD using a cutoff score ofor=16. The two-question screen had a sensitivity of 78% (95% confidence interval [CI] = 73% to 84%) and specificity of 82% (95% CI = 77% to 87%) for depressive symptoms compared with the CESD. The single-question screen had a sensitivity of 56% (95% CI = 50% to 63%) and specificity of 93% (95% CI = 90% to 96%) compared with the CESD.The two-question screen is a sensitive and specific initial screen for depressive symptoms in adolescents being seen in the PED. This quick, simple instrument would be ideal for use in the busy PED setting and would allow clinicians to identify adolescents who require more extensive psychiatric evaluation.

The objective of this study was to elucidate factors that predicted the initiation of HIV postexposure prophylaxis (PEP) for blood or body fluid exposures evaluated at Rhode Island emergency departments (EDs). The study involved a retrospective review of patient visits to all civilian Rhode Island EDs for these exposures from 1995 to mid-2001. Multivariate logistic regression models were created to evaluate predictors of the offering and the acceptance and receipt of HIV PEP from 1996 to 2001. The search identified 3622 patients who sustained a blood or body fluid exposure. Of these, 43.8% were health care workers (HCWs) and 57.2% were not HCWs. Most (52.0%) of the exposures were nonsexual. HIV PEP was offered to 21.0% and accepted and received by 9.4% of all patients. HIV PEP was offered more often after significant exposures, exposures to known HIV-infected sources, when time elapsed after the exposure was shorter, if the patients were HCWs, adults, presented to a teaching hospital, presented during the latter years of the study, or sustained nonsexual exposures. Once offered HIV PEP, patients who were male, adult, sustained a significant exposure, knew the source was HIV infected, sustained a nonsexual exposure, or were HCWs had a greater odds of accepting and receiving HIV PEP. Even when controlling for exposure significance, HIV status, and time elapsed since the exposure, several factors such as gender and type of hospital that are unrelated to the exposure appeared to influence the initiation of HIV PEP. ED providers should ensure that these factors do not inappropriately restrict its initiation.

Introduction:The demography of healthcare workers (HCWs) and non- HCWs seeking medical care at emergency departments after a non-percutaneous potential exposure to human immunodeficiency virus (HIV) duringcardiopulmonary resuscitation (CPR), the types and body locations of their exposures, the time elapsed from exposure to emergency department presentation, and usage of HIV-post-exposure prophylaxis (PEP) for these exposures are described.Methods:A retrospective study of emergency department patients who were exposed to blood or body fluids during CPR in Rhode Island from January 1995–June 2001 was performed.The demography, characteristics of the exposure, and HIV-PEP usage for these patients were compared, and the elapsed time from exposure to evaluation inthe emergency department was calculated.Results:Of the 39 patients exposed to non-percutaneous blood or body fluid during CPR, 22 were healthcareworkers (HCWs) and 17 were non-HCWs. Thirty-four patients sustained mucous membrane exposures. Most of the patients (69.2%) were exposed to saliva or sputum (p p p Conclusions:Nearly half of the patients who presented with non-percutaneous exposures acquired during CPR were not HCWs. Most of the exposures were to saliva or sputum and occurred on their mucous membranes. Continuing education programs on maintaining universal precautions to prevent blood or body fluid exposures and appreciating the benign nature of most non-percutaneous exposures possible during CPR are needed.

Objectives:Evidence and consensus on best practices on the management of human bite injuries is lacking. Our objective was to identify factors that are associated with delay to emergency department (ED) presentation, antibiotic usage and patient admission.Methods:We present a retrospective chart review of adults treated for human bites. Multivariable logistic regression models used demographic characteristics and bite circumstances and characteristics as factors associated with ED presentation more than 24 hours after the bite, antibiotic usage and hospital admission.Results:Of the 388 patients evaluated for a human bite, 66.5% were bitten during an altercation; 23.8% presented more than 24 hours after the bite; 50.3% were bitten on the hands or fingers, 23.5% on an extremity and 17.8% on the head or neck. Only 7.7% of all patients sustained closed-fist injuries; the majority had occlusional or other kinds of bites. The majority of patients (77.3%) received antibiotics and 11.1% were admitted to hospital. Patients who had greater odds of presenting more than 24 hours after the bite were black (odds ratio [OR] 1.79, 95% confidence interval [CI] 1.02–3.13), Hispanic (OR 2.68, 95% CI 1.22–5.89) and those who had a non-occupational bite (OR 3.87, 95% CI, 1.68–8.90). Patients had a greater chance of receiving antibiotics if they were bitten during an altercation (OR 1.87, 95% CI, 1.09–3.20) and were bitten on the hands or fingers (OR 2.23, 95% CI 1.31-3.80). Patients had a greater chance of being admitted to the hospital if they were bitten during an altercation (OR 4.91, 95% CI 1.65–14.64), bitten on the hands or fingers (OR 5.26, 95% CI, 1.74–15.87) and if they presented ≥ 24 hours after the bite.Conclusion:Most patients presented to the ED within 24 hours of their injury and received antibiotics. The circumstances surrounding the bite appeared to be associated with delay to ED presentation, receipt of antibiotics and admission to the hospital. There are ethnic background differences in delay to ED presentation. ED clinicians in our study favour antibiotic usage and admission based on the body location of the bite, despite little evidence to support these practices.

A substantial number of the injured patients who go to the emergency department (ED) for treatment will have alcohol use issues and may be intoxicated at the time of treatment (Becker et al., 1995; Cherpitel, 1999; Freedland et al., 1993), and they are at risk from continuing to experience adverse health and psychosocial consequences (D’Onofrio et al., 2005; Dinh-Zarr et al., 2000; McDonald et al., 2004). As ED treatment presents a brief opportunity for the health-care professional to screen for and to address patients’ hazardous alcohol use, it is important that emergency-care professionals determine the most efficacious way to help individuals who may be amenable to changing their alcohol-related behaviors and reduce the likelihood of future negative consequences (Crawford et al., 2004; D’Onofrio et al., 2005). Brief motivational interviewing (BMI) is a therapeutic approach that has been employed to change patient behavior in a variety of clinical settings. This brief client-centered intervention focuses on helping patients identify behaviors that need to be changed, increased patient ambivalence, motivation, and planning for changing identified behaviors (Field et al., 2005;Miller and Rollnick, 2002). A meta-analysis on the use of BMI in randomized clinical trials in primary care settings has demonstrated effectiveness in reducing alcohol consumption in patients when compared with standard care (Burke et al., 2003; Moyer et al., 2002; Rounsaville et al., 2001). A more recent review of the literature suggests that BMI is generally more effective than standard care in the ED for reducing hazardous alcohol use, such as binge drinking, and in reducing alcohol-related injuries and negative consequences (P. Nilsen et al., unpublished data). However, treatment regimes such as BMI should not only demonstrate efficacy but also should provide an explanation of the mechanisms by which the treatment facilitates change in the patient (Rounsaville et al., 2001). In reviewing research on the treatment components for alcohol use, Miller (1985) concluded that therapist characteristics were seldom investigated as a predictor of patient motivation and treatment compliance (Miller, 1985). In explaining how treatments such as BMI work, it is important to understand what works by also looking at therapists’ behaviors, rather than only focusing on the patient characteristics that moderate treatment. This current investigation was conducted as a secondary analysis of data from a large-scale randomized clinical trial which demonstrated that injured ED patients, randomized into BMI with a booster BMI session, experienced better long-term outcomes than ED patients receiving standard care (SC), including reduced alcohol-related injuries and negative consequences (Longabaugh et al., 2001). Using an intention-to-treat model, the original study compared treatment effectiveness of injured ED patients (n = 539) randomization to: (1) SC (n = 188), (2) 1 session of BMI delivered in the ED setting (BI; n = 182), and (3) 2 sessions of BMI (BIB; n = 169) (one delivered in the ED and a later booster session delivered outside of the ED). In the original study, all recruited participants were at-risk drinkers by virtue of having a score of 8 or greater in the AUDIT, and/or having alcohol in their system at the time of injury or ED visit. The Institutional Review Board of the hospital and university approved the original research protocol. The outcome data analyzed in the present paper were provided by those participants recruited into the study who were interviewed 12 months after their initial injury-related ED visit (84%). These follow-up interviews were conducted by research assistants who were blinded to participant group assignment. The original study demonstrated that those participants assigned to the BIB condition as compared with those assigned to the SC, had fewer alcohol-related injuries and negative consequences during the 12 months after treatment (Longabaugh et al., 2001). However, because only 68% of the participants assigned to the BIB condition actually received the second BMI session, it is important to examine why some participants did not complete the second BMI session and to determine if completion of the second BMI was important in predicting treatment outcomes. As participants were demonstrating a selection bias in deciding whether or not to return to complete the treatment assignment, it was important (1) to determine what effect that this lack of treatment completion had on participant outcomes and (2) to examine what therapists behaviors may have influenced the decisions of the participants to complete treatment, after we have ruled out competing theories of participant characteristics that were associated with treatment completion.

Female Emergency Department (ED) patients were surveyed to determine their comprehension of the concept of emergency contraception (EC), to assess how often they had used EC in comparison to other forms of contraception, and to learn which patients want the ED to provide EC services. Most of the 761 respondents were aged < 35 years (62.1%), never married (42.9%), had been pregnant at least once (70.2%), had never had an abortion (76.1%), had never used EC (90.6%), and had sex with a man within the past month (70.7%). Respondents were 2.5 times more likely to have had an abortion than to have used EC; 85.3% could not correctly answer two questions that assessed comprehension of the concept of EC; 43.1% wanted the ED to offer EC, 55.6% to provide information about EC, and 52.6% to refer patients for EC. Younger patients, those who attended religious services infrequently, patients who had ever used EC, and those at risk of pregnancy were more likely to want the ED to provide EC services.

The Preventive Health Task Force of the Society for Academic Emergency Medicine and other groups advocate for the implementation of selected preventive health screening procedures in emergency departments (EDs).1,2 Patient questionnaires are frequently used to assess knowledge about and need for these procedures. Policy decisions, such as the selection of interventions to create opportunities to provide these procedures in the ED, are partially based on presumptions about the quality of information that is obtained. Flaws, inadequacies, and limitations of question design affect the accuracy of participant responses, and thus the interpretation and understanding of respondent knowledge on the topics studied. Incorrect information impedes effective intervention design. Two common question formats utilized in patient surveys are the agree-disagree and the multiple-choice formats. Both formats have inherent problems that limit their effectiveness in evaluating respondent comprehension of the concepts being assessed. Agree-disagree questions restrict responses to a narrow range of extremes (agree vs. disagree) that do not allow respondents to fully express their beliefs, knowledge, or opinions; moreover, respondents have a tendency to choose “agree” when they do not fully understand the question.3,4 For multiple-choice question formats, limitations include the propensity to encourage guessing through selection of any response, the tendency of respondents to select either the first or last response options dependent on question length and complexity, and the inclination of respondents to select an incorrect response when the response option has part of the correct answer embedded in the response.3,4 Despite the limitations of multiple-choice format questions, they have a number of advantages. Multiple-choice format questions tend to be more reliable than true-false and open-ended response questions, allow for greater items of measurement within a single question, tend to be better predictors of performance than open-ended response questions, and can force the examinee to discriminate among alternatives to show degree of understanding of concepts.5–8 Acquiescence bias (also known as agreeing-response bias, agreement bias, or “yea-saying”) is the propensity for participants to agree with a statement or assertion presented to them regardless of its contents.9,10 This type of bias is believed to be an important limitation of agree-disagree and other Likert-scale format questions. Several study investigators have observed that the level of acquiescence bias in surveys varies by demographic group. Acquiescence bias in surveys has been shown to be more prominent among persons of lower socioeconomic class,11,12 nonwhite persons,13–17 those with fewer years of formal education,18,19 older persons,12 and among some cultures.10,12 Most of these studies use agree-disagree or other Likert scales to examine attitude and opinions and utilization of services instead of knowledge-based assessment. Some studies offer conflicting conclusions on the importance of demographic characteristics, and investigators debate whether or not acquiescence bias truly affects interpretation of survey results.9,20,21 It is at least likely that the role of acquiescence bias varies by type of survey, context of the survey, nature of the question, response format, and form of administration of the survey. We were interested in examining whether female ED patients were more likely to respond correctly to questions in a preventive health knowledge survey relevant to women when they were in an agree-disagree format instead of a multiple-choice format. The survey included an assessment of participant knowledge on three cancer screening (Pap smears, breast self-examinations [BSEs], and mammograms) and two contraceptive (birth control pills [BCPs] and emergency contraception [EC]) measures. The survey evaluated participant knowledge on these topics using paired versions of questions in two formats: the agree-disagree format and the multiple-choice format. We also investigated if patient demography (age group, race/ethnicity, and insurance status) was associated with a tendency for participants to correctly respond to knowledge-based questions when they were in an agree-disagree format instead of a multiple-choice format. We hypothesized that if an agree-disagree formatted survey was relied on to obtain participant knowledge about the preventive health measures, respondent knowledge would be overestimated, that is, acquiescence bias would be a limiting factor in assessing knowledge when an agree-disagree response format was used. Further, important differences in participant knowledge according to patient demography would be masked.

This study examined predictors of readiness to quit smoking among emergency chest pain patients admitted to the observation unit (OU) to rule out myocardial infarction. While in the OU, patients (n=543) completed surveys assessing smoking history, nicotine dependence, readiness to quit, and other relevant variables. Participants smoked an average of 18.8 (SD=12.6) cigarettes per day. More than half (58%) had made at least 1 serious quit attempt > or = 24 hours) in the past year. Most had never used nicotine replacement medications. Nicotine dependence, perceived risk from smoking, and patient perceptions that smoking might be related to their chest pain were significantly associated with readiness to quit (P

Little is known about treatment process for behavior change for brief interventions. Patient ratings of treatment process during a patient-centered brief intervention for alcohol were used to predict post-treatment alcohol use and consequences. We use data from a randomized clinical trial that compared the effects of BI (1 session) to BI and booster (BIB, 2 sessions) to reduce harmful drinking and alcohol consequences. Subjects were n=167 (BI) and n=82 (BIB). Five of the 12 ratings were rated significantly higher by those in the BIB condition compared to BI. The only predictor of reduced alcohol consequences at 12-months was higher ratings of, “I have obtained some new understanding,” for BIB participants (t=−2.50, p < .05). Patient perspectives on treatment may have a role in patient outcomes and should be explored as a dimension of treatment process.

Alcohol, tobacco, and other drug use is a significant societal problem. Individuals who use these substances are frequently seen in emergency departments at rates disproportionately greater than their population prevalence. This article highlights the impact of these drugs on patients and on emergency departments, including common presenting problems of individuals on these substances. Also discussed is how to help and to refer individuals with substance abuse problems through brief motivational interventions.

Objectives: Emergency Medicine Patients' Access To Healthcare (EMPATH) was a cross-sectional, observational study conducted to identify the principal reasons why patients seek care in hospital emergency departments (EDs) in the United States. Methods: Twenty-eight U.S. hospitals, stratified by geographic region and hospital characteristics, participated in this study. Demographic, clinical, and insurance data were collected for a 24-hour period at each site, using chart reviews and a structured interview administered to all consenting adult patients seeking treatment during that period. Patients' reasons for presenting to the ED were assessed by their level of agreement (on a three-point Likert scale) with 21 carefully worded statements designed to capture a range of possible reasons for seeking care in the ED. Factor analysis was used to consolidate highly correlated responses and to identify the principal factors explaining patients' reasons for coming to the ED. Results: A total of 1,579 patient interviews and 2,004 chart reviews were obtained from a diverse sample that was 55.4% female, 58.3% white, 28.3% African American, 7.0% Hispanic, and 6.0% other ethnic groups. This exploratory analysis yielded five factors characterizing patients' principal reasons for seeking ED care, with medical necessity the most frequent, followed by ED preference, convenience, affordability, and limitations of insurance. Conclusions: Use of the ED is, for most people, an affirmative choice over other providers rather than a last resort; it is often a choice driven by lack of access to or dissatisfaction with other sources of care.

Although the Short Index of Problems (SIP) is often used, little is known about the psychometric properties of the SIP in special populations. The present study seeks to determine the following: (1) whether it is possible to substitute items to enhance the psychometric properties of the SIP and (2) whether the SIP, or improved scale, is as sensitive as the Drinker Inventory of Consequences (DrInC) to assess intervention effectiveness.The sample consisted of 404 injured patients who were treated in the Emergency Department (ED) of a major teaching hospital that serves southern New England. Three approaches were used to guide development of the 3-month SIP-R, the potential alternative to the SIP. Cronbach's alpha assessed intrascale reliability; hierarchical multiple regression assessed construct validity; performance of the scales assessing intervention change were compared to the total 3-month DrInC as a function of intervention using analysis of covariance (ANCOVA).There was no evidence that changing the current SIP items will significantly improve performance. The 3-month SIP performed as well as the 3-month DrInC-2R in predicting 12-month DrInC scores and in determining intervention change at 12 months. Of the 45 DrInC items, 31 also predicted a difference across intervention groups.These results suggest there is no advantage to changing the current SIP items. The 3-month SIP is a psychometrically sound measure for assessing consequences of alcohol consumption in an ED sample and is almost as sensitive to intervention change as the full DrInC.

Study objective: This study compares the effect of a brief motivational intervention for alcohol plus a booster given to emergency department (ED) patients with subcritical injuries from a motor vehicle crash with the effect of brief motivational intervention for alcohol plus a booster in patients treated for non-motor vehicle crash-related injuries. Methods: A randomized controlled trial (n=539) was conducted at an urban Level I trauma center of brief intervention (1 ED session of brief intervention), brief motivational intervention for alcohol plus a booster (1 ED session plus booster session), or standard care for injured ED patients with an alcohol use problem who were being discharged home. At 12 months, alcohol-related negative consequences and injuries were measured. We performed a secondary analysis comparing motor vehicle crash–injured patients and non–motor vehicle crash–injured patients in the study sample. Results: Subcritically injured ED patients with harmful or hazardous alcohol use who received brief motivational intervention for alcohol plus a booster had fewer alcohol-related negative consequences and alcohol-related injuries than those receiving brief intervention or standard care at 12-month follow-up (previously reported). A secondary analysis of this result showed that motor vehicle crash patients (n=133) given brief motivational intervention for alcohol plus a booster (n=34) had fewer alcohol-related injuries than those receiving standard care (n=46; P=.001). Moreover, there were no significant differences in alcohol-related injuries among the non–motor vehicle crash–injured patients who received brief intervention or standard care. Conclusion: Brief motivational intervention for alcohol plus a booster is a useful intervention for subcritically injured ED patients with harmful or hazardous alcohol use. Objectives: To determine rates of alcohol and tobacco use among independent elder emergency department (ED) patients and assess the extent of health care use of this population. Methods: A convenience sample of independent elders (age ≥65 years) in an urban academic ED was enrolled. Patients were excluded if they were medically unstable or had a change in mental status. The Fagerstrom Test for Nicotine Dependence, and the Alcohol Use Disorders Identification Test (AUDIT) scales were used to measure tobacco and alcohol use. Subjects completed questionnaires about their health and use of the health care system. Data were analyzed by using t-tests to compare independent variables. Results: A total of 565 subjects completed the study. Of these, 296 (52.4%) were male and 269 (47.6%) were female; mean age was 77.1 years. Fifty-four (9.5%) were smokers, and 22 (3.9%) were nicotine-dependent by the Fagerstrom test (Fagerstrom+). Alcohol use was reported at least once monthly by 176 (31.2%) and twice monthly by 76 (13.5%) patients; 12 (2.1%) were alcohol-dependent by the AUDIT scale (AUDIT+). Two (0.35%) were both Fagerstrom+ and AUDIT+. Fagerstrom+ subjects visited a physician less often than Fagerstrom− subjects (3.9 vs. 4.6 annual visits, p < 0.0009). AUDIT+ subjects visited a primary care physician less (3.3 vs. 4.2 annual visits, p < 0.007) or “any” physician less (3.9 vs. 4.6 annual visits, p < 0.01) than AUDIT− subjects. AUDIT+ and Fagerstrom+ subjects did not differ from AUDIT− and Fagerstrom− subjects in number of annual ED visits, self-reported general health, physical symptoms (except nervousness, p < 0.004), comorbid illnesses, hospital admissions, and injuries requiring treatment. Conclusions: Elder ED patients have low rates of nicotine and alcohol dependence. Nicotine- or alcohol-dependent elders use outpatient providers less often than nondependent elders but use EDs at the same rate and report similar health patterns.

To compare the "Bellows on Sternum Resuscitation" (BSR) device that permits simultaneous compression and ventilation by one rescuer with two person cardiopulmonary resuscitation (CPR) with bag-valve-mask (BVM) ventilation in a single blind crossover study performed in the laboratory setting. Tidal volume and compression depth were recorded continuously during 12-min CPR sessions with the BSR device and two person CPR. Six CPR instructors performed a total of 1,894 ventilations and 10,532 compressions in 3 separate 12-min sessions. Mean tidal volume (MTV) and compression rate (CR) with the BSR device differed significantly from CPR with the BVM group (1242 mL vs. 1065 mL, respectively, p = 0.0018 and 63.2 compressions per minute (cpm) vs. 81.3 cpm, respectively, p = 0.0076). Error in compression depth (ECD) rate of 9.78% was observed with the BSR device compared to 8.49% with BMV CPR ( p = 0.1815). Error rate was significantly greater during the second half of CPR sessions for both BSR and BVM groups. It is concluded that one-person CPR with the BSR device is equivalent to two-person CPR with BVM in all measured parameters except for CR. Both groups exhibited greater error rate in CPR performance in the latter half of 12-min CPR sessions.

Objective: The study aim was to test whether a brief mo- tivational intervention, with or without a booster session, would improve drinking-related outcomes more than standard Emergency Department (ED) treatment. Method: The study population consisted of 539 (78% male) injured patients treated in the ED and discharged to the commu- nity following their treatment. Injured patients met inclusion criteria if they were assessed as hazardous or harmful drinkers by scoring eight or more on the AUDIT and/or having alcohol in their system at the time of their injury or ED visit. Patients were randomly assigned to either standard care (SC), brief intervention (BI) or brief intervention plus a booster session (BIB). At 1-year follow-up, 447 patients (83% of the sample) were re-interviewed to measure alcohol-related negative con- sequences, injuries and drinking. Results: Patients receiving BIB, but not BI patients, reduced alcohol-related negative consequences and alcohol-related injuries more than did those in the SC group. All three groups reduced their days of heavy drinking. Patients with histories of hazardous drinking responded to BIB, whether or not they had consumed alcohol prior to their injury. Conclusions: Together, these results indi- cate that the effects of a booster session that is added to a brief inter- vention in the ED can be helpful to injured patients with a history of hazardous or harmful drinking, irrespective of whether they have con- sumed alcohol prior to their injury. Alcohol and substance use constitute a significant health risk for people with disabilities. Rates of alcohol and substance use are higher among individuals with disabilities, often play a contributing role in the onset of injury-related disability, and severely disrupt the effectiveness of post-injury rehabilitation. This ethnographic study of 24 youths participating in a 5-week residential rehabilitation program confirms a high prevalence of problematic use of alcohol and substances that create psychological and physiological barriers to rehabilitation. Family history and pre-injury alcohol and substance use were common among study participants; however, social needs appeared to significantly contribute to continued alcohol and substance use. This paper presents the results of this research employing individual case examples to explore these dimensions of alcohol and substance use among college-age youths with disabilities and discusses opportunities for alcohol and substance use intervention.

The feasibility of the emergency department (ED) as a setting for smoking interventions was examined among 159 adult patients presenting with chest pain (38% were smokers). Subjects had been admitted to a 24-h observation unit (OU) to rule out myocardial infarction. We examined the frequency and extent of physician interventions for smoking using the AHCPR guidelines as a model. We also assessed patients' perceptions of risk from smoking and motivation to quit. Results indicate that ED physicians provided incomplete and inconsistent intervention. While most patients were asked if they smoked, only half were advised to quit, and few were offered assistance with quitting. Perceived risk from smoking was low among almost half of all smokers. However, over three-quarters were willing to receive smoking cessation counseling while in the ED. The ED may be an opportune setting in which to initiate smoking cessation counseling for certain high-risk populations.

Objective. To determine the prevalence of domestic violence against mothers in a pediatric emergency department and the relationship of their children to the abusers. Design. Cross-sectional survey of a convenience sample of mothers seeking treatment for their children. Setting. An urban pediatric emergency department. Participants. A total of 157 mothers with children Results. A total of 52% of women reported histories of adult physical abuse, 21% reported adult sexual abuse, and 28% reported childhood sexual abuse. A total of 10% of women were in abusive relationships in the past year. Victims of adult physical abuse were more likely to report histories of adult sexual abuse (relative risk [RR]: 4.93) or childhood sexual abuse (RR: 3.13). Intimate partners perpetrated 67% of physical abuse and 55% of sexual abuse. Relatives perpetrated 66% of childhood sexual abuse. Women who revealed histories of childhood sexual abuse were more likely to report adult sexual abuse (RR: 4.93). A total of 40% of the perpetrators of adult physical abuse, 73% of the perpetrators of past year physical abuse, and 10% of the perpetrators of adult sexual abuse had regular contact with their victims' children. Health care providers screened only 21% of the women for past violence. Victims of domestic violence were no more likely to have been screened than those without histories of physical or sexual abuse. Conclusions. Mothers of young patients in a pediatric emergency department are often victims of domestic violence. Perpetrators are often close relatives and thus place the victims' children at risk for abuse and for the psychological trauma of witnessing violence. Given the prevalence of domestic violence, families may benefit from routine violence screening and interventions in pediatric emergency departments.

Objectives: 1 ) To cluster patients according to self-reported drinking patterns using cluster analysis; 2) to externally validate clustered groups on variables related to drinking but not used in the cluster analysis; and 3) to use the clustered patients’ responses to alcohol consumption questions to develop a brief screening tool emergency physicians can use to identify patients in need of referral or intervention related to potentially hazardous alcohol consumption. Methods: A self-report battery was administered to 95 subcritically injured patients. Patients also were saliva alcohol-tested upon arrival to the ED. Using the patients’ self-reported quantity, frequency of alcohol consumption, and frequency of having 26 drinks on a drinking occasion, patients were categorized into 3 groups using cluster analysis. The 3 clusters were externally validated using injury-related variables, alcohol-related consequences, and the patients’ reported readiness to change drinking. A screening tool was developed using cutoff values reported by the patients’ answers to drinking pattern questions. Results: Fifty-nine patients were alcohol-negative, and 36 tested alcohol-positive (i.e., >4 mmol/L [>20 mgl dL]) or had elevated scores on an alcohol problem screening instrument. Three distinct drinking pattern clusters were found. Clusters were validated using discriminant function analysis and multivariate analyses of variance to confirm cluster classifications. Steady and high-intensity drinkers reported more alcohol-related negative consequences, and high-intensity drinkers indicated they would consider changing their drinking. The screening tool correctly classified 97% of the patient sample into their respective clusters. Conclusions: Using the drinking pattern questions in the clustering procedure was effective for grouping injured patients into clusters that could be differentiated on other drinking-related variables. The resulting screening tool can be used in the ED setting to screen patients for further assessment and intervention. The readiness-to-change results support the assertion that the injury event provides a “teachable moment” for subcritically injured patients whose injury may be related to their alcohol consumption.

A study was undertaken to determine the relationship between temperature and delivery rate of warmed intravenous fluid using standard intravenous infusion equipment and tubing. One-liter bags of 0.9% NaCl were warmed to 60 degrees C and run through standard microdrip tubing for 1 hour at rates of 1,000, 800, 600, and 400 mL/h. Thermistor probes were placed into the bag and into the tubing at 0, 100, 180, 230, and 280 cm from the intravenous bag. Separate fluid bags were also warmed to 39.3 degrees and 75 degrees C, and the fluid was run through the same apparatus at 1,000 mL/h and 200 mL/h, respectively. Temperatures were recorded at each site at the start of the infusion and every 10 minutes thereafter for 1 hour, Subsequently, 60-mL syringes of fluid warmed to 39.5 degrees C were eluted through 50 cm tubing over 10 minutes at 300 mL/h and 360 mL/h. Mean delivery temperature over each 10-minute infusion was determined. Fluid preheated to 39.3 degrees C approached room temperature at delivery even at a flow rate of 1,000 mL/h and tubing lengths as short as 100 cm. Fluid preheated to 60 degrees C was delivered at near 37 degrees C using tubing lengths as long as 280 cm when eluted at 1,000 mL/h. Fluid preheated to 39 degrees C in 60-mL syringes and eluted through 50 cm of tubing over a period of 10 minutes at 300 mL/h or 360 mL/h was delivered near a mean temperature of 37 degrees C. These results show that warmed fluid can be delivered through standard intravenous tubing at or near 37 degrees C if the fluid is preheated to 60 degrees C and eluted through long tubing (280 cm) at high flow rates (1,000 mL/h). Alternatively, fluid warmed to 37 degrees C to 42 degrees C can be delivered at or near 37 degrees C via intermittent bolus through short tubing (50 cm) either by hand or syringe pump. The latter approach would be particularly beneficial in the pediatric population, in whom it is not advisable to administer fluid at flow rates as high as 1,000 mL/h.

A study was conducted to determine the accuracy of tympanic thermometers for measuring the temperature of warmed fluids in fluid bags and in tubing at the delivery site (ie, beside the intravenous [IV] catheter). One-liter 0.9% saline bags were warmed in a microwave oven. A thermocouple electronic temperature probe was then used to measure the reference temperature. The probe was inserted into each bag and bathed in the fluid. Temperature changes were recorded simultaneously over a 20-minute period using the probe and a First Temp Tympanic Thermometer (Intelligent Medical Systems, Inc, Carlsbad, CA). The warmed fluid was then allowed to run through microdrip IV tubing. Temperature of the effluent was measured in the tubing using the tympanic thermometer externally and the probe internally at the same point. The two measures were compared using linear regression and Student's t tests. Overall, the correlation between the two probes was r = 0.99 for both the fluid bags and the IV tubing. The overall mean differences were small, 0.7°C and 1.2°C for the bags and tubing, respectively, but they were statistically different ( P > .05). Data were analyzed in three temperature ranges: 41°C. Again, small differences were found on the order of 1°C. It was concluded that infrared thermometry is an accurate method for measuring the initial and delivery temperature of warmed fluids. Although tympanic thermometer measurements were statistically different from reference readings in certain temperature ranges, these differences were small and not clinically significant. Tympanic thermometers can measure the temperature of warmed fluid bags and lavage and IV effluent adjacent to the catheter site, ensuring that hypothermic patients receive fluid at therapeutic temperatures.

This exhibit will use DVD footage, written material, and live real-time interaction to demonstrate an innovative methodology to teaching residents and medical students to gain comfort and expertise in communicating with and taking a history from challenging patients in the ED. I teach this course at our institution every year. Professional actors with improvisational experience play out unscripted interactions with students based on a character or characters, a set scene, and predefined goals and endpoints. Their dialogue and choices evolve from the student's response and the tone, direction, and approach that the student takes. These scenarios are much more realistic and educational than OSCEs which are often described as artificial and flat. Some of the scenarios that I will present include: Daughter trying to make End-of-life choices for her mother, type A man with chest pain trying to sign out, a woman with “dental pain” who is seductive and drug-seeking, an intoxicated attending MD brought in from a motor vehicle crash (MVC) who uses power in a manipulative way. Many of the scenarios contain potential ethical, sexual, gender, racial issues that the student must address along with the medical problems.

Study objectives: A broad sample of 18- to 55-year-old women seeking treatment at a high-volume, urban emergency department (ED) were invited to participate in a study with the goals of (1) defining the extent of their knowledge on the purpose and suggested intervals of preventive health screening modalities and contraceptive and sexually transmitted disease preventive techniques relevant to women; and (2) determining whether demographic characteristics and medical history predict their knowledge on these topics. Methods: Using published surveys on women's health topics as a template, the authors created an anonymous, written, multiple-choice, self-administered, English-language survey about Papanicolaou tests, breast self-examinations, mammograms, condoms, emergency contraception, and birth control. After the instrument was pilot tested, surveys were administered to 18- to 55-year-old English-speaking women seeking treatment in our ED during 66 8-hour shifts in July and August 2002, 32 4-hour shifts in October and November 2002, and 32 4-hour shifts in April and May 2003. To reflect the patient volume patterns for the ED, each day of the week was sampled equally, and shifts were sampled so that 25% of the 7 am to 11 am shifts, 50% of the 11 am to 11 pm shifts, and 25% of the 11 pm to 7 am shifts were represented. In addition to demographic questions, the women were asked about their medical history, including use of screening modalities, contraceptive and sexually transmitted disease preventive techniques, and knowledge-based questions on the "who, why, when, and how often" of the screening and preventive techniques recommended use. Surveys were entered into an Epi Info 2002 database and analyzed in STATA 7. Analyses included summary statistics and constructing multivariable logistic regression models to predict correct responses to the knowledge-based questions by using demographic characteristics and medical history variables as potential predictors. Results: Of 1,203 women approached to participate in the survey, 1,101 spoke English, and 780 (70.8%) of these agreed to complete all or part of the questionnaire. The majority of respondents were white, single, Catholic, and without health insurance. Most (62%) of the women in the sample were younger than 35 years ( P P Conclusion: Although women in this age group demonstrated strong knowledge on some preventive health topics, there were clear areas of knowledge deficit, particularly about mammograms and emergency contraception. Younger, nonwhite women without private insurance appear less likely to understand the purpose of and recommended intervals for preventive health screening modalities and contraceptive and sexually transmitted disease preventive techniques. During interaction with these subgroups, emergency physicians should be mindful of potential misunderstandings on these topics and address them accordingly.
