Abstract

Background There are no Italian data regarding the strategies for preventing neonatal group B streptococcal (GBS) infection. We conducted a national survey in order to explore obstetrical, neonatal and microbiological practices for the GBS prevention. Methods Three distinct questionnaires were sent to obstetricians, neonatologists and microbiologists. Questionnaires included data on prenatal GBS screening, maternal risk factors, intrapartum antibiotic prophylaxis, microbiological information concerning specimen processing and GBS antimicrobial susceptibility. Results All respondent obstetrical units used the culture-based screening approach to identify women who should receive intrapartum antibiotic prophylaxis, and more than half of the microbiological laboratories (58%) reported using specimen processing consistent with CDC guidelines. Most neonatal units (89 out of 107, 82%) reported using protocols for preventing GBS early-onset sepsis consistent with CDC guidelines. Conclusions The screening-based strategy is largely prevalent in Italy, and most protocols for preventing GBS early-onset sepsis are consistent with CDC guidelines. However, we found discrepancies in practices among centers that may reflect the lack of Italian guidelines issued by public health organizations.

Abstract

Leber's hereditary optic neuropathy (LHON) affects the optic nerve as well as other structures of the body, including the heart. A striking feature of the genetic disorder is the rapidity with which vision is destroyed, in contrast to many inherited disorders which progress more slowly. LHON results from a genetic defect in the mitochondria, and therefore the disease is inherited only from carrier females. A male sufferer cannot pass the condition to his children, since the mitochondria in the sperm do not contribute at all to a resulting embryo. To learn more of the genetic and physiological aspects of LHON, a study was conducted on three patients all belonging to the same Italian family. Genetic studies of the DNA from the mitochondria revealed a mutation within the DNA located at the same point as mutations that have been reported for patients from families in North America and Japan. Since mitochondria are responsible for the oxidative metabolism of food energy sources to produce a cell's energy, it might be expected that biochemical abnormalities might be present in the cells of patients with LHON. It was not possible to identify any biochemical abnormalities in these patients. However, using magnetic resonance spectroscopy, it was possible to show that the response of the muscle to exercise was abnormal in the three patients. Similarly, using magnetic resonance spectroscopy on the living brain to measure the relative amounts of different forms of phosphate in the brain, it was found that the brain had decreased chemical energy reserves in the patients with LHON. These changes are consistent with a mutation in mitochondrial genes. The mutation in LHON appears to alter a protein which serves as a key part of a biochemical structure called complex I. This structure is critical to the function of the mitochondria and the production of energy; it seems likely that in LHON complex I is altered but not destroyed. These observations do not, however, explain the peculiar finding that LHON is more common in men than women. Since all cells contain and require mitochondria, why men should be more susceptible to the consequences of mitochondrial abnormalities is not clear. Abstract

Background: Persistent inflammation, immunosuppression and catabolism syndrome (PICS) has been described in critically ill adults and may contribute to unfavourable outcomes. The present study aimed to describe and characterise PICS in critically ill children (PICS‐ped) and to verify its association with clinical outcomes. Methods: A prospective longitudinal study was conducted in a paediatric intensive care unit (PICU) with children aged between 3 months and 15 years. PICS‐ped, based on adult definition, was described. PICS‐ped was defined as PICU length of stay >14 days; C‐reactive protein > 10.0 mg L−1; lymphocytes <25%; and any reduction of mid‐upper arm circumference Z‐score. Clinical, demographic, nutritional status, nutrition therapy parameters and clinical outcomes were assessed. Statistical analysis comprised Mann–Whitney and Fisher's chi‐squared tests, as well as logistic and Cox regression. P < 0.05 was considered statistically significant. Results: In total, 153 children were included, with a median age of 51.7 months (interquartile range 15.6–123.4 months), and 60.8% male. The mortality rate was 10.5%. The prevalence of PICS‐ped was 4.6%. Days using vasoactive drugs and days using antibiotics were associated with PICS‐ped. PICS‐ped was associated with mortality in crude (odds ratio = 6.67; P = 0.013) and adjusted analysis (odds ratio = 7.14; P = 0.017). PICS‐ped was also associated with PICU and hospital length of stay, as well as duration of mechanical ventilation. Similar results were found in a subset of critically ill children who required mechanical ventilation for more than 48 h. Conclusions: Children with PICS‐ped required antibiotics or vasoactive drugs for a longer period. PICS‐ped was associated with poor clinical outcomes in critically ill children. 96. MicroRNA expression analysis identifies a subset of downregulated miRNAs in ALS motor neuron progenitors

Abstract

Amyotrophic lateral sclerosis (ALS) is a fatal neurological disorder that is characterized by a progressive degeneration of motor neurons (MNs). The pathomechanism underlying the disease is largely unknown, even though increasing evidence suggests that RNA metabolism, including microRNAs (miRNAs) may play an important role. In this study, human ALS induced pluripotent stem cells were differentiated into MN progenitors and their miRNA expression profiles were compared to those of healthy control cells. We identified 15 downregulated miRNAs in patients' cells. Gene ontology and molecular pathway enrichment analysis indicated that the predicted target genes of the differentially expressed miRNAs were involved in neurodegeneration-related pathways. Among the 15 examined miRNAs, miR-34a and miR504 appeared particularly relevant due to their involvement in the p53 pathway, synaptic vesicle regulation and general involvement in neurodegenerative diseases. Taken together our results demonstrate that the neurodegenerative phenotype in ALS can be associated with a dysregulation of miRNAs involved in the control of disease-relevant genetic pathways, suggesting that targeting entire gene networks can be a potential strategy to treat complex diseases such as ALS.

Abstract

Multiple System Atrophy is a severe neurodegenerative disorder which is characterized by a variable clinical presentation and a broad neuropathological spectrum. The pathogenic mechanisms are almost completely unknown. In the present study, we established a cellular model of MSA by using fibroblasts’ primary cultures and performed several experiments to investigate the causative mechanisms of the disease, with a particular focus on mitochondrial functioning. Fibroblasts’ analyses (7 MSA-P, 7 MSA-C and 6 healthy controls) displayed several anomalies in patients: an impairment of respiratory chain activity, in particular for succinate Coenzyme Q reductase (p < 0.05), and a reduction of complex II steady-state level (p < 0.01); a reduction of Coenzyme Q10 level (p < 0.001) and an up-regulation of some CoQ10 biosynthesis enzymes, namely COQ5 and COQ7; an impairment of mitophagy, demonstrated by a decreased reduction of mitochondrial markers after mitochondrial inner membrane depolarization (p < 0.05); a reduced basal autophagic activity, shown by a decreased level of LC3 II (p < 0.05); an increased mitochondrial mass in MSA-C, demonstrated by higher TOMM20 levels (p < 0.05) and suggested by a wide analysis of mitochondrial DNA content in blood of large cohorts of patients. The present study contributes to understand the causative mechanisms of Multiple System Atrophy. In particular, the observed impairment of respiratory chain activity, mitophagy and Coenzyme Q10 biosynthesis suggests that mitochondrial dysfunction plays a crucial role in the pathogenesis of the disease. Furthermore, these findings will hopefully contribute to identify novel therapeutic targets for this still incurable disorder.

Abstract

Background: To assess Guillain-Barré syndrome (GBS) incidence we relied on the Italian Network for the study of GBS (ITANG) established in 2010 in 7 Italian regions to analyse the association between influenza vaccination and GBS. Methods: All individuals aged ≥18 years, presenting with clinical manifestations that suggested GBS according to the universally accepted Asbury's diagnostic criteria (1990) were prospectively notified to a centralised database by ITANG neurologists over the period October 1, 2010-September 30, 2011. Through a telephone survey, 9 trained interviewers followed up the cases to diagnosis and then for 1 year since hospital discharge. Validation of case reporting was performed with the support of administrative data in 5 regions. Results: We found 365 cases fulfilling the definition for GBS or one of its variants over 19,846,068 population ≥18 years of age, yielding an annual incidence rate of 1.84 per 100,000 (95% CI 1.65-2.03), 2.30 (95% CI 1.99-2.60) in men and 1.41 (95% CI 1.18-1.64) in women. A highly significant peak of incidence was observed in February 2011 as compared to reference month (September 2011, rate ratio 3.3:1, p < 0.01). Conclusions: In Italy, GBS incidence was among the highest reported in Europe and higher than previously observed in Italian studies.
