Abstract

Diagnosis of cow’s milk allergy (CMA) in infants is based on reproducible symptoms and absence of symptoms on elimination of cow’s milk protein.1,2 Previous research has raised concerns that CMA guidelines published by national or international organisations could be driving CMA overdiagnosis.1 Personal testimonies by mothers suggest that a diagnosis of suspected CMA may reduce breastfeeding confidence, especially when maternal dietary restrictions are advised,2 and the potential for this to lead to cessation of breastfeeding has been suggested by earlier comments by experts in the fields of breastfeeding and CMA.2,3 Dietary restrictions are not likely to be needed for most breastfed infants with CMA, since quantities of cow’s milk protein transferred to the infant via breastmilk are much smaller than thresholds of reactivity in children with CMA.

Abstract

Background: The European Academy of Allergy and Clinical Immunology (EAACI) is in the process of updating the guidelines on the diagnosis and management of food allergy. The existing guidelines are based on a systematic review of the literature until 30 September 2012. Therefore, a new systematic review must be undertaken to inform the new guidelines. This systematic review aims to assess the accuracy of index tests to support the diagnosis of IgE-mediated food allergy. Methods: The databases Cochrane CENTRAL (Trials), MEDLINE (OVID) and Embase (OVID) will be searched for diagnostic test accuracy studies from 1 October 2012 to 30 June 2021. Inclusion and exclusion criteria will be used to select appropriate studies. Data from these studies will be extracted and tabulated, and then reviewed for risk of bias and applicability using the QUADAS-2 tool. All evaluations will be done in duplicate. Studies with a high risk of bias and low applicability will be excluded. Meta-analysis will be performed if there are three or more studies of the same index test and food. Results: A protocol for the systematic review and meta-analyses is presented and was registered using Prospero prior to commencing the literature search. Discussion: Oral food challenges are the reference standard for diagnosis but involve considerable risks and resources. This protocol for systematic review aims to assess the accuracy of various tests to diagnose food allergy, which can be useful in both clinical and research settings.

Abstract

Background There is uncertainty about the influence of diet during pregnancy and infancy on a child's immune development. We assessed whether variations in maternal or infant diet can influence risk of allergic or autoimmune disease. Methods and findings Two authors selected studies, extracted data, and assessed risk of bias. Grading of Recommendations Assessment, Development and Evaluation (GRADE) was used to assess certainty of findings. We searched Medical Literature Analysis and Retrieval System Online (MEDLINE), Excerpta Medica dataBASE (EMBASE), Web of Science, Central Register of Controlled Trials (CENTRAL), and Literatura Latino Americana em Ciências da Saúde (LILACS) between January 1946 and July 2013 for observational studies and until December 2017 for intervention studies that evaluated the relationship between diet during pregnancy, lactation, or the first year of life and future risk of allergic or autoimmune disease. We identified 260 original studies (964,143 participants) of milk feeding, including 1 intervention trial of breastfeeding promotion, and 173 original studies (542,672 participants) of other maternal or infant dietary exposures, including 80 trials of maternal (n = 26), infant (n = 32), or combined (n = 22) interventions. Risk of bias was high in 125 (48%) milk feeding studies and 44 (25%) studies of other dietary exposures. Evidence from 19 intervention trials suggests that oral supplementation with nonpathogenic micro-organisms (probiotics) during late pregnancy and lactation may reduce risk of eczema (Risk Ratio [RR] 0.78; 95% CI 0.68-0.90; I.sup.2 = 61%; Absolute Risk Reduction 44 cases per 1,000; 95% CI 20-64), and 6 trials suggest that fish oil supplementation during pregnancy and lactation may reduce risk of allergic sensitisation to egg (RR 0.69, 95% CI 0.53-0.90; I.sup.2 = 15%; Absolute Risk Reduction 31 cases per 1,000; 95% CI 10-47). GRADE certainty of these findings was moderate. We found weaker support for the hypotheses that breastfeeding promotion reduces risk of eczema during infancy (1 intervention trial), that longer exclusive breastfeeding is associated with reduced type 1 diabetes mellitus (28 observational studies), and that probiotics reduce risk of allergic sensitisation to cow's milk (9 intervention trials), where GRADE certainty of findings was low. We did not find that other dietary exposures-including prebiotic supplements, maternal allergenic food avoidance, and vitamin, mineral, fruit, and vegetable intake-influence risk of allergic or autoimmune disease. For many dietary exposures, data were inconclusive or inconsistent, such that we were unable to exclude the possibility of important beneficial or harmful effects. In this comprehensive systematic review, we were not able to include more recent observational studies or verify data via direct contact with authors, and we did not evaluate measures of food diversity during infancy. Conclusions Our findings support a relationship between maternal diet and risk of immune-mediated diseases in the child. Abstract

Acute food protein-induced enterocolitis syndrome ('FPIES') is a potentially severe type of non IgE-mediated food allergy affecting mainly infants usually when foods are introduced [1]. Acute FPIES triggered by multiple unrelated foods ('multiple food FPIES') has been reported in up to two thirds of patients, particularly in the USA [2,3]. FPIES reactions are often traumatic experiences for parents and weaning leads to significant anxiety, as there is no test to identify safe new foods. This has led to complex weaning recommendations in children with FPIES in an attempt to support parents [4]. However, evidence suggests that multiple food FPIES is rare in other regions, such as Southern Europe [5,6], which questions the applicability of such weaning advice in this population. Studies including a detailed dietary history in children with FPIES are lacking.

94. Association of frequent moisturizer use in early infancy with the development of food allergy

Abstract

BackgroundFood allergy is thought to develop through transcutaneous sensitization, especially in the presence of skin barrier impairment and inflammation. Regular moisturizer application to infant skin could potentially promote transcutaneous sensitization and the development of food allergy.ObjectivesWe tested this hypothesis in the Enquiring About Tolerance (EAT) study population.MethodsThe EAT study was a population-based randomized clinical trial conducted from January 15, 2008, to August 31, 2015, and recruited 1303 exclusively breastfed 3-month-old infants and their families from England and Wales. At enrollment at 3 months, families completed a questionnaire that included questions about frequency and type of moisturizer applied, use of corticosteroid creams, and parental report of dry skin or eczema. Infants were examined for visible eczema at the enrollment visit.ResultsA statistically significant dose-response relationship was observed between parent-reported moisturization frequency at 3 months of age and the subsequent development of food allergy. Each additional moisturization per week was associated with an adjusted odds ratio of 1.20 (95% CI, 1.13-1.27; P [less than].0005) for developing food allergy. For infants with no visible eczema at the enrollment visit, the corresponding adjusted odds ratio was 1.18 (95% CI, 1.07-1.30; P = .001) and for those with eczema at the enrollment visit, 1.20 (95% CI, 1.11-1.31; P [less than] .0005). Moisturizer frequency showed similar dose-response relationships with the development of both food and aeroallergen sensitization at 36 months.ConclusionsThese findings support the notion that regular application of moisturizers to the skin of young infants may promote the development of food allergy through transcutaneous sensitization.

Abstract

The objective of this work was to develop design concepts for a cooled ceramic vane to be used in the first stage of the High Pressure Turbine(HPT). To insure that the design concepts were relevant to the gas turbine industry needs, Honeywell International Inc. was subcontracted to provide technical guidance for this work. The work performed under this contract can be divided into three broad categories. The first was an analysis of the cycle benefits arising from the higher temperature capability of Ceramic Matrix Composite(CMC) compared with conventional metallic vane materials. The second category was a series of structural analyses for variations in the internal configuration of first stage vane for the High Pressure Turbine(HPT) of a CF6 class commercial airline engine. The third category was analysis for a radial cooled turbine vanes for use in turboshaft engine applications. The size, shape and internal configuration of the turboshaft engine vanes were selected to investigate a cooling concept appropriate to small CMC vanes.
