Background: Narrative may provide a useful way in which to assess the language ability of adolescents with specific language impairment and may be more ecologically valid than standardized tests. However, the language of this age group is seldom studied and, furthermore, the effect of narrative genre has not been explored in detail. Methods & Procedures: A total of 99 typically developing adolescents and 19 peers with specific language impairment were given two different types of narrative task: a story-telling condition and a conversational condition. Four areas of narrative (productivity, syntactic complexity, syntactic errors and performance) were assessed. Outcomes & Results: The group with specific language impairment was poorer on most aspects of narrative confirming recent research that specific language impairment is a long-term disorder. A number of measures also showed interactions between group and genre, with story-telling proving to be a disproportionately more difficult task for the specific language impairment group. Error analysis also suggested that the specific language impairment group was making qualitatively different errors to the typically developing group, even within a genre. Conclusions: Adolescents with specific language impairment are not only poorer at both types of narrative than peers, but also show different patterns of competence and error, and require more support from the narrative-partner. Clinical Implications: Assessments of adolescents are less frequent than at younger ages. This is partly because of the sparsity of tests available in this age range. Qualitative analysis of narrative might prove a useful alternative. The findings suggest that in every-day conversation, young people with specific language impairment manage their difficulties more discreetly and this might make them harder to identify in a mainstream setting. There is a debate about whether the language of children with primary language disorders and normal cognitive levels is qualitatively different from those with language impairments who have low or borderline non-verbal IQ (NVIQ). As children reach adolescence, this distinction may be even harder to ascertain, especially in naturalistic settings. Narrative may provide a useful, ecologically valid way in which to assess the language ability of adolescents with specific language impairment (SLI) who have intact or lowered NVIQ and to determine whether there is any discernable difference in every day language. Nineteen adolescents with a history of SLI completed two narrative tasks: a story telling condition and a conversational condition. Just under half the group (n = 8) had non-verbal IQs of 85. The remaining 11 had NVIQs in the normal range or above. Four areas of narrative (productivity, syntax, cohesion and performance) were assessed. There were no differences between the groups on standardized tests of language. However, the group with low NVIQ were poorer on most aspects of narrative, suggesting that cognitive level is important, even when language is the primary disorder. The groups showed similar patterns of differences between story telling and conversational narrative. It was concluded that adolescents with a history of SLI and poor cognitive levels have poorer narrative skills than those with normal range NVIQ even though these may not be detected by standardized assessment. Their difficulties present as qualitatively similar to those with normal range NVIQ and narratives appear impoverished rather than inaccurate.

The present study explores the characteristics and concerns of a group of mothers of young people with a history of specific language impairment (SLI). Altogether, 52 young people and their mothers participated. Psychometric tests were used to assess mothers' cognitive and literacy performance and socio-economic status was also recorded. Maternal concerns about their young people were taken from the Autism Diagnostic Interview. Young people were also assessed for language, literacy and cognitive performance. Number and type of concerns as well as the ways in which child and mother characteristics relate to maternal concern were examined. The findings revealed that mothers of those with SLI were no more likely to have language difficulties than those in the general population. The majority of mothers reported either one concern (36.5%), or two concerns (36.5%) for their adolescents. A smaller group reported more concerns (11.5%) and eight mothers reported no concerns for their adolescents (15%). The most common reported primary maternal concern was the future, followed by social and educational concerns. Interestingly, few mothers reported speech and language concerns. Furthermore, maternal concerns for their adolescents were not related to the young people's own characteristics. Whilst most parent characteristics were also not associated with their concerns, type of concern reported appeared to be related to mothers' own skills: higher overall IQ and literacy scores were found for parents reporting concerns about their adolescent's social skill.

Background: Traditionally, autism and specific language impairment (SLI) have been regarded as distinct disorders but, more recently, evidence has been put forward for a closer link between them: a common set of language problems, in particular receptive language difficulties and the existence of intermediate cases including pragmatic language impairment. The present study aimed to examine the prevalence of autism spectrum disorders in a large sample of adolescents with a history of SLI. Method: The presence of autism spectrum disorders was examined in seventy-six 14-year-olds with a confirmed history of SLI. A variety of instruments were employed, including the Autism Diagnostic Interview-Revised (ADI-R), the Autism Diagnostic Observation Schedule (ADOS) and the Family History Interview (FHI). Results: The prevalence of autism spectrum disorders in young people with SLI was found to be 3.9%, about 10 times what would be expected from the general population. In addition, a much larger number of young people with a history of SLI showed only some autism spectrum symptoms or showed them in a mild form. Conclusions: Young people with SLI have an increased risk of autism. The magnitude of this risk is considerable. In addition, a larger proportion (a quarter of individuals) present with a number of behaviours consistent with autism spectrum disorders.

Three groups of children with communication disorders were assessed using the CELF (Clinical Evaluation of Language Fundamentals). The results were used to compare the language skills of children with specific language impairment (SLI), autistic spectrum disorder (ASD) and a middle group who shared some characteristics of both SLI and ASD (Shared group). The children were aged between 5 years 10 months and 10 years 7 months. Each child was assessed on either the CELF UK 3 or the CELF Preschool, depending on their age and ability. The overall language profiles were compared and differences were found between the three groups. A more detailed analysis of the individual subtests was made, which showed that all three groups had the least difficulty with "Listening to Paragraphs" (which involves abstracting and remembering information from two short texts). It also showed that all three groups had difficulty with "Recalling Sentences" (which involves repeating sentences of increasing length and complexity). This paper discusses the value of using a more comprehensive range of linguistic assessments when considering the similarities and differences between groups of children within this population, it also considers the educational and clinical implications and makes suggestions for future research.

Background: The study is concerned with the cognitive abilities of children with specific language impairment (SLI). Previous research has indicated that children with SLI demonstrate difficulties with certain cognitive tasks despite normal non-verbal IQ scores. It has been suggested that a general processing limitation might account for the pattern of language and cognitive difficulties seen in children with SLI. The performances on a visuo-spatial short-term memory task and a visuo-spatial processing task were considered in a group of young children with SLI. Verbal short-term memory was also measured. Aims: To identify whether children with SLI demonstrate difficulties with visuo-spatial memory as well as verbal short-term memory. To see whether a visuo-spatial processing task without short-term memory requirements is problematic for children with SLI. To consider performance on these tasks over time. Methods & Procedures: Nine children with SLI (mean age 3;9 years at the study outset) and nine typically developing children (mean age 3;9 years at the study outset) were visited on three occasions over 1 year. Verbal short-term memory, visuo-spatial short-term memory and visuo-spatial processing tasks were administered to the children, and performance over time was compared between the two groups. Outcomes & Results: The children with SLI performed at a lower level than the typically developing children on the verbal short-term memory task. Both groups showed similar development on the verbal short-term memory task and the visuo-spatial processing task over time. Only the visuo-spatial short-term memory task showed slower development over time in the children with SLI relative to the typically developing children. Conclusions: Children with SLI demonstrated slower development on a visuo-spatial short-term memory task relative to typically developing children of the same chronological age. This finding has implications for speech and language therapists and other professionals working with children with SLI. It may mean that only certain types of visual support are suitable, and that children with SLI will have difficulty with tasks requiring a high level of processing, or a number of mental manipulations.

Background: Specific language impairment (SLI) is currently partly defined by the presence of non-verbal IQ scores in the normal range. However, not only is there a debate concerning where "normal thresholds" should be, but increasing information about the presence of processing deficits in SLI have led some researchers to question the use of IQ criteria in clinical diagnosis. In particular, little is known about the longitudinal and developmental patterns of cognitive performance in this population. Method: Data from a long-term follow-up study of SLI was examined in 82 children defined at original participation as having SLI who had IQ measurements at 7, 8, 11 and 14 years. Results: Analyses revealed a significant fall between 7 and 14 years of over 20 IQ points. This fall took place mainly between 8 and 11 years but was still continuing between 11 and 14 years. Further investigation revealed different groups of children showing different developmental patterns in IQ, even after controlling for baseline measurement. These groups also showed significantly different language outcomes at 14 years. Analyses controlling for IQ at 7 were also performed that suggested a dynamic process between language and cognitive development. Conclusions: These findings appear to be in agreement with a model of impairment that views "Residual Normality" as unlikely (Karmiloff-Smith, 1998; Thomas & Karmiloff-Smith, 2002). The implications are therefore discussed in relation to the dynamic development of systems along with the possible cognitive mechanisms (such as working memory) that might interact with language to create an SLI profile.

Background: Semantic and inferencing abilities have not been fully examined in children with communication difficulties. Aims: To investigate the inferential and semantic abilities of children with communication difficulties using newly designed tasks. Methods & Procedures: Children with different types of communication disorder were compared with each other and with three groups of typically developing children: those of the same chronological age and two groups of younger children. In total, 25 children aged 11 years with specific language impairment and 22 children, also 11 years of age, with primary pragmatic difficulties were recruited. Typically developing groups aged 11 (n=35; age-match), and those aged 9 (n=40) and 7 (n=37; language similar) also participated as comparisons. Outcomes & Results: For Semantic Choices, children with specific language impairment performed significantly more poorly than 9- and 11-year-olds, whilst the pragmatic difficulties group scored significantly lower than all the typically developing groups. Borderline differences between specific language impairment and pragmatic difficulties groups were found. For inferencing, children with communication impairments performed significantly below the 11-year-old peers, but not poorer than 9- and 7-year-olds, suggesting that this skill is in line with language ability. Six children in the pragmatic difficulties group who met diagnosis for autism performed more poorly than the other two clinical groups on both tasks, but not statistically significantly so. Conclusions: Both tasks were more difficult for those with communication impairments compared with peers. Semantic but not inferencing abilities showed a non-significant trend for differences between the two clinical groups and children with pragmatic difficulties performed more poorly than all typically developing groups. The tasks may relate to each other in varying ways according to type of communication difficulty.

Background: Public awareness of Developmental Language Disorder (DLD) is lower than other neurodevelopmental disorders, despite its high prevalence of 7.6%. This lower awareness means recruitment for DLD research studies is difficult. DLD is both underfunded and under‐researched, resulting in relatively limited research investigating individuals with DLD. Engage with Developmental Language Disorder (E‐DLD) is a response to these considerations. E‐DLD is the first international participant database of those affected by DLD. Parents of children with DLD under 16 and young people and adults over 16 from anywhere in the world can sign up to be a part of the E‐DLD. Aims: This paper aims to describe the families of children with DLD and adults with DLD in the database thus far. Methods & Procedures: E‐DLD members sign up via our website, reporting demographic characteristics as part of this procedure. We request all E‐DLD members subsequently fill in a yearly survey. The content of the yearly survey changes dependent on the age of the child, while the yearly survey for adults remains consistent. We measure a wide range of domains, such as speech and language therapy (SLT) support, school support, socialisation skills, and early developmental milestones for our youngest members, and health care support and mental well‐being measurements for our adults. We also collect parent and self‐reported reflections on strengths and challenges for the person with DLD using open‐ended questions and the Strengths and Difficulties Questionnaire. Outcomes & Results: The database currently consists of 196 parents of children with DLD and 20 individuals over the age of 16 with DLD or suspected DLD across a range of socioeconomic status (SES) backgrounds. Our initial results confirm that E‐DLD members meet the linguistic profile of DLD in relation to self‐ or parent‐rated language difficulties. Both children and adults show increased rates of psychosocial difficulties compared to established norms, consistent with past research on clinical samples of people with DLD. Conclusions & Implications: The findings indicate that a participant database for DLD research is feasible and useful. The rates of emotional, behavioural and sleep difficulties among the child probands are higher than reported rates amongst typically developing children. Initial data indicate that adults with DLD have poorer well‐being than their peers. The E‐DLD is a useful collection of data on those affected by DLD and is a promising method for connecting people with DLD with academic researchers. What This Paper Adds: What is already known on this subject: Developmental Language Disorder (DLD) is characterised by expressive and/or receptive language difficulties in the absence of another biomedical condition that could explain these difficulties. It is critically under‐researched and underfunded. As such, there is a lack of public awareness and difficulty recruiting sufficient sample sizes for DLD research studies. What this paper adds to existing knowledge: Engage with Developmental Language Disorder (E‐DLD) is the first international participant database of individuals with DLD. This paper provides a preliminary report on the profile of linguistic and psychosocial skills among the individuals on the database, adding to current understanding of DLD across age groups. What are the potential or actual clinical implications of this work?: Our aim is that the E‐DLD will provide much‐needed facilitation of research into DLD. E‐DLD will enable those with DLD and their families more readily to shape research agendas and to participate in studies that interest them. Specific language impairment is sometimes thought to be associated with concurrent difficulties in the area of social and behavioral development (N. Bolting & G. Conti-Ramsden, 2000; D. P. Cantwell & L. Baker, 1987; M. Fujiki, B. Brinton, & C. Todd, 1996; S. Redmond & M. Rice, 1998). The present study follows a group of 242 children, initially studied at age 7 years when they attended language units in England, and assesses their social and behavioral status at age 11 years.In total, 64% of the children were found to have scores on the Rutter behavioral questionnaire (M. Rutter, 1967) of 9 or above (clinical threshold); 34% scored over the threshold for the Strengths and Difficulties questionnaire (R. Goodman, 1997); and 39% scored below average on the Peer Competence subscale of the Harter Perceived Competence Scale (S. Harter & R. Pike, 1984). On further analysis, these generalized difficulties were characterized mainly by poor social competence. In addition, 36% of the cohort were at risk of being regular targets for victimization compared to 12% of a comparison sample of typically developing peers.Few associations were found between social outcome and other measures, including nonverbal intelligence, overall linguistic skill, gender, and longitudinal measures taken previously. Importantly, however, pragmatic language difficulties measured on the Children's Communication Checklist (D. V. M. Bishop, 1998) were most strongly related to poor social outcome and to expressive language related to victimization.

Background: The pragmatic skills of children with communication disorders and their assessment are currently an issue for speech and language therapy and educational placement. Aims: To explore whether different subgroups of children with communication disorders score differently on the Children's Communication Checklist (CCC) and to study how they compare with published normative data. Methods & Procedures: A sample of 161 eleven-year-old children with a history of communication disorders was assessed using the CCC. The main use of this questionnaire was to establish whether pragmatic impairments were part of a child's communication difficulty. Although the checklist was originally designed for research purposes, normative data for this scale have been recently published as well as group data from a number of different clinical groups. Whilst the present CCC data have been previously reported descriptively for a wider sample, they have not been examined in terms of subgroups or compared directly with normative information and similarly diagnosed individuals from other studies. Outcomes & Results: Of the children assessed, 52 (33%) scored in the normal range (within 1 SD) on the pragmatic scale, 40 (26%) fell between 1 and 2 SD below the normative mean, and 64 (41%) scored below 2 SD of the mean of typically developing children (aged 6-16 years). Thus, the majority (67%) scored out of the normal range for pragmatic skill at 11 years of age. The cohort was separated into four diagnostic subcategories: those with a definite diagnosis of autistic spectrum disorder (n=15); those with typical specific language impairment (n=82); generally impaired (n=37); and those with a clinical history of primary pragmatic language impairment (independent of CCC score, n=27). Conclusions: Findings show that those generally impaired and with specific language impairment were less impaired than the other groups on the CCC pragmatic scale. There was a significant trend for those with autistic spectrum disorders to score lowest through pragmatic language impairment, generally impairment to specific language impairment. It is argued that a cut-off of 140 may prove more useful at this age than the 132 level previously published for 8 year olds. Results suggest that the CCC can be used as a clinical tool, but in conjunction with other reliable measures.

A four-year investigation of 200 British children with language impairments found that they performed all of the British National Curriculum's Key Stage 2 assessments poorly. After full-time placement in language units at 7, 171 of the children were found to continue education in a variety of placements with provision for special education needs at age 11. A study examined the stability of the six subgroups of 242 English children with language impairment at age 8. Findings indicate considerable stability in the patterns of difficulties delineated by the classification system. Poorer stability was evident across time with 45% the children moving across subgroups. (Author/CR)

Research examines four potential psycholinguistic markers for specific language impairment in children, including a sentence repetition task, a third person singular task, a nonword repetition task, and a past tense task. Findings suggest that sentence repetition is the most useful for identifying language impairment.

The Child and Adolescent Psychiatric Assessment parent interview was utilized to compare a sample of very low birthweight (VLBW) children with peers having a number of psychiatric disorders such as depression, anxiety, antisocial behavior and attention deficit/hyperactivity disorder (ADHD). VLBW children were more likely to exhibit symptoms of depression and generalized anxiety. They also showed a psychiatric disorder of some type compared to their peers. Results indicated that VLBW children are at increased risk of psychiatric symptoms especially ADHD.

Background: Improved discourse production is a priority for all key stakeholders in aphasia rehabilitation. A Cochrane review of randomised controlled trials (RCTs) for aphasia found speech and language therapy treatment to be effective for improving the ability to communicate in everyday interaction. However, this large-scale review did not focus exclusively on treatment for discourse production and did not include other treatment research designs. Thus, the extent of the evidence base addressing discourse interventions is currently unclear. Objective: The present study undertakes the first systematic review of research on treatment for discourse production in aphasia, appraises the quality of the evidence base; characterises the methods for measuring outcomes; and describes discourse treatment in terms of both content and efficacy. Design: Scopus, Medline, and EmBase databases were searched, providing 334 records. Twenty-five studies (reporting on 127 participants) met inclusion criteria and were reviewed with the following research questions: What is the quality of the study designs used? How complete is the intervention reporting? What is the range, type, and content of outcome measures used? What is the range, type, and content of discourse treatments reported to date? Are discourse treatments efficacious? Results: Seven of the 25 studies met the criteria for quality review, with 3 RCTs scoring moderately well and 3 (of 4) case studies scoring moderate-low. Most studies had adequate levels of completeness of treatment reporting, with 3 scoring highly. There were 514 different outcome measures reported across the 25 studies, with measures of words-in-discourse the most common. Studies were grouped into six treatment categories: "word production in discourse", "sentence production in discourse", "discourse macrostructure", "discourse scripts", "multi-level", and "no consensus". Twenty-two studies reported post-treatment gains, most commonly noted in increased word production. Changes in sentence production and discourse macrostructure were present but infrequently assessed. Conclusions: Discourse treatment is an emerging field of research. Despite limitations in the evidence base, there are clear positive signs that discourse treatment is efficacious. There is emerging evidence for beneficial effects on word and sentence production in discourse, for improved discourse macrostructure, and for treatments working at multiple levels of language. To strengthen the evidence in this field and improve outcomes for people with aphasia, we need more discourse treatment research using an explicit theoretical rationale, high-quality study designs, more complete reporting, and agreed treatment and assessment methods. Children and adolescents with developmental language disorder (DLD) are, overall, vulnerable to difficulties in emotional adjustment and in peer relations. However, previous research has shown that different subgroups follow different trajectories in respect to the quality of peer relations. Less is known about the trajectories of emotional development. We consider here the possibility that development in these two domains is interrelated: that is, the trajectories of emotional and peer problems will proceed in parallel. We conducted longitudinal joint trajectories analyses of emotional and peer relations in a sample of young people identified as having DLD at the age of 7 years and seen at intervals up to 16 years. Potential influences on joint trajectory group membership were examined. Findings revealed five distinct joint trajectories. Emotional and peer difficulties do occur together from childhood to adolescence for just over half of the sample, but not all. The variables most clearly associated with group membership were pragmatic language ability, prosociality and parental mental health. This is the first study to examine joint longitudinal trajectories of emotional and peer difficulties in individuals with DLD. We demonstrate that development in individuals with DLD is heterogeneous and identify three key variables associated with personal and social adjustment from childhood to adolescence. Important emerging measures of academic impact are article download and citation rates. Yet little is known about the influences on these and ways in which academics might manage this approach to dissemination. Three groups of papers by academics in a center for speech-language-science (available through a university repository) were compared. The first group of target papers were blogged, and the blogs were systematically tweeted. The second group of connected control papers were nonblogged papers that we carefully matched for author, topic, and year of publication. The third group were papers by different staff members on a variety of topics-Unrelated Control Papers. The results suggest an effect of social media on download rate, which was limited not just to Target Papers but also generalized to Connected Control Papers. Unrelated Control Papers showed no increase over the same amount of time (main effect of time, F(1,27) = 55.6, p < .001); Significant Group×Time Interaction, F(2,27) = 7.9, p = .002). The effect on citation rates was less clear but followed the same trend. The only predictor of the 2015 citation rate was downloads after blogging ( r = 0.450, p = .012). These preliminary results suggest that promotion of academic articles via social media may enhance download and citation rate and that this has implications for impact strategies. Background: There is a need for clinical tools that capture the real-life impact of aphasia. This study reports on a psychometric investigation of two self-report tools: the Communicative Activities Checklist and the Social Activities Checklist (COMACT; SOCACT), which assess the dimensions of communication activity and social participation in aphasia. Aims: (1) To investigate internal consistency (IC), convergent, and known validity of the COMACT and SOCACT and (2) to investigate the impact of personal contextual factors: gender, age, years in education, linguistic ability, and emotional health on communicative and social activities. Methods & Procedures: Thirty participants with mild-moderate chronic aphasia (people with aphasia (PWA); mean age 71 years, mean time post onset 41 months, mean years in education 10.77) and 75 control neurologically healthy participants (NHP; mean age 74 years, mean years in education 13.18) completed the COMACT and SOCACT reporting how frequently they engaged in particular activities. The COMACT has 45 communication activities with subscales ofTalking, Listening, Reading, andWriting. The SOCACT contains 20 social activities with subscales ofLeisure, Informal, andFormal. IC was examined using Cronbach’s alpha (α). Correlations with published assessments, Western Aphasia Battery (WAB) and Communication Activities of Daily Living were computed for COMACT only. Multiple regression models were examined for differences in participant (PWA vs. NHP) performance on COMACT and SOCACT. Outcomes & Results: Total COMACT IC was 0.83 (PWA) and 0.84 (NHP). Following deletion of four items, to further improve subscale ICs, total COMACT IC was 0.83 (PWA) and 0.86 (NHP). COMACT total score and WAB AQ were moderately correlated (r = 0.55). Total SOCACT IC was 0.58 (PWA) and 0.63 (NHP). Following single item deletion, total IC was 0.65 (PWA) and 0.64 (NHP). Statistical analysis revealed PWA, in comparison to NHP, participated in significantly fewer communication and social activities. Personal contextual factors impacted both groups differently; particular aspects were associated with communication activity (age and language severity) and social activity (age only). For NHP, ageing, emotional health and years in education were significant predictors of social and communication activity. Conclusions: This study finds the COMACT to be a reliable, valid measure of communication activity. The SOCACT had “questionable” IC and requires further psychometric investigation. Both tools demonstrate known group validity. Relationships between impairment-level and personal contextual factors for communication activity and social participation are highlighted.
