Abstract

Methods Sixty-nine patients with sustained ventricular tachyarrhythmias were followed up to evaluate the predictive value of functional capacity (i.e. New York Heart Association class and peak oxygen consumption) and resting left ventricular function (i.e. radionuclide left ventricular ejection fraction, angiographic left ventricular wall motion score and echocardiographic dimensions) with respect to arrhythmia recurrence. Results During a mean follow-up of 19 months 18 patients (26%) had an arrhythmia recurrence. Parameters of functional capacity and echocardiographic dimensions were not related to arrhythmia recurrence. Left ventricular ejection fraction and wall motion score were worse in patients with a recurrence compared with the arrhythmia-free patients: 30 +/- 16% versus 40 +/- 19% (mean +/- SD, P = 0.035) and 25 +/- 7 versus 20 +/- 7 (P = 0.01), respectively. Multivariately the most powerful parameter was left ventricular wall motion score (odds ratio 1.12, 95% CI 1.02-1.23). Conclusions Arrhythmia recurrence in ventricular tachyarrhythmia patients relates to resting left ventricular function and not to functional capacity. Since angiographic left ventricular wall motion score is prognostically more important than ejection fraction this parameter should be considered for risk stratification in these patients.

Abstract

Propranolol in slow-release form has been the first-line treatment in long QT (LQT) until it was withdrawn from the market. We describe two cases where a switch to bisoprolol resulted in worsening of arrhythmia control: A man with LQT2, asymptomatic on propranolol, experienced syncope after switching to bisoprolol 5 mg daily. He switched back to propranolol and has remained asymptomatic during subsequent 12 months. A man with classical Jervell Lange-Nielsen syndrome, previous gangliectomy, and ICD implantation, switched to bisoprolol 5 mg daily. Four months later he experienced a tachycardia storm. He switched back to propranolol and has remained free from arrhythmias during subsequent 12 months., (©2015 Wiley Periodicals, Inc.)

Abstract

We replicated a randomized clinical trial (RCT) that investigated the effectiveness of a beta-blocker therapy in reducing cardiac events among 400 patients undergoing major non-cardiac surgeries to determine if observational studies can achieve comparable results to RCTs. From the electronic medical records of Cleveland Clinic using the same inclusion criteria as the RCT, 200 cases were matched to 200 controls by risk set. Our study achieved comparable results as the RCT: no significant differences in cardiac events existed between cases and controls based on t-tests and chi-square tests, although different predictors were identified by logistic regression (age, myocardial infarction, and valvular heart disease in our study vs. systolic blood pressure and hypotension in the RCT). The effectiveness of the therapy was then investigated on a larger sample (358 cases vs. 1614 controls) matched by propensity scores. The short-term postoperative survival was essentially the same between cases and controls based on a Cox proportional hazard model.
