Abstract

Incidence of epilepsy increases rapidly after age 65; recent studies indicate that approximately 10% of nursing home residents are being treated with antiepileptic drugs (AEDs). Almost all are being treated with first generation AEDs. The average nursing home patient receives six medications, has age-related changes in protein binding, decreases in hepatic and renal clearance, and may have alterations in gastrointestinal absorption. AEDs that do not have drug-drug interactions, are not metabolized by the liver, and are readily absorbed may offer benefits in this population. New studies are demonstrating that the first generation AEDs have a number of shortcomings for treating older patients, whereas some of the newer AEDs may overcome these limitations. This paper reviews the present knowledge base and compares properties of the first generation AEDs with newer agents to develop a more rational approach for drug selection in older adults. Abstract

Following blockade of membrane potassium conductance with tetraethylammonium ions or 3-aminopyridine, long-duration action potentials were recorded from mouse spinal cord neurons in primary dissociated cell culture. The action potentials were calcium-dependent since they: (1) were not blocked by the sodium-channel blocker tetrodotoxin, (2) could be recorded in sodium-free, calcium-containing medium (3) could not be evoked in sodium-containing, calcium-free medium, (4) were blocked by calcium channel blockers manganese and cobalt and (5) had overshot amplitudes that varied linearly with the log of the extracellular calcium concentration (slope of 27.5 mV/decade change in calcium concentration).

67. Inhibition Modifies the Effects of Slow Calcium-Activated Potassium Channels on Epileptiform Activity in a Neuronal Network Model

Abstract

Generation of epileptiform activity typically results from a change in the balance between network excitation and inhibition. Experimental evidence indicates that alterations of either synaptic activity or intrinsic membrane properties can produce increased network excitation. The slow Ca2+-activated K+ currents (sI AHP) are important modulators of neuronal firing rate and excitability and have important established and potential roles in epileptogenesis. While the effects of changes in sI AHP on individual neuronal excitability are readily studied and well established, the effects of such changes on network behavior are less well known. The experiments here utilize a defined small network model of multicompartment pyramidal cells and an inhibitory interneuron to study the effects of changes in sI AHP on network behavior. The benefits of this model system include the ability to observe activity in all cells in a network and the effects of interactions of multiple simultaneous influences. In the model with no inhibitory interneuron, increasing sI AHP results in progressively decreasing burst activity. Adding an inhibitory interneuron changes the observed effects; at modest inhibitory strengths, increasing sI AHP in all network neurons actually results in increased network bursting (except at very high values). The duration of the burst activity is influenced by the length of delay in a feedback loop, with longer loops resulting in more prolonged bursting. These observations illustrate that the study of potential antiepileptogenic membrane effects must be extended to realistic networks. Network inhibition can dramatically alter the observations seen in pure excitatory networks.

Abstract

An imbalance between excitation and inhibition can play an important role in the generation of epileptiform activity. Experimental evidence indicates that alterations of either synaptic activity or intrinsic membrane properties may contribute to this imbalance. The slow Ca2+ - activated K+ currents (sIAHP) limit neuronal firing rate and excitability and are therefore of great interest for their potential role in epileptogenesis. The sIAHP is found in both excitatory and inhibitory neurons, and its effect on these neurons can influence the network behavior. Simulations show that the increased excitability caused by reduction of inhibition by the sIAHP for inhibitory interneuron generates recurrent bursting activity.

Abstract

Inhibitory interneurons play crucial roles in the regulation of patterns of activity in the hippocampus, and some types are thought to be vulnerable in epilepsy. The connections between excitatory and inhibitory synapses are important for generation of bursting activity in pyramidal neurons. The present study investigates the influences of changes in the connectivity of interneurons on the patterns of bursting in several excitatory connections using a multicompartmental pyramidal cell model. Simulations show that bursting activity depends upon changes in the connectivity of the inhibitory interneuron, and the location of the inhibitory synapses on excitatory neurons.

84. Mechanisms of Action of Clostridial Neurotoxins on Dissociated Mouse Spinal Cord Neurons in Cell Culture

Abstract

Experiments with tetanus toxin in the dissociated spinal cord neuronal system have thus far revealed that the toxin binds to the neurons and after a dose-dependent latent period produces convulsant activity manifest by paroxysmal depolarizing events (PDE), abrupt depolarizing shifts (typically 5-20 mV) of membrane potential associated with triggered action potentials lasting 200-2000mS. This convulsant period is produced by the reduction and ultimate block of synaptic inhibition by the toxin. Tetanus toxin has been demonstrated to block release of neurotransmitter at a presynaptic locus (Bergey et al., 1983, 1987). After the convulsant period produced by the toxin, the frequency of the PDE is reduced and ultimately these events disappear. This parallels the apparent disappearance of all spontaneous postsynaptic potentials, excitatory as well as inhibitory. Tetanus toxin has been demonstrated by quantal analysis to block the presynaptic release at excitatory synapses (Bergey et al, 1987). Therefore, while tetanus toxin has a preferential effect on inhibitory neurotransmission, all synaptic transmission appears to be blocked by the toxin.

Abstract

The balance between inhibition and excitation plays a crucial role in the generation of synchronous bursting activity in neuronal circuits. In human and animal models of epilepsy, changes in both excitatory and inhibitory synaptic inputs are known to occur. Locations and distribution of these excitatory and inhibitory synaptic inputs on pyramidal cells play a role in the integrative properties of neuronal activity, e.g., epileptiform activity. Thus the location and distribution of the inputs onto pyramidal cells are important parameters that influence neuronal activity in epilepsy. However, the location and distribution of inhibitory synapses converging onto pyramidal cells have not been fully studied. The objectives of this study are to investigate the roles of the relative location of inhibitory synapses on the dendritic tree and soma in the generation of bursting activity. We investigate influences of somatic and dendritic inhibition on bursting activity patterns in several paradigms of potential connections using a simplified multicompartmental model. We also investigate the effects of distribution of fast and slow components of GABAergic inhibition in pyramidal cells. Interspike interval (ISI) analysis is used for examination of bursting patterns. Simulations show that the inhibitory interneuron regulates neuronal bursting activity. Bursting behavior patterns depend on the synaptic weight and delay of the inhibitory connection as well as the location of the synapse. When the inhibitory interneuron synapses on the pyramidal neuron, inhibitory action is stronger if the inhibitory synapse is close to the soma. Alterations of synaptic weight of the interneuron can be compensatory for changes in the location of synaptic input. The relative changes in these parameters exert a considerable influence on whether synchronous bursting activity is facilitated or reduced. Additional simulations show that the slow GABAergic inhibitory component is more effective than the fast component in distal dendrites. Taken together, these findings illustrate the potential for GABAergic inhibition in the soma and dendritic tree to play an important modulatory role in bursting activity patterns.
