Abstract

Since the first report of the use of a patient's own vein to bridge a defect in an artery (bypass surgery), there has been extensive research into finding an artificial graft that functions as well. There have been attempts at using nylon, Teflon (registered trademark), and Dacron (registered trademark) grafts, but these attempts have been abandoned. Today, the most commonly used arterial grafts are made from the patient's own veins (rerouting the blood from the artery through a vein and back), polytetrafluoroethylene (PTFE), and denatured, or preserved, umbilical vein. While there seems little doubt that using the patient's own vein for graft material is ideal, the relative merits of other options are still controversial. The results of 373 bypass operations between 1980 and 1988 were reviewed. Overall, the results suggest that artificial grafts are a suitable alternative to the use of the patient's vein when the surgical site is above the knee, but the vein should always be used, if available, in grafts below the knee joint. Abstract

Patients with severe peripheral vascular disease that has progressed to the point where the survival of the leg is threatened as a result of ischemia (lack of adequate blood supply), may need to undergo vascular bypass surgery. The first choice for a conduit for this bypass surgery is the patient's own leg vein, the long saphenous vein. This is particularly true if the bypass is performed below the level of the knee. When the bypass involves the upper leg, or when the saphenous vein is not suitable, a prosthetic vascular conduit, usually made of polytetrafluoroethylene, may have to be used. Some surgeons prefer using the prosthetic graft rather than the saphenous vein for bypass performed in the upper leg. This allows the saphenous vein to be left in the leg, and it is available if it is needed at a later time for a longer bypass such as in the lower leg. This decision to preserve the saphenous vein assumes that use of a prosthetic graft in the upper leg does not have a detrimental effect on the saphenous vein in that leg. A study was undertaken to evaluate the suitability of the saphenous vein for future surgery, following prosthetic bypass. Over a 10-year period 137 above-the-knee bypass operations using prosthetic graft were performed; 74 patients were available for further study. Evaluation of the vein was performed using duplex scanning, a noninvasive study used to measure blood flow and provide detail of the vessel wall structure. At the time of initial surgery 16 patients were found to have unsuitable saphenous veins; these veins were still unsuitable at follow-up. Of the remaining 58 veins, 97 percent were still suitable to be used as bypass conduits. These findings indicate that above-the-knee prosthetic graft use does not have a detrimental effect on the future suitability of the long saphenous vein.
