Abstract

Background: Foot surgery is common in patients with RA but research into surgical outcomes is limited and conceptually flawed as current outcome measures lack face validity: to date no one has asked patients what is important to them. This study aimed to determine which factors are important to patients when evaluating the success of foot surgery in RA Methods: Semi structured interviews of RA patients who had undergone foot surgery were conducted and transcribed verbatim. Thematic analysis of interviews was conducted to explore issues that were important to patients. Results: 11 RA patients (9 ♂, mean age 59, dis dur = 22yrs, mean of 3 yrs post op) with mixed experiences of foot surgery were interviewed. Patients interpreted outcome in respect to a multitude of factors, frequently positive change in one aspect contrasted with negative opinions about another. Overall, four major themes emerged. Function: Functional ability & participation in valued activities were very important to patients. Walking ability was a key concern but patients interpreted levels of activity in light of other aspects of their disease, reflecting on change in functional ability more than overall level. Positive feelings of improved mobility were often moderated by negative self perception ("I mean, I still walk like a waddling duck”). Appearance: Appearance was important to almost all patients but perhaps the most complex theme of all. Physical appearance, foot shape, and footwear were closely interlinked, yet patients saw these as distinct separate concepts. Patients need to legitimize these feelings was clear and they frequently entered into a defensive repertoire ("it's not cosmetic surgery; it's something that's more important than that, you know?”). Clinician opinion: Surgeons' post operative evaluation of the procedure was very influential. The impact of this appraisal continued to affect patients' lasting impression irrespective of how the outcome compared to their initial goals ("when he'd done it ... he said that hasn't worked as good as he'd wanted to ... but the pain has gone”). Pain: Whilst pain was important to almost all patients, it appeared to be less important than the other themes. Pain was predominately raised when it influenced other themes, such as function; many still felt the need to legitimize their foot pain in order for health professionals to take it seriously ("in the end I went to my GP because it had happened a few times and I went to an orthopaedic surgeon who was quite dismissive of it, it was like what are you complaining about”). Conclusions: Patients interpret the outcome of foot surgery using a multitude of interrelated factors, particularly functional ability, appearance and surgeons' appraisal of the procedure. While pain was often noted, this appeared less important than other factors in the overall outcome of the surgery. Abstract

Objective: Countries worldwide, including Canada, need tools for informed decision making on the adoption of ICD-11. The purpose of the current study is to create a cost and outcome estimation framework for the transition from ICD-10-CA to ICD-11 in Canada and anticipate the benefits/outcomes and international considerations of ICD-11 adoption. Methods: This paper follows a cost and outcome evaluation framework. The costs of transitioning to the ICD-11 coding system are based on literature reviews and interviews adapted for the Canadian context, while the outcomes have been created using a novel methodology with face and criterion validity. Results: While it was infeasible to include in the framework all possible categories and variables identified by the interviewees, this paper provides a comprehensive guideline of the outcome and cost estimation methodology. Conclusion: The estimation technique adopted in this paper is unique and may be used in other countries as a benchmark methodology to evaluate adoption of the ICD-11 coding system. 63. Two Studies of Illinois Public Libraries: An Adult User Survey; and Fines, Fees and Charges Levied. Illinois Library Statistical Report No. 7.

Abstract

Two surveys were conducted in 1981/82 and 1982 respectively to determine if Illinois public libraries were satisfying their patrons in terms of book use, and how much money Illinois public libraries received directly from their patrons in library fines, fees, and charges. In the first study, 2,133 adults returning a book at a random sample of 71 public libraries were surveyed to determine their satisfaction with books borrowed, their major book selection methods, and the actual audience the books reached. It was found that 90% of the respondents read all or more than half of the book returned, that 99% rated it good or fair, that 46% selected the book by browsing, and that each book borrowed was read by 1.5 users (due to book sharing). In the second study, questionnaires on library fees were mailed to 83 Illinois public libraries, with a response rate of 94%. It was found that the average amount received in library fees was $7,323 per year, or over 7% of library operating budgets; that the average number of charges for services per library was 17; and that more money was earned from user fees, e.g., rental of books or film projectors, than from penalty charges. 67. Exploring the Use of 'Output Measures for Public Libraries' in North Carolina Public Libraries.

Abstract

This study was designed to determine: (1) whether the 5-year planning cycle described in the 1980 manual "A Planning Process for Public Libraries" is being used in North Carolina's public libraries; (2) whether output measures described in a second publication released in 1982 are being collected in the state's public libraries; (3) the levels of performance that are being achieved on the various output measures; and (4) whether variations in selected measures are associated with other underlying factors. The supposition that larger libraries should be adopting such innovations in planning and accountability as the methods described in "A Planning Process for Public Libraries" and "Output Measures for Public Libraries" at a faster rate than smaller libraries is also examined. Based on an analysis of the responses of 61 North Carolina libraries to a questionnaire distributed to the 106 library directors in the state, study results indicate that a quarter of the directors were using the recommended 5-year planning cycle, fewer than half of the directors were using at least some of the output measures, and no library collected data on all of the measures. It was tentatively concluded that North Carolina libraries appear to be following a national pattern of non-use of the two manuals, and it is recommended that steps be taken so that public libraries in North Carolina will recognize the importance of planning and evaluation. Used clustered sample household survey of 329 males and females aged 14 to 17, and 470 of their parents to examine influence of parental factors on adolescent sexual behavior and contraceptive use. The synthesis of enantioenriched allenes utilizing a Pd(0)-catalyzed cross-coupling reaction of propargylic carbonates and phosphates with alkenyl trifluoroborates is described. Di-, tri- and tetrasubstituted allenes were synthesized in moderate to high optical yields and several racemic allenes possessing various functional groups were also synthesized.

Interactive Objects Inc. of Redmond, WA, has reduced its workforce by nearly 50% to 14 by laying off 11 positions as it consolidated operations into its much-larger office in Seattle, WA. The software firm increased its staff earlier in 1998 after projecting rapid growth, but failed to secure adequate cash infusion. Interactive Objects cited adverse market conditions in shelving plans to raise an additional $6 million in debt financing, despite landing $6 million from foreign investors earlier in 1998. Interactive CEO Steve Wollach said the firm is expected to post sales of around $3 million in 1998, rather than previously anticipated $7 million.
