Abstract

BACKGROUND: Neurostimulation is a potentially interesting treatment option for children and adolescents with psychiatric disorders. AIM: To review the efficacy of two non-invasive neurostimulation techniques, namely repetitive transcranial magnetic stimulation (rtms) and transcranial direct current stimulation (tdcs), in the treatment of child and adolescent psychiatric disorders. METHOD: We searched the literature research using PubMed. RESULTS: There is some evidence that rtms is being used to treat unipolar depression, psychosis, autism spectrum disorder, attention deficit hyperactivity disorder and tic disorder. So far, however, very little research has been done on tdcs. CONCLUSION: Further research is vitally important in order to ensure the safety and efficacy of rtms and tdcs.

Abstract

BACKGROUND Thecausal influence of cortico-subcortical connectivity by means of brain stimulation seems to bean effective biological treatment in psychiatric patients. AIM To review the working mechanisms and moderating factors of two non-invasive brain stimulation techniques (NIBS), namely repetitive transcranial magnetic stimulation (rTMS) and transcranial direct current stimulation (toes). METHOD We reviewed the current literature on the use of NIBS in neuropsychiatrie disorders. RESULTS First of all, stimulation parameters (location of the stimulation, intensity and duration of the stimulation, number of sessions) are important for the effect of NIBS. Secondly, it is important to consider the non-specific neuroplasticity that results from NIBS.Thirdly, recent studies suggest that NIBS should be combined with neurobehavioral interventions, namely cognitive interventions, for the purpose of modulating specific neural processes (i.e. specific neuroplasticity). CONCLUSION If we want to Improve the NIBS treatment in neuropsychiatrie patients, we need to consider the factors that influence the patients' response to treatment with ITMS and tDCS.

Item does not contain fulltext, Antidepressant medication is the most common treatment for major depressive disorder (MDD), however, the precise working mechanism underlying these treatments remains unclear. Recent neuromodulation treatments demonstrate that direct stimulation of the dorsolateral prefrontal cortex (DLPFC), dorsomedial prefrontal cortex (DMPFC), and subgenual anterior cingulate (sgACC) relate to clinical improvement, suggesting connectivity alterations of the DLPFC-DMPFC-sgACC network to mediate antidepressant response. The international Study to Predict Optimized Treatment in Depression (iSPOT-D) is an international multicentre study that collected EEG data for 1008 MDD patients, randomized to 3 different antidepressant medications (N=447 MDD with complete pre- and post-treatment data and N=336 non-MDD). Treatment response was defined by a decline of >50% on the Hamilton Rating Score for Depression (HRSD17). We investigated whether connectivity in alpha and theta frequencies of the DLPFC-DMPFC-sgACC network changed from pre- to post-treatment between: (i) patients and controls, and (ii) responders (R) and non-responders (NR). Women exhibited higher alpha and theta connectivity compared to males, both pre- and post-treatment. Furthermore, theta, but not alpha, hypo-connectivity was found for MDD patients. A decreased alpha connectivity after treatment was found only for male responders, while non-responders and females exhibited no changes in alpha connectivity. Decreasing alpha connectivity could potentially serve as a treatment emergent biomarker, in males only. Furthermore, it could be useful to a priori stratify by gender for future MDD studies.

Abstract

In hun korte bijdrage gaan Schutter en Van den Hoven (2015) in op essentiële ethische overwegingen over een relatief nieuwe techniek die steeds meer ingang vindt in de psychiatrie: neurostimulatie. De reflecties van auteurs gaan verder dan het louter gebruik van informed consent en good medical practice. De auteurs richten zich meer in het bijzonder op een relatief nieuwe niet-invasieve behandelvorm voor majeure depressie: repetitieve transcraniële magnetische stimulatie (rtms). Zij beargumenteren dat met de huidige kennis en ervaring niet enkel de klinische effecten meer onderzocht moeten worden, maar dat er ook specifiek aandacht nodig is naar rtms-effecten op authenticiteit, wilsbekwaamheid en autonomie van de patiënt. Hierin hebben de auteurs gelijk. Niet alleen de ‘neuronale’ effecten na zulke toepassingen zijn onvoldoende bekend, maar ook het feit dat deze nieuwe techniek aan ‘uitbehandelde’ patiënten met een depressieve stoornis als een ‘last resort’ wordt aangeboden, geeft aanleiding tot ethische afwegingen.

Abstract

For nearly two decades now, transcranial magnetic stimulation (TMS) has been available as a noninvasive clinical tool to treat patients suffering from major depression. In this period, a bulk of animal and human studies examined TMS parameters to improve clinical outcome. However, the neurobiological mechanisms underlying mood changes remain an important focus of research. In addition to having an effect on neuroendocrinological processes, neurotransmitter systems, and neurotrophic factors, TMS may not only affect the stimulated cortical regions, but also those connected to them. Therefore, we will review current human data on possible neurobiological mechanisms of repetitive (r) TMS implicated in the deregulated neurocircuitry present in unipolar depression. Furthermore, as the rTMS application can be considered as a "top-down" neuronal intervention, we will focus on the neuronal pathways linked with the stimulated area and we will present an integrative model of action.

Abstract

A group of European experts was commissioned to establish guidelines on the therapeutic use of repetitive transcranial magnetic stimulation (rTMS) from evidence published up until March 2014, regarding pain, movement disorders, stroke, amyotrophic lateral sclerosis, multiple sclerosis, epilepsy, consciousness disorders, tinnitus, depression, anxiety disorders, obsessive-compulsive disorder, schizophrenia, craving/addiction, and conversion. Despite unavoidable inhomogeneities, there is a sufficient body of evidence to accept with level A (definite efficacy) the analgesic effect of high-frequency (HF) rTMS of the primary motor cortex (M1) contralateral to the pain and the antidepressant effect of HF-rTMS of the left dorsolateral prefrontal cortex (DLPFC). A Level B recommendation (probable efficacy) is proposed for the antidepressant effect of low-frequency (LF) rTMS of the right DLPFC, HF-rTMS of the left DLPFC for the negative symptoms of schizophrenia, and LF-rTMS of contralesional M1 in chronic motor stroke. The effects of rTMS in a number of indications reach level C (possible efficacy), including LF-rTMS of the left temporoparietal cortex in tinnitus and auditory hallucinations. It remains to determine how to optimize rTMS protocols and techniques to give them relevance in routine clinical practice. In addition, professionals carrying out rTMS protocols should undergo rigorous training to ensure the quality of the technical realization, guarantee the proper care of patients, and maximize the chances of success. Under these conditions, the therapeutic use of rTMS should be able to develop in the coming years.
