Giant cell arteritis (GCA) is an inflammatory vasculitis affecting large and medium‐sized arteries, leading to complications such as arterial dissection, blindness, and stroke. Rarely, GCA presents with Horner's syndrome due to sympathetic neuron involvement from arterial inflammation. This case report discusses an 82‐year‐old female with hypertension, atrial fibrillation, and arthritis who presented with a 24 h history of right eye ptosis, blurred vision, dizziness, and aching eye pain. She had a mild headache and tenderness over the right temporomandibular joint but no temporal artery tenderness. Examination revealed right eye ptosis and miosis, indicative of Horner's syndrome, with no other neurological deficits. Lab results showed elevated ESR (68 mm/h) and CRP (16 mg/L). MRI with contrast revealed mild to moderate stenosis and enhancement in bilateral MCAs and basilar artery with inflammation in the right distal extracranial ICA, suggesting an inflammatory process. The patient was started on prednisone 40 mg daily. A temporal artery biopsy confirmed GCA with characteristic histopathological findings. Her prednisone dosage was increased to 60 mg/day, and she was started on tocilizumab. This case underscores the need to consider GCA in patients with Horner's syndrome and the importance of vessel wall imaging, as early corticosteroid treatment can prevent complications like vision loss and stroke. BackgroundThe therapeutic significance of the cervical pessary has been confirmed by several studies. However, the underlying mechanism by which pessaries reduce the risk of a preterm birth remains elusive. The aim of this study is to investigate the hypothesis whether the application of a cervical pessary may stabilize the ectocervical stiffness in order to achieve a cervical arrest.MethodsThis is a prospective, controlled, non-interventional, post-market, monocentric, longitudinal, cohort study in a tertiary maternity hospital to determine ectocervical stiffness and its changes measured before and after placement of a pessary in singleton pregnancies with cervical shortening in the mid trimester. In order to assess reference values for cervical stiffness, we measured also singleton pregnancies with normal cervical length in the same gestational week spectrum. The cervical stiffness measured with the Pregnolia System as the Cervical Stiffness Index (CSI, in mbar) shall be the primary endpoint, whilst patient delivery data (gestational age, mode of delivery and complications) will be the secondary endpoint. In this pilot study, up to 142 subjects will be enrolled to have a total of 120 subjects (estimated dropout rate of 15%) to complete the study; pessary cohort: 60 (up to 71 recruited), control group: 60 (up to 71 recruited).DiscussionOur hypothesis is that patients with cervical shortening will present with lower CSI values and that pessary placement will be able to stabilize the CSI values through further prevention of cervical remodelling. The measurement of controls with normal cervical length shall serve as a reference.

Developing resistance to the currently available antimicrobial agents, particularly anti-Bacterial, is a worrying issue that rises highly worldwide. Communities, health workers, and government officials around the world are joining in, calling for overuse and misuse of antibiotics to stop. The phrase “de-marketing” indicates a strategy that drives to organize the form and the level of the current and future demand; not only that, but rationalizes it by organizations (profit, non-profit organizations as well as governments) specified to discourage and minimize activities of organizations about selling, distribution, using, and advertising… etc. It is to conduct an investigation of the potential impact of de-marketing in both controlling and minimizing the antibiotic misusage in Jordan. The essential purpose of the current analysis research is to manifest the contribution of the general demarketing strategy in influencing the attitude of consumers towards rationalization of taking antibiotics through regular medications, which is under the supervision and recommendations of professional doctors. In this article, we focused on those secondary resources derived from the published literature. Besides, we relied on the output of 450 consumers’ interactions and responses to our 19 questions distributed as a survey with consideration to the general ethics. The model of our study was designed based on the preliminary information collected from exploration study. The insight statistical analysis confirmed a attitude towards rationalization of taking antibiotics without doctor supervision (off prescription) that properly will raise the issue of antibiotics misuse along with its severe and dangerous side effects which will have a possible reduction of the undesirable and toxic effects after misusing the drugs. Moreover, this strategy would have a positive impact in lowering eco-toxicity of antibiotics misuse to words non-target aquatic organisms; plants or sediment and soil-inhabiting organisms. In conclusion, The problem of antibiotic resistance can be minimized only by concerted efforts of all members of society for ensuring the continued efficiency of antibiotics, the demarketing strategy shows a promising approach which could be used by any interested parties working to develop policies, strategies, interventions or campaigns to reduce antibiotic resistance.

ABSTRACT: Objectives: Extended-spectrum β-lactamase (ESBL)-producing Escherichia coli are an increasingly significant cause of hospital- and community-acquired infections worldwide. Whereas several reports have highlighted their increased prevalence also in North African countries, genomic data on isolates associated with these infections are still scarce. This study aimed to provide data on ESBL-producing E. coli isolates from patients with extraintestinal infections at the Military Teaching Hospital Mohamed V of Rabat, Morocco. Methods: Whole-genome sequencing was carried out on 18 ESBL-producing extraintestinal pathogenic E. coli (ExPEC) isolates for analysis of phylogenomic evolution, virulence factors and antimicrobial resistance genes. Data were compared with ExPEC lineages from several surrounding countries using multilocus sequence typing (MLST) and single nucleotide polymorphism-based phylogenetic approaches. Results: The majority of E. coli isolates were ST131 (n = 15), followed by ST617 (n = 2) and a novel sequence type (ST10703) that is closely related to the pandemic ST405 clone. All ST131 isolates belonged to the O25b-ST131 pandemic clone. They harboured more virulence genes than their non-ST131 counterparts. IncF plasmid replicons and the blaCTX-M-15 β-lactamase gene were identified in all isolates. No ESBL-producing E. coli isolates carried any known carbapenemase gene. Conclusion: Our findings underscore the pre-eminence of ST131 as the major factor driving the expansion of ExPEC in the Rabat region while highlighting the potential links with isolates circulating in other neighbouring countries.

Abstract The coronavirus pandemic, which appeared in Wuhan, China, in December 2019, rapidly spread all over the world in only a few weeks. Faster testing techniques requiring less resources are key in managing the pandemic, either to enable larger scale testing or even just provide developing countries with limited resources, particularly in Africa, means to perform tests to manage the crisis. Here, we report an unprecedented, rapid, reagent-free and easy-to-use screening spectroscopic method for the detection of SARS-CoV-2 on RNA extracts. This method, validated on clinical samples collected from 280 patients with quantitative predictive scores on both positive and negative samples, is based on a multivariate analysis of FTIR spectra of RNA extracts. This technique, in agreement with RT-PCR, achieves 97.8% accuracy, 97% sensitivity and 98.3% specificity while reducing the testing time post RNA extraction from hours to minutes. Furthermore, this technique can be used in several laboratories with limited resources.

Abstract: This study aims at introducing the local cultural heritage through popular healing practices in Algeria, It depends on the nature of the social and economic changes taking place in society and people's beliefs about the methods they use in order to heal, Recalls what is a therapeutic method in which forms of folk remedies are practiced, whether physical or natural remedies related to the folk medicine, to include the principle of cultural identity in a sense a number of values ​​and standards, and constitute culture the human being and his knowledge in a number of different fields, this knowledge and awareness of the issues surrounding him in society, represent the intellectual and cognitive heritage of him. This research study seeks to: 1- Detection and monitoring of traditional folk medicine forms in Algeria. 2- Finding out the most important local traditional healing practices that are rooted in the principle of cultural identity. 3- Explain the status of medical tourism in preserving the nature of the local folklore. 4- To uncover methods of intellectual efficiency and field practices that would enhance the cognitive and cultural interaction within the Algerian society.

The present paper, in spite of the difficulties to be foreseen, intends to lead a comparative discussion on some theoretical propositions, in particular typological ones, that have been developed according to distinct methods around two different objects: on the one hand, the mental states (intentions, desires, emotions...) that philosophy, now backed by psychology, labels and discerns; on the other hand, the modalities (alethic, epistemic, deontic...) described by linguistics. It seems plausible indeed that the association of the notion of will and the verb to want, or of the notion of belief and the verb to believe, to mention only the most obvious cases, leads to overlapping interests between the philosophical approach of mental states and the linguistic and semiotic approach of modalities. Three authors will guide this discussion: linguist Laurent Gosselin, phenomenological philosopher Uriah Kriegel and semiotician Jacques Fontanille. The presentation of their respective models will reveal two common grounds: first, the way in which each of these theoretical models integrates as a typological differentiator that very thing from which its object differs, namely, a “world” contained or represented in mental states as in modalities; second, the appeal to a certain “presence”, which, however equivocal or problematic the notion may be, seems essential when it comes to accounting for differences between the types of object considered.
