61. A rare cause of salt-wasting in early infancy: Transient pseudohypoaldosteronism

Abstract

Three infants aged between 38 days and 43 days all presented with poor weight gain, hyponatremia, hyperkalemia, and were diagnosed as having urinary tract infections, which were accompanied by urinary tract malformations in our cases. Hydration and infection treatments were given. A few days after admission, hormonal studies revealed normal cortisol and 17-hydroxy progesterone levels and markedly high aldosterone levels, thus the patients were diagnosed as having transient pseudohypoaldosteronism. After the proper treatment was given, the transient pseudohypoaldosteronism resolved. In conclusion, when an infant with urinary tract infection or malformation has electrolyte abnormalities, pediatricians should consider the diagnosis of transient pseudohypoaldosteronism.

Abstract

Amaç: Fizik Tedavi ve Rehabilitasyon Servislerinde yatan hastaların disfaji prevalansını tespit etmektir. Ve Fizik Tedavi ve Rehabilitasyon servislerinde Dil ve Konuşma Terapistlerinin gerekliliğini ortaya koymaktır. Gereç ve Yöntem: Demografik bilgileri alınıp çalışmaya dahil edilmeye uygun görülen hastalara Eating Assessment Tool-1 (EAT-10) ve Disfaji Handikap İndeksi (DHİ) anketleri uygulanmıştır. EAT-10 anketinden 3 ve üzeri paun alan hastalara Volüm Viskozite Yutma Testi (V-VST) testi uygulanmıştır. Veriler Microsoft Excel 2010 programı ile analiz edilmiştir. Bulgular: Çalışmaya 21’si erkek, 30’u kadın 51 hasta alınmıştır. Hastalar 21 yaş –90 yaş aralığındadır. Hastaların 24’i nörolojik, 27’si ortopedik tanı ile serviste yatmaktadır. 2 hasta PEG ile beslenmektedir. 40 hasta EAT-10 anketinde 3 puan altı almış ve yutma işlemlerinin güvenli olduğu saptanmıştır. 9 hastaya V-VST testi uygulanmış ve değişen parametreler, hacim ve kıvamlarda yutma güvenliğinin bozulduğu tespit edilmiştir. Sonuç: Çalışmaya katılan 51 hastanın 11’ine disfdaji tanısı konmuştur. Bu hastaların 9’u kadın, 2’si erkektir. 10’u nörolojik, 1’i ortopedik tanı ile serviste yatmaktadır. Fizik tedavi ve rehabilitasyon servisi disfaji prevalansı %21.5 olarak saptanmıştır. Aim: It was aimed to determine the prevalence of dysphagia in patients hospitalized in the physical therapy and rehabilitation service of a hospital in this study. Material and Method: The data were collected in physical therapy and rehabilitation service in Fatih Sultan Mehmet Training and Research Hospital The data were collected via EAT-10 and DHI questionnaires. V-VST test was applied if the participants got 3 or more points from EAT-10. Microsoft Excel 2010 analysis program was used for the other necessary analyzes. Results: A total of 51 participants, 21 men and 30 women, were included in the study. Patients were between the ages of 21 and 90. 24 of the patients were hospitalized with neurological diagnosis and 27 of the patients were in the hospital with orthopedic diagnosis. 2 patients were fed with PEG. 40 patients scored less than 3 points on the EAT-10 questionnaire and swallowing procedures were found to be safe. V-VST test was applied to 9 patients and it was seen that swallowing safety was impaired in changing parameters, volume and consistency. Conclusion: Dysphagia was diagnosed in 11 of total 51 participants. 9 of these participants were female and 2 are male. The prevalence of physical therapy and rehabilitation service dysphagia was found to be 21.5%.

Abstract

Diabetes care at school has recently appeared on the agenda of international diabetes organizations, the basic principles of which have been newly determined. The aim of this review was to summarize the activities and output of the Diabetes at School Program - a program that has been delivered in Turkey for the last 10 years - and to focus on different aspects of Diabetes Care at School through a national model. Recently, a detailed set of national regulations, including the basic principles proposed by the International Society for Pediatric and Adolescent Diabetes and the experience in Turkey, was prepared and has come into force. The future agenda includes giving priority to socio-economically disadvantaged regions, provision of an Individual Treatment Plan at School for each child with diabetes and ensuring that each school has an action plan for the care of children with diabetes. We believe that if all countries have programs and structured national regulations similar to the Diabetes at School Program, this will enable significant progress in the level of care delivered to children with diabetes., NA

Abstract

Amaç: Yetişkinlerde AMH ile antral folikül sayısı arasında pozitif korelasyon vardır. Peripubertal sağlıklı kızlarda da AMH ile toplam folikül sayısı ve folikül büyüklüğü arasında ilişki saptanmıştır. Çalışmamızda; santral puberte prekoks (SPP) tanısı almış kızlarda AMH düzeyi ile folikül sayı ve büyüklüğünün ilişkisi araştırıldı.Gereç Ve Yöntemler: Santral puberte prekoks tanılı 32 kız olgu çalışmaya alındı. Hastalardan AMH için kan örneği alınarak pelvik ultrasonografi ile overde folikül sayımı yapıldı. Foliküller büyüklüklerine göre; küçük (1-4,4 mm), orta (4,5-9,4 mm) ve büyük (≥9,5mm) olarak sınıflandırıldı. Hastalar puberte evrelerine ve antral folikül sayısına göre gruplandırıldı.Bulgular: Hastaların AMH düzeyi ile küçük, orta, büyük folikül sayısı, küçük/orta folikül oranı, en büyük folikül boyutu ve toplam folikül sayısı arasında anlamlı bir ilişki saptanmadı (p > 0,05). Hastalar meme evrelerine göre sınıflandırıldığında da grupların AMH düzeyleri, küçük, orta ve büyük folikül sayısı, küçük/orta folikül oranı, en büyük folikül boyutu ve toplam folikül sayısı farklı değildi (p>0,05). Birden fazla antral folikülü olanların AMH düzeyi [median:0,23 ng/ml, (0,06-0,56)], en fazla bir antral folikülü olanların AMH düzeyinden [median:0,17ng/ml (0,05-0,48)] anlamlı olarak daha yüksekti (p:0,035).Sonuç: Puberte prekokslu kızlarda AMH düzeyi ile folikül sayı ve boyutu arasındaki ilişkinin ilk defa incelendiği çalışmamızda; AMH düzeyi ile over hacmi, folikül sayı ve büyüklüğü arasında anlamlı bir ilişki saptanamamış olması hastalarımızın erken pubertal evrede olmasından kaynaklanıyor olabilir. Bununla birlikte; puberte prekoksta folikül gelişim paterninin normal puberteden farklı olabileceği ya da folikül sayısı dışında AMH’ya etkili başka faktörlerin de olabileceği ihtimali de dışlanamaz.

Abstract

Gender incongruence (CI) is a condition in which there is a marked discrepancy between one's experienced/expressed gender and sex assigned at birth for at least 6 months, accompanied by clinically significant distress. In adolescents whose diagnosis for gender incongruence is confirmed after psychiatric evaluations, medical treatments are provided in three stages. The first stage is suppression of puberty with gonadotropin-releasing hormone (GnRH) analogue therapy, the second stage is the addition of sex steroids to the treatment, the third stage is surgical correction after the age of 18 and continued sex steroid therapy. The gender adjustment process in individuals with CI is quite long and difficult. However, as social awareness and experience of clinicians increase, it will be easier for these individuals to access necessary treatments. [ABSTRACT FROM AUTHOR]

Abstract

It is well-known that in children with type 1 diabetes (T1D), the frequency of Celiac disease (CD) is increased due to mechanisms which are not fully elucidated but include autoimmune injury as well as shared genetic predisposition. Although histopathologic examination is the gold standard for diagnosis, avoiding unnecessary endoscopy is crucial. Therefore, for both clinicians and patients’ families, the diagnosis of CD remains challenging. In light of this, a joint working group, the Type 1 Diabetes and Celiac Disease Joint Working Group, was convened, with the aim of reporting institutional data and reviewing current international guidelines, in order to provide a framework for clinicians. Several controversial issues were discussed: For CD screening in children with T1D, regardless of age, it is recommended to measure tissue transglutaminase-immunoglobulin A (tTG-IgA) and/or endomysial-IgA antibody due to their high sensitivity and specificity. However, the decision-making process based on tTG-IgA titer in children with T1D is still debated, since tTG-IgA titers may fluctuate in children with T1D. Moreover, seronegativity may occur spontaneously. The authors’ own data showed that most of the cases who have biopsy-proven CD had tTG-IgA levels 7-10 times above the upper limit. The decision for endoscopy based solely on tTG-IgA levels should be avoided, except in cases where tTG-IgA levels are seven times and above the upper limit. A closer collaboration should be built between divisions of pediatric endocrinology and gastroenterology in terms of screening, diagnosis and follow-up of children with T1D and suspicious CD. Abstract

We aimed to determine the prevalence of early puberty in girls with premature pubarche and analyze the time interval between their pubarche and succeeding thelarche. This study included 60 female children with premature pubarche. We retrospectively collected clinical, laboratory, and radiological findings from all participants. We investigated the time interval between pubarche and thelarche in cases wherein premature pubarche was followed by thelarche. The mean age at onset of pubarche was 6.93 ± 0.79 yr old. Among the participants, 16.7% were preterm, 20% were small for gestational age (SGA), and 55% were overweight or obese. The mean time interval between pubarche and thelarche was 11.20 ± 7.41 mo. The mean serum DHEA-S level was higher in the preterm group (p = 0.016), and DHEA-S levels were generally higher in the SGA group (p = 0.004). This study documented the presence of being overweight or obese and having more advanced growth than their genetic potential in half of the patients who had premature pubarche. In addition to these identified risk factors, obesity-independent DHEA-S levels were observed to be higher in patients who had early puberty with the first six months of their follow-up considered to be the most critical time in predicting early puberty.
