In Brazil, despite the improvements that have occurred in the health of mothers and children, indicators of maternal and neonatal mortality are still far from desirable. To reverse this situation, it is important to rethink the process of work, considering the hospital and perinatal care network planning, and focus on institutional democratization. This study aimed to discuss the process of application of the "strategic demarche" approach in a State public maternity with a view to coordinated network of services. The period was from June to September 2009 and included the direct participation of professionals / stakeholders, including the hospital board. The results showed the method's ability to question the work processes and the analysis of hospital management. Based on the analyses, the experience pointed action strategies validated by different actors. The method can be considered innovative and effective to produce more cohesive health teams and co-responsible with changing practices movements. It allowed the creation of communication channels between maternity professionals, and between these actors and the external network. The study showed the potential of the approach as a tool for situational analysis and hospital management to review the institutional mission in search of quality and solidarity within the network, committed to attend users in a universal and equitable way. However, the commitments expressed in the propositional synthesis must be constantly reviewed and renegotiated by the actors, in order to achieve permanent institutional changes. Information about several articles on health planning, presented at the seminar held in Brazil by the National School of Public Health Sergio Arouca (ENSAP), the Department of Administration and Health Planning (DAPS), and the National Council for Scientific and Technological Development (CNPq), is presented. Topics include the methodological approaches of planning in the national and international scenarios. Moreover, it acknowledges the presence of Professor Michel Crémadez.

The area of Policy, Planning and Health Management (PPG) express the intersection between research, intervention and political action. The article analyzes the dynamics of knowledge production about PPG in the Journal Ciência & Saúde Coletiva (C&SC) from 1996 to 2019. The study articulates a bibliometric and qualitative approach to explore the profile of articles in three dimensions: thematic, methodological and authorship/institutional partnerships. 1680 PPG manuscripts were identified, corresponding to 28.3% of the total publications. Thematic analysis showed a strong influence of the context and the implementation of Unified Health System (SUS). Qualitative empirical studies predominated, followed by quantitative and quanti-qualitative studies. The combination of methodological procedures was frequent, with bibliographic review and interviews being the most cited. The average number of authors per article and national institutional partnerships increased. As most articles were published in Portuguese, the challenge of internationalization persists. The PPG area stands out in the Journal C&SC and in the production of scientific knowledge relevant to SUS.

The objective was to analyze the discourses of adolescents and health professionals in a state of Venezuela concerning this age group's access to information on sexual and reproductive health. Interviews were conducted with 12 adolescents and 12 health professionals and processed with Fairclough's Critical Discourse Analysis. The results were structured in three themes: gender inequalities; strategies used by adolescents; and difficulties in the health services. The arguments revealed that matricentric family culture limits girls' access to information on sexual health. Greater effort is required of girls and homosexual adolescents to access information. Both of them seek health services outside their area of residence, due to fear of the family or lack of trust in professional secrecy. The prime sources for adolescents to information are friends and the Internet. One of the main challenges is to recognize the institutional devices acting in Venezuelan society, with greater force in power relations in the family, school, and health services. Such barriers reinforce the "sexual double standard", perpetuating models of patriarchal domination. The naturalization of such models allows the silent reproduction of beliefs and attitudes, causing gender inequalities in access to health services. Changes in the provision of services offered to adolescents involve renegotiation of traditional subjective positions in user-provider interaction and deconstruction of gender relations. Possible changes will be engendered with participation by new generations of adolescents, health professionals, parents, and teachers, producing more equitable gender discourses and norms.

The article analyzes the application of four criteria for the (re)distribution of basic health teamwork at the local level. This was a descriptive, cross-sectional, case study conducted in Nueva Esparta, Venezuela, in 2016. An instrument was designed to consolidate in a single database the local data on basic health teamwork in 170 popular clinics serving 447 communities. The target criteria were: (1) basic health teamwork/population ratio, calculating the number of complete and incomplete basic health teamwork and the density of health professionals per 10,000 inhabitants; (2) distance between the popular clinics and the population; (3) characteristics of the health professionals comprising the basic health teamwork, by sex, age, and health profession; (4) analysis of the metrics for inequality as an innovative criterion, calculating two summary measures of gap and one health inequality gradient to assess the proposals for (re)distribution of the basic health teamwork. There were differences within states at the municipal and local community levels in the distribution of health professionals and a shortage of nurses and health promoters. Of the 317 required basic health teamwork, there were 52 complete basic health teamwork. The measures of inequality reflect an eco-social equidistribution of human resources, favoring the poorer populations. Combined application of the criteria allowed identifying geographic areas and populations with the greatest need and shortage of basic health teamwork and optimizing their equitable (re)distribution at the local level. The limitation of lack of disaggregated information was overcome by using a single data repository with consensual information among the state and local health teamwork. In conclusion, investments should be made in efficient information systems.

This study shows research results on the persistence of RhD alloimmunization in pregnant women seen in the public healthcare network in Rio de Janeiro State, Brazil, through patient file analysis and interviews with administrators, health professionals, and patients. We analyzed 289 patient files of RhD-negative pregnant women seen from 2004 to 2012 at the State Reference Center. Individual interviews were held with 15 individuals. The interviews revealed factors contributing to persistence of the problem, such as: loss of restructuring of the Program for the Prevention of RhD Alloimmunization (PPARhD); lack of a system for monitoring the Program; low linkage between the referral and counter-referral network during prenatal care; insufficient training of the health professionals treating pregnant women at risk; and insufficient visibility of the program for RhD alloimmunization prevention. These critical points highlight the need for the State Health Department to reinforce prioritization of the program for alloimmunization prevention and invest in referral and counter-referral in the healthcare network. The study showed that the problem has multiple causes and requires interdisciplinary and complex measures related to comprehensive prenatal care.

This article discusses the application of theories of argumentation and communication to the field of medicine. Based on a literature review, the authors compare Toulmin's model, pragma-dialectics, and the work of Todd and Fisher, derived from American sociolinguistics. These approaches were selected because they belong to the pragmatic field of language. The main results were: pragma-dialectics characterizes medical reasoning more comprehensively, highlighting specific elements of the three disciplines of argumentation: dialectics, rhetoric, and logic; Toulmin's model helps substantiate the declaration of diagnostic and therapeutic hypotheses, and as part of an interpretive medicine, approximates the pragma-dialectical approach by including dialectical elements in the process of formulating arguments; Fisher and Todd's approach allows characterizing, from a pragmatic analysis of speech acts, the degree of symmetry/asymmetry in the doctor-patient relationship, while arguing the possibility of negotiating treatment alternatives.

Society's participation in health policymaking is an underlying principle of the Brazilian public health system. The study analyzed participants' discourses on the role and representativeness of municipal health conferences. The study was conducted in five municipalities (counties) in Mato Grosso State, Brazil, in 2009-2010. Thirty social actors were interviewed, including municipal health council members and health system administrators, planners, city council members, and members of the office of the public prosecutor. Data were analyzed using the Qualiquantisoft software for the elaboration of the collective subject's discourse. The discourses identified by the study showed a consensus on the importance of the municipal health council, even among those who questioned the effectiveness of its planning and administrative decisions. The studied revealed discourses resulting from interpretations based on different theoretical traditions. The collective discourses presented both complementary and conflicting arguments, seeking greater resonance and influence in the public health sphere and political system and greater administrative power.

This study is based on an adaptation of the Strategic Démarche Approach applied to the Evandro Chagas Institute of Clinical Research (IPEC) in Brazil, from April to July 2009. The results are related to the experience of the Leishmaniasis Laboratory. A strategic analysis of four homogeneous segments was performed, considering the administration of care and specific teaching and research indicators. In both portfolios, all segments showed competitiveness and an appropriate response to mission goals. However, the second portfolio (teaching/research) showed a greater concentration of production in one segment. A highlight was the presence in the work team of cultural traits consistent with the objectives of cultural change. This experience, by operating indirectly in furtherance of the features of the new culture that emerges, revealed the potential of the method in question, to effect policies, processes and resources in a democratic and communicative way, which contributes to the operationalization of the guidelines of the Brazilian Unified Health System.

The article discusses the appropriation of the theory developed by Habermas to analyze health policies and management. The fundamental concepts of the discursive theory of democracy as a deliberative policy, procedural democracy, the public sphere and civil society are analyzed. An attempt is made to demonstrate that the concepts of deliberative policies are grounded on basic theoretical categories of Habermas's conception of language, namely the theory of communicative action (TCA): lifeworld and system; communicative action and discourse; the ideal speech situation. The possibility of translating the categories presented in analytical categories, such as the experiences of social participation in deliberative forums and the results for the formulation and implementation of policies and health management is discussed. The conclusion drawn is that the theoretical categories reveal great explanatory potential and analytical categories are important provided that they are mediated and contextualized.

The objective of the article was to propose a model of communication management of networks for the Health Innovation System in Brazil. The health production complex and its relationship with the nation's development are addressed and some suggestions for operationalization of the proposed model are also presented. The discussion is based on Habermas' theory and similar cases from other countries. Communication strategies and approaches to commitment dialogue for concerted actions and consensus-building based on critical reasoning may help strengthen democratic networks.

This article presents the results of a study of the type before and after training in Shelter with Risk classification compared to the humanization of social representations of health among 111 health professionals in an emergency hospital. Data collection was performed using the technique of free evocation and analysis was done using the EVOC software. The results showed changes in the symbolic meaning attributed to terms inducers towards incorporation of the right perspective in the representation of the Unified Health System, the translation of Humanization in health as Shelter, and the progression of understanding the Shelter, the humanistic focus to the qualification of processes of care of the users demand at emergencies. The results indicate that the differences found in the core before and after training were due to an apprenticeship, which based in peripheral elements, was able to question the core elements and interchange between the central and peripheral system, recognizing the functional complementarity between these two systems and the relationships between representations and practices. However, the method does not assert the persistence of such changes in the social representations of the objects studied in depth or measure the changes in daily practices.

This article aims to present a condensed overview of major trends in Brazil, establish a general taxonomy of models of health management and planning, based on the international scene and support the proposal for a communicative planning. In a context of democratization,argues for a pluralistic conception and concludes that the various currents, albeit with different perspectives and referrals theoretical and methodological dialogue in a process of mutual exchange and learning.

The article discusses the results of operationalizing Situational Strategic Planning adapted to the local level in health, considering the communicative approach and equity in a parish in Venezuela. Two innovative criteria were used: estimated health needs and analysis of the actors' potential for participation. The problems identified were compared to the corresponding article on rights in the Venezuelan Constitution. The study measured inequalities using health indicators associated with the selected problems; equity criteria were incorporated into the action proposals and communicative elements. Priority was assigned to the problem of "low case-resolving capacity in the health services network", and five critical points were selected for the action plan, which finally consisted of 6 operations and 21 actions. The article concludes that the combination of epidemiology and planning expands the situational explanation. Incorporation of the communicative approach and the equity dimension into Situational Strategic Planning allows empowering health management and helps decrease the gaps from inequality.
