Abstract

Prenatal stress exposure increases vulnerability to virtually all forms of psychopathology. Based on this robust evidence base, we propose a "Mental Health, Earlier" paradigm shift for prenatal stress research, which moves from the documentation of stress‐related outcomes to their prevention, with a focus on infant neurodevelopmental indicators of vulnerability to subsequent mental health problems. Achieving this requires an expansive team science approach. As an exemplar, we introduce the Promoting Healthy Brain Project (PHBP), a randomized trial testing the impact of the Wellness‐4‐2 personalized prenatal stress‐reduction intervention on stress‐related alterations in infant neurodevelopmental trajectories in the first year of life. Wellness‐4‐2 utilizes bio‐integrated stress monitoring for just‐in‐time adaptive intervention. We highlight unique challenges and opportunities this novel team science approach presents in synergizing expertise across predictive analytics, bioengineering, health information technology, prevention science, maternal–fetal medicine, neonatology, pediatrics, and neurodevelopmental science. We discuss how innovations across many areas of study facilitate this personalized preventive approach, using developmentally sensitive brain and behavioral methods to investigate whether altering children's adverse gestational exposures, i.e., maternal stress in the womb, can improve their mental health outlooks. In so doing, we seek to propel developmental SEED research towards preventive applications with the potential to reduce the pernicious effect of prenatal stress on neurodevelopment, mental health, and wellbeing. Abstract

Background - Addressing the increasing obesity rates in children living in the United Kingdom has become a priority. A public health level approach as opposed to an individual approach is potentially one way forward. The wider food environment should be designed so that the ‘healthier choice’ is the easiest choice; this includes public sector settings such as hospitals. Many hospital outlets sell and promote food and drinks high in sugar, fat and salt undermining health messages developed by the UK National Health Service. Financial incentives have been provided to encourage hospitals to promote healthier food choices; however, few outlets have complied with the targets.\ud \ud The aim of this qualitative interview study was to determine the dietary perceptions and needs of parents whose children attend a large children’s hospital in the North East of England and to identify potential barriers and facilitators to eating healthily in a hospital setting.\ud \ud Methods - Eighteen parents whose children attended the hospital as an in- or out-patient were recruited through either ward research nurses, information posters or a parent hospital Facebook page to participate in a one-to-one in-depth interview.\ud \ud Results - Parents reported a lack of affordable healthy options for sale both for themselves and visiting children. Although parents wanted to see more healthy options available for sale they did not feel it was appropriate to ban or restrict sales of any food types. Parents of frequent or long-term in-patients found it difficult to adequately feed themselves.\ud \ud Conclusions - The ways in which visitors and staff can be encouraged to choose the healthier option in an NHS hospital setting warrants further investigation. The use of ‘nudge theory’, which has gained particular momentum in areas such as health promotion, may be a tool which can be utilised by hospitals to facilitate the promotion of healthy eating.

Abstract

Wearable devices monitoring food intake through passive sensing is slowly emerging to complement self-reporting of users' caloric intake and eating behaviors. Though the ultimate goal for the passive sensing of eating is to become a reliable gold standard in dietary assessment, it is currently showing promise as a means of validating self-report measures. Continuous food-intake monitoring allows for the validation and refusal of users' reported data in order to obtain more reliable user information, resulting in more effective health intervention services. Recognizing the importance and strength of wearable sensors in food intake monitoring, there has been a variety of approaches proposed and studied in recent years. While existing technologies show promise, many challenges and opportunities discussed in this survey, still remain. This paper presents a meticulous review of the latest sensing platforms and data analytic approaches to solve the challenges of food-intake monitoring, ranging from ear-based chewing and swallowing detection systems that capture eating gestures to wearable cameras that identify food types and caloric content through image processing techniques. This paper focuses on the comparison of different technologies and approaches that relate to user comfort, body location, and applications for medical research.
