Spontaneous intracranial hypotension is an important cause of new daily persistent headaches in young and middle-aged individuals. The diagnosis is made based on low cerebrospinal fluid pressure with characteristic findings upon brain and spinal magnetic resonance imaging (MRI). We present the case of a 15-year-old boy with spontaneous intracranial hypotension. Although his brain MRI was normal, his lumbar spinal MRI showed clustering of the nerve roots characteristic of arachnoiditis. Radionuclide cisternography revealed an epidural leak, which was treated with an epidural blood patch. The patient reached a near-full recovery within 24 h, and the lumbar spinal MRI findings mimicking arachnoiditis disappeared. [ABSTRACT FROM AUTHOR]

Acute cerebellitis is an inflammatory syndrome characterized by cerebellar dysfunction with acute onset. Here, the clinical findings, conventional and diffusion-weighted MR imaging features of five cases are reported. The most common clinical findings were headache, vertigo, nausea and vomiting. Neurological examination revealed cerebellar ataxia, nystagmus, dysmetria and dysdiadokinesia. Analysis of the CSF showed an elevated protein content and pleocytosis. Bilateral cerebellar cortical involvement and leptomeningeal enhancement along the folia represent the most common imaging presentations. Diffusionweighted MR imaging showed restricted diffusion in two patients. In one patient, diffusionweighted MR imaging did not show restricted diffusion. All patients were treated medically. Additionally, external ventricular drainage was performed in one patient. Follow-up MR imaging revealed cerebellar atrophy in 1 patient and gliosis in 2 patients. Two patients completely resolved after medical therapy. In conclusion, brain MR imaging and diffusionweighted MR imaging are valuable imaging methods in diagnosis of cerebellitis, assessment of complications and sequel lesions. [ABSTRACT FROM AUTHOR]

Hypothalamic hamartomas (HH) are rare non-neoplastic congenital malformations. The aims of this study are to demonstrate the brain MR findings and clinical manifestations in children with HH and the literature is reviewed. We have evaluated 6 children with HH diagnosed by brain MR imaging. We analyzed age at seizure onset, seizure types, neuropsychological assessment, and the presence of precocious puberty, electroencephalogram and MR findings. Among 6 HH patients, 2 manifested precocious puberty, 2 had experienced seizures, and 1 presented with both precocious puberty and seizures, and 1 presented with hyperactivity. MR imaging showed hypothalamic mass that was isointense to brain parenchyma on T1- weighted images and isointense-or hyperintense on T2-weighted images, without contrast enhancement. The size of the lesions does not change in the course of follow-up. Typical clinical presentation, peculiar location and MR imaging features strongly favour the HH diagnosis. Accurate diagnosis and classification is important for the selection of treatment modality. [ABSTRACT FROM AUTHOR]

Colonic herniation through the esophageal hiatus is an unusual condition. Isolated trans-hiatal herniation of the transverse colon is very rare and only five cases have been reported. All these reported patients were symptomatic, and had dysphagia, epigastralgia, vomiting, dyspnea, cough, or palpitations. Hiatal hernia is an important clinical problem. Because of the severe complications of hiatal hernia, including bleeding, strangulation, and perforation, early diagnosis and treatment are important. A 75-year-old man who had prostate carcinoma was evaluated with multidetector computed tomography (MDCT). In this case, CT findings and clinical emphasis of the incidentally discovered isolated transverse colonic hiatal herniation was reviewed. [ABSTRACT FROM AUTHOR]

Inflammatory bowel disease (IBD) is an idiopathic inflammatory disease of the gastrointestinal tract in which the pathogenesis is not known wholly. Thrombosis is an important risk that may be encountered in this disease and it is thought to be associated with the pathogenesis of IBD. Deep venous thrombosis and pulmonary emboli are the most common thrombotic complications in IBD patients, however, cerebral vascular involvement is rare. In these patients, although thromboembolic complications are uncommon, they are important because of their high mortality. In this report, the clinical picture with a fatal course of a patient, newly diagnosed with ulcerative colitis, who developed a cerebral venous sinus thrombosis is discussed. [ABSTRACT FROM AUTHOR]

Gallstone-induced ileus is a rare complication of cholelithiasis, and gastric outlet obstruction is even rarer. We describe the multidetector computed tomographic diagnosis of small bowel obstruction resulting from a gallstone impacted in the distal ileum and of gastric outlet obstruction from a gallstone impacted in the pyloric antrum (Bouveret syndrome).

Background/aims: Spontaneous intestinal hematoma is a rare complication of anticoagulant therapy, and small bowel obstruction caused by intramural hematoma secondary to anticoagulant therapy is even rarer. The first symptom is usually abdominal pain, frequently accompanied by nausea and vomiting. A history of anticoagulant use with prolonged international normalized ratios in patients presenting with abdominal pain should alert physicians to search for this entity. Typical findings on abdominal computed tomography yield the diagnosis. Early diagnosis is crucial because most patients are treated non-operatively with a good outcome. Herein, we present the non-enhanced and enhanced computed tomography findings of two cases who were admitted to the emergency clinic with acute abdominal pain and diagnosed as spontaneous intramural ileal hematomas and partial small bowel obstruction secondary to intramural ileal hemorrhage.
