Abstract Chronic myelomonocytic leukaemia is an atypical myeloproliferative disorder with a natural history of progression to acute myeloid leukaemia, a complex and poorly understood response by the bone marrow to stress. Cardiac surgery activates many inflammatory cascades and may precipitate a systemic inflammatory response syndrome. We present a case of undiagnosed chronic myelomonocytic leukaemia who developed rapidly fatal multi-organ dysfunction following cardiac surgery due to an acute leukaemoid reaction.

Abstract Background The beneficial effect of aspirin after coronary surgery is established; however, a recent study reported the inability of low doses (100 mg) to inhibit postoperative platelet function. We conducted a double-blind randomised trial to establish the efficacy of low dose aspirin and to compare it against medium dose aspirin. Methods Patients undergoing coronary surgery were invited to participate and consenting patients were randomised to 100 mg or 325 mg of aspirin daily for 5 days. Our primary outcome was the difference in platelet aggregation (day 5 – baseline) using 1 μg/ml of collagen. Secondary outcomes were differences in EC50 of collagen, ADP and epinephrine (assessed using the technique of Born). Results From September 2002 to April 2004, 72 patients were randomised; 3 patients discontinued, leaving 35 and 34 in the low and medium dose aspirin arms respectively. The mean aggregation (using 1.1 μg/ml of collagen) was reduced in both the medium and low dose aspirin arms by 37% and 36% respectively. The baseline adjusted difference (low – medium) was 6% (95% CI -3 to 14; p = 0.19). The directions of the results for the differences in EC50 (low – medium) were consistent for collagen, ADP and epinephrine at -0.07 (-0.53 to 0.40), -0.08 (-0.28 to 0.11) and -4.41 (-10.56 to 1.72) respectively, but none were statistically significant. Conclusion Contrary to recent findings, low dose aspirin is effective and medium dose aspirin did not prove superior for inhibiting platelet aggregation after coronary surgery.

This article proposes a tri–band Multiple Input Multiple Output (MIMO) antenna operating at 5.2 – 5.7 GHz (5G Wi–Fi), 11.8 GHz – 17.3 GHz (Ku–band), and 23.4 GHz – 37.3 GHz (5G millimeter–Wave) frequency bands. The proposed MIMO antenna consists of a 2 × 2 configuration with a defective ground structure on Rogers–5880 substrate. An innovative passive decoupling structure, featuring thin microstrip lines positioned between the radiating elements, has been meticulously engineered to ensure a high level of isolation, achieving values of less than −20 dB. The peak gain of the MIMO antenna is found to be 3.05 dB, 5.27 dB, and 6.97 dB at 5.5 GHz, 17 GHz, and 35.5 GHz frequency bands, respectively. Measured ECC is less than 0.007, and DG is greater than 9.96. The radiation efficiency is above 90 %, TARC is less than –10 dB and channel capacity loss is below 0.4 Bits/s/Hz in all the bands of operation. The simulated and measured results are found to be in good agreement and the proposed MIMO antenna offers numerous advantages, including extensive frequency coverage, broad bandwidth, exceptional isolation, high gain, and efficiency. View large Download slide View large Download slide OBJECTIVES Donor organ utilization and shortage remain the major limitations to the opportunity of a lung transplantation (LTx). Donation after circulatory determined death (DCD) has been adopted as a source of additional organs worldwide. However, concerns about organ quality and ischaemia–reperfusion injury have limited its application. The aim of this study was to retrospectively analyse a single-centre experience in the DCD LTx and compare early and mid-term outcomes with those from a standard donation after brain death (DBD). METHODS During the 6-year study period, 186 LTxs were performed: 147 bilateral LTxs (79%) and 39 single LTxs (21%). Of these, 23 recipients received organs retrieved from DCD donors (12.4%). RESULTS No differences were found between the 2 groups of recipients except for age and cystic fibrosis as an underlying disease. No differences in terms of duration of mechanical ventilation, incidence of postoperative extracorporeal membrane oxygenation support, intensive care unit stay, hospital length of stay, airway anastomotic complications, incidence and grade of rejection and freedom from bronchiolitis obliterans syndrome were demonstrated. There was a non-statistically significant trend towards older age in the DCD group. Actuarial survival in the subgroup of bilateral LTx at 1 year and 5 years was 75% and 51% for the DCD group and 82% and 61% for the DBD group, respectively (P  =   0.12). CONCLUSIONS Short- and medium-term outcomes after the DCD LTx are comparable with those achieved after transplantation from the DBD donors, despite a tendency to use DCD lungs for older recipients. We study a nonlinear generalized Sawada-Kotera equation of fractional order via the exp(-φ(η))-expansion method and Shifted modified Chebyshev Wavelet technique. We obtain abundant exact solutions and approximate solution of the equation. The results of the study shows that the exp(-φ(η))-expansion method is very effective and proficient for solving nonlinear fractional partial differential equations. The solitary wave solutions are obtained through the hyperbolic, trigonometric, exponential and rational functions. Graphical representations along with the numerical data reinforce the efficacy of the used procedure. The specified idea is very expedient for fractional PDEs, and could be extended to other physical problems.
