This case-control study's purpose is to investigate serum irisin. levels and their role in people with type 2 diabetes mellitus (T2DM) who don't have hypothyroidism. A total of 60 patients with T2DM were chosen as the) case group (based on the grounds for inclusion and exclusion, and a 1:1 ratio was determined based on the patients' inclusion rate. This cohort, which was created at the Medical City / Baghdad Teaching Hospital, consisted of adults aged 30 to 60. The control group consisted of 60 healthy volunteers of the same gender and age. Clinical data on the participants was collected in order to determine their medical history, BMI, fasting blood glucose (FBS), hemoglobin (HbA1c), insulin, and serum lipid levels. An enzyme-linked immunosorbent test was used to measure the amount of irisin in the blood (ELISA). The data was analyzed using SPSS 23.0 software. only one variable. The amount of irisin in the T2DM group was substantially higher than in the control group (12.77±2.236 ng /mL and 4.121±0.308 ng /mL, respectively, according to single factor analysis. BMI, fasting blood sugar (FBS), HbA1c, insulin, total cholesterol (TC), and triglyceride (TG) levels were all higher with statistically significant differences, the T2DM group outperformed the control group. Background: Diabetes mellitus (DM) is a metabolic disorder in which hyperglycemia is a characteristic feature due to impairment in insulin secretion, defective insulin action, or both reasons. One long-term complication is the ischemic heart disease (IHD) which is a major cause of morbidity and mortality. Insulin and fetuin-A are hormones that play a role in glucose metabolism regulation. Insulin is responsible for regulating glucose passage to the cells, while fetuin-A blocks insulin binding to its receptor, which is linked to insulin resistance in metabolic syndrome. Higher fetuin-A level patients have 4 times greater risk of developing IHD compared to patients with lower fetuin-A levels. Studies found that both hormones participate in the development of T2DM and IHD, beside their role in the pathophysiology of metabolic syndrome. Aim of the study: Evaluate serum fetuin-A concentrations in type2 diabetic (T2D) patients with and without IHD and compare them with healthy individuals. Assess serum glucose, insulin levels, and HOMA-IR in T2D patients and IHD and compare them with healthy individuals. Subjects and Methods: A case control study, that included 120 patients (60 have T2DM with IHD and 60 having T2DM without IHD age ranged from 30 to 70 years patient, who attended Al-Yarmouk Teaching Hospital and Iraqi Center for Myocardial Infarction, Medical City Hospital, Baghdad during the period from November 2020 until February 2021 and compare result with 60 healthy control which age ranged from 30 to 70 years. Female: male ratio is almost 1:1 in the three studied groups. The diagnosis of MI is based on medical reports, laboratory, and clinical tests for heart disease. Patients with neuropathy, retinopathy, thyroid dysfunction and liver diseases were excluded from the study. BMI, FBS, serum insulin, HOMA-IR, and fetuin-A were measured or calculated for each participant. Result: a statistical difference in BMI between participants is found; 46.70% of T2DM in the IHD group are obese. No significant association between disease duration and development of IHD is found. All the included markers showed statistical differences in mean ± SD between the diabetic and control group; fetuin-a mean ± SD level was 27.63 13.31, 12.39 ± 5.61 and 9.93 ± 5.1 in T2DM with IHD, T2DM without IHD and control group, respectively. There was no statistical association between the markers and fetuin-A except a moderate positive correlation with insulin. Conclusion: Fetuin-A can be used as a predictor of IHD development in diabetic patients.

In this paper, we propose a method for automatic generation of fuzzy rules for data classification. The proposed method is based on subtractive clustering optimized using genetic algorithm. It searches for the FIS structure and number of rules that have the highest fitness value. Multiple performance measures are incorporated into the fitness function to address the problem of imbalanced data. Fitness function includes both training and validation to avoid data over-fitting. Classification performance of the proposed method is evaluated using different data sets and results are compared to those of a number of models generated by fuzzy cmeans clustering with various cluster numbers. Results show that the proposed method has better accuracy and a well compromised sensitivity and specificity.

Background: Positron Emission Tomography (PET) and Computed Tomography (CT) are devices used for diagnosis purposes. One of their helpful uses is in oncology. This study compares the effective dose of PET and CT scans for the brain and thyroid. Materials and Methods: One hundred individuals participated in this study: 50 patients with brain tumors and 50 with thyroid tumors. The tumors of each patient type are separated into two equal groups. Each group had twenty-five patients. The first group was evaluated using a PET scan, whereas the second group was inspected with a CT scan. The patients were previously diagnosed by an oncologist and then sent for PET/CT and CT scans. Before administering the radiopharmaceutical for PET, the blood glucose content is measured in all PET and CT patients who have been fasting for at least six hours. Results: The CT scan shows that the brain shows higher current (mAs) than the thyroid. The effective dose of x-ray reached the brain was the highest, followed by the thyroid. The scanning time (minutes) was significantly the highest in the thyroid, followed by the brain. There was no significant difference among the organs in effective dose or current (mAs). For PET scan, the effective dose (mSv), scan time (minute), radiation activity (mCi), and SUV (MBq/ml) shows a highly significant difference between the two studied organs (brain and thyroid). The effective dose shows to be the highest in the brain, followed by the thyroid. The brain, followed by the thyroid, was taken for the minimum examination time. The activity of the x-ray radiation in mCi was found in the thyroid and brain. The brain's standard uptake value (SUV) was higher than the thyroid's. The effective dose for the CT scan was significantly higher than the PET scan for the brain and thyroid. Graves' disease (GD) is an autoimmune thyroid disorder and recent studies have proposed a role for interleukin (IL)-37, IL-38, and vitamin D (VitD) in the pathophysiology of disease. Therefore, this study investigated the expression of IL-37, IL-38, and VitD in the serum of GD patients and correlations of their levels with some demographic and clinical characteristics.Serum IL-37, IL-38, and VitD levels were evaluated in 90 women with GD and 93 control women using enzyme-linked immunosorbent assay kits. Depending on therapy, six patients were newly diagnosed (ND; untreated), and 50 patients were receiving only carbimazole (CMZ), while 34 patients were also on CMZ but also received one (31 patients), two (one patient), or three (two patients) doses of radioactive iodine (RAI).IL-37 levels were significantly higher in GD patients than in controls, while IL-38 and VitD levels were significantly decreased. As indicated by the area under the curve (AUC), receiver operating characteristic curve analysis demonstrated the potential of IL-37, IL-38, and VitD as biomarkers to distinguish GD patients from controls (AUC = 0.953, 0.959, and 0.793, respectively). Multinomial logistic regression analysis showed that altered levels of IL-37, IL-38, and VitD were most likely associated with the pathogenesis of GD. IL-37 was negatively correlated with IL-38 and VitD, while IL-38 and VitD were positively correlated.Serum Il-37 levels were upregulated in women with GD, while IL-38 and VitD levels showed downregulated levels. The latter two were positively correlated while they showed a negative correlation with IL-37.

Indigenous Peoples in Canada are a non-homogenous group consisting of First Nations, Metis, and Inuit Peoples representing the original settlers of a given land or a geographical area (Parrott, 2018). Based on geographical location, there are unique names used to describe a given subset or group of Indigenous Peoples around the world. Despite their proximity, they originate from different nations, tribes, and communities and remain distinct in their spoken language, history, and way of life. Although there has been a notable growth in the literature on Indigenous Peoples, relatively little is published about their understanding of healthy aging. Similarly, there is a dearth of literature about the specific needs and wishes of Indigenous Peoples in Canada to facilitate a healthy aging process.

Few therapies exist to treat severe COVID-19 respiratory failure once it develops. Given known diffuse pulmonary microthrombi on autopsy studies of COVID-19 patients, we hypothesized that tissue plasminogen activator (tPA) may improve pulmonary function in COVID-19 respiratory failure.A multicenter, retrospective, observational study of patients with confirmed COVID-19 and severe respiratory failure who received systemic tPA (alteplase) was performed. Seventy-nine adults from seven medical centers were included in the final analysis after institutional review boards' approval; 23 were excluded from analysis because tPA was administered for pulmonary macroembolism or deep venous thrombosis. The primary outcome was improvement in the PaOtPA was associated with significant PaOThese data suggest tPA is associated with significant improvement in pulmonary function in severe COVID-19 respiratory failure, especially in patients whose pulmonary function is in decline, and has an acceptable safety profile in this patient population.
