Abstract Background Many patients with T2DM on insulin are not optimally controlled despite receiving standard diabetes education counselling. Poor insulin adherence may be a contributing factor. We developed and evaluated a new module [Universiti Sains Malaysia-Insulin Adherence Module (USM-IAM)] on insulin-treated patients with poorly controlled diabetes. Methods Eligibility criteria are those diagnosed with T2DM, aged between 18 and 65 years, with HbA1c between 8 and 15% and on insulin therapy for 1 year. Patients were randomly allocated to receive either the USM-IAM-based counselling or the standard counselling (SC) at baseline and the second visit. Patients were instructed to adjust insulin doses based on blood glucose levels. Outcomes were changes in adherence score, FBS and HbA1c levels from baseline to 3 months and baseline to sixth month. Results Ninety patients were randomised to each group. The baseline sociodemographic and clinical characteristics were homogenous among groups. Ninety patients were analysed for each group. Adherence score changes between baseline to 3 months were − 8.30 (− 11.47, − 5.14) in USM-IAM-based counselling group (USM-IAM) and − 7.64 (− 10.89, − 4.40) in standard counselling group (SCG), between baseline to sixth month were − 10.21 (− 13.40, − 7.03) in USM-IAM and − 10.79 (− 14.64, − 6.97) in SCG. FBS changes between baseline to 3 months were 1.374 (0.25, 2.50) in USM-IAM and 0.438 (− 0.66, 1.54) in SCG, and between baseline to sixth month were 1.713 (0.473, 2.95) in USM-IAM and 0.998 (− 0.02, 2.01) in SCG. HbA1c changes between baseline to 3 months were 1.374 (0.25, 2.50) in USM-IAM and 0.547 (0.12, 0.98) in SCG, and between baseline to sixth month were 1.03 (0.65, 1.41) in USM-IAM and 0.617 (0.20, 1.03) in SCG. Between-subjects effects for all outcomes were not statistically significant. Conclusion Both groups had significant improvements in adherence score and HbA1c with time, with higher improvement in patients receiving the USM-IAM. FBS reductions were significant in the intervention group but not in the control group. The coastal area is a dynamic region influenced by continuous interactions between land and the sea. Changes in coastal compositions are closely related to shoreline instability. This study analyzes coastline changes at the Olie Pier Heritage Site, Manggar, East Belitung, Indonesia, using statistical-based techniques EPR (End Point Rate) and NSM (Net Shoreline Movement) 2015 to 2023. The DSAS (Digital Shoreline Analysis System) was employed to calculate the shoreline alterations in every transect with a distance of about 100 m. The study area is generally predominated by a moderate abrasion. The EPR and NSM values indicate a potential for future shoreline changes, considering the present status and future estimations of oceanographic parameters (currents, wind, waves, tides). The highest erosion is identified in the Lalang Village with a shoreline retreat (NSM) of -65.38 m and a retreat rate (EPR) of 8.78 m/year. On the other hand, the highest accretion is observed in the Baru Village with a NSM of -56.68 m and EPR of 7.61 m/year. The implications of these shoreline changes on the heritage site and the coastal environment contribute to the management of coastal areas amidst global environmental challenges. This study provides valuable insights into conservation and sustainable development efforts in the coastal region of East Belitung.

This study examines two downscaling techniques, convolutional neural networks (CNNs) and feedforward neural networks for predicting precipitation and temperature, alongside statistical downscaling as a benchmark model. The daily climate predictors were extracted from the European Center for Medium-range Weather Forecast (ECMWF) ERA5 dataset spanning from 1979 to 2010 for Tabriz city, located in the northwest of Iran. The biases in precipitation data of ERA5 predictors were corrected through the empirical quantile mapping method. Also, two nonlinear predictor screening methods, random forest and mutual information, were employed, alongside linear correlation coefficient. While these methods facilitate identification of dominant regional climate change drivers, it is essential to consider their limitations, such as sensitivity to parameter settings, assumptions about data relationships, potential biases in handling redundancy and correlation, challenges in generalizability across datasets, and computational complexity. Evaluation results indicated that CNN, when applied without predictor screening, achieves coefficient of determination of 0.98 for temperature and 0.71 for precipitation. Ultimately, future projections were employed under two shared socioeconomic pathways (SSPs), SSP2-4.5 and SSP5-8.5, and concluded that the most increase in temperature by 2.9 °C and decrease in precipitation by 3.5 mm may occur under SSP5-8.5. In this paper, we combine decision fusion methods with four metaheuristic algorithms (Particle Swarm Optimization (PSO) algorithm, Cuckoo search algorithm, modification of Cuckoo Search (CS McCulloch) algorithm and Genetic algorithm) in order to improve the image segmentation. The proposed technique based on fusing the data from Particle Swarm Optimization (PSO), Cuckoo search, modification of Cuckoo Search (CS McCulloch) and Genetic algorithms are obtained for improving magnetic resonance images (MRIs) segmentation. Four algorithms are used to compute the accuracy of each method while the outputs are passed to fusion methods. In order to obtain parts of the points that determine similar membership values, we apply the different rules of incorporation for these groups. The proposed approach is applied to challenging applications:MRI images, gray matter/white matter of brain segmentations and original black/white images Behavior of the proposed algorithm is provided by applying to different medical images. IntroductionThe purpose of this study is to culturally adapt the Awareness and Beliefs about Cancer (ABC) measure for use in the Hispanic/Latino population living in the United States (US).MethodsIn accordance with Patient Reported Outcomes (PRO) Consortium guidelines for cross-cultural adaptation of measures for content and linguistic validity, we conducted: two forward-translations, reconciliation, two back-translations, revision and harmonization, six cognitive interviews, revision, external expert review, and finalization of the version. We used a mixed methods approach, conducting cognitive interviews with Hispanic/Latino community members while also convening an expert panel of six clinicians, health professionals, and community representatives and including the in the entire process. After cross-culturally adapting the ABC measure, we assessed the psychometric properties of the instrument using item response theory analysis. Item parameters, discrimination and category thresholds, and standard errors were calculated. For each of the adapted subdomains, we used item information curves to report the graphical profile of item effectiveness.ResultsTwenty-two Hispanic/Latino community members were enrolled in cognitive interviews, and Hispanics/Latinos fluent in Spanish completed the measure to assess its psychometric properties. Cognitive interviews revealed opportunities to improve items. Key changes from the original measure include the inclusion of gender inclusive language and an inquiry into e-cigarette use on items related to smoking habits. Psychometric property analyses revealed that the anticipated delay in seeking medical help, general cancer beliefs, and cancer screening beliefs and behaviors subdomains had some slope parameters that were < 1; this implies that those items were not able to adequately discriminate the latent trait and had poor performance.DiscussionThe adapted ABC measure for US Hispanics/Latinos meets content and linguistic validity standards, with construct validity confirmed for cancer symptom recognition and barriers to symptomatic presentation subdomains, but revisions are necessary for others, highlighting the need for ongoing refinement to ensure the cultural appropriateness of instruments.

Authoritative citations are critical to ensure information integrity, especially in encyclopedias like Wikipedia. To date, research on automating citation worthiness detection has largely focused on the most resourceful language, English Wikipedia, neglecting the applicability to smaller Wikipedias. In addition, previous research proposed models that analyze the content inherent to a sentence to determine its citation worthiness, overlooking the potential of additional context to improve the prediction. Addressing these gaps, our study proposes a transformer-based contextualized approach for smaller Wikipedias, presenting a novel method to compile high-quality datasets for the Albanian, Basque, and Catalan editions. We develop the Contextualized Citation Worthiness (CCW) model, employing sentence representations enriched with adjacent sentences and topic categories for enhanced contextual insight. Empirical experiments on three newly created datasets demonstrate significant performance improvements of our contextualized CCW model, with 6%, 3% and 6% absolute improvements over the baseline for Albanian, Basque and Catalan datasets, respectively. We conduct an in-depth analysis to understand the influence and extent to which preceding and succeeding context as well as topic categories contribute to the accuracy of citation-worthiness predictions. Our findings suggest that incorporating such contextual information aids in the automatic identification of sentences in need of citations, not least when both the preceding and succeeding context are incorporated. This has implications for supporting Wikipedia projects across low-resource languages, promoting better article validation and fact-checking.

The circular economy (CE) is reasoned to organize complex systems supporting sustainable resilience by distinguishing between waste materials and economic growth. This is crucial to the electronic waste (e-waste) industry of developed countries, and e-waste operation management has become their top priority because e-waste contains toxic materials and valuable sources of elements. In the UK, although London Metropolitan city boasts an ambitious sustainable resilience target underlying the context of CE, practical implementation has yet to be feasible, with few investigations detailing if and how the existing target implications enable industrial and social-ecological sectors to continue their performance functionalities in the face of undesired disruptions. In this paper, a dynamic Bayesian Network (dynamic BN) approach is developed to address a range of potential risks. The existing London e-waste operation management is considered as an application of study for sustainable resilience development. Through the utilization of dynamic BN, a comprehensive analysis yields a Resilience Index (RI) of 0.5424, coupled with a StdDev of 0.01350. These metrics offer a profound insight into the intricate workings of a sustainable system and its capacity to swiftly rebound from unexpected shocks and disturbances. This newfound understanding equips policymakers with the knowledge needed to navigate the complexities of sustainable e-waste management effectively. The implications drawn from these in-depth analyses furnish policymakers with invaluable information, enabling them to make judicious decisions that advance the cause of sustainable e-waste management. The findings underscore that the absorptive capacity of a sustainable and resilient e-waste operation management system stands as the foremost defense mechanism against unforeseen challenges. Furthermore, it becomes evident that two pivotal factors, namely “diversifying the supply chain” and “enhancing supply chain transparency,” play pivotal roles in augmenting the sustainability and resilience of e-waste operation management within the context of London's ambitious sustainability targets. These factors are instrumental in steering the trajectory of e-waste management towards a more sustainable and resilient future, aligning with London's aspirations for a greener and more eco-conscious future.

Abstract Biological samples are often frozen and stored for years and/or thawed multiple times, thus assessing their stability on long-term storage and repeated freeze–thaw cycles is crucial. The study aims were to assess:—the long-term stability of two major enzymatic and non-enzymatic metabolites of arachidonic acid, i.e. urinary 11-dehydro-thromboxane-(Tx) B2, 8-iso-prostaglandin (PG)F2α, and creatinine in frozen urine samples;—the effect of multiple freeze–thaw cycles. Uvod: Ovisno o rezultatima ambulantnog praćenja krvnog tlaka (ABPM), pacijenti imaju različite profile krvnog tlaka, a neke velike međunarodne studije pokušale su ispitati i usporediti profil kontinuiranog monitoringa krvnog tlaka (KMAT) pacijenata u različitim zemljama. U našoj studiji pokušali smo utvrditi profile arterijske hipertenzije naših pacijenata. Materijali i metode: Ovo je retrospektivna studija presjeka od listopada 2022. do travnja 2023. Ispitani su demografski, klinički i podatci o 24-satnom krvnom tlaku. Bolesnici su podijeljeni u nekoliko kategorija prema fenotipovima hipertenzije i dipping profilima. Analize su provedene koristeći mjere deskriptivne statistike. Rezultati: U studiju su bila uključena 94 bolesnika. Tijekom praćenja 12 pacijenata je umrlo. Hipertenzija bijele kute bila je najčešći fenotip hipertenzije u naših bolesnika (30,8%), a druga najveća skupina bila je trajna nekontrolirana hipertenzija (29,7%). Također, pronašli smo visoku prevalenciju non-dippers sistoličkog (43,6%) i obrnutog dipper dijastoličkog tlaka (42,5%). Rezultati pokazuju visoko značajne razlike između skupine koja je bila živa i skupine koja je umrla, s p < 0,001, što ukazuje da je profil krvnog tlaka povezan s ishodima preživljenja. Zaključci: Prevalencija visokorizičnog profila povišenoga krvnog tlaka u bubrežnih bolesnika je visoka. Zbog povišenoga noćnog krvnog tlaka u liječenju samo praćenje krvnog tlaka nije dovoljno. KMAT bi trebao postati zlatni standard za potvrdu odgovarajuće kontrole krvnog tlaka u bolesnika s bolešću bubrega.

The present article reveals that silicate solubilizing microbes are essential part of weathering silicate minerals. Alumino-silicate group appears the most minerals to weathering by several bacteria. The contribution of biological agents in weathering has attracted increased interests as several genera of microorganisms, therfore referred as ' silicate bacteria' were found to decompose aluminosilicate minerals to increase potassium, silicon and aluminum in an available forms for plant nutrition.Thus, the magnitude of the role of such bacteria in the decomposition of alumino-silicates and releasing the elements contained therein as well as in the elements uptake by plants was found of great interest . The article concluded that effect of silicate bacteria in minerals weathering, soil formation, elements cycling and plant nutrition. Therefore, the speculate that the classic weathering sequence is not just chemical weathering. But should be interpreted as a mixture of chemical and biochemical weathering. [ABSTRACT FROM AUTHOR]

The co-delivery of microRNAs (miRNAs) and protein-coding RNA presents an opportunity for a combined approach to gene expression and gene regulation for therapeutic applications. Protein delivery is established using long mRNA, self-, and trans-amplifying RNA (taRNA), whereas miRNA delivery typically uses short synthetic oligonucleotides rather than incorporating it as a precursor into long RNA. Although miRNA delivery into the cell cytoplasm using long genomes of RNA viruses has been described, concerns have remained regarding low processing efficiency. However, miRNA precursors can be released from long cytoplasmic alphaviral RNA by a cytoplasmic fraction of Drosha. taRNA, a promising vector platform for infectious disease vaccination, uses a nonreplicating mRNA expressing an alphaviral replicase to amplify a protein-coding short transreplicon-RNA (STR) in trans. To investigate the possibility of simultaneously delivering protein expression and gene silencing, we tested whether a taRNA system can carry and release functional miRNA to target cells. Here, we show that mature miRNA is released from STRs and silences specific targets in a replication-dependent manner for several days without compromising the expression of STR-encoded proteins. Our findings suggest that incorporating miRNAs into the taRNA vector platform has the potential for gene regulation alongside the expression of therapeutic genes.

This study explores the prevalence of adherent-invasive Escherichia coli (AIEC) in colorectal cancer (CRC) patients and investigates the potential of effective intracellular antibiotics as a therapeutic strategy for CRC patients with AIEC infections. Considering the pivotal role of integrons in bacterial antibiotic resistance, the frequency of class 1 and 2 integrons in AIEC isolated from CRC patients, in one of the referenced 3 gastroenterology clinics in Isfahan, Iran was examined. AIEC strains were isolated from the colorectal biopsies and their antimicrobial sensitivity was assessed using the disc diffusion method. Polymerase chain reaction (PCR) was employed to detect intl1 and intl2. The multilocus sequence typing (MLST) method was utilized to type 10 selected isolates. Of the 150 samples, 24 were identified as AIEC, with the highest number isolated from CRC2 (33.4%) and CRC1 (29.16%), and the least from the FH group (8.3%) and control group (12.5%). int1 in 79.2% and int2 in 45.8% of AIEC strains were found and 41.6% of strains had both integrons. AIEC isolates with int1 exhibited the highest sensitivity to trimethoprim-sulfamethoxazole (57.9%), while those with int2 showed the highest sensitivity to ciprofloxacin (63.6%). A significant association between resistance to rifampin and integron 2 presence in AIEC isolates was observed. Furthermore, a significant correlation between integron 1 presence, invasion, survival, and replication within macrophages in AIEC strains was identified. MLST analysis revealed ST131 from CC131 with integron 1 as the most common sequence type (ST). The emergence of such strains in CRC populations poses a serious public health threat. The distribution pattern of STs varied among studied groups, with pandemic STs highlighting the importance of examining and treating patients infected with these isolates. Comprehensive prospective clinical investigations are warranted to assess the prognostic value of detecting this pathovar in CRC and to evaluate therapeutic techniques targeting drug-resistant AIECs, such as phage therapy, bacteriocins, and anti-adhesion compounds, for CRC prevention and treatment.

Peroxidasin (PXDN) is a secreted heme peroxidase that catalyzes the oxidative crosslinking of collagen IV within the extracellular matrix (ECM) via intermediate hypobromous acid (HOBr) synthesis from hydrogen peroxide and bromide, but recent findings have also suggested alternative ECM protein modifications by PXDN, including incorporation of bromide into tyrosine residues. In this work, we sought to identify the major target proteins for tyrosine bromination by HOBr or by PXDN-mediated oxidation in ECM from mouse teratocarcinoma PFHR9 cells. We detected 61 bromotyrosine (BrY)-containing peptides representing 23 proteins in HOBr-modified ECM from PFHR9 cells, among which laminins displayed the most prominent bromotyrosine incorporation. Moreover, we also found that laminin α1, laminin β1, and tubulointerstitial nephritis antigen-like (TINAGL1) contained BrY in untreated PFHR9 cells, which depended on PXDN. We extended these analyses to lung tissues from both healthy mice and mice with experimental lung fibrosis, and in lung tissues obtained from human subjects. Analysis of ECM-enriched mouse lung tissue extracts showed that 83 ECM proteins were elevated in bleomycin-induced fibrosis, which included various collagens and laminins, and PXDN. Similarly, mRNA and protein expression of PXDN and laminin α/β1 were enhanced in fibrotic mouse lung tissues, and also in mouse bone-marrow-derived macrophages or human fibroblasts stimulated with transforming growth factor β1, a profibrotic growth factor. We identified 11 BrY-containing ECM proteins, including collagen IV α2, collagen VI α1, TINAGL1, and various laminins, in both healthy and mouse fibrotic lung tissues, although the relative extent of tyrosine bromination of laminins was not significantly increased during fibrosis. Finally, we also identified 7 BrY-containing ECM proteins in human lung tissues, again including collagen IV α2, collagen VI α1, and TINAGL1. Altogether, this work demonstrates the presence of several bromotyrosine-modified ECM proteins, likely involving PXDN, even in normal lung tissues, suggesting a potential biological function for these modifications.
