Mutations in ATP13A2 (PARK9), an autophagy-related protein, cause Kufor–Rakeb syndrome, an autosomal recessive, juvenile-onset form of parkinsonism. α-Synuclein (α-syn) is a presynaptic neuronal protein that forms toxic aggregates in Parkinson’s disease (PD). We studied α-syn aggregation and autophagic flux in ATP13A2-knockdown Drosophila expressing either wild-type (WT) or mutant α-syn. Dopaminergic (DA) neuron loss was studied by confocal microscopy. Sleep and circadian activity were evaluated in young and old flies using a Drosophila activity monitor. Thirty-day-old ATP13A2-RNAi A53T-α-syn flies had increased Triton-insoluble α-syn levels, compared to control A53T-α-syn flies without ATP13A2-RNAi. Whole-brain staining revealed significantly fewer dopaminergic (DA) neurons in the PPL2 cluster of 30-day-old ATP13A2-RNAi flies expressing WT-, A30P-, and A53T-α-syn than in that of controls. In ATP13A2-RNAi A53T-α-syn flies, autophagic flux was decreased, as indicated by increased accumulation of Ref(2)P, the Drosophila p62 homologue. ATP13A2 silencing decreased total locomotor activity in young, and enhanced sleep features, similar to PD (decreasing bout length), in old flies expressing A53T-α-syn. ATP13A2 silencing also altered the circadian locomotor activity of A30P- and A53T-α-syn flies. Thus, ATP13A2 may play a role in the autophagic degradation of A53T-α-syn.

Background:Uncontrolled blood pressure (BP) is a major healthcare issue and responsible for high risk of cardiovascular disease (CVD) mortality and morbidity. Physicians of Bangladesh had been usingamlodipine and Olmesartan combination for a long period of time but have very limited evidence-based information regarding the efficacy and safety of this combination. The aim of this study was to evaluate the efficacy and safety of fixed dose combination of olmesartan/amlodipine in hypertensive patients. Methods:This open-label, non-randomized, non-comparative observational study was conducted in different centers of Bangladesh during the period from January 2020 to November 2020. In total 443 Bangladeshi adult patients with hypertension were selected as the study population. Proper written consent was taken from all the participants before collecting data. Inter-group differences in the baseline characteristics were compared using analysis of variance (ANOVA). Two-tailed p-values of less than 0.05 were considered to be statistically significant. Statistical analyses were conducted by using SPSS for Windows, version 25 (SPSS Inc., Chicago, IL, USA). Results:Among all the participants, more than 70% were above 46 years of age and 55% were male. More than 60% of them were either overweight or obese. Among them, dyslipidemia, diabetes, and Chronic renal failure (CRF) was found among 14.22%, 28.44%, and 4.29% participants respectively Mean systolic (SBP) and diastolic blood pressures (DBP) were reduced to 36.61 and 18.69 mm of Hg respectively after 12 weeks of therapy. Besides these, the targeted SBP was achieved among 92% and targeted DBP was achieved among 98.3% of participants after 12 weeks. Achievement of BP goal in respect to Body mass index (BMI) of the patient (P-value 0.019), family history of hypertension (P-value 0.028), and dose of the drug in initial visit (P-value 0.001) was found statistically significant. There was no discontinuation of study medication due to intolerance or adverse events. Conclusion:Olmesartan/Amlodipine is an effective combination for reducing the blood pressure along with achievement of BP goal. Future studies on Olmesartan/Amlodipine in Bangladesh could be conducted with a longer treatment duration to examine the maintenance of effect and long-term safety.

Absolute diffusion rates of KMnO4in vertical and flattened diffusimeters show the concentration gradient force as being stronger than the gravitational force. Hot water molecules move downward on self-diffusion against buoyancy. Diffusive convection (DC) in warm water and double-diffusive convection (DDC) in warm, saline water take place inside the diffusimeter with DDC transferring more heat than DC. In the diffusing medium the original reagents change or retain their compositions to give the diffusate molecules to diffuse. In water, the change is mostly hydration. The syngener BaCl2.2H2O separately with congeners 3CdSO4.8H2O, ZnSO4.7H2O, and ZnSO4.H2O presents two distinct pairs of overlapping concentration versus rate curves, first for having very close MWs of BaCl2and CdSO4and second for having ZnSO4.H2O as the common congener for both the zinc sulfates. Chlorides of Li, Na, and K diffusing at hindered rates in glucose solution show the least rate for LiCl inevitably on grounds of low mass and high Li+hydration radius. Diffusion blocking occurs at higher glucose concentration. Diffusion of 0.6M AgNO3-0.6M NH4Cl standardizes this diffusimeter. Mass transfer of HCl, H2SO4, and H2C2O4show oxalic acid diffusing as hydrate and 88 percentage transfer of sulfuric acid in 5 minutes. The Superdiffusive Anti Graham’s Law,Vd, is further consolidated by Ca (NO3)2-M2CO3(M = Na, K, NH4+) and Ca (NO3)2-Na2HPO4diffusions. Odd and even diffusions are illustrated by AgNO3-NH4Cl and AgNO3-BaCl2diffusions.

Determination of accurate operating states of the power system is one of the most challenging tasks due to integration of large number of solar PVs into the power system. Since these sources of energy are inertialess generations, hence may cause instability issues if highly penetrated. Hence, precise monitoring and control of such systems with higher PV penetration is a critical issue to address both in terms of the total number of PV sources in the system and the percentage of penetration. Phasor Measurement Units (PMUs), with their time synchronizing abilities, have made this task a bit easier. However, traditional centralized data handling architecture at control centers are becoming redundant due to various limitations such as data handling, computational constraints etc. To overcome this, a distributed PMU-PDC architecture approach is utilized in this paper. Since local PDC (Phasor Data Concentrator) in an n-area power system can run separate optimization algorithms, by combining the results of these optimization algorithms of n-area PDCs, we can get a much more accurate global consensus about the current operating state of the system. The presented work is divided into two parts. First, the analysis of power system stability is performed in terms of the total number of Photovoltaic Sources (PV) in the system and the percentage of PV penetration. Then, the accuracy of the method is tested both in terms of the number of PMUs in each area along with their channel capacities. Low-frequency oscillations have been simulated on IEEE-68 standard bus system using MATLAB, and the modes of oscillation are estimated using the Alternating Direction Method of Multipliers (ADMM) algorithm.

The study was undertaken for understanding the potential of deep aquifers as a source of safe drinking water and for assessing the status of groundwater aquifer near Madunaghat area, Chattogram. The specific issues are salinity, interconnectivity of the shallow and deep aquifers, mixing of groundwater with the adjacent Halda river water, recharge condition and groundwater age. The isotopic data suggest that most of the groundwater results from a mixture between recent recharge and an older component recharge under climatic conditions cooler than at present. The interconnectivity between shallow aquifers and river waters are mostly found in the line wells installed particularly in shallow depth (16 m) close to Halda river as evidenced from the similar tritium values of sampled line well water with that of the Halda river water. The groundwater in Madunaghat well field area is not affected by salinity, as it is evidenced by higher values of Na/Cl ratio of the groundwater samples compared to the sea water fresh water mixing line. The geochemistry of intermediate and deep groundwater is dominated by Na-Mg-HCO3 and Na-HCO3 type waters. Even the relationship between chloride and oxygen-18 (Cl-δ18O) depicts that the waters from the deep, intermediate and shallow wells do not fall on the seawater mixing line. The Carbon-14 contents of intermediate to deep groundwater samples vary from 16.2 to 59.3 pMC indicating the residence time in the range of 4300 to 15,000 years BP, i.e., the sourced water recharged the aquifers a long time ago. The intermediate and deep wells have water with arsenic concentrations less than the detection limit of 3.0 μg/L. Only a few shallow wells have arsenic concentrations greater than the detection limit varying from 13.7 - 47.4 μg/L, which is less than the DoE permissible limit (50 μg/L) implying that the groundwater at Madunaghat area is not affected by Arsenic contamination.

Papillon-Lefèvre syndrome (PLS) an autosomal recessive disorder characterized by diffuse transgradient palmar-plantar hyperkeratosis, with premature loss of deciduous and permanent teeth, along with the calcification of the dura mater. This results in teeth radiographically appearing as “floating” in the soft tissue. Genetic studies of patients with PLS have mapped the major gene locus to chromosome 11q24-q21 and revealed mutation and loss of function of the cathepsin gene. It affects 1- 4 people per 1 million population with no gender or racial predilection. Dermatological manifestations, usually occurs before four years of age, include hyperkeratosis of palms and soles, nail dystrophy, hyperhidrosis and keratinization on elbows and knees with the lesions appearing as white, yellow-like or red plaques or patches that then develop cracks, crusts, or deep fissures. Adipogenesis is regulated by a cascade of signals that drive transcriptional reprogramming in adipocytes. Here, we report that nuclear actin regulates the chromatin states that establish tissue- specific expression during adipogenesis. To study the role of β-actin in adipocyte differentiation, we conducted RNA sequencing on wild-type and β-actin knockout mouse embryonic fibroblasts (MEFs) after reprograming to adipocytes. We found that β-actin depletion affects induction of several adipogenic genes during transcriptional reprograming. This impaired regulation of adipogenic genes is linked to reduced expression of the pioneer factor Cebpa and is rescued by reintroducing NLS-tagged β-actin. ATAC-Seq in knockout MEFs revealed that actin-dependent reduction of Cebpa expression correlates with decreased chromatin accessibility and loss of chromatin association of the ATPase Brg1. This, in turn, impairs CEBPB’s association with its Cebpa promoter-proximal binding site during adipogenesis. We propose a role for the nuclear β-actin pool in maintaining open chromatin for transcriptional reprogramming during adipogenic differentiation.

Ankylosing Spondylitis Disease Activity Score (ASDAS) is a very important tool for measuring disease activity in patients with axial spondyloarthritis (axSpA). This study was aimed to develop a culturally adapted, valid and reliable Bengali version of questionnaire for the ASDAS. The original English ASDAS was translated into Bangla and adapted in the local socio-cultural context, following standard international recommendations. Content validity of the adapted Bangla version was assessed by the item-level & scale level content validity indices (I-CVI & S-CVI). A total 120 patients with axSpA were assessed in the study. For construct validity, correlation between ASDAS with BASDAI, ESR, CRP, Maastricht enthesitis score, physician's global assessment and BASMI was assessed by Spearman's rank correlation coefficient. Internal consistency and test-retest reliability were assessed using Cronbach's alpha coefficient and intraclass correlation coefficient respectively. I-CVI for each item was 1 and S-CVI was 1. ASDAS-ESR and ASDAS-CRP showed strong correlation (Spearman's correlation coefficient >0.5) with BASDAI, ESR, MASES and physician global assessment of disease activity. The scale demonstrated excellent internal consistency (Crohnbach's alpha=0.703) and test-retest reliability (ICC=0.98). The adapted Bangla version of the ASDAS demonstrated acceptable psychometric properties (validity & reliability) in Bangladeshi patients with axSpA. Faridpur Med. Coll. J. Jul 2019;14(2): 62-66

The advancements in the field of wind turbine, solar photovoltaic (PV), fuel cell coupled with improved power electronics have increased reliability of renewable energy sources. The environmental benefits of these sources have forced the power industry to switch over for more distributed generations to meet the increasing load demand. Islanding in distribution system occurs when a portion of distribution system gets isolated from the rest of the grid and continues to supply the local load. Practically, all distributed generations (DG) are required to be disconnected immediately after the formation of island. It is done primarily to take care of safety of the operating personnel and to prevent power quality issues. Effective detection of islanding is an important area of concern. Prior to the integration of DG to the main electrical grid, each DG must be equipped with a suitable anti-islanding detection technique. In this paper, phasor measurement unit (PMU)-based islanding detection technique is presented. The requirement of channel limits of PMUs is also incorporated, and significant results of industry and utility have been obtained. The study has been carried out using MATLAB/Simulink (version 2018a) creating several islanding and non-islanding cases in a PV integrated distribution grid.

Background : Pilonidal sinus was first described by Dr. A.W. Anderson in 1847 and is often seen in the intergluteal region. It is a disease of mainly younger male and rarely affects after 40. Main causes for the formation of this sinus are hirsutism, sweating in the area, repeated maceration due to trauma, leading to breakage of the skin barrier, attracting hair inside which initiates a foreign body reaction leading to infection with abscess or sinus formation. Most common embarrassing situation is discharging sinus. The diagnosis is mainly on clinical ground. Though there are a lot of modalities of treatment but recurrence is still a challenge. We have adopted Limberg flap technique since 2012 with a least recurrent chance. Objectives : To prove that this procedure will be end up with lower morbidity & least chance of recurrence Methods : This prospective study was done jointly by the Department of Surgery of Central Medical College and Comilla Medical College Hospital. In this series 50 patients were selected from July 2012 to June 2017. Results : Out of 50 patients operated by Limberg flap technique 42(84%) were males and 8(16%) were females. Mean age was 27.3 years (range 18–37years). Of them,12(24%) were driver, 6(12%) police, 6(12%) teacher, 6(12%) student, 5(10%) worker, 4(8%) millitary army, 4(8%) housewife, 4 (8%)service holder, 2(4%) computer operator, 1(2%) doctor and 1(2%) were barber. In our study most common mode of presentation were chronic discharging sinuses 15(30%), beside this 12(24%) had multiple sinuses, 10(20%) had multiple pits,7(14%) had acute abscess and 6(12%) had unpleasant smell. Associated hirsutism were present in 30(60%) and absent in 20(40%) patients. One (2%) male patient had wound gap & discharge and six(12%) patients had flap edema. We have found no recurrence in follow up period. Conclusion : Pilonidal sinus is a notoriously recurrent disease, so treatment by this flap technique is the only hope of cure with lower morbidity. - Abstract
Over the past few decades, there has been a growing interest in passive design strategies in the built environment, such as daylight. Using sunlight as the source, passive daylighting aims to provide effective natural lighting into the building while retaining maximum visual comfort without using mechanical equipment. Various research has shown the benefits of the natural environment to aid in human recovery, including daylighting. The majority of the biophilic design studies focused on workplace and healthcare environments such as hospital design. While biophilic architecture brings the meaning of nature to the internal environment, the positive effects of this biophilia are particularly concerned on healthcare facilities as it puts an element of extreme pressure on the space. This paper aims to understand the relationship between the patients and staffs’ experiences of their physical environment, specifically addressing the use of daylighting as aiding the recovery of patients and restorative impact for the users. This paper investigates the effects of daylighting in the inpatient ward at Seri Manjung Hospital through a set of questionnaires distributed to the 50 respondents of male and female users aged 18 and above. This inpatient ward space is built with windows and utilising natural lighting during the day. The participants were asked to share their experience on the lighting conditions in the ward space and the impact on their health and recovery. The outcome of this study has shown that passive daylighting technique and biophilic design in inpatient wards help to increase users' connectivity with nature, improve physical and psychological well-being and accelerate the recovery process. Consequently, incorporating passive daylighting and biophilic design in a healthcare setting will shift the hospital's design to be more sustainable and responsive to the environment.

This study aimed to compare the efficacy of low-dose prednisolone with conventional high-dose regimen in proliferative lupus nephritis (LN) for remission. This open-label randomized clinical trial was conducted in the Department of Rheumatology, Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka, Bangladesh. A total of 32 LN patients were randomized into low-dose (experimental) and high-dose (control) groups. All subjects received standard dose of intravenous (I/V) methylprednisolone and pulse I/V cyclophosphamide. Oral prednisolone, 0.5 mg/kg/d and 1 mg/kg/d were given to experimental and control groups respectively for initially 4 weeks then tapered. The patients were followed for 24 weeks. The rates of renal remission (complete and partial) were assessed at 24 weeks. The disease activity, biochemical markers, and quality of life were evaluated at baseline and at 24 weeks.Complete renal remission was achieved by 66.7% of patients in each group (P = .99). Renal remission (partial/complete) was achieved by 86.7% and 83.3% of patients in the prednisolone low-dose group and high-dose group respectively (P = .99). In between groups, no significant difference was observed in the improvement of active urinary sediments, serum creatinine level, anti-double-stranded DNA level, complements level, disease activity and Short Form-12 score. The prednisolone dose-related adverse events like cushingoid facies, abdominal stria, infections and serious adverse events like death occurred more in the high-dose prednisolone group. It has been observed that low-dose prednisolone regimen may be effective in LN. Steroid dose-related side effects and rate of infections were lower in this group.

Peripheral giant cell granuloma is a relatively uncommon benign reactive gingival lesion of the oral cavity. A 9-year boy presented with a painless, slow-growing, reddish-blue, soft tissue lesion on attached gingiva adjacent to maxillary right first and second premolars, which was interfering with eating. A periapical radiograph demonstrated focal alveolar bone loss and slight teeth displacement adjacent to the lesion. Diagnosis of peripheral giant cell granuloma was made through clinical and radiographic evaluation, by its typical presentation and correlation with histopathologic findings. Complete excision was carried out down to the underlying bone. A follow-up visit was scheduled after 7 days and deep scaling was performed. There was no recurrence three months post-excision. Timely detection and excision of this lesion is important to avoid future dentoalveolar problems. Key Words: Giant cell granuloma, Benign, Peripheral, Child.

OBJECTIVE: To evaluate the effect of pre-operative pain on the success of pulpotomy in primary teeth and to assess whether formocresol (FC) or MTA performs better in patients with pre-operative pain. METHODOLOGY: Data was collected retrospectively from the records of Department of Pediatric Dentistry, Children Hospital, Islamabad. Only patients with complete pre-operative and one-year follow-up clinical and radiographic records were included. A total of 60 teeth were selected for the study on which pulpotomy was performed. Among the selected teeth, thirty teeth were symptomatic at time of treatment and thirty were asymptomatic. Fifteen teeth in each group were treated with FC and fifteen with MTA as pulpotomy medicament. RESULTS: After one-year, the clinical and radiographic success of pulpotomized teeth with positive history of pain was found to be 70% and 53.3% respectively. Among the teeth with positive pre-operative pain, 60% of the teeth treated with FC and 80% of the teeth treated with MTA showed clinical success. The radiographic success rate was found to be 33.3% and 73.3% for FC and MTA respectively. CONCLUSION: The results from the current study suggest the presence of pre-operative pain negatively affects the success of pulpotomy in primary molars and could be used as an important clinical indicator to govern the choice of pulpotomy agent in clinical practice. Lymphedema is a life-long sequelae to several neglected tropical diseases (NTD). In Bangladesh the main cause is lymphatic filariasis (LF) and Ethiopia is endemic for both LF and podoconiosis. The World Health Organization (WHO) recommends daily self-care including meticulous washing and drying of affected skin and attention to entry lesions, limb exercises and elevation. Adherence to this regime reduces secondary infections which cause disabling episodes of acute dermato-lymphangitis (ADL). Self-care practices must be integrated into family life, supported by community and monitored by health staff; however, little is known about the influence of personal and socio-demographic factors on adherence. People affected by lymphedema (n=272), adult caregivers (n=272), and health workers (n=68) in Bangladesh and Ethiopia were trained in lymphedema management according to WHO recommendations. Surveys on the causes and management of lymphedema were collected at baseline and 24-weeks, and patients completed a daily journal of self-care activities and symptoms. At baseline knowledge on causes and management of lymphedema was greater among health workers (>70%) than patients and caregivers (

Bovine rotavirus (BRV) is considered the leading cause of calf diarrhea worldwide, including Bangladesh. In this study we aimed to identify risk factors for BRV infection and determine the G and P genotypes of BRV strains in diarrheic calves. Fecal samples were collected from 200 diarrheic calves in three districts between January 2014 and October 2015. These samples were screened to detect the presence of BRV using rapid test-strips BIO K 152 (RTSBK). The RTSBK positive samples were further tested by polyacrylamide gel electrophoresis and the silver staining technique to detect rotavirus dsRNA. Risk factors were identified by multivariable logistic regression analysis. The G and P genotypes of BRV were determined by RT-PCR and sequencing. A phylogenetic tree was constructed based on the neighbor-joining method using CLC sequence viewer 8.0. About 23% of the diarrheic calves were BRV positive. The odds of BRV infection were 3.8- (95% confidence interval [95% CI]: 1.0–14.7) and 3.9-times (95% CI:1.1–14.2) higher in Barisal and Madaripur districts, respectively, than Sirrajganj. The risk of BRV infection was 3.1-times (95% CI: 1.5–6.5) higher in calves aged ≤ 5 weeks than those aged >5 weeks. Moreover, the risk of BRV infection was 2.6-times (95% CI:1.1–5.8) higher in crossbred (Holstein Friesian, Shahiwal) than indigenous calves. G6P[11] was the predominant genotype (94.4%), followed by G10P[11] (5.6%). The BRV G6 strains were found to be closest (98.9–99.9%) to Indian strains, and BRV G10 strains showed 99.9% identities with Indian strain. The VP4 gene of all P[11] strains showed >90% identities to each other and also with Indian strains. The most frequently identified BRV genotype was G6P[11]. About 23% of calf diarrhea cases were associated with BRV. To control disease, high-risk areas and younger crossbred calves should be targeted for surveillance and management. The predominant genotype could be utilized as the future vaccine candidate or vaccines with the dominant genotype should be used to control BRV diarrhea in Bangladesh.

Background: Dental caries remains to be the most common oral disease, caused by the unbalance between demineralization effect of bacteria and remineralization process. In the early phase of caries, this balance can be restored and the demineralization effect reversed using an enhanced approach to remineralization. Objective: The aim of the study is directed to evaluate the degree of demineralization and remineralization in subclinical carious lesion using chitosan and conventional remineralizing agent. Materials and methods: A total number of 40 sound non-carious human anterior teeth were collected from oral surgery clinic, free of cracks and any developmental defects, were used in this study in age range (20-40) years old. Teeth were stored in distilled water at room temperature until use. The teeth were divided into four main groups (10 teeth each). Results: In the present study specimens treated with chitosan after six months storage time (the best result) recorded the highest mean value of remineralization while fluoride record the lowest mean value of remineralization (the worst result) through the remineralizing agents chitosan, casein, and fluoride. The specimens subjected to Diagnodent fluorescent reading proved that for all remineralizing agents (chitosan, casein and fluoride) and also control group the remineralization to tooth structure was increased by increasing the storage time. Conclusion: Chitosan is considered a valuable biomimetic remineralizing agent. Mineral precipitation to tooth structures by using remineralizing agents is a continuous dynamic process.

In quest of prospective rizospheric bacteria of agricultural importance, one of the isolates from bean (Lablab niger Medikus) was identified as Bacillus siamensis based on morphological, biochemical and 16S rRNA gene sequencing data. Study was carried out to evaluate growth promotion of two tomato cultivars, in vitro and in vivo. Experiments conducted for two consecutive years, following seed treatments revealed that the bacterial isolate increased plant height by 14.66‒15.68%, shoot fresh weight by 34.5‒65.09% and root fresh weight by 75.3‒92.48% over the non-treated control. The bacterial strain showed encouraging results for plant growth promotion in pot study and hence may be useful for the growth enhancement of tomato plant.
