Background: Despite the increase in the desire to consume herbal medicines and the growing concern of people about their health, as well as the increase in the number of specialized herbal medicine manufacturing companies and pharmacies, these products still have a small market share in the domestic market. Therefore, this research seeks to identify and categorize the marketing problems of herbal medicines with a systemic approach from the perspectives of actors active in this chain (companies producing herbal medicines, pharmaceutical distribution companies, pharmacies and consumers) and provide practical solutions to improve these problems. Methods: The current research is exploratory in nature and qualitative in type. To better identify the marketing problems of herbal medicines, semi-structured interviews were used, and using purposive and snowball sampling methods, interviews were conducted with knowledgeable people in 14 manufacturing companies, 13 pharmaceutical distribution companies, 22 pharmacies and 40 consumers of herbal medicines in 2022. Quantitative content analysis was also used to analyze the text of the interviews. Results: The identified problems are in three short-term, medium-term and long-term time frames, and from another perspective, these problems are located at the three organizational, industrial and macro level. About half of the marketing problems of herbal medicines are located in the three houses of the matrix, i.e. in the houses of organization-mid-term (16 %), industry-short-term (15 %) and organization-short-term (14 %). Also, based on the normalized data, about half of the marketing problems of herbal medicines are related to the non-availability of these medicines (7.7 %), lack of scientific visits by doctors (6.8 %), lack of appropriate and sufficient advertising (6.2 %), returns due to expiration dates (5 %), lack of confidence in the effectiveness (4.8 %), low offers and discounts (4.4 %), high prices (4.1 %), difficulty in collecting claims (3.6 %), low variety of products (2.9 %) and long production time (2.8 %). Conclusion: The government managers of the country can address macro-level issues by controlling the inflation rate, accelerating the settlement time of insurance and government organizations with distribution companies, reducing the time of collecting claims, and raising public awareness regarding herbal medicines. In the industry sector, there is a need to establish national distribution companies for herbal medicines and enhance the research, supply, production, and distribution processes. It is also important to improve the collaboration between manufacturers and pharmacies, as well as facilitate communication with international scientific societies. At the organizational level, managers can boost productivity by enhancing the quality of products and packaging, conducting scientific visits, and publishing credible scientific articles to convince doctors and consumers about the benefits of herbal medicines.

Abstract Comparing the abundance of microbial communities between different groups or obtained under different experimental conditions using count sequence data is a challenging task due to various issues such as inflated zero counts, overdispersion, and non-normality. Several methods and procedures based on counts, their transformation and compositionality have been proposed in the literature to detect differentially abundant species in datasets containing hundreds to thousands of microbial species. Despite efforts to address the large numbers of zeros present in microbiome datasets, even after careful data preprocessing, the performance of existing methods is impaired by the presence of inflated zero counts and group-wise structured zeros (i.e. all zero counts in a group). We propose and validate using extensive simulations an approach combining two differential abundance testing methods, namely DESeq2-ZINBWaVE and DESeq2, to address the issues of zero-inflation and group-wise structured zeros, respectively. This combined approach was subsequently successfully applied to two plant microbiome datasets that revealed a number of taxa as interesting candidates for further experimental validation.

Ultrasonic metal welding is a solid-state welding process being widely employed for joining the interconnections of lithium-ion batteries in hybrid and plug-in hybrid vehicles. Examining the microstructure of the joint is of crucial importance to evaluate the performance. This analysis allows a comprehensive understanding of the phenomena influencing the joint strength. In this study, ultrasonic welding was utilized to join copper to aluminum sheets of 0.3 mm thickness, and then the macro- and microstructural characteristics of the weld cross-section and interface were thoroughly investigated. Finally, the microstructural changes at different values of ultrasonic welding parameters were correlated with the mechanical strength characteristics. This research identified mechanical interlocking and interface diffusion as the primary welding mechanisms. With an increase in welding time from 0.2 to 0.8 s, the bond density rose, the post-weld thickness decreased, and the average Vickers hardness at various points along the weld interface decreased by 10%.

Zearalenone (ZEN) and its metabolites are a potent component with estrogenic potential that can enter milk. ZEN and its metabolites have the ability to disturb the function of endocrine glands. The aim of this systematic review was to estimate the level of ZEN and its metabolites in milk. This study was performed with these keywords; zearalenone, ZEN, bovine milk, ruminant milk, milk, dairy products, and milk product in various databases. 946 manuscripts were collected from databases and at the end, 17 manuscripts were reviewed according to the inclusion criteria. ZEN was identified in 59 % of studies. The most common methods of analysis were UHPLC, HPLC and ELISA. Meta-analysis was performed with CMA (Comprehensive Meta-Analysis) software. No publication bias was observed in meta- analysis. But, heterogeneity was recorded between studies. The measurement method was identified as one of the sources of heterogeneity through meta-regression tests and subgroup analysis. Furthermore, in meta- analysis test, the total estimate of milk contamination with this mycotoxin was 0.036±0.017 µg/L. So far, the permissible limit for this compound in milk has not been announced, but these compounds have the ability to disturb the endocrine glands in low amounts. Therefore, it is necessary to regularly measure and control this mycotoxin and its metabolite in milk with valid methods.

Background: By modifying the lifestyle of patients, the risk factors of Coronary Artery Diseases (CADs) are adjusted. This study was performed with the aim to investigate the role of nurses in the lifestyle of coronary artery patients. Materials and Methods: This scoping review was conducted with the question of the role of nurses in improving the lifestyle of coronary artery patients. This study was conducted in databases and search engines of ScienceDirect, Springer, Scopus, PubMed, MEDLINE, and Google Scholar for English texts, and in the Magiran and Scientific Information Database (SID) databases and search engines for the texts printed in Persian. The search for articles printed from 2012 to 2022 was conducted based on the keywords used in the title and abstract of the articles. The keywords used to search domestic databases included nursing, lifestyle and healthy lifestyle, and heart diseases (based on MeSH). Results: Nurses have made significant efforts and performed interventions to improve the lifestyle of coronary artery patients through guidance and education regarding the improvement of their diet and mobility, compliance with their treatment regimen, and reduction of the risk factors of CAD. In the psychological dimension, they had a positive effect in some fields such as stress management and mental health. However, in the spiritual dimension, effective studies and interventions have not been conducted by nurses. Conclusions: The role of nurses in the lifestyle of coronary artery patients is effective in the physical dimension, limited in the psychological dimension, and neglected in the spiritual dimension.

Background and aimsAllergic asthma has a considerable burden on the quality of life. A significant portion of moderate-to-severe allergic asthma patients need omalizumab, an anti-immunoglobulin-E monoclonal antibody, as an add-on therapy. In this phase III clinical trial P043 (Zerafil®, CinnaGen, Iran) efficacy, safety, and immunogenicity were compared with Xolair® (the originator omalizumab). The primary outcome was the rate of protocol-defined asthma exacerbations.MethodsExacerbation rates, Asthma Control Test (ACT) results, spirometry measurements, immunogenicity, and safety were evaluated. Each subject received either medication with a dose ranging from 150 to 375 mg based on pre-treatment serum total IgE level (IU/mL) and body weight (kg) every two or four weeks for a duration of 28 weeks.ResultsExacerbation rates were 0.150 (CI: 0.079-0.220) in the P043 group, and 0.190 (CI: 0.110-0.270) in the omalizumab group (per-protocol). The least squares mean differences of predicted Forced Expiratory Volume in the First second (FEV1) were -2.51% (CI: -7.17-2.15, P=0.29) and -3.87% (CI: -8.79-1.04, P=0.12), pre- and post-bronchodilator use. The mean ± SD of ACT scores at the screening and the last visit were 10.62 ± 2.93 and 20.93 ± 4.26 in P043 and 11.09 ± 2.75 and 20.46 ± 5.11 in the omalizumab group. A total of 288 adverse events were reported for the 256 enrolled participants. Among all, “dyspnea” and “headache” were the most reported ones. The overall incidence of adverse events (P=0.62) and serious adverse events (P=0.07) had no significant differences between the two groups. None of the samples were positive for anti-drug antibodies.ConclusionP043 was equivalent to omalizumab in the management of asthma in reduction of exacerbations. Osmopriming has been shown to improve the germination and growth of bread wheat (Triticum aestivum L.). This study explores the impact of various priming agent NaCl (3g l-1), proline (1 mM), ZnSO4 (1 mM), and their combination on wheat performance during the summer season (Jul-Aug 2022) at the greenhouse of Payame Noor University, Tabriz. Wheat seeds treated with a combination of priming agent demonstrated significantly enhanced performance compared to untreated seeds. Chlorophyll fluorescence measurements taken 35 days post-cultivation revealed a higher Photosystem Performance Index (PIabs) in osmoprimed seeds, particularly those treated with combined priming agent. Furthermore, primed plants demonstrated elevated concentrations of chlorophyll a, b, and carotenoids. Osmopriming also modulated the oxidative status of enzymes such as glutathione peroxidase (GPX), catalase (CAT), and superoxide dismutase (SOD). Genetic analysis showed that osmopriming could influence the expression of NHX2, a gene linked to improving plant growth, water uptake, and yield in stress conditions.

Background: Following the COVID-19 global emergency, health students were faced with increased workloads, university closures, study interruptions, loss of peer support networks, and the challenges of volunteer work in hospitals. These factors caused health students to experience significant stress and anxiety, highlighting the necessity of psychological interventions for this group. Several studies have reported that a mindfulness-based stress reduction (MBSR) protocol offers valuable coping skills for traumatic events. This study aimed to investigate the impact of Mindfulness-Based Stress Reduction (MBSR) on stress, anxiety, and depression among nursing students at Qom University of Medical Sciences during the COVID-19 pandemic. Method: This experimental study was conducted on 72 nursing students from the nursing faculty of Qom University of Medical Sciences in 2020. The sampling was conducted using stratified sampling, and the allocation method employed was simple randomization. MBSR intervention based on social networks was implemented for the experimental group. The Stress-Anxiety-Depression Assessment Questionnaire (DASS-21) was completed by both groups before the study commenced, immediately after, and 2 months post-intervention. Abstract Background One of the clinical responsibilities and goals of hospitals is to provide patients with comfort and security. The present study aims to assess patients’ sense of security among patients in Iranian hospitals. Methods The present research employed a cross-sectional design. The sample consisted of 830 patients visiting public, private, and social security hospitals in Mazandaran in the North of Iran. The required data were collected using a questionnaire developed by the researcher of this study.This questionnaire consisted of 4 dimensions:nursing, medical, advanced facilities and patient rights. The participants were selected using a proportional stratified random sampling method. Exploratory factor analysis, confirmatory factor analysis, descriptive statistics, and ANOVA were used for data analysis using SPSS version 22. Results The mean scores of patients’ sense of security in social security, private, and public hospitals were 4.16 ± 0.89, 3.78 ± 0.67, and 3.60 ± 0.89, respectively. Medical factors with a mean and standard deviation of 3.92 ± 0.76, advanced facilities and equipment with 3.89 ± 0.89, nursing factors with 3.87 ± 0.73, and patient rights with 3.71 ± 0.90 were the highest to the lowest scores, respectively. Abstract Targeted drug delivery is one of the attractive ways in which cancer treatment can significantly reduce side effects. In the last two decades, the use of antibodies as a tool for accurate detection of cancer has been noted. On the other hand, the binding of drugs and carriers containing drugs to the specific antibodies of cancer cells can specifically target only these cells. However, the use of whole antibodies brings challenges, including their large size, the complexity of conjugation, the high cost of production, and the creation of immunogenic reactions in the body. The use of nanobodies, or VHHs, which are a small part of camel heavy chain antibodies, is very popular due to their small size, high craftsmanship, and low production cost. In this article, in addition to a brief overview of the structure and characteristics of nanobodies, the use of this molecule in the targeted drug delivery of breast cancer has been reviewed.

Background & Aims: There are variables whose influence on the risk of tuberculosis (TB) recurrence change over time. Therefore, this study aimed to assess the time-dependent effects of these variables on the hazard of TB recurrence. Materials & Methods: In this historical cohort study, data were collected from 4,564 TB patients who were referred to the TB research center of Dr. Masih Daneshvari Hospital, Tehran, from 2005 to 2015, in order to evaluate factors affecting the hazard of TB recurrence in terms of time dependency or time constancy. Data were analyzed in STATA 14 software using a statistical test based on Schoenfeld residuals, the time-dependent effects method, and the time-varying effects model (considering time function as f (t) = t). Results: The results showed that only the impact of the variables of drug adverse effects and passive smoker were inconstant over time and had time-dependent effects, and they also influenced the hazard of TB recurrence. Also, the effect of the two mentioned variables on the hazard of TB recurrence displayed a decreasing and increasing trend with time, respectively. Conclusion: Using the time-varying effects model in the study of the hazard of TB recurrence allows evaluating the time-dependent effects of the studied variables and also can differentiate them from the time-independent variables.

Abstract The application of a new class of surfactants such as ionic liquids (ILs) compared with the conventional surfactants and their interactions with each other concomitant and alkaline under salinities is not well examined based on the best knowledge of the authors. So, the current work focused on the impact of sodium lauryl sulfate (SDS), sodium dodecyl benzene sulfonate (SDBS), 1-dodecyl 3-methyl imidazolium chloride (C12mim][Cl]), 1-octadecyl 3-methyl imidazolium chloride ([C18mim][Cl]) in the presence and absence of alkali namely sodium tetraborate known as borax (Na2B4O7) on the IFT variation while the salinity was changed 0–82,000 ppm (ionic strength of 0–1.4 M). The results showed the positive impact of salinity on the pH reduction and reduced the alkaline effect for pH reduction. Also, the measurements showed that the presence of surfactant reduces the role of alkaline for pH variation as it moved from 9.2 to 6.63 for the solution prepared using SLS and SDBS. The measured IFT values showed that not only alkali has a significant impact as it combined with SLS and SDBS due to a desired synergy between these chemicals, it can reduce the critical micelle concentration (CMC) for the SDBS from 1105 to 852 ppm and much higher for [C12mim][Cl].

Abstract Epigenetic changes may fill a critical gap in our understanding of kidney disease development, as they not only reflect metabolic changes but are also preserved and transmitted during cell division. We conducted a genome-wide cytosine methylation analysis of 399 human kidney samples, along with single-nuclear open chromatin analysis on over 60,000 cells from 14 subjects, including controls, and diabetes and hypertension attributed chronic kidney disease (CKD) patients. We identified and validated differentially methylated positions associated with disease states, and discovered that nearly 30% of these alterations were influenced by underlying genetic variations, including variants known to be associated with kidney disease in genome-wide association studies. We also identified regions showing both methylation and open chromatin changes. These changes in methylation and open chromatin significantly associated gene expression changes, most notably those playing role in metabolism and expressed in proximal tubules. Our study further demonstrated that methylation risk scores (MRS) can improve disease state annotation and prediction of kidney disease development. Collectively, our results suggest a causal relationship between epigenetic changes and kidney disease pathogenesis, thereby providing potential pathways for the development of novel risk stratification methods.

Communication is associated to the effectiveness and efficiency of any sports team function due to its informative, motivational and evaluative role. However, the effects of communication skills within the physical education context are rarely investigated. Hence, the aim of this study was to investigate the effects of communication skills in the physical education on enjoyment and involvement of high school students. It was hypothesized that communication skills within the physical education class have positive effects on enjoyment and involvement of high-school students. The method used in this study was a structural equation modelling. The participants included 384 adolescents age 16 to 18 years old (mean 16.96±0.57 years old) who attended in regular high-schools. Teacher Communication Behavior Questionnaire (TCBQ) and Fragebogens zur Erfassung von Freude am Schulsport im Jugendalter (FEFS-J), and Student Engagement Instrument (SCI) were used for assessing communication skills, enjoyment, and involvement. To analyze data, we used Pearson correlation test and structural equation modeling. Results showed that communication skills had significant effects on enjoyment (T=4.182) and involvement (T=6.082). In addition, enjoyment had significant effects on involvement (T=3.291). Results of evaluating fitness of good showed that our model has a good fit (GOF=0.93). Communication skills are very important for engagement and learning of students.

The present study aims to compare the creativity of students in virtual and classroom courses at University of Isfahan and study its prediction based on the personality traits of these students. The statistical population of the study consisted of all faculty members and students at University of Isfahan in academic year 2017-2018. Among them, 150 subjects were selected using Morgan Table and random sampling method. The data collection was done through Abedi's Creativity Questionnaire (1993) and Costa and McCrae Personality Questionnaire (1992). Data were analyzed by T-test for independent groups, Pearson correlation coefficient and multiple regression analysis. Results show that there is a significant difference in creativity scores of students attending virtual courses and those attending classroom courses, as well as between female and male students. However, the relationship between personality traits and creativity was significantly stronger among classroom students than the virtual students, and these traits were better predictors of creativity in classroom students than in virtual students. Based on findings, it can be concluded that more usage of the information and communication technologies such as internet, wireless networks, cell phone, etc.(ICTs) by virtual students not only increases their creativity, but also, as an important environmental and intrapersonal factor, affects the relationship between personality traits and creativity among these students and weakens it. This finding shows the great role of acquisitive-digital factors in students' creativity.
