AIM: To evaluate the histological changes after transepithelial corneal crosslinking (CXL) using partial thickness excimer laser ablation or epithelial ethanol application in an experimental rabbit study.METHODS: Right eyes of twenty-four rabbits were studied. Four eyes received total epithelial debridement (group I). Four eyes received partial thickness epithelial ablation with excimer laser (group II). Twelve eyes were treated with different durations (30s and 60s) and concentrations (18% to 48%) of ethanol (group III). Riboflavin was applied for 30min intervals along with topical proparacaine drops with benzalkonium chloride, and 370 nm irradiation was performed for 30min, while riboflavin was instilled every 3min. Four eyes (group IV) received 48% ethanol for 30s without riboflavin and irradiation. Eyes were collected after 24h and examined histologically.RESULTS: All eyes in group I showed keratocyte loss in the superficial 300 µ of corneal storma. In group II, 1-4 layers of epithelium were preserved and no keratocyte loss occurred. In group III, CXL after treatment with ethanol up to 24% concentration and up to 60s revealed no keratocyte loss. CXL after treatment with 48% and higher ethanol concentrations yielded keratocyte loss in the superficial 200 µ to 300 µ of cornea.CONCLUSION: Incomplete excimer laser ablation of the epithelium or treatment with ethanol up to 24% concentration and up to 60s duration yielded no stromal keratocyte loss. To get the same histological appearance seen in epithelial debridement group, partial thickness excimer laser epithelial ablation or ethanol application is not adequate for transepithelial CXL.

Bu randomize, çift kör, plasebo kontrollü çalışma, tavşan gözlerinde sütür ile indüklenmiş kornea neovaskülarizasyonunda subkonjonktival bevacizumab ile potansiyel biyobenzer bevacizumab olan Anti-Human VEGF GU01'in, tedavilerinin etkinliklerinin karşılaştırılması amacıyla yapılmıştır. On beş tane Yeni Zelanda tavşanının sağ gözüne, KNV oluşturmak amacıyla 7-0 siyah ipek sütür atıldı. Birinci haftada, yeterli KNV oluştuğu izlendikten sonra, bütün sütürler alındı. Ameliyat mikroskobu ile korneaların görüntüleri alındı. KNV ImageJ programı yardımıyla hesaplandı. Tavşanlar randomize olarak üç gruba ayrıldı. Kontrol grubuna 0,1 ml dengeli tuz solüsyonu (n=5), bevacizumab grubuna 0,1 ml/2,5 mg bevacizumab (n=5), biyobenzer grubuna 0,1/2,5 mg Anti-Human VEGF GU01 (n=5) subkonjonktival olarak uygulandı. 1 hafta sonra alınan görüntülerdeki KNV alan değişimi yüzde olarak hesaplandı. Sakrifasyon sonrası histolojik ve immunohistokimyasal değerlendirmeler, hematoksilen-eozin ve CD31 boyalarıyla yapıldı. On dördüncü günde, KNV alanındaki gerileme oranları bevacizumab (%82,2) ve biyobenzer (%83,6) grubunda, kontrol grubuna göre (%24.6) daha yüksek bulundu (p = 0,009). Bevacizumab ve biyobenzer gruplarındaki KNV gerileme oranları arasında bir fark izlenmedi (p = 0,347). Histolojik değerlendirmede bevacizumab ve biyobenzer gruplarında, kontrol grubuna göre daha az damar yapıları izlendi. Sonuç olarak çalışmamızda, tavşan gözlerinde, sütür ile indüklenmiş kornea neovaskülarizasyonunda, bevacizumab ile Anti-Human VEGF GU01 etkili bulundu. Bu veriler, Anti-Human VEGF GU01'in biyobenzer bevacizumab olarak geliştirme çalışmalarının devam etmesini desteklemektedir. İleri preklinik ve klinik çalışmalara ihtiyaç duyulmaktadır. The Effect Of A New Biosimilar Bevacizumab On Corneal Neovascularization This study is aimed to compare the effect of subconjuntival bevacizumab and Anti-Human VEGF GU01 (potential biosimilar) on suture induced corneal neovascularization (CNV) in rabbit eyes. CNV was induced by sutures (7-0 silk) in the right eyes of 15 rabbits. On seventh day, corneal sutures were removed after sufficient neovascularization was confirmed. Photographs were taken a surgical microscope (Möller-Wedel) . CNV area is calculated with ImageJ program. Rabbits randomly were divided into three groups and were treated with sunconjuctival 0,1ml balanced salt solution (control group, n=5), 0,1 ml/2,5 mg bevacizumab (bevacizumab group, n=5) and 0,1 ml/2,5 mg Anti-Human VEGF GU01 (biosimilar group, n=5). One week after suturation, CNV area changes were calculated as a percentage. After sacrification, histological and immunohistochemical staining were made by using hematoxlin and eosin and anti CD31. On day 14, the mean percentages of regression areas of CNV in bevacizumab (%82,2) and biosimilar (%83,6) groups were higher compared control group (%24,6) (p = 0,009). There were no difference between bevacizumab and biosimilar groups (p = 0,347). Histopathology showed that bevacizumab and biosimilar groups presented less neovascularization than the control group. These results suggest that bevacizumab and Anti-Human VEGF GU01 have same effect fort he inhibition of CNV. These data support the development process of Anti-Human VEGF GU01 as a potential biosimilar to bevacizumab. Further preclinic and clinic studies are needed.

METHODS: This is an observational, case -control study. Patients with keratoconus in one eye and forme fruste keratoconus in the fellow eye were compared with sex and age matched with controls healthy subjects. All subjects were evaluated with rotating scheimpflug imaging system. The receiver -operating -characteristic curves were analyzed to evaluate the sensitivity and specificity of the parameters. RESULTS: Twenty-seven patients with keratoconus in one eye and forme fruste keratoconus in the fellow eye were compared with 40 eyes of 40 normal subjects. Kırkiki yaşında erkek hasta, yaklaşık 2 haftadır özellikle okur-ken aniden görme bulanıklaşması ile kliniğimize başvurdu. Detaylı hikâyesinde hastanın diş ağrısı nedeniyle diş hekimine başvurduğu öğrenildi. Hastanın okuma ile ilgili şikâyetleri diş çekiminden 2 hafta sonra başlamıştı. Hasta diş çekilmesinden 1 ay öncesinde amoksisilin, sonrasında 2 hafta süreyle non-steroid anti-inflamatuar ilaç kullanmıştı. Bu vakada diş çekimi sonra-sında görülen çift taraflı SSKR olgusu sunulmuştur, A 42-year-old male presented to our clinic with acute blurring of vision in both eyes especially in reading for about 2 weeks. Detailed history revealed that the patient recently applied to his dentist because of tooth ache. The patient's complaints abo-ut reading had started 2 weeks after tooth extraction. He had a medication of amoxicillin for a month prior to tooth removal and non-steroidal anti-inflammatory drugs after extraction. In this case we report a bilateral CSCR case after tooth extraction
