Sir .—A life-table analysis of circumcision and the risk of cancer of the penis has been published in the AJDC , 1 using circumcision figures based on a sample of adult American men. This yielded a lifetime risk for cancer of the penis of one in 600 for uncircumcised men. Due to the extreme variability of circumcision in the United States in the older age groups in which cancer of the penis generally occurs, and the inevitable unreliability of incidence figures in this population, a lifetime risk Table is herewith submitted based on a population known to be essentially totally uncircumcised, namely, that of Denmark. 2 This shows a lifetime risk in Denmark of one in 909 for cancer of the penis. THE incidence of carcinoma in situ (CIS) of the genitalia either is increasing or is being diagnosed more frequently. It may resemble a variety of benign cutaneous abnormalities, and the clinician may not appreciate the malignant biological features of these lesions unless a biopsy is performed. We describe the multiple clinical appearances of CIS occurring on the male and female genitalia. We describe the clinical features of 19 cases and review the relevant literature on this subject. Patients and Methods The 19 patients reported in this series were seen for diagnosis and treatment of cutaneous genital lesions. The relevant clinical data are summarized in the Table. All patients had biopsy confirmation of CIS made by light microscopy. The histologic diagnosis of CIS was made by consulting dermatopathologists and pathologists. • The phenomenon of multiple lesions of Bowen's disease appearing on the genitalia of young adults is becoming increasingly recognized. We have seen two patients who had genital lesions with histologic features of squamous carcinoma in situ, but with an entirely different clinical appearance. In both patients, varying degrees of spontaneous regression occurred. A conservative approach to management of these lesions is suggested. ( Arch Dermatol 114:1698-1699, 1978)

The more common causes of priapism are neurogenic stimuli (which include psychic, central nervous system, and local reflex stimulation), hematological disorders, (chronic myelogenous leukemia and certain hemoglobinopathies), and local mechanical or traumatic conditions. 1 Priapism secondary to metastatic disease involving the penis is rare. The purpose of this report is to describe a patient with priapism as the initial manifestation of a renal malignancy. Patient Summary A 66-year-old white man was admitted to the Atlanta Veterans Administration Hospital with a 2½-week history of persistent priapism, Eighteen days prior to admission, after having lifted a 50-lb weight, he noted a burning sensation in his scrotum, followed by the appearance of a tender, brownish nodule on the right side of the penis. Pearly penile papules share histologic features in common with other acral angiofibromas: adenoma sebaceum, subungual and periungual fibromas, fibrous papule of the nose (face), acquired acral angiofibroma, and oral fibroma.

ERYTHROPLASIA is a rare disease and was first described in 1893 by Fournier and Darier. 1 Since Queyrat's 2 studies in 1911, the disease has been known as erythroplasia of Queyrat. Sulzberger and Satenstein, 3 in 1933, reported the first case in the American literature. Approximately 50 additional cases have since been presented at various dermatologic societies in this country, in addition to reports by Stiles 4 and by Irgang and Alexander. 5 In spite of the rarity of cases, there has been considerable discussion concerning this disease. It is our purpose in this paper to report our observations in 10 cases. We intend to show that in these instances there was no clinical or microscopic evidence of malignancy or premalignancy and that lesions develop not only on mucous membranes but also on glabrous skin. Pearly white, dome-shaped papules of the penis were observed in two cases. These were located on the coronal margin and in the sulcus. The lesions were persistent, asymptomatic, and had been present many years. Histologically, these lesions showed a rich vascular network surrounded by dense connective tissue. Clinically, these lesions were easily confused with verruca acuminata and heterotopic sebaceous glands, but they could be differentiated by history, response to treatment, and histological appearance.

Background Human papillomavirus is associated with urogenital carcinogenesis such as penile and uterine cervix cancer. On the other hand, association between human papillomavirus infection and risk of bladder and prostatic cancer remains controversial. Case presentation We report a rare case of a 67-year-old Japanese man with synchronous triple urogenital cancer including bladder small cell carcinoma, prostatic ductal adenocarcinoma, and penile squamous cell carcinoma, who presented with a history of asymptomatic gross hematuria. Bladder small cell carcinoma and prostatic ductal adenocarcinoma were diagnosed by histopathological examination after transurethral resection of the tumor. Moreover, he was diagnosed with penile carcinoma based on the exfoliative cytodiagnosis of nodular and papillary tumors inside the preputial collar. He was treated with laparoscopic radical cystectomy, urethrectomy, partial penectomy with pelvic lymph node dissection, and ileal conduit urinary diversion. To identify a common pathogenesis, we considered human papillomavirus as an etiologic factor because it is a known risk factor for penile carcinoma. Human papillomavirus deoxyribonucleic acid bands were detected by polymerase chain reaction in the three tumors. There was a possibility that human papillomavirus was involved in the carcinogenesis of the triple cancer. Conclusions To the best of our knowledge, this is the first report of the synchronous triple urogenital cancer of small cell carcinoma of the bladder, ductal adenocarcinoma of the prostate and penile squamous cell carcinoma. We believe that human papillomavirus may have been involved in the carcinogenesis of the triple urogenital cancer described in this case.
