This paper reviews the contribution of qualitative methods to health services research (HSR) and discusses some of the issues involved in recognizing quality in such work. The place of qualitative work is first defined by reference to Archie Cochrane's agenda for HSR and the limitations of the recent focus on randomized trials as the standard method. Health care practice involves large elements of improvisation which cannot be captured by evidence-based approaches. Qualitative methods offer ways of understanding this improvisation and of identifying more efficient and effective practices, as well as considering the traditional topics of equity and humanity. The methodological procedures of qualitative work reflect a long-established inductive tradition in scientific practice. The logic of grounded theory provides a contemporary specification. In its application, it is quite different from the methodological anarchy of postmodernism. The use of qualitative research and the theoretically stated generalizations which arise from it inform reflective work by health service managers, planners and clinicians.

Background During the COVID-19 pandemic, it has been essential for occupational health services (OHS) providers to react rapidly to increased demand and to utilize resources in novel ways. The impact of the COVID-19 pandemic on the psychological well-being of staff is already identified as an area of high risk; therefore, providing timely access to psychological support may be vital, although limited evidence is available on how these risks are best managed. Aims To describe implementation and analysis of a psychology-led COVID-19 telephone support line in a National Health Service OHS. Methods Data from calls made to the support line were collected over the first 4 weeks of service implementation. Numerical data including frequency of calls and average waiting time were first considered. A content analysis was then conducted on call notes to identify prevalence of themes. Results Six hundred and fifty-five calls were received, and 362 notes included sufficient information for use within the content analysis. Frequency of calls peaked within the first week followed by a reduction in the number of calls received per day over time. Most calls included discussion around clarification of guidance (68%) with a smaller subset of calls offering support around anxiety (29%). Prevalence of themes did not appear to change over time. Conclusions Clear and timely information is vital to support the well-being of healthcare staff. A psychologically informed telephone support line was a good use of occupational health service resources in the interim while more tailored advice and services could be established.

An experienced, wise teacher suggested that the parents were trying to compensate to the children for other materialistic and psychological disadvantages. Nevertheless, the pocket money created problems for the children. When it was not lost or stolen, it was spent on dill pickles and soft drinks for breakfast or, worse yet, on paper tattoos, wax teeth, or some other toy that distracted attention in the classroom.

Redox modifications play an important role in many cellular processes, including cell death. Ischemic preconditioning (IPC) has been shown to involve redox signaling and protein S-nitrosylation (SNO) is greatly increased following myocardial IPC. Furthermore, inhibition of GSNO-reductase has also been shown to increase protein SNO and is cardioprotective. SNO is thought to provide cardioprotection, in part, by modulating enzyme activity. SNO may also provide cardioprotection by reducing cysteine oxidation during ischemia/reperfusion (IR) injury. In order to test the hypothesis that SNO provides cardioprotection by providing direct protection against cysteine oxidation following IR injury, we developed a method to measure protein oxidation using resin-assisted capture (Ox-RAC). This method is similar to the SNO-RAC method used in the quantification of SNO. Langendorff perfused hearts were subjected to various perfusion protocols (control, IPC, IR, IPC-IR) and homogenized. Each sample was divided into two equal aliquots, and subjected to the SNO-RAC/Ox-RAC procedure in order to simultaneously analyze SNO and oxidation. Using the SNO-RAC protocol, we identified 44 different proteins that were SNO with IPC. With the Ox-RAC protocol, we identified nearly 200 oxidized proteins following IR injury. Interestingly, IPC significantly reduced protein oxidation by more than 30% following ischemia and early reperfusion as determined via label free peptide analysis. Additionally, more than half of the proteins which showed significantly increased SNO with IPC, also showed a significant decrease in cysteine oxidation following IPC-IR at the same site when compared to IR alone. These results support the hypothesis that SNO provides cardioprotection by shielding cysteine residues from ROS-induced oxidation during IR injury. Therefore, the level of protein SNO plays a critical role in IR injury, where ROS production is increased.

Nursing leadership is instrumental in providing structure, consistency, and specific feedback on progress as well as real time CLABSI rates to the team. CLABSIs are common and can be fatal in the transplant population. Continued surveillance of techniques and practices learned are ongoing and monitored for compliance. Improvements in all aspects of nursing carewith meticulous infection control techniques and environmental monitoring is essential for improved patient outcomes and reducing CLABSI rates in a bone marrow transplant population.

Introduction Takayasu's Arteritis, formerly known as "pulseless disease", is a chronic idiopathic vasculitis which affects the large vessels in the body. First described in the 1800's, this rare condition is more commonly found in Asian women in their 40's. The aorta and its main branches are the primary vessels affected, with the most typical features reflected as ischemia or aneurysm formation. With Takayasu's Arteritis being a rare condition and its acute phase presentation often similar to other conditions, diagnosis is often difficult. Case Presentation A 48 year old Irish Caucasian female, who presented as a typical history of an aortic dissection (chest pain radiating to her back in an interscapular region and a systolic blood pressure differential of 50 mmHg between her right and left upper limbs), was investigated with a number of imaging modalities and diagnosed with Takayasu's Arteritis, involving arteries affecting a number of organs. She was treated as per protocol for Takayasu's Arteritis. A diagnosis of cervical cancer quickly followed. Conclusion This case report highlights that a differential diagnosis should never be dispelled based upon a "typical" history. The importance of modern day imaging techniques such as CT, MRI and angiography, can often be paramount to confirming a diagnosis and the extent of the pathology. A possible link between Takayasu's Arteritis and gynaecological malignancies may exist.
