Background: Pseudophakic presbyopic correction is among the prevalent methods for regaining near vision capacity. The purpose of this study is to compare the impact of pseudophakic monovision correction and multifocal lens implantation on the performance in a series of activities of daily living (ADL) of presbyopic patients., Methods: An ADL research framework (10 ADLs) was constructed and validated in a sample of patients divided into three validation groups according to their near visual acuity. Sixty-two participants that underwent mini-monovision (MoG) cataract extraction and 60 that underwent bilateral multifocal lenses implantation (MfG) populated study groups and addressed the ADLs. Binocular uncorrected distant (BdUVA) and near (BnUVA) visual acuity were associated with ADL scores and with subjective satisfaction using the VF-14 questionnaire., Results: Test-retest reliability [all Intraclass Correlations Coefficients (ICC) >0.90] and construct validity (all p  < 0.05) tests indicated sufficient psychometric performance of the ADL framework. Both study groups presented comparable mean ADL scores ( p  = 0.07) however, MoG patients had lower performance in demanding ADLs ( p  = 0.02). ADL scores demonstrated significant correlation with BnUVA (r 2  = -0.67, p  < 0.01) VF-14 scores (r 2  = 0.53, p  < 0.01)., Conclusions: Both methods provide sufficient near vision capacity for the majority of activities of daily living. Purpose: To assess the effectiveness and safety of a multifocal intraocular lens (IOL) with +2.5 diopter (D) additional power compared with a monofocal IOL., Setting: Fifteen sites in the United States., Design: Prospective randomized patient- and observer-masked clinical trial., Methods: Randomized patients received multifocal or monofocal IOLs bilaterally. Visual acuity (33 cm, 40 cm, 53 cm, 60 cm, 4 m) was measured; safety was assessed through adverse event rates. Patient-reported visual outcomes were evaluated using the Visual Tasks questionnaire. The frequency and severity of visual disturbances were evaluated using the Assessment of Photic Phenomena and Lens EffectS questionnaire., Results: The multifocal IOL (n = 155) provided better corrected distance visual acuity at 53 cm than the monofocal IOL (n = 165) (0.322 versus 0.512 logMAR; between-group difference, -0.190 logMAR; P < .0001) and 40 cm but not at 4 m. Ocular adverse event rates were less than 3.84% in both groups. Serious adverse event rates were comparable between the 2 IOL types. Patients with multifocal IOLs reported less difficulty with near tasks (with and without correction) and intermediate tasks (without correction). Difficulty with extended-intermediate and distance tasks was similar between groups. The most frequently reported self-rated severe phenomena were halos, starbursts, and glare. Most patients (monofocal ≥72%; multifocal ≥73%) reported never experiencing blurred, distorted, or double vision., Conclusions: The +2.5 D multifocal IOL provided better vision at 40 cm and 53 cm and similar vision at 4 m compared with the monofocal IOL. Purpose: To evaluate visual acuity and contrast sensitivity after bilateral implantation of AcrySof® IQ ReSTOR® +2.50 D or +3.00 D., Methods: A total of 56 eyes with cataract had bilateral implantation of a multifocal IOL. Inclusion criteria were corneal astigmatism less than 1.0 diopters, axial length between 22.0 and 24.0 mm, and no associated ocular disease. Outcome measures recorded 12 months postoperatively were binocular uncorrected and corrected distance, intermediate and near visual acuities, and contrast sensitivity. Postoperative quality of life and patient satisfaction were assessed with the VF-14 Questionnaire., Results: Twenty months postoperatively, the mean uncorrected and best-corrected distance visual acuities, UCIVA at 60 cm, and contrast sensitivity were similar in all two groups. UCNVA were better in the +3.00 IOL group than in the +2.50 D group. Patients reported good performance for visual tasks., Conclusion: Apodized multifocal IOLs seem a good compromise for non-spectacles life in elderly patients; our study indicates that this solution has a good long-term stability on patients' visual function.

Purpose: To compare the postoperative quality of vision between different bilateral placements of near segments of rotationally asymmetric refractive multifocal intraocular lenses (IOLs) and to determine how this affects visual performance., Setting: Cathedral Eye Clinic, Belfast, Northern Ireland, United Kingdom., Design: Retrospective comparative case series., Methods: The study enrolled consecutive patients having refractive lens exchange and implantation of rotationally asymmetric multifocal IOLs. Group 1 received bilateral SBL-3 IOLs and Group 2 received bilateral Lentis Mplus LS-312 MF30 IOLs, with the near segments placed inferonasally in each group. Group 3 received a Lentis Mplus LS-312 MF20 IOL in the dominant eye with the near segment positioned superotemporal and a Lenstec SBL-3 IOL positioned inferonasally in the fellow eye. Binocular uncorrected (UDVA) and corrected distance visual acuities, binocular uncorrected near (UNVA) and intermediate (UIVA) visual acuities, binocular distance-corrected near and intermediate visual acuities, and quality of vision were evaluated over 3 months postoperatively., Results: The study enrolled 180 patients (360 eyes). There was no significant difference between the groups in binocular UDVA, UIVA, and UNVA; however, there was a significant difference between the groups in quality of vision (P ≤ .001). Group 3 had significantly better overall quality of vision., Conclusion: When implanting rotationally asymmetric multifocal IOLs, a combination of superotemporal placement of the near segment (+2.00 diopter [D] addition [add]) in the dominant eye with inferonasal placement of the near segment (+3.00 D add) in the fellow eye yielded consistent, high overall quality of vision and uncorrected visual acuity., Financial Disclosure: None of the authors has a financial or proprietary interest in any material or method mentioned., (Copyright © 2016 ASCRS and ESCRS. Purpose: A retrospective analysis of visual function and postoperative complaints in patients after posterior vitrectomy and multifocal Mplus or MplusX intraocular lens implantation., Material and Methods: 14 patients (14 eyes) were enrolled in the study. All patients underwent posterior vitrectomy due to underlying retinal pathologies. A combined procedure of phacovitrectomy was performed in some patients, whereas others underwent a separate cataract extraction as a second stage of treatment. The Mplus or MplusX intraocular lenses were implanted in all cases and the postoperative visual function and patient complaints were analysed., Results: Multifocal lens explantation was not necessary in any case. Good uncorrected distance and near visual acuity was achieved in most cases. When implanted prior to vitrectomy, the intraocular lenses did not limit the view into the posterior segment, so precise manipulations within the macula and peripheral retina were feasible. The right choice between Mplus and MplusX minimises the rate of postoperative complaints. Conslusions: The multifocal Mplus and MplusX intraocular lenses offer a good alternative for patients who need posterior vitrectomy and still want to achieve good uncorrected distance and near visual acuity.

Purpose: To study the impact of loss in optical quality from glistenings in diffractive multifocal intraocular lenses (IOLs) using ray tracing in a model eye., Setting: Independent research laboratory, Irvine, California, USA., Design: Experimental study., Methods: A pseudophakic eye model was constructed in Zemax, an optical ray-tracing program, using the Arizona eye model as the basis. The Mie scattering theory was used to describe the intensity and direction of light as it scattered for a spherical particle immersed in a diffractive multifocal IOL. To evaluate the impact of glistening scatter, a more advanced eye model was constructed in Fred, a nonsequential optical ray-tracing software. An evaluation of scatter and modulation transfer function (MTF) was performed for a hydrophobic biomaterial with a refractive index of 1.54 for various sizes and densities of glistenings under mesopic conditions., Results: As predicted by the Mie theory, the amount of scatter was a function of the change in the refractive index, size of the scatterer, and volume fraction of the scatterers. This modeling showed that an increase in density of glistenings can lead to a significant drop of MTF of the IOL. This effect was more pronounced in multifocal IOLs than in monofocal IOLs., Conclusions: Mathematical modeling showed that glistenings in multifocal IOLs lead to a reduction in MTF of the IOL and the pseudophakic eye. Unlabelled: A comprehensive review of the effect of multifocal intraocular lens (IOL) designs on postoperative ocular straylight was performed. Studies reporting straylight values obtained with the natural pupil using the C-Quant device after uneventful multifocal IOL implantation were included. The IOLs were categorized based on their material characteristics; that is, hydrophobicity and presence of colored chromophores. Age adjustment was achieved using the straylight age-dependency norm for pseudophakic eyes. This norm also served as a reference for comparing mean straylight levels of the various IOLs. The literature review identified 10 studies reporting 9 multifocal IOL designs. The hydrophilic IOLs showed less straylight than the hydrophobic IOLs by 0.08 log(s) (P = .001). Blue violet light-filtering IOLs showed less straylight than standard IOLs by 0.04 log(s), which was not statistically significant (P = .32). Purpose: The aim of the study was evaluation and comparison of rotational stability and the effect of multifocal toric intraocular lens rotation (T-MIOL) at visual quality of two types of intraocular lens AcrySof IQ ReSTOR Toric SND1Tx (Alcon Laboratories, Inc.) and AT LISA toric 909M (Carl Zeiss Meditec AG). There was evaluated a pseudophakic dysphotopsia based on patients subjective feelings as well as the independence on glasses and overall satisfaction with the visual quality after T-MIOLs were implanted., Methods: The retrospective study included 68 eyes (100 %) of 34 patients, who underwent an uncomplicated cataract surgery with multifocal toric intraocular lens was implanted. 68 eyes were divided into two groups. Group A consisted of 48 eyes (70,6 %) with AcrySof IQ ReSTOR Toric SND1Tx and group B included 20 eyes (29,4 %) with AT LISA toric 909M multifocal toric intraocular lenses were implanted. The T-MIOL position, planned axis and misalignment were evaluated one week and 6 months after surgery. Surgical lens reposition was indicated in case of worsening visual quality due to T-MIOL rotation from its planned position., Results: In the group of 68 eyes, the mean uncorrected distance visual acuity (UDVA) before surgery was 0,08 (n = 48) in group A and 0,09 (n = 20) in group B. The mean UDVA after T-MIOL were implanted improved to 0,83 ± 0,06 (n = 48) in group A and 0,75 ± 0,08 (n = 20) in group B. The mean UDVA in patients before T-MIOL surgical rotation was 0,57 ± 0,03 (n = 5) in group A and 0,59 ± 0,08 (n = 5) in group B. At six months after T-MIOL surgical rotation, the uncorrected distance visual acuity was 1,0 in both groups (p = 0.70). The surgical rotation of T MIOL was performed in 10 eyes (14,7 %) of 68 eyes (p = 0.48), in group A (n = 48) in 5 eyes (10,4 %) and in group B (n = 20) in 5 eyes (25,0 %). Mean deviation from the planned axis was 8,2 ± 1,78 degrees in group A and 13,4 ± 3,04 degrees in group B. At six months postoperatively, the T-MIOL rotational stability showed less than 5 degrees for all surgically rotated lenses in both groups. Mean cylinder decreased in all patients from -2,92 ± 1,85Dcyl to -0.85 ± 0.61Dcyl after surgery. The mean spherical equivalent for all patients decreased from 0,56 ± 4,75D to 0,06 ± 1,33D after surgery. Presence of disturbing fotic phenomenon had reported 11 (32.4 %) of 34 patients, 9 of them they were subsequently indicated T-MIOL reposition. All 34 patients were able to perform normal daily tasks independently on spectacle correction. At six months after surgery, there was very high satisfaction with the quality of postoperative vision in both groups., Conclusion: Implantation of toric multifocal intraocular lens, both of AcrySof ReSTOR Toric SND1Tx (Alcon Laboratories, Inc.) and AT LISA toric 909M (Zeiss Meditec AG Carl) in cataract patients is safe and effective method to correct corneal astigmatism and maintain the visual multifocality. Early surgical T-MIOL reposition should be performed to put the T-MIOL to planned axis in steepest meridian if unwanted lens rotation is identified., Key Words: cataract surgery, toric multifocal intraocular lens, rotation, rotational stability, quality of vision.

Objective: To present the theoretical and experimental characterization of the halo in multifocal intraocular lenses (MIOL)., Method: The origin of the halo in a MIOL is the overlaying of 2 or more images. Using geometrical optics, it can be demonstrated that the diameter of each halo depends on the addition of the lens (ΔP), the base power (P(d)), and the diameter of the IOL that contributes to the «non-focused» focus. In the image plane that corresponds to the distance focus, the halo diameter (δH(d)) is given by: δH(d)=d(pn) ΔP/P(d), where d(pn) is the diameter of the IOL that contributes to the near focus. Analogously, in the near image plane the halo diameter (δH(n)) is: δH(n)=d(pd) ΔP/P(d), where d(pd) is the diameter of the IOL that contributes to the distance focus. Patients perceive halos when they see bright objects over a relatively dark background. In vitro, the halo can be characterized by analyzing the intensity profile of the image of a pinhole that is focused by each of the foci of a MIOL., Results and Conclusions: A comparison has been made between the halos induced by different MIOL of the same base power (20D) in an optical bench. As predicted by theory, the larger the addition of the MIOL, the larger the halo diameter. For large pupils and with MIOL with similar aspheric designs and addition (SN6AD3 vs ZMA00), the apodized MIOL has a smaller halo diameter than a non-apodized one in distance vision, while in near vision the size is very similar, but the relative intensity is higher in the apodized MIOL. When comparing lenses with the same diffractive design, but with different spherical-aspheric base design (SN60D3 vs SN6AD3), the halo in distance vision of the spherical MIOL is larger, while in near vision the spherical IOL induces a smaller halo, but with higher intensity due to the spherical aberration of the distance focus in the near image. In the case of a trifocal-diffractive IOL (AT LISA 839MP) the most noticeable characteristic is the double-halo formation due to the 2 non-focused powers., (Copyright © 2013 Sociedad Española de Oftalmología. Purpose: To discuss the various static and dynamic surgical approaches which attempt to give presbyopes good vision at far, intermediate and near viewing distances., Content: Static methods broadly adopt the same optical techniques as those used in presbyopic contact lens correction and aim to satisfy the needs of the presbyope by increasing binocular depth-of-focus, often using monovision as well as simultaneous-imagery. Dynamic methods generally attempt to make use of at least some of the still-active elements of the accommodation system. They include procedures which are supposed to modify the relative geometry of the ciliary muscle and lens, or which reduce the stiffness of the presbyopic lens either by replacing it with other natural or man-made material or by subjecting it to femtosecond laser treatment. Alternatively the natural lens may be replaced by some form of intraocular lens which changes power as a result of forces derived from the still-active ciliary muscle, zonule and capsule, or other sources., Conclusions: At present, multifocal intraocular lenses appear to offer the most consistent and reliable surgical approach to surgical presbyopic correction. They have obvious advantages in convenience and stability over optically-similar, simultaneous-image presbyopic contact lenses but this must be balanced against their relative inflexibility in cases of patient dissatisfaction. Dynamic methods remain largely experimental. Purpose: To evaluate intraindividual visual acuity, wavefront errors and modulation transfer functions in patients implanted with two diffractive multifocal intraocular lenses., Methods: This prospective study examined 40 eyes of 20 cataract patients who underwent phacoemulsification and implantation of a spherical multifocal ReSTOR intraocular lens in one eye and an aspheric Tecnis ZM900 multifocal intraocular lens in the other eye. The main outcome measures, over a 3-month follow-up period, were the uncorrected photopic distance and near visual acuity and the defocus curve. The visual acuity was converted to logMAR for statistical analysis and is presented in decimal scale. The wavefront error and modulation transfer function were also evaluated in both groups., Results: At the 3-month postoperative visit, the mean photopic distance uncorrected visual acuity (UCVA) was 0.74 +/- 0.20 in the ReSTOR group and 0.76 +/- 0.22 in the Tecnis group (p=0.286). The mean near UCVA was 0.96 +/- 0.10 in the ReSTOR group and 0.93 +/- 0.14 in the Tecnis group (p=0.963). The binocular defocus curve showed measurements between the peaks better than 0.2 logMAR. The total aberration, higher-order aberration and coma aberration were not significantly different between the groups. The spherical aberration was significantly lower in the Tecnis group than in the ReSTOR group. (p=0.004). Both groups performed similarly for the modulation transfer function., Conclusion: The ReSTOR SN60D3 and Tecnis ZM 900 intraocular lenses provided similar photopic visual acuity at distance and near. The diffractive intraocular lenses studied provided a low value of coma and spherical aberrations, with the Tecnis intraocular lens having a statistically lower spherical aberration compared to the ReSTOR intraocular lens. In the 5 mm pupil diameter analyses, both intraocular lens groups showed similar modulation transfer functions.
