Vertebral compression fractures (VCF) are common in patients older than 50 years but are often undiagnosed. Zebra Medical Imaging developed a VCF detection algorithm, with machine learning, to detect VCFs from CT images of the chest and/or abdomen/pelvis. In this study, we evaluated the diagnostic performance of the algorithm in identifying VCF. We conducted a blinded validation study to estimate the operating characteristics of the algorithm in identifying VCFs using previously completed CT scans from 1200 women and men aged 50 years and older at a tertiary-care center. Each scan was independently evaluated by two of three neuroradiologists to identify and grade VCF. Disagreements were resolved by a senior neuroradiologist. The algorithm evaluated the CT scans in a separate workstream. The VCF algorithm was not able to evaluate CT scans for 113 participants. Of the remaining 1087 study participants, 588 (54%) were women. Median age was 73 years (range 51-102 years; interquartile range 66-81). For the 1087 algorithm-evaluated participants, the sensitivity and specificity of the VCF algorithm in diagnosing any VCF were 0.66 (95% confidence interval [CI] 0.59-0.72) and 0.90 (95% CI 0.88-0.92), respectively, and for diagnosing moderate/severe VCF were 0.78 (95% CI 0.70-0.85) and 0.87 (95% CI 0.85-0.89), respectively. Implementing this VCF algorithm within radiology systems may help to identify patients at increased fracture risk and could support the diagnosis of osteoporosis and facilitate appropriate therapy. © 2023 Amgen, Inc. Introduction: Due to loss of brain buoyancy, spontaneous spinal cerebrospinal fluid (CSF) leaks cause orthostatic headaches but also can cause symptoms indistinguishable from behavioral variant frontotemporal dementia (bvFTD) due to severe brain sagging (including the frontal and temporal lobes), as visualized on brain magnetic resonance imaging. However, the detection of these CSF leaks may require specialized spinal imaging techniques, such as digital subtraction myelography (DSM)., Methods: We performed DSM in the lateral decubitus position under general anesthesia in 21 consecutive patients with frontotemporal dementia brain sagging syndrome (4 women and 17 men; mean age 56.2 years [range: 31-70 years])., Results: Nine patients (42.8%) were found to have a CSF-venous fistula, a recently discovered type of CSF leak that cannot be detected on conventional spinal imaging. All nine patients underwent uneventful surgical ligation of the fistula. Complete or near-complete and sustained resolution of bvFTD symptoms was obtained by all nine patients, accompanied by reversal of brain sagging, but in only three (25.0%) of the twelve patients in whom no CSF-venous fistula could be detected ( P = 0.0011), and who were treated with non-targeted therapies., Discussion: Concerns about a spinal CSF leak should not be dismissed in patients with frontotemporal brain sagging syndrome, even when conventional spinal imaging is normal. Aim: Aim of the current research is to establish and assess the microleakage in bulk-fill composite, nanohybrid ormocer-based resins, and nanofilled composite resin core build-up materials employing the dye-penetration technique., Materials and Methods: Sixty human mandibular first premolar teeth with a solitary root canal without dental caries were chosen for this research. Each specimen was subjected to decoronation of 2 mm from the cementoenamel junction (CEJ), following which the root canal treatment procedure was rendered complete. A space for the post was made for all the 60 samples. Following positioning of the post, specimens were allocated into one of the following three investigational groups (20 specimens in every group) on the basis of the core build-up materials used as group I: bulk-fill composites, group II: nanohybrid ormocer-based resins, and group III: nanofilled resin composites. Direct composite was used for core build-up and subjected to light-curing. Following this, the specimens were immersed in 1% methylene blue solution for 24 hours interval. Each section was evaluated for dye diffusion employing a stereomicroscope with software at a magnifying power of 40× and surface contact between dentin and base of the material was evaluated under scanning electron microscope., Results: Nanohybrid ormocer-based composites exhibited the least microleakage at 1.12 ± 0.14, in pursuit by nanofilled composite resins at 1.79 ± 0.09, and finally the bulk-fill composites at 2.85 ± 0.11, amid the investigational groups studied. A statistically significant difference amid the three dissimilar cores buildup substances was found upon analysis of variance., Conclusion: Despite the study limitations, this research came to a conclusion that each of the three investigated core build-up substances exhibited microleakage. However, amid the three, nanohybrid ormocer-based composites depicted the lowest amount of microleakage in pursuit by the nanofilled resins and the bulk-fill composites., Clinical Significance: Core build-up is an important requirement as the remaining tooth substance following root canal treatment reduces and needs reinforcement with core build-up to sustain the tooth structure and provide resistance. A vital mandate for enduring efficiency of the restoration in the mouth is high-quality adhesive bond of these agents to cavity walls with diminished microleakage.

Context: Pediatric hydrocephalus (PH) results in significant clinical and psychosocial morbidity in pediatric population., Aims: The aims of the study are to evaluate clinical, surgical, and outcome perspective of PH patients of age <12 years., Settings and Design: This is a retrospective cohort study., Materials and Methods: This study includes 117 pediatric patients (age ≤12 years) of hydrocephalus due to various etiology admitted in our department between September 2018 and December 2020. Demographic profile, etiology, clinical presentation, management, complications and postoperative outcome characteristics were evaluated. Survival analysis was done with respect to etiology and age group., Statistical Analysis Used: P < 0.05 was considered statistically significant. Unpaired t -test and Chi-square test were used. Kaplan-Meier curve plotting and survival analysis were also done., Results: Male-to-female ratio was 1.3:1. Most frequent etiology of PH was postinfectious (35%). Posterior fossa pilocytic astrocytoma (34.2%) was the most common neoplastic etiology. Surgical procedure performed for PH was ventriculoperitoneal shunting ( n = 103), Ommaya reservoir ( n = 2) placement, and endoscopic third ventriculostomy (ETV) ( n = 8). Mortality was significantly ( P = 0.0139) more in patients of neoplastic etiology. Cognitive deficits and delayed developmental milestones were significantly ( P < 0.05) more in congenital hydrocephalus etiology. There was a nonsignificant difference in survival between age groups ( P = 0.1971). However, a significant survival difference was evident ( P = 0.0098) for etiology., Conclusions: Disease-specific mortality is main cause of mortality in PH. Neoplastic etiology PH has poor survival when compared to others. Objective: To determine the frequency of multiple spinal CSF leaks in a recent group of patients with spontaneous intracranial hypotension (SIH) who were investigated with digital subtraction myelography (DSM)., Methods: This observational study was conducted using data from a prospectively maintained data base of patients who meet the International Classification of Headache Disorders, third edition, criteria for SIH. The patient population consisted of a consecutive group of 745 patients with SIH who underwent DSM between March 2009 and February 2020. Based on the results of DSM, participants were classified according to the type and number of spinal CSF leaks., Results: Among 398 patients with SIH and extradural CSF on spinal imaging, multiplicity of CSF leaks was observed in none of 291 patients with type 1a ventral leaks and in 4 (6.2%) of 65 patients with type 1b (postero-) lateral leaks. Among 97 patients with SIH from spinal CSF-venous fistulas (type 3 leaks) who did not have extradural CSF on spinal imaging, 9 patients (9.3%) had multiple fistulas ( p < 0.0001 for comparison between groups). Type 3 and type 1a or 1b CSF leaks coexisted in an additional 5 patients., Conclusions: Among patients with SIH, multiplicity of CSF leaks was observed radiographically in none of the patients with ventral leaks, in 6% of patients with lateral leaks, and in 9% of patients with CSF-venous fistulas. These results suggest that patients with SIH can be reassured that the occurrence of multiple CSF leaks is negligible to uncommon at most, depending on the type of CSF leak., (© 2021 American Academy of Neurology.)

Background Context: The North American Spine Society's (NASS) Evidence Based Clinical Guideline for the Diagnosis and Treatment of Low Back Pain features evidence-based recommendations for diagnosing and treating adult patients with nonspecific low back pain. The guideline is intended to reflect contemporary treatment concepts for nonspecific low back pain as reflected in the highest quality clinical literature available on this subject as of February 2016., Purpose: The purpose of the guideline is to provide an evidence-based educational tool to assist spine specialists when making clinical decisions for adult patients with nonspecific low back pain. This article provides a brief summary of the evidence-based guideline recommendations for diagnosing and treating patients with this condition., Study Design: This is a guideline summary review., Methods: This guideline is the product of the Low Back Pain Work Group of NASS' Evidence-Based Clinical Guideline Development Committee. The methods used to develop this guideline are detailed in the complete guideline and technical report available on the NASS website. In brief, a multidisciplinary work group of spine care specialists convened to identify clinical questions to address in the guideline. The literature search strategy was developed in consultation with medical librarians. Upon completion of the systematic literature search, evidence relevant to the clinical questions posed in the guideline was reviewed. Work group members utilized NASS evidentiary table templates to summarize study conclusions, identify study strengths and weaknesses, and assign levels of evidence. Work group members participated in webcasts and in-person recommendation meetings to update and formulate evidence-based recommendations and incorporate expert opinion when necessary. The draft guideline was submitted to an internal and external peer review process and ultimately approved by the NASS Board of Directors., Results: Eighty-two clinical questions were addressed, and the answers are summarized in this article. The respective recommendations were graded according to the levels of evidence of the supporting literature., Conclusions: The evidence-based clinical guideline has been created using techniques of evidence-based medicine and best available evidence to aid practitioners in the diagnosis and treatment of adult patients with nonspecific low back pain. Imaging plays a multifaceted role in the diagnosis and characterization of head and neck oncological patients and is integral to their care. Given the complexity of treatment, a multimodality approach is often necessary. With the advent of new technologies, imaging can also be used to predict tumor behavior and treatment response. In this chapter, with selected case examples, we describe the various imaging modalities available and offer suggestions on their utilization.

Low-layered, transparent graphene is accessible by a chemical vapor deposition (CVD) technique on a Ni-catalyst layer, which is deposited on a <100> silicon substrate. The number of graphene layers on the substrate is controlled by the grain boundaries in the Ni-catalyst layer and can be studied by micro Raman analysis. Electrical studies showed a sheet resistance (R(sheet)) of approximately 1435 Ω per □, a contact resistance (R(c)) of about 127 Ω, and a specific contact resistance (R(sc)) of approximately 2.8×10(-4)  Ω cm(2) for the CVD graphene samples. Transistor output characteristics for the graphene sample demonstrated linear current/voltage behavior. A current versus voltage (I(ds)-V(ds)) plot clearly indicates a p-conducting characteristic of the synthesized graphene. Gas-sensor measurements revealed a high sensor activity of the low-layer graphene material towards H(2) and CO. At 300 °C, a sensor response of approximately 29 towards low H(2) concentrations (1 vol %) was observed, which is by a factor of four higher than recently reported., (Copyright © 2012 WILEY-VCH Verlag GmbH & Co. Tumor size is often determined from computed tomography (CT) images to assess disease progression. A study was conducted to demonstrate the advantages of the fuzzy C-means (FCM) algorithm for volumetric analysis of colorectal liver metastases in comparison with manual contouring. Intra-and interobserver variability was assessed for manual contouring and the FCM algorithm in a study involving contrast-enhanced helical CT images of 43 hypoattenuating liver lesions from 15 patients with a history of colorectal cancer. Measurement accuracy and interscan variability of the FCM and manual methods were assessed in a phantom study using paraffin pseudotumors. In the clinical imaging study, intra-and interobserver variability was reduced using the FCM algorithm as compared with manual contouring (P = 0.0070 and P = 0.0019, respectively). Accuracy of the measurement of the pseudotumor volume was improved using the FCM method as compared with the manual method (P = 0.047). Interscan variability of the pseudotumor volumes was measured using the FCM method as compared with the manual method (P = 0.04). The FCM algorithm volume was highly correlated with the manual contouring volume (r = 0.9997). Finally, the shorter time spent in calculating tumor volume using the FCM method versus the manual contouring method was marginally statistically significant (P = 0.080). These results suggest that the FCM algorithm has substantial advantages over manual contouring for volumetric measurement of colorectal liver metastases from CT.
