Background: Iran is a country with very high incidences of stomach cancer, especially in Northern parts. Here we assessed prognostic value of serum screening biomarkers among people >50 years old for early detection of precancerous lesions in a hot spot for gastric carcinoma in Guilan Province, North Iran., Methods: A cross- sectional population-based survey was conducted on 1,390 residents of Lashtenasha city with the mean age (SD) of 61.8 (9.02) years old (50.8% females) to assess the association of gastrin and the pepsinogen (PG) I/II ratio with premalignant gastric lesions. Blood samples were taken for CBC, blood group, and serologic exams (PGI, PGII, and gastrin 17) from each subject. Expert gastroenterologists performed upper GI endoscopy and ROC curves were generated to determine appropriate cutoff points., Results: Mean values of PGI, PGII, PGI/PGII and gastrin were significantly different between patients with and without atrophy or metaplasia (P<0.05). To diagnose atrophy and intestinal metaplasia, a significantly higher AUC was observed for the PGI/PGII ratio (70 and 72%, respectively) compared to the PGI (56, 55%), PGII (63, 64%) and gastrin (59, 61%) (all p<0.001)., Conclusions: Biomarker tests such as the PGI/II ratio can be used in the screening and diagnosis of subjects at high gastric cancer risk in our region.

The aim of this study was to evaluate the influence of using Citric Acid on false negative rates induced by PPIs during (14)C-UBT in dyspeptic patients with H. pylori infection. In a crossover randomized controlled clinical trial, one hundred dyspeptic patients (46 females and 54 males) with determined H. pylori infection who referred to gastrointestinal Outpatient Clinic of Razi Hospital-Rasht, Iran. All the patients underwent a (14)C-UBT HELIPROBE(®) baseline test and the positive ones entered the second phase. They were divided randomly to two groups and started PPIs treatment by Pantoprazole (20 mg/daily Pantozol(®) Nycomed Company) and underwent two other UBTs in days 12-13 and 14-15 with and without 4 grams Citric Acid. In group I who underwent UBT with Citric Acid in days 12-13 (UBT2) and UBT without Citric Acid in days 14-15 (UBT3), there was no significant difference between the UBT results with and without Citric Acid (P≥0.05). In group II who underwent UBT without Citric Acid in days 12-13 and UBT with Citric Acid in days 14-15, the false negative rates were not significantly different between UBT with and without Citric Acid (P≥0.05). In both groups the results of UBT without Citric Acid after Pantoprazole consumption didn't change significantly compared to the baseline UBT. Also the difference between the false negative rates of baseline UBT and UBT without citric acid was not significant (P>0.05). These results suggest that acidification of gastric environment during (14)C-UBT cannot prevent false negative results and do not increase the accuracy of the test in patients taking PPIs.

Background: It is generally accepted that gastric carcinomas are preceded by a sequential multistage process that includes chronic gastritis, gastric atrophy, usually with intestinal metaplasia (IM), and dysplasia. This series of changes in gastric carcinogenesis is often initiated by Helicobacter pylori (H pylori) infection. The aim of the present study was determination of gastric histopathologic changes in IM patients after at least one year in Guilan province, Iran., Materials and Methods: This case-series study was conducted in Guilan Gastrointestinal and Liver Disease Research Center (GLDRC) during 2010 to 2011. Gastric biopsy was performed for all 71 known cases of IM and precanceric lesions including gastric atrophy, IM, dysplasia and H pylori infection were determined after at least one year., Results: Of the total of 71 patients with established IM who were enrolled, 50 had complete-type IM and 21 had incomplete-type IM. Fifty two people had H pylori infection. H pylori eradication was achieved in 39 patients (75%). Secondary pathology findings of patients with IM were complete metaplasia (39.4%), incomplete metaplasia (32.4%), dysplasia (23.9%) and other precanceric lesions (4.2%). Dysplasia (20%vs 33%) occurred in patients who had complete and incomplete IM at baseline respectively (p>0.05). Age, gender, family history of gastric cancer(GC); smoking habits and NSAIDs use were not associated with gastric premalignant lesions in initial and secondary pathologies (p>0.05). The difference became statistically significant between H pylori infection in patients with more than 3 years diagnostic intervals (p<0.05). Statistical difference between eradicators and non-eradicators was not significant., Conclusions: We found that incomplete IM increased the risk of subsequent dysplasia in this study.

Objective: Thyroid hormones profile in patients with hepatic cirrhosis due to chronic HBV and HCV infections was evaluated in order to find any relationship between thyroid hormones and severity of liver damage., Material and Methods: Patients with the diagnosis of hepatic cirrhosis due to hepatitis B or C were screened for thyroid function status. Child-Pugh and model for end-stage liver disease (MELD) scores were calculated. Considering each thyroid function test, patients were divided into two groups with lower than normal and normal range of thyroid hormones, separately for each (for TSH, normal and upper than nor- mal). The correlation between thyroid function tests and severity of liver disease was taken into account., Results: Number of patients with a T3 level lower than normal range (70-110 ng/dL) significantly increased along with Child-Pugh scores A, B and C. A negative correlation was found between Child-Pugh scores and total serum T3 level (r = -0.453, P < 0.001). Also a reverse correlation was observed between MELD score and T3 levels (r = -0.305, P = 0.14)., Conclusion: In conclusion serum T3 concentration is a good index of hepatic function, decreasing by the severity of liver damage.

Aim: To study knowledge levels and attitudes of health care providers toward patients with hepatitis C virus infection in Guilan, a northern province of Iran., Methods: This cross-sectional study was performed on 239 health care professionals from the Razi Hospital, including doctors, nurses, and operating room technicians. The questionnaires consisted of questions on demographic characteristics, knowledge levels, and attitudes toward hepatitis C patients. The questionnaire was tested in a pilot study and validated by Cronbach's alpha coefficient. Data were analyzed using SPSS16 software., Results: The mean ± SD knowledge score was 17.43 ± 2.65 (from a total of 22). 51.9% of the participants achieved scores higher than the mean. There was a significant relationship between knowledge score and age (P = 0.001), gender (P = 0.0001), occupational history (P = 0.0001), and educational history (P = 0.027). There was also a significant relationship between attitude level and age (P = 0.002), gender (P = 0.0001), occupational history (P = 0.0001), and educational history (P = 0.035). Physicians were significantly more knowledgeable and showed more positive attitudes. There was a positive correlation between knowledge and attitude scores (P = 0.02)., Conclusion: Discriminatory attitudes are common among health care providers toward hepatitis C patients. It is therefore necessary to improve their knowledge level and attitude toward this disease.

Hemodialysis (HD) patients are vulnerable to transfusion-transmitted infections such as hepatitis B virus (HBV). HBV infection with undetectable hepatitis B surface antigens (HBsAg) is described as occult HBV and can lead to serious complications. The aim of this study was to evaluate the prevalence of occult HBV and concomitant factors in HD patients. Using a cross-sectional design, clinical and epidemiological data were obtained from May to September 2009 in 11 different HD units in Guilan province in northern Iran. After serological testing for HBV surface antigens in 514 HD patients using a third-generation enzyme-linked immunosorbent assay kit (Diapro, Milano, Italy), HBsAg-negative patients were tested for HBV DNA using a Qiagen PCR kit (Artus Qiagen GmbH, Hilden, Germany). After omission of seven HBsAg-positive patients, 507 patients were included in the study, 280 (55.2%) of whom were male and 227 (44.8%) were female. Patients ranged in age from 16 to 66 years (mean 53.2 years). No HBV DNA was detected in HBsAg-negative patients. Some 59 patients (11.6%) were anti-hepatitis C virus positive and 32 (6.3%) were hepatitis C virus positive according to polymerase chain reaction. The study results indicate that occult HBV infection is not a significant health problem in HD patients in Guilan province.

Background: Psychiatric disorders are common in irritable bowel syndrome (IBS) patients. The prevalence of psychiatric disorders in IBS patients varies in different cultures. We conducted this study to determine the prevalence of psychiatric disorders, Methods: In a cross-sectional study, 256 IBS patients were selected (using the criteria of Rome III) and evaluated for psychiatric disorders. In the first phase, subjects were screened using the General Health Questionnaire 28 (GHQ28). In the second phase, those who had scores ≥ 23 were assessed through semi-structured psychiatric interviews., Results: Thirty out of 256 subjects had no significant psychiatric symptoms after performing GHQ28. In further psychiatric evaluation of the remaining subjects (226) who suffered from some degree of a psychiatric problem, 36 were diagnosed without Anxiety/Depressive disorder. Thus 66 subjects (25.8%) were known as a group without any significant psychiatric problem. A total of 190 subjects (74.2%) with anxiety-depressive problems were diagnosed; 89 were suffering from pure anxiety disorders, 41 were suffering from depressive disorders and 60 had co-morbid anxiety-depressive disorders. When comparing anxiety-depressive patients (n = 190) with normal subjects (n = 66), gender (P = 0.016), occupation (P = 0.002) and intensity of IBS (P < 0.001) showed statistically significant differences., Conclusion: The high prevalence of anxiety-depressive disorders in this study indicates the necessity of psychiatric assessment, early diagnosis and treatment of the patients with IBS. It may improve management of the patients suffering from IBS.

Background and Objectives: Gastric cancer is a leading cause of cancer-related deaths in both sexes in Iran. This study was designed to assess upper GI endoscopic findings among people>50 years targeted in a mass screening program in a hot-point region., Methods: Based on the pilot results in Guilan Cancer Registry study(GCRS), one of the high point regions for GC - Lashtenesha - was selected. The target population was called mainly using two methods: in rural regions, by house-house direct referral and in urban areas using public media. Upper GI endoscopy was performed by trained endoscopists. All participants underwent biopsies for rapid urea test (RUT) from the antrum and also further biopsies from five defined points of stomach for detection of precancerous lesions. In cases of visible gross lesions, more diagnostic biopsies were taken and submitted for histopathologic evaluation., Results: Of 1,394 initial participants, finally 1,382 persons (702 women, 680 men) with a mean age of 61.7 ± 9.0 years (range:50-87 years) underwent upper GI endoscopy. H.pylori infection based on the RUT was positive in 66.6%. Gastric adenocarcinoma and squamous cell carcinoma of esophagus were detected in seven (0.5%) and one(0.07%) persons, respectively. A remarkable proportion of studied participants were found to have esophageal hiatal hernia(38.4%). Asymptomatic gastric masses found in 1.1% (15) of cases which were mostly located in antrum (33.3%), cardia (20.0%) and prepyloric area (20.0%). Gastric and duodenal ulcers were found in 5.9% (82) and 6.9% (96) of the screened population., Conclusion: Upper endoscopy screening is an effective technique for early detection of GC especially in high risk populations. Further studies are required to evaluate cost effectiveness, cost benefit and mortality and morbidity of this method among high and moderate risk population before recommending this method for GC surveillance program at the national level.

Background and Objectives: The most northern and northwestern regions of Iran are at a high risk for gastric cancer. The aim of this study was to assess the general population's awareness about risk factors, symptoms and signs, preventive methods and management of gastric carcinoma in a high prevalence city in the North of Iran., Methods: A cross-sectional population-based telephone survey which was conducted on 3,457 residents of Rasht, the capital city of Guilan Province, to assess their awareness regarding gastric carcinoma. The questionnaires contained demographic data and statements on respondents' knowledge about risk factors, symptoms and signs, prevention and management of gastric cancer which were filled by general practitioners after asking the subjects. Data were analyzed in SPSS14. P<0.05 was considered significant., Results: The mean knowledge score of the respondents was 5.05±1.37 regarding risk factors of gastric carcinoma, 4.39±1.99 regarding symptoms and signs, 6.0±1.22 regarding preventive strategies, and 1.6±1.16 regarding management. Totally the mean knowledge level of the respondents toward gastric carcinoma would be 17.1±3.97 from the maximum grade of 29. The age group of 45-55 y/o, bachelor degree and higher, physicians and nurses, those who had cancer history in friends or had the history of gastrointestinal diseases showed significantly higher knowledge scores (P=0.001)., Conclusion: There is a general lack of awareness of cancer risk factors, symptoms and signs, methods of prevention, and importance of early diagnosis and treatment. Educational programs should be developed to promote adherence to recommended screening guidelines.

Background: Northern Iran counts as one of the highest prevalence regions for esophageal cancer (EC) worldwide. This study was designed to assess the epidemiologic aspects of EC in north central and northwest Iran over a 10 year period., Materials and Methods: The Guilan cancer registry study (GCRS) is a population- based cancer registry study featuring retrospective (1996-2003) and prospective (2004-2005) phases. A detailed questionnaire based on WHO standards for cancer registratration was applied to gather the required information. Two trained physicians coded information using ICD-O-3 in close coordination with an expert pathologist., Results: A total of 19,936 cases of malignancy (mean age 55.4±18.0 years, range: 1-98 years) were registered, including 1,147 cases (670 males, 447 female; mean age: 64.0±11.5 years) of EC. In 1996 the male/female ratio among patients with EC was 1.25 which increased to 1.53 in 2005. The lower third of the esophagus still remained the most common site of tumors. The average age-standardized rate (ASR) was 6.9 and 4.1 per 105 men and women, respectively. In 1996, the ASRs were 7.2 and 5.2 per 105 men and women which decreased to 6.9 and 4.1 per 105 in 2004-2005. Squamous cell carcinoma (SCC) was the most prevalent histological subtype of EC accounting over 80% of cases., Conclusions: However the prevalence of adenocarcinoma (ADC) showed an increase to 18.4%. Guilan province may be considered a relatively low incidence region for EC.

Background. Celiac disease (CD) is closely associated with other autoimmune endocrine disorders, particularly autoimmune thyroid disease. The aim of this study was to find the frequency of celiac disease in patients with hypothyroidism in Guilan province, north of Iran. Methods. A total of 454 consecutive patients with hypothyroidism underwent celiac serological tests antiGliadin antibodies (AGA), antitissue transglutaminase antibodies (IgA-tTG) and antiendomysial antibodies (EMA-IgA). Small intestinal biopsy was performed when any of celiac serological tests was positive. Results. Eleven (2.4%) patients were positive for celiac serology, and two patients with documented villous atrophy were diagnosed with classic CD (0.4%; 95%). Two patients with classic CD had Hashimoto's thyroiditis (HT) (0.6%; 95%). Six (54.5%) of 11 were suffering from overt hypothyroidism and 45.5% from subclinical hypothyroidism. Six (54.5%) had HT, and 45.5% had nonautoimmune hypothyroidism. Conclusions. In this study, prevalence of CD was lower than other studies. Most of the patients with CD were suffering from HT, but there was no significant statistical relation between CD and HT.

Background and Objectives: In patients with gastric cancer, the most frequently reported family history of cancer also involves the stomach. The aim of this study was to assess the presence of gastric precancerous lesions in first-degree relatives of patients with gastric cancer and to compare the obtained results with those of individuals with no such family history., Methods: Between 2007 and 2009, 503 consecutive persons more than 30 years old were enrolled in the study covering siblings, parents or children of patients with confirmed adenocarcinoma of stomach. The control group was made up of 592 patients who were synchronously undergoing upper gastrointestinal endoscopy for evaluation of dyspepsia without gastric cancer or any family history. All subjects were endoscopically examined., Results: The overall prevalence of Helicobacter pylori was 77.7% in the cancer relatives and in 75.7% in the control group. Chronic gastritis was found in 90.4% vs. 81.1% (P<0.001). Regarding histological findings, 37(7.4%) of the study group had atrophy vs. 12(1.7%) in the control group (P<0.001), while no difference was observed for intestinal metaplasia (20.3%vs. 21.6%, P=0.58). Dysplasia were shown in 4% of cancer relatives but only 0.4% of the control group (P<0.001). There was no gender specificity., Conclusions: Findings of our study point to great importance of screening in relatives of gastric cancer patients in Iran.

Unlabelled: Colonoscopy is a procedure that used largely, in assessment, screening & management of lower gastrointestinal disease. Some of these diseases that assessed are colorectal cancer, polyps & inflammatory bowel disease. The goal of this study is determining of frequency of lower GI disease among patient who have elective colonoscopy. This cross-sectional study was conducted retrospective. Samples collected from the census method of colonoscopy patients in Razi hospital, from March 21th in 2006 to March 20th in 2009. Patients' data including age, sex, location and type of lesion found by colonoscopy and pathology diagnosis were collected and at the end necessary information collected into the SPSS software (version 16) for analyzing with the help of statistical tests Chi-square. Significant findings: Of 1398 patients, 683 patients (48/8 %) were male and 715 (51/2 %) were female 494 patients (35/3%) had normal results. Hemorrhoids (22/6 %) and polyps (14/8 %) were the most abundant lesions discovered by colonoscopy. The most common site of lesions among patients with abnormal colonoscopy findings, were anal canal and anus (43/8 %) and rectosigmoid (26%). The findings in both males and females, revealed no statistical differences., Conclusion: Colonoscopic detection of hemorrhoids is a common finding in patient undergone colonoscopy. The most frequent site for lesions which found in anal canal and anus. In addition to, normal findings had a high rate and it indicates that unnecessary colonoscopy have been required by physicians.

BACKGROUND This study aims to elucidate the knowledge and practice of general practitioners (GP) and internists regarding diagnosis and treatment of Helicobacter pylori (H. pylori ) infection in a high prevalent area, with the intent to assist with future educational strategies for H. pylori infection. METHODS In this cross-sectional study in Guilan, a Northern Province of Iran, all GPs and internists in the city of Rasht were included. Questionnaires consisted of questions on demographic characteristics in addition to physicians' knowledge and practice regarding H. pylori infection. The questionnaire was verified by a panel of experts, validated by the test-retest method, and distributed among participants. Scores higher than the mean score indicated good knowledge or attitude. Those lower than the mean score indicated poor knowledge and attitude. Data were collected and analyzed by SPSS version 14 software. RESULTS The mean (SD) knowledge and practice score of physicians was 12.1±3.13 and 2.37±1.54, respectively. Overall, 67.9% of GPs and 91.7% of internists exhibited good knowledge scores, while 72.4% of GPs and 95.8% of internists showed good practice. Physicians who used books or educational programs and had working histories of less than 10 years scored significantly higher in terms of mean knowledge. The mean practice score of physicians who worked in public units and had working histories of more than 5 years and those who had used books or educational programs was significantly higher. CONCLUSION Since H. pylori infection is prevalent in Iran and GPs' practices are directly under the influence of knowledge, it is necessary to attempt to increase the level and quality of information among GPs by educational and Continuing Medical Education programs and seminars.

Background: The purpose of the present study is to investigate the association of nonalcoholic fatty liver disease (NAFLD) with the doppler waveform pattern of hepatic veins and portal vein doppler indices., Objectives: This assay may be useful in evaluating the natural course of NAFLD and monitor treatment efficacy on follow-up., Patients and Methods: This case control study was performed in 31 patients with NAFLD and 31 normal healthy adults who served as the control group. The patients presented with elevated liver enzymes levels (ALT/AST) and hyperechogenic livers in the B-mode ultrasonography examination. Eleven patients had a liver biopsy. After an 8-hour fast,B-mode and duplex doppler ultrasonography were performed, and the waveform patterns of the right hepatic vein, portal vein diameter, grade of fatty liver, portal vein pulsatility index (VPI), and mean flow velocity (MFV) were measured., Results: VPI and MFV values were 0.42 ± 0.92 and 17.27 ± 5.34 cm/second, respectively, in the control group and 0.25 ± 0.50 and 12.82 ± 4.32 cm/second in patients with NAFLD (P< 0.01). The frequency of abnormal hepatic vein doppler waveform patterns (biphasic or monophasic) was significantly higher in patients with NAFLD (55.2%) versus control subjects (3.2%) (P < 0.001). There was no correlation between the degree of fat infiltration and VPI (P = 0.714), MFV (P = 0.911), or hepatic vein waveform pattern (P = 0.197). We found no correlation between liver enzyme levels and MFV or VPI. However, the rate of abnormal hepatic vein was higher in patients with enzyme levels that exceeded twice the normal value (P = 0.05)., Conclusions: Patients with NAFLD have a high rate of abnormal hepatic vein doppler waveform patterns, and decreased VPI and MFV are suggestive of reduced vascular compliance in the liver. Elevated liver enzymes levels do not influence VPI or MFV, but patients with abnormal enzymes have higher rates of abnormal hepatic vein doppler waveform patterns.

Background: Hepatitis C virus (HCV) and hepatitis B virus (HBV) infection are especially problematic in patients with end-stage renal disease who are undergoing hemodialysis (HD)., Objectives: To determine the prevalence of HCV and HBV infection in HD population in Guilan, north of Iran., Patients and Methods: In a cross-sectional study, from May to September 2009, in 11 different hemodialysis units in Guilan province, North of Iran, clinical data such as age, gender, duration of dialysis, HBsAg and anti-HCV antibody of 514 HD patients were recorded. Patients with positive antibodies against HCV were tested for HCV RNA., Results: From 514 patients, 286 (55.64%) were male. 61 (11.9%) patients were anti-HCV-positive and 31 (50.8%) were HCV PCR-positive. There was significant relationship between HCV Ab-positivity with gender and HD duration (p < 0.05). There was significant difference between the mean HD duration in anti-HCV-positive and anti-HCV-negative patients (p < 0.05). Also, significant relationship was found between HCV RNA-positivity with gender and HD duration (p < 0.05). Seven (1.4%) patients were positive for HBsAg. Two (0.38 %) were found positive for both HBsAg and anti-HCV antibody., Conclusions: There is low a prevalence of HCV and HBV in HD patients in our region. The rate can be decreased by HBV vaccination of end-stage renal disease patients before setting chronic HD, antiviral treatment and isolation of infected individuals.

Background. We encountered repeatedly, in our clinical practice, discordant results between UBT and histopathology about H. pylori infection. Goal. To study the diagnostic accuracy of Heliprobe (14)C-urea breath test ((14)C-UBT) for detection of H. pylori infection in an Iranian population. Study. We enrolled 125 dyspeptic patients in our study. All of them underwent gastroscopy, and four gastric biopsies (three from the antrum and one from the corpus) were obtained. One of the antral biopsies was utilized for a rapid urease test (RUT), and three others were evaluated under microscopic examination. Sera from all patients were investigated for the presence of H. pylori IgG antibodies. The (14)C-UBT was performed on all subjects using Heliprobe kit, and results were analyzed against the following gold standard (GS): H. pylori infection considered positive when any two of three diagnostic methods (histopathology, RUT, serology) are positive. Results. According to data analysis, the Heliprobe (14)C-UBT had 94% sensitivity, 100% specificity, 93% negative predictive value (NPV), 100% positive predictive value (PPV), and 97% accuracy, compared with GS. Conclusion. The Heliprobe (14)C-UBT is an easy-to-perform, rapid-response, and accurate test for H. pylori diagnosis, suitable for office use.

Background: The aim was to investigate the H. pylori recurrence rate one year after successful eradication in dyspeptic patients in Guilan province, northern Iran., Material/methods: Patients with chronic dyspepsia were enrolled in our gastroenterology clinic. Gastric biopsies were obtained by endoscopy and H. pylori infection was diagnosed using the Rapid Urease Test (RUT). Patients with H. pylori infection were prescribed clarithromycine-based quadruple therapy for 10 days. The (13)C urea breath test ((13)C-UBT) was used to assess H. pylori status 10 weeks after completing H. pylori therapy. Patients who H. pylori-negative had a second (13)C-UBT after 1 year to establish the recurrence rate., Results: Of 269 dyspeptic patients with RUT positivity for H. pylori in endoscopy who were treated with the quadruple regimen for 10 days, 240 had negative (13)C-UBT 10 weeks after the end of therapy, with an eradication rate of 89.2% (95%CI: 82.2-97.2%) by per-protocol analysis. Of this group, 153 patients (63.8%) were female. The (13)C-UBT at 1 year was positive in 12 (5.0%) of the 240 patients. A mean age of >35 year, being female, living in an urban area, and a household size of more than 5 person were more frequent in the (13)C-UBT-positive patients at 1 year, but they were not statistically significant. In this study there was also no significant relationship between, job, educational status, and the source of domestic water with recurrence of H. pylori infection., Conclusions: According to these data, the H. pylori recurrence rate in our region is 5% after successful eradication.

Background: Chronic hepatitis B virus (HBV) infection remains a major health problem. The aim of this study was to determine the serum HBV DNA levels in HBeAg-negative HBV patients and look for a relationship between serum HBV DNA level and liver histology., Material/methods: In a cross-sectional study, 70 patients with positive serum hepatitis B surface antigen (HBsAg) and normal ALT for at least 6 months were enrolled. Quantification of HBV DNA was performed by real-time PCR. Liver biopsy specimens were performed for grading and staging of chronic hepatitis., Results: Fifty-four patients (34 males, 20 females) were included. Mean + or - SD serum HBV DNA level was 282,280.46 + or - 1,474,295 copies/ml, fibrosis (0-6) 2.37 + or - 1.263, necroinflammation (0-18) 0.33 + or - 0.476, and BMI 26.65 + or - 4.9. The mean serum HBV DNA level had significant differences between grade <4 and grade > or = 4 cases (P<0.05). The relationship between serum HBV DNA level and liver grade was confirmed by the Kendall test (P<0.05). No significant relationship between serum HBV DNA level and liver histological stage, gender, age, BMI, or HBeAg was observed in these patients (P>0.05)., Conclusions: It is advantageous to measure serum HBV DNA level quantitatively in patients who are inactive carries of hepatitis B. If they have HBV DNA levels > or = 104 copies/ml, it will be necessary to perform a liver biopsy and apply therapy accordingly.

Objective: To evaluate the correlation between anti CagA antibody (Ab) and presence of gastric cancer., Methods: In a descriptive cross-sectional study during October 2003 to October 2005, in the Gastrointestinal Subspeciality Center in Razi Hospital, Rasht, Iran, we assessed anti-Helicobacter Ab immunoglobulin G (IgG) and anti CagA Ab IgG by the enzyme-linked immunosorbent assay (ELISA) method in 52 patients with gastric adenocarcinoma and 57 patients with nonulceric dyspepsia (NUD)., Results: Among 52 patients with gastric cancer, anti-Helicobacter pylori Ab was negative in 13 (25%) and positive in 39 persons (75%). Among 57 patients with NUD, anti-Helicobacter pylori Ab was negative in 5 (8.5%) and positive in 52 patients (91.2%) (p=0.043). This significant difference did not remain after logistic regression for adjustment of confounders (p=0.068). The CagA Ab was positive in 22 (42.3%) patients with cancer and 32 (56.1%) of the NUD group. There was no difference in this regard before (p=0.212) and after (p=0.131) logistic regression analysis., Conclusion: Anti CagA Ab does not have a significant value as an independent predictive factor in gastric adenocarcinoma.

Aim: Mothers' understanding and co-operation is an important factor supporting primary prevention and secondary prophylaxis for controlling rheumatic fever and rheumatic heart disease. We sought to determine mothers' knowledge about different aspects of rheumatic fever., Method: Participation of 500 mothers referred to healthcare centers in Guilan province was sought for the study which used a questionnaire to evaluate their knowledge about five different aspects of rheumatic fever., Results: Among the 443 respondents, good knowledge about treatment was high (86 per cent); fewer mothers had a good level of knowledge about other aspects of treatment: epidemiology 34 per cent, symptoms 4 per cent, route of infection 27 per cent and complications 10.5 per cent. More educated women had significantly better knowledge about rheumatic fever., Conclusion: Expanded health education is required to improve knowledge about this condition among the population.
