Wildfire emissions are challenging to measure and model, but simple and realistic estimates can benefit multiple disciplines. We evaluate the potential of MODIS (Moderate Resolution Imaging Spectroradiometer) data to address this objective. A total of 11004 fire pixels detected over 92 days were clustered into 242 discrete fire events in a mountainous region of North America. Burned areas were estimated with spatial buffers around the MODIS detections, and all events were matched and compared with administrative fire records based on their location and duration. Linear regression between recorded and estimated burned areas showed excellent agreement (slope=0.93 and R2=0.96). Aerosol emission rates were estimated for each MODIS detection using its fire radiative power measurement. Results were compared with estimates from the Canadian Fire Behaviour (CANFB) prediction system in Canada and the US Emissions Production Model (USEPM) for detections in the US. Median emission rates were similar for the MODIS and CANFB methods (600 and 579gs-1respectively) but not for the MODIS and USEPM methods (575 and 382gs-1respectively). The MODIS rates were much more variable in both comparisons. Linear regression on emission rates summed daily across the study area shows that the MODIS method is more consistent with CANFB (slope=0.71, R2=0.71) than with USEPM (slope=0.24, R2=0.68). We conclude that simple calculations based on remote sensing data can yield results that are comparable with those obtained with more complex methods. [ABSTRACT FROM AUTHOR]

Laboratory studies evaluated the performance of a combined gas and particle sampling instrument. Results of these investigations indicated that the system had the ability to reliably sample atmospheric gases. The sampler consists of a glass inlet/impactor, two Na 2 CO 3 -coated annular denuders to collect gaseous SO 2 , HNO 3 and HNO 2 , a citric acid-coated annular denuder to trap NH 3 , and a filter pack to collect fine particles and artifact gases. The inlet walls may be extracted to measure the extent of gas deposition on the inlet surfaces. Inlet transmission efficiencies and denuder gas collection efficiencies indicated excellent performance under a wide range of simulated conditions. Laboratory experiments with SO 2 , HNO 3 and NH 3 at various concentrations and relative humidities (10−90%) indicated that the annular denuders achieve collection efficiencies near 100%. Tests to investigate the deposition of SO 2 , HNO 3 and NH 3 on the walls of the glass inlet showed a small loss ( 3 and NH 3 . No loss of SO 2 on inlet walls was detected. By extracting the inlet walls the deposited HNO 3 and NH 3 can be recovered.

Effects of ozone exposure on outdoor farm workers in the Fraser Valley of British Columbia, Canada, were investigated. Fifty-eight workers underwent spirometry daily before and after each 8- to 14-h workday from June 23 to August 26, 1993. The mean daily maximum (1 h) ambient ozone concentration was 40 ppb (range: 13 to 84 ppb). Concentrations of acid aerosols and fine particulates, potential confounders of ozone effects, were very low. In individual regressions of evening FEV1 and FVC on maximum daily ozone concentration, 47 of 53 workers with valid data (46 of 53 for FVC) had negative slopes. The average slopes (weighted by the inverse SE of the regression coefficients) were -3.3 and -4.7 ml for FEV1 and FVC, respectively, for each ppb increase in ozone (p < 0.001). Following correction for an individual's mean lung function level, date, and temperature, regression of either the afternoon or the daily change (afternoon-morning) corrected for the morning measurement of FEV1 and FVC on ozone showed similar magnitudes of effect. These associations were still apparent on the following morning, suggesting a persistent ozone effect. These results indicate that exposure of a population of outdoor workers to ambient ozone concentrations below 85 ppb is associated with decreased lung function over the day, which persists to the following day.

Nitrogen dioxide (NO2) concentrations in an ice arena were evaluated before and after a three-way catalytic converter and fuel management system were retrofitted to a 7-year-old ice resurfacing machine. Continuous monitoring with a chemiluminescence NO2 analyzer for 68 and 71 consecutive hours before and after the retrofit indicated an 87% reduction in airborne NO2 concentrations. Simultaneous multiple location sampling with Palmes tube diffusive samplers confirmed this reduction following the retrofit. In the ice arena used for testing, the average NO2 concentration was reduced from 1.9 to 0.25 ppm. In addition, carbon monoxide (CO) levels were reduced by 57% from 14 to 6 ppm. These findings indicate that there is a practical and affordable solution to reduce N02 and CO concentrations in ice skating facilities that can be used with other measures to improve air quality.

OBJECTIVES. The common operation of fuel-powered resurfacing equipment in enclosed ice skating rinks has the potential for producing high concentrations of carbon monoxide and nitrogen dioxide. Exposures to these gaseous combustion products may adversely affect the health of those inside the rink. Little information is available on pollutant concentrations under normal operating conditions. METHODS. One-week average nitrogen dioxide concentrations in 70 northeastern US rinks were measured with passive samplers during normal winter season conditions. RESULTS. The median nitrogen dioxide level inside rinks was 180 ppb, more than 10 times higher than the median outdoor concentration. One-week average nitrogen dioxide concentrations above 1000 ppb were measured in 10% of the rinks. CONCLUSIONS. Considering that short-term peak concentrations were likely to have reached two to five times the measured 1-week averages, our results suggest that nitrogen dioxide levels were well above short-term air quality guidelines and constitute a public health concern of considerable magnitude.
611. Assessment of indoor aerosols with an integrating nephelometer

A portable nephelometer was used to assess particulate levels inside and outside of homes with indoor air impacted by biomass combustion. Simultaneous sampling with PM10 and PM2.5 inertial impactors was undertaken to determine the relationship between particle light scattering coefficient (sigma sp) and particle mass concentration. Measurements were conducted in 22 homes in rural Mexico and in 6 homes in rural British Columbia, Canada. In both areas, biomass combustion resulted in high levels of fine particulates, mainly indoors in Mexico and outdoors in British Columbia. Comparisons were made between the average of the nephelometer sigma sp (unheated nephelometer inlet stream) and the measured particle mass (PM10 and PM2.5) during a defined sampling duration. The two measurements were highly correlated (r2 values of 0.79-0.96) over a wide range of particle mass concentrations (10-1600 micrograms/m3). A stronger correlation was observed for PM2.5 than for PM10. Colocated outdoor nephelometer and impactor measurements were poorly correlated (r2 = 0.50). Substantially different relationships were observed between the Mexico and British Columbia particulate, indicating the importance of particle composition and ambient conditions as factors affecting light scattering, even indoors. However, in a given series of similar indoor environments, the relationship between particle light scattering and mass concentration is stable enough to use independent nephelometer measurements as estimates of mass concentrations. The fast-response, continuous measurement, portability and datalogging capabilities of this nephelometer, combined with its ease of use, make this an important measurement tool to be used in conjunction with traditional filter sampling in the assessment of indoor particle levels and the evaluation of sources.

Recent evidence suggests that chronic exposure to high levels of ambient particulate matter (PM) is associated with decreased pulmonary function and the development of chronic airflow obstruction. To investigate the possible role of PM-induced abnormalities in the small airways in these functional changes, we examined histologic sections from the lungs of 20 women from Mexico City, a high PM locale. All subjects were lifelong residents of Mexico City, were never-smokers, never had occupational dust exposure, and never used biomass fuel for cooking. Twenty never-smoking, non-dust-exposed subjects from Vancouver, British Columbia, Canada, a low PM region, were used as a control. By light microscopy, abnormal small airways with fibrotic walls and excess muscle, many containing visible dust, were present in the Mexico City lungs. Formal grading analysis confirmed the presence of significantly greater amounts of fibrous tissue and muscle in the walls of the airways in the Mexico City compared with the Vancouver lungs. Electron microscopic particle burden measurements on four cases from Mexico City showed that carbonaceous aggregates of ultrafine particles, aggregates likely to be combustion products, were present in the airway mucosa. We conclude that PM penetrates into and is retained in the walls of small airways, and that, even in nonsmokers, long-term exposure to high levels of ambient particulate pollutants is associated with small airway remodeling. This process may produce chronic airflow obstruction.

Nitrous acid (HONO) is formed both indirectly from the reaction of nitrogen dioxide (NO2) with water on indoor surfaces, and directly during combustion. This gaseous pollutant may be a previously unrecognized causal factor in assessments of nitrogen oxide exposure effects. The present study is the first attempt to evaluate exposure effects of HONO on the human airways and the mucous membranes of the eyes and nose. Fifteen healthy adult nonsmokers were exposed for 3.5 h in a double-blind, balanced protocol to clean air, 77, and 395 ppb HONO. Each exposure was preceded by a 1-h baseline measurement period, and exposures were separated by 1 wk. There was a 10-min exercise period during exposure. Effects measurements included assessment of bronchial reactivity, measurement of specific airway conductance, spirometry, acoustic rhinometry, nasal lavage, tear-fluid cytology, a CO2 eye- provocation test, evaluation of eye redness, and subjective sensations. Effects of HONO exposure on the eyes were found as exposure-related changes in tear-fluid cytology. In particular, the number of squamous cells increased by 20, 67, and 80% following exposure to clean air, 77, and 395 ppb HONO, respectively (p = 0.004). Possible indications of exposure effects on sensitivity to CO2 eye provocation and on specific airway conductance were also measured. For specific airway conductance there was an approximate 10% decrease in conductance following exercise in association with HONO exposure, compared with a 20% decrease with clean air (p = 0.038).

Background Understanding the shape of the relationship between long-term exposure to ambient fine particulate matter (PM2.5) concentrations and health risks is critical for health impact and risk assessment. Studies evaluating the health risks of exposure to low concentrations of PM2.5 are limited. Further, many existing studies lack individual-level information on potentially important behavioural confounding factors. Methods A prospective cohort study was conducted among a subset of participants in a cohort that linked respondents of the Canadian Community Health Survey to mortality (n = 299,500) with satellite-derived ambient PM2.5 estimates. Participants enrolled between 2000 and 2008 were followed to date of death or December 31, 2011. Cox proportional hazards models were used to estimate hazard ratios (HRs) for mortality attributed to PM2.5 exposure, adjusted for individual-level and contextual covariates, including smoking behaviour and body mass index (BMI). Results Approximately 26,300 non-accidental deaths, of which 32.5 % were due to circulatory disease and 9.1 % were due to respiratory disease, occurred during the follow-up period. Ambient PM2.5 exposures were relatively low (mean = 6.3 μg/m3), yet each 10 μg/m3 increase in exposure was associated with increased risks of non-accidental (HR = 1.26; 95 % CI: 1.19-1.34), circulatory disease (HR = 1.19; 95 % CI: 1.07–1.31), and respiratory disease mortality (HR = 1.52; 95 % CI: 1.26–1.84) in fully adjusted models. Higher hazard ratios were observed for respiratory mortality among respondents who never smoked (HR = 1.97; 95 % CI: 1.24–3.13 vs. HR = 1.45; 95 % CI: 1.17–1.79 for ever smokers), and among obese (BMI ≥ 30) respondents (HR = 1.76; 95 % CI: 1.15-2.69 vs. HR = 1.41; 95 % CI: 1.04–1.91 for normal weight respondents), though differences between groups were not statistically significant. A threshold analysis for non-accidental mortality estimated a threshold concentration of 0 μg/m3 (+95 % CI = 4.5 μg/m3). Conclusions Increased risks of non-accidental, circulatory, and respiratory mortality were observed even at very low concentrations of ambient PM2.5. HRs were generally greater than most literature values, and adjusting for behavioural covariates served to reduce HR estimates slightly. Acidic sulfate aerosols were sampled from the atmosphere with the Harvard Impactor-Honeycomb Denuder System. This sampler incorporates a citric-acid-coated denuder to remove NH 3 and an impactor with a 50% size cutoff of 2.5 μm ( d a ). Fine particles were collected on a Teflon filter. The sampling (24 h) lasted from December 1990 until June 1992. The sampling sites were in Erfurt and Sokolov, two eastern European cities with high SO 2 concentrations. At the same time approximately 230 asthmatic children and adults were followed by daily peak flow measurements and symptom diaries. Despite the high SO 2 pollution with daily averages of 62 μg m −3 and peaks of nearly 500 jug m −3 , the mean SO 4 −2 concentration for Erfurt, 9.8 μg m −3 , and that for Sokolov, 8.9 μg m −3 , were moderate. In addition, the average H + concentration over the entire period was low in Erfurt (0.3 μg m −3 , H + as H 2 SO 4 ) as well as in Sokolov (0.4 μg m −3 ). H + concentrations tended, however, to be higher during the cold winter months and the hot summer months. In Sokolov, there were three episodes in the winter when measurements exceeded 3.5 μg m −3 . The measurements suggest that the environmental conditions in Erfurt and Sokolov produce a low SO 2 oxidation rate and have a high H 2 SO 4 neutralization potential. Daily PM 10 concentrations (59.90 μg m −3 in Erfurt and 53.8 μg m −3 in Sokolov) were in the same range for both cities with higher values in winter. Of note was the decrease of all pollutants during the period of observation.

Outdoors, in the absence of direct sunlight, significant concentrations of the nitrate radical can result from the reaction between ozone and nitrogen dioxide. The nitrate radical will rapidly equilibrate with dinitrogen pentaoxide, and both of these species can further react to produce nitric acid. We suggest that similar chemistry can occur indoors. A recent study supports this suggestion. Measurements of acidic species in the Boston area indicate significant indoor sources of nitric acid during the summer, but not during the winter

Background National levels of personal health-care access and quality can be approximated by measuring mortality rates from causes that should not be fatal in the presence of effective medical care (ie, amenable mortality). Previous analyses of mortality amenable to health care only focused on high-income countries and faced several methodological challenges. In the present analysis, we use the highly standardised cause of death and risk factor estimates generated through the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) to improve and expand the quantification of personal health-care access and quality for 195 countries and territories from 1990 to 2015. Methods We mapped the most widely used list of causes amenable to personal health care developed by Nolte and McKee to 32 GBD causes. We accounted for variations in cause of death certification and misclassifications through the extensive data standardisation processes and redistribution algorithms developed for GBD. To isolate the effects of personal health-care access and quality, we risk-standardised cause-specific mortality rates for each geography-year by removing the joint effects of local environmental and behavioural risks, and adding back the global levels of risk exposure as estimated for GBD 2015. We employed principal component analysis to create a single, interpretable summary measure-the Healthcare Quality and Access (HAQ) Index-on a scale of 0 to 100. The HAQ Index showed strong convergence validity as compared with other health-system indicators, including health expenditure per capita (r= 0.88), an index of 11 universal health coverage interventions (r= 0.83), and human resources for health per 1000 (r= 0.77). We used free disposal hull analysis with bootstrapping to produce a frontier based on the relationship between the HAQ Index and the Socio-demographic Index (SDI), a measure of overall development consisting of income per capita, average years of education, and total fertility rates. This frontier allowed us to better quantify the maximum levels of personal health-care access and quality achieved across the development spectrum, and pinpoint geographies where gaps between observed and potential levels have narrowed or widened over time. Findings Between 1990 and 2015, nearly all countries and territories saw their HAQ Index values improve; nonetheless, the difference between the highest and lowest observed HAQ Index was larger in 2015 than in 1990, ranging from 28.6 to 94.6. Of 195 geographies, 167 had statistically significant increases in HAQ Index levels since 1990, with South Korea, Turkey, Peru, China, and the Maldives recording among the largest gains by 2015. Performance on the HAQ Index and individual causes showed distinct patterns by region and level of development, yet substantial heterogeneities emerged for several causes, including cancers in highest-SDI countries; chronic kidney disease, diabetes, diarrhoeal diseases, and lower respiratory infections among middle-SDI countries; and measles and tetanus among lowest-SDI countries. While the global HAQ Index average rose from 40.7 (95% uncertainty interval, 39.0-42.8) in 1990 to 53.7 (52.2-55.4) in 2015, far less progress occurred in narrowing the gap between observed HAQ Index values and maximum levels achieved; at the global level, the difference between the observed and frontier HAQ Index only decreased from 21.2 in 1990 to 20.1 in 2015. If every country and territory had achieved the highest observed HAQ Index by their corresponding level of SDI, the global average would have been 73.8 in 2015. Several countries, particularly in eastern and western sub-Saharan Africa, reached HAQ Index values similar to or beyond their development levels, whereas others, namely in southern sub-Saharan Africa, the Middle East, and south Asia, lagged behind what geographies of similar development attained between 1990 and 2015. Interpretation This novel extension of the GBD Study shows the untapped potential for personal health-care access and quality improvement across the development spectrum. Amid substantive advances in personal health care at the national level, heterogeneous patterns for individual causes in given countries or territories suggest that few places have consistently achieved optimal health-care access and quality across health-system functions and therapeutic areas. This is especially evident in middle-SDI countries, many of which have recently undergone or are currently experiencing epidemiological transitions. The HAQ Index, if paired with other measures of health-systemcharacteristics such as intervention coverage, could provide a robust avenue for tracking progress on universal health coverage and identifying local priorities for strengthening personal health-care quality and access throughout the world., Bill & Melinda Gates Foundation.

Summary Background In September, 2015, the UN General Assembly established the Sustainable Development Goals (SDGs). The SDGs specify 17 universal goals, 169 targets, and 230 indicators leading up to 2030. We provide an analysis of 33 health-related SDG indicators based on the Global Burden of Diseases, Injuries, and Risk Factors Study 2015 (GBD 2015). Methods We applied statistical methods to systematically compiled data to estimate the performance of 33 health-related SDG indicators for 188 countries from 1990 to 2015. We rescaled each indicator on a scale from 0 (worst observed value between 1990 and 2015) to 100 (best observed). Indices representing all 33 health-related SDG indicators (health-related SDG index), health-related SDG indicators included in the Millennium Development Goals (MDG index), and health-related indicators not included in the MDGs (non-MDG index) were computed as the geometric mean of the rescaled indicators by SDG target. We used spline regressions to examine the relations between the Socio-demographic Index (SDI, a summary measure based on average income per person, educational attainment, and total fertility rate) and each of the health-related SDG indicators and indices. Findings In 2015, the median health-related SDG index was 59·3 (95% uncertainty interval 56·8–61·8) and varied widely by country, ranging from 85·5 (84·2–86·5) in Iceland to 20·4 (15·4–24·9) in Central African Republic. SDI was a good predictor of the health-related SDG index (r2=0·88) and the MDG index (r2=0·92), whereas the non-MDG index had a weaker relation with SDI (r2=0·79). Between 2000 and 2015, the health-related SDG index improved by a median of 7·9 (IQR 5·0–10·4), and gains on the MDG index (a median change of 10·0 [6·7–13·1]) exceeded that of the non-MDG index (a median change of 5·5 [2·1–8·9]). Since 2000, pronounced progress occurred for indicators such as met need with modern contraception, under-5 mortality, and neonatal mortality, as well as the indicator for universal health coverage tracer interventions. Moderate improvements were found for indicators such as HIV and tuberculosis incidence, minimal changes for hepatitis B incidence took place, and childhood overweight considerably worsened. Interpretation GBD provides an independent, comparable avenue for monitoring progress towards the health-related SDGs. Our analysis not only highlights the importance of income, education, and fertility as drivers of health improvement but also emphasises that investments in these areas alone will not be sufficient. Although considerable progress on the health-related MDG indicators has been made, these gains will need to be sustained and, in many cases, accelerated to achieve the ambitious SDG targets. The minimal improvement in or worsening of health-related indicators beyond the MDGs highlight the need for additional resources to effectively address the expanded scope of the health-related SDGs. - Abstract
Although the attention of the world and the global health community specifically is deservedly focused on the COVID-19 pandemic, other determinants of health continue to have large impacts and may also interact with COVID-19. Air pollution is one crucial example. Established evidence from other respiratory viruses and emerging evidence for COVID-19 specifically indicates that air pollution alters respiratory defense mechanisms leading to worsened infection severity. Air pollution also contributes to co-morbidities that are known to worsen outcomes amongst those infected with COVID-19, and air pollution may also enhance infection transmission due to its impact on more frequent coughing. Yet despite the massive disruption of the COVID-19 pandemic, there are reasons for optimism: broad societal lockdowns have shown us a glimpse of what a future with strong air pollution measures could yield.
