Purpose: This network meta-analysis (NMA) of randomized controlled trials (RCTs) aimed to identify effective initial conservative treatment strategies for patients with temporomandibular joint disorders (TMD)., Study Selection: RCTs comparing treatment options for TMD published between January 2000 and July 2021 were retrieved from the databases of PubMed and Embase via a comprehensive electronic search. Patients diagnosed with myalgia (muscle pain) or arthralgia (joint pain) according to pain-related Diagnostic Criteria for Temporomandibular Disorders (DC/TMD) and the Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD) were eligible for inclusion. Twelve treatment options and a placebo were included in the mutual comparisons. The risk of bias was assessed using Risk of Bias 2.0. Forest plots of direct comparisons between individual studies were created using MetaInsight. NMA was performed using R statistical software (netmeta)., Results: Twenty-four RCTs involving 1336 patients assessing pain and 12 RCTs involving 614 patients assessing maximal mouth opening were identified. Low-level laser therapy (standard mean difference [SMD]: -2.12, 95% confidence interval [CI]: -3.18, -1.06), self-exercise (SMD: -1.51, 95% CI: -2.82, -0.2), and stabilization splints (SMD: -1.16, 95% CI: -2.02, -0.29) were effective in improving pain; however, the certainty of evidence was very low. Self-exercise (SMD: 0.71, 95% CI: -0.58, 2.01), stabilization splints (SMD: 0.65, 95% CI: -0.09, 1.39), and low-level laser therapy (SMD: 0.63, 95% CI: -0.34, 1.6) were effective in improving maximal mouth opening; however, the certainty of evidence was very low., Conclusions: Stabilization splints, self-exercise, and low-level laser therapy may be effective in the initial treatment of TMD.

Cervical auscultation is a simple, noninvasive method for diagnosing dysphagia, although the reliability of the method largely depends on the subjectivity and experience of the evaluator. Recently developed methods for the automatic detection of swallowing sounds facilitate a rough automatic diagnosis of dysphagia, although a reliable method of detection specialized in the peculiar feature patterns of swallowing sounds in actual clinical conditions has not been established. We investigated a novel approach for automatically detecting swallowing sounds by a method wherein basic statistics and dynamic features were extracted based on acoustic features: Mel Frequency Cepstral Coefficients and Mel Frequency Magnitude Coefficients, and an ensemble learning model combining Support Vector Machine and Multi-Layer Perceptron were applied. The evaluation of the effectiveness of the proposed method, based on a swallowing-sounds database synchronized to a video fluorographic swallowing study compiled from 74 advanced-age patients with dysphagia, demonstrated an outstanding performance. It achieved an F1-micro average of approximately 0.92 and an accuracy of 95.20%. The method, proven effective in the current clinical recording database, suggests a significant advancement in the objectivity of cervical auscultation. However, validating its efficacy in other databases is crucial for confirming its broad applicability and potential impact.

Purpose: Botulinum neurotoxin (BoNT) is a biological toxin produced by Clostridium botulinum. BoNT is a potent toxin extensively used in therapeutic interventions. This review provides an updated overview of the mechanisms of action and clinical applications of BoNT in head and facial region., Study Selection: MEDLINE/PubMed searches were conducted using the terms "botulinum neurotoxin" and "dentistry" along with a combination of other related terms. In addition, studies were manually selected from reference lists of the selected articles., Results: The Food and Drug Administration in the United States initially approved BoNT to treat strabismus, blepharospasm, and hemifacial spasms. The use of BoNT in dermatology and cosmetics has been widely established and has created a revolution in these fields. Over the years, its applications in various medical specialties have expanded widely. Owing to its safety, efficacy, and long duration of action, it is well-accepted by patients. BoNT/A and BoNT/B are widely used in clinical practice. Several off-label uses of BoNT in the dental fraternity have yielded promising results. We have elaborated on the speculated mechanism of action, dosage, effective sites of injection, and adverse effects of each therapeutic application. The various clinical indications for BoNT include bruxism, myofascial pain, temporomandibular joint dislocation, hemifacial pain, orofacial dystonia, facial paralysis, chronic migraine, and trigeminal neuralgia., Conclusions: BoNT is a safe treatment that can be used effectively, provided that the clinician has adequate knowledge regarding the mechanism, injection techniques, and local and systemic side effects and that it is administered cautiously and purposefully.

Few standards and guidelines to prevent health problems have been associated with tablet use. We estimated the effects of posture and tablet tilt angle on muscle activity and posture in healthy young adults. Seventeen healthy young adults (age: 20.5 ± 3 years) performed a cognitive task using a tablet in two posture (sitting and standing) and tablet tilt angle (0 degrees and 45 deg) conditions. Segment and joint kinematics were evaluated using 16 inertial measurement unit sensors. Neck, trunk, and upper limb electromyography (EMG) activities were monitored using 12 EMG sensors. Perceived discomfort, kinematics, and EMG activities were compared between conditions using the Friedman test. The perceived discomfort in the standing-0 deg condition was significantly higher than in the remaining three conditions. Standing posture and tablet inclination significantly reduced the sagittal segment and joint angles of the spine, compared with sitting and flat tablet conditions. Similarly, standing posture and tablet inclination significantly reduced EMG activities of the dorsal neck, upper, and lower trunk muscles, while increasing EMG activity of shoulder flexors. Standing posture and tablet inclination reduced the sagittal flexion angle, and dorsal neck, upper, and lower trunk muscle activities, while potentially increasing the muscle activity of arm flexors.

Neck pain is one of the most common musculoskeletal complaints. Evidence suggests that increased activities of neck and trunk muscles are one of the mechanisms related to neck pain. Jaw clenching and sitting posture may modulate the muscle activity in neck and trunk muscles during typing. The present study aimed to assess the effects of different postural positions and clenching conditions on neck and trunk muscle activities. Thirteen healthy adults (39.8 ± 5.0 years) performed computer typing tasks in four conditions (two postural positions [upright vs slouched] and two jaw clenching conditions [clenching vs non-clenching]). Integrated surface electromyography (iEMG) was measured in sternocleidomastoid (SCM), upper trapezius (uTP) and middle trapezius (mTP) muscles and compared between conditions. The Friedman and Wilcoxon signed-rank tests with Bonferroni's corrections were used to estimate the condition-specific differences in the iEMG data. The statistical significance level was set at 5%. In both postural positions, iEMG SCM was significantly greater under the jaw clenching than under the non-clenching condition (χ 2  = 21.700, P < .01). Under both jaw clenching conditions, iEMG uTP was significantly greater in the slouched than in the upright postural position (χ 2  = 23.182, P < .01). No significant differences in iEMG mTP were seen across conditions (χ 2  = 5.018, P = .10). Purpose: Rhythmic masticatory muscle activity (RMMA), a biomarker of sleep bruxism (SB), has been associated with mild hypoxia and/or big breaths in some adults with non-sleep-disordered breathing. The purpose of this study was to investigate that concurrent oxygen and carbon dioxide fluctuations are among the physiological variables that contribute to RMMA onset., Methods: Twelve subjects (5 female, 7 male, mean age: 43 ± 11) underwent polysomnography recording in a sleep laboratory. RMMA index and apnea-hypopnea index were calculated. Oxygen saturation (SpO 2 ) was estimated by finger pulse oximeter and end-tidal CO 2 (ETCO 2 ) by nasal airflow cannula before and after RMMA onset. Given the expected response time delay between actual arterial hypoxemia and fingertip pulse detection, we adjusted the SpO 2 desaturation onset to the onset of masseter muscle activity using a 17 s criterion based on ETCO 2 shifts., Results: SpO 2 was slightly but significantly lower than at baseline (max: -0.6%) in the 6-4 s before RMMA onset and significantly higher in the 6-18 s after onset (0.9%; p < 0.05). Although ETCO 2 before RMMA onset did not differ from baseline, it decreased at 8-10 s after onset (-1.7 mmHg: p < 0.05). No changes in SpO 2 or ETCO 2 in relation to RMMA onset reached a critical clinical threshold., Conclusions: The mild transient hypoxia observed before RMMA onset was not associated with a change in ETCO 2. The mild and brief oxygen fluctuations before RMMA onset may reflect a physiological response that seems to have little influence on SB genesis., (Copyright © 2019 Japan Prosthodontic Society. Optimizing interfacial contact between graphene and a semiconductor has often been proposed as essential for improving their charge interactions. Herein, we fabricated bismuth vanadate-reduced graphene oxide (BiVO 4 /rGO) composites with tailored interfacial contact extents and revealed their disparate behavior in photoelectrochemical (PEC) and powder suspension (PS) water oxidation systems. BiVO 4 /rGO with a high rGO coverage on the BiVO 4 surface (BiVO 4 /rGO HC) exhibited an 8-fold enhancement in the PEC photocurrent density with respect to neat BiVO 4 at 0 V versus Ag/AgCl, while BiVO 4 /rGO with a low rGO coverage (BiVO 4 /rGO LC) gave a lesser 3-fold enhancement. In contrast, BiVO 4 /rGO HC delivered a detrimental effect, while BiVO 4 /rGO LC exhibited an enhanced performance for oxygen evolution in the PS system. The phenomenon is attributed to changes in the hydrophobicity of the BiVO 4 /rGO composite in conjunction with the interfacial contact configuration. A better BiVO 4 /rGO interfacial contact was found to improve the charge separation efficiency and charge transfer ability of the composite material, explaining the superior PEC performance of BiVO 4 /rGO HC. Additionally, optimization of the interfacial contact extent was revealed to further improve the energetics of the composite material, as evidenced by a Fermi level shift to a more negative potential. However, the high hydrophobicity of BiVO 4 /rGO HC arising from the higher rGO reduction extent triggered poor water miscibility, reducing the surface wettability and therefore hampering the photocatalytic O 2 evolution activity of the sample. The study underlines water miscibility as a governing issue in the PS system.

This work reports the template-free fabrication of mesoporous Al 2 O 3 nanospheres with greatly enhanced textural characteristics through a newly developed post-synthesis "water-ethanol" treatment of aluminium glycerate nanospheres followed by high temperature calcination. The proposed "water-ethanol" treatment is highly advantageous as the resulting mesoporous Al 2 O 3 nanospheres exhibit 2-4 times higher surface area (up to 251 m 2 g -1 ), narrower pore size distribution, and significantly lower crystallization temperature than those obtained without any post-synthesis treatment. To demonstrate the generality of the proposed strategy, a nearly identical post-synthesis "water treatment" method is successfully used to prepare mesoporous monometallic (e.g., manganese oxide (MnO 2 )) and bimetallic oxide (e.g., CuCo 2 O 4 and MnCo 2 O 4 ) nanospheres assembled of nanosheets or nanoplates with highly enhanced textural characteristics from the corresponding monometallic and bimetallic glycerate nanospheres, respectively. When evaluated as molybdenum (Mo) adsorbents for potential use in molybdenum-99/technetium-99m ( 99 Mo/ 99m Tc) generators, the treated mesoporous Al 2 O 3 nanospheres display higher molybdenum adsorption performance than non-treated Al 2 O 3 nanospheres and commercial Al 2 O 3 , thereby suggesting the effectiveness of the proposed strategy for improving the functional performance of oxide materials. Study Design: This is a retrospective cohort study., Background: Hemimetameric segmental shift (HMMS) is defined as a hemivertebral deformation in which 2 or more hemivertebrae exist on both sides of the spine and are separated by at least 1 normal vertebra. Reports of HMMS are rare and based on simple anterior x-ray images. No reports have used 3-dimensional computed tomography (3D-CT) to analyze both the anterior and posterior elements. The objective of this study was to analyze the morphology and clinical features of HMMS 3 dimensionally., Methods: HMMS was confirmed in 32 (6.6%, 16 males and 16 females) of 483 patients diagnosed with congenital scoliosis at the study institution between 1998 and 2013. The average age at the first visit was 6 years and 3 months. 3D-CT imaging was performed for 30 patients older than 2 years (average age: 9 y and 8 mo) and used to classify cases according to posterior elements., Results: With regard to the number of hemivertebrae present, 21 patients had 2 hemivertebrae, 7 patients had 3 hemivertebrae, and 2 patients had 4 hemivertebrae. Patients with 2 hemivertebrae predominantly had hemivertebrae in the thoracolumbar spine. Patients were classified into 2 categories: malformation existing at an equal level in anterior and posterior sides (unison HMMS) and malformation existing at nonequal levels (discordant HMMS). Nine patients had unison HMMS and all of them had 2 hemivertebrae (average: 4.6 vertebrae). Twenty-one patients had discordant HMMS, with 12 having 2 hemivertebrae, 7 having 3 hemivertebrae, and 2 having 4 hemivertebrae., Conclusions: Through 3D-CT analysis, HMMS was categorized as unison or discordant. Discordant HMMS was observed in 21 of 30 (70%) patients and in all patients with >3 hemivertebrae. Diagnosing HMMS, whether unison or discordant, is clinically important and should be done with careful analysis of bone models and/or radiologic images to determine the correct spinal levels., Level of Evidence: Level IV-diagnostic study.

Introduction: The purpose of this study is to demonstrate the clinical characteristics and thoracic factors such as sternal tilt angle and Haller index in patients with idiopathic or syndromic scoliosis associated with pectus excavatum., Methods: We performed a retrospective review on a cohort of 70 patients (37 males and 33 females) diagnosed with idiopathic and syndromic scoliosis associated with pectus excavatum between 1985 and 2014. We investigated age, location and Cobb angle of the main curve, and thoracic factors including sternal deviation and tilting angle and Haller index using radiographs and computed tomography of the chest., Results: Patients' mean age at the first visit to our hospital was 10.3 years (1-18 years old). There were 41 patients with idiopathic scoliosis and 29 with syndromic scoliosis. Main curve locations were thoracic in 52 patients, thoracolumbar in 10, and lumbar in 8. The mean Cobb angle of the main curve was 45.0 degrees (11-109 degrees). The sternum was displaced on the left side in 72% of patients, central in 23%, and right in 5%. Mean sternal tilt angle was 12.4 degrees (2.3-34 degrees), and mean Haller index score was 4.9 (2.9-9.2). There was no significant correlation between Cobb angle and sternal tilt angle/Haller index. However, a significant difference was found between sternal tilt angle and Haller index., Conclusion: Most patients with both scoliosis and pectus excavatum have left side deviated sternum and a higher Haller index score; therefore this can negatively impact cardiac function. Prone positioning and the corrective force applied during scoliosis surgery as well as thoracic compression during cast or brace treatment may have a negative effect on cardiac function in these patients., Competing Interests: Conflicts of Interest: The authors declare that there are no conflicts of interest.

The aim of this study was to determine the validity of peer evaluation for team-based learning (TBL) classes in dental education in comparison with the term-end examination records and TBL class scores. Examination and TBL class records of 256 third- and fourth-year dental students in six fixed prosthodontics courses from 2013 to 2015 in one dental school in Japan were investigated. Results of the term-end examination during those courses, individual readiness assurance test (IRAT), group readiness assurance test (GRAT), group assignment projects (GAP), and peer evaluation of group members in TBL classes were collected. Significant positive correlations were found between all combinations of peer evaluation, IRAT, and term-end examination. Individual scores also showed a positive correlation with group score (total of GRAT and GAP). From the investigation of the correlations in the six courses, significant positive correlations between peer evaluation and individual score were found in four of the six courses. In this study, peer evaluation seemed to be a valid index for learning performance in TBL classes. To verify the effectiveness of peer evaluation, all students have to realize the significance of scoring the team member's performance. Clear criteria and detailed instruction for appropriate evaluation are also required.

Aims: To explore whether traumatic brain injury (TBI) patients have a higher prevalence of sleep bruxism (SB) and a higher level of orofacial muscle activity than healthy controls and whether orofacial muscle activity in the context of mild TBI (mTBI) increases the risk for headache disability., Methods: Sleep laboratory recordings of 24 mTBI patients (15 males, 9 females; mean age ± standard deviation [SD]: 38 ± 11 years) and 20 healthy controls (8 males, 12 females; 31 ± 9 years) were analyzed. The primary variables included degree of headache disability, rhythmic masticatory muscle activity (RMMA) index (as a biomarker of SB), and masseter and mentalis muscle activity during quiet sleep periods., Results: A significantly higher prevalence of moderate to severe headache disability was observed in mTBI patients than in controls (50% vs 5%; P = .001). Although 50% and 25% of mTBI patients had a respective RMMA index of ≥ 2 episodes/hour and ≥ 4 episodes/hour, they did not present more evidence of SB than controls. No between-group differences were found in the amplitude of RMMA or muscle tone. Logistic regression analyses suggested that while mTBI is a strong predictor of moderate to severe headache disability, RMMA frequency is a modest but significant mediator of moderate to severe headache disability in both groups (odds ratios = 21 and 2, respectively)., Conclusion: Clinicians caring for mTBI patients with poorly controlled headaches should screen for SB, as it may contribute to their condition.

Study Design: A retrospective study., Objective: To investigate whether proximal junctional kyphosis (PJK) or obvious proximal junctional angle (PJA) changes in the sagittal plane develops following short fusion in children younger than 10 years of age with congenital scoliosis, and to investigate the possible risk factors., Summary of Background Data: PJK following long spinal fusion in adolescents and adults is a serious postoperative complication. Although the same problem may occur in patients with early-onset scoliosis who have undergone spine fusion, few studies have been reported any relationship between PJK and spinal fusion in young children with congenital scoliosis., Materials and Methods: Thirty-seven children treated in a single institution between 1998 and 2010 were reviewed retrospectively. The inclusion criteria included (1) younger than 10 years of age at the time of operation; (2) simple congenital deformity; (3) hemivertebra treated by posterior hemivertebrectomy with short fusion at a maximum of 5 motion segments; and (4) minimum follow-up for 2 years. The PJA from the caudal endplate of the upper instrumented vertebra (UIV) to the cephalad endplate of the vertebra adjacent to the UIV, thoracic kyphosis (T5-T12), lumbar lordosis (T12-S1), global sagittal balance, and magnitude of scoliosis of the major curves and upper compensated curves were measured on lateral radiographs. PJK was defined by a PJA>10 degrees during the follow-up and at least 10 degrees greater than the preoperative or early postoperative measurement. Wilcoxon tests were performed for statistical analysis., Results: PJK occurred in 7 of 37 patients (18.9%), during an average of 4.5±3.2 years of follow-up (2-12 y). The UIV level of children with PJK was on T9 in 4 patients, and T11, T12, and L1 in 1. Screw malposition at UIV was confirmed by postoperative computed tomography images in 6 patients. Only 1 patient with a screw deviation did not develop PJK during the follow-up period. None of the patients with PJK was symptomatic, and no patients required revision surgery because of PJK. PJK occurred and progressed during the first 6 months after surgery followed by almost no progression or slight improvement in patients that could be followed up beyond 6 months postoperatively; in association with an increase of the lumbar lordosis., Conclusions: PJK occurred in pediatric patients with simple congenital deformities following hemivertebrectomy and short fusion. PJK was more common in patients with (1) greater immediately postoperative segmental kyphosis and PJA; (2) screw malposition on the UIV; and (3) hemivertebra located on the lower thoracic or the thoracolumbar region.

Objective: Hypersomnia is frequently reported after mild traumatic brain injury (mTBI), but its cause(s) remain elusive. This study examined sleep/wake activity after mTBI and its association with pain, a comorbidity often associated with insomnia., Methods: Actigraphy recording was performed for 7 ± 2 consecutive days in 56 individuals at one month post-mTBI (64% male; 38 ± 12 years), 24 individuals at one year post-mTBI (58% male; 44 ± 11years), and in 20 controls (50% male; 37 ± 12 years). Pain intensity and its effect on quality of life was assessed with a visual analogue scale and the Short Form Health Survey (SF-36) bodily pain subscale., Results: Overall, few differences in sleep/wake patterns were found between mTBI patients and controls. However, higher percentages of mTBI individuals with moderate-to-severe pain were found to require more than eight hours of sleep per day (37% vs11%; p = 0.04) and to be frequent nappers (defined as those who took three or more naps per week) (42% vs 22%; p = 0.04) compared to those with mild or no pain at one month postinjury. Correcting for age and depression, The SF-36 score was found to be a significant predictor of sleep duration exceeding eight hours per day at one month (odds ratio = 0.95; 95% confidence interval = 0.92-0.99; p = 0.01), but not at one year post-mTBI. Pain and increased sleep need (in terms of hours per day or napping frequency) were found to co-exist in as much as 29% of mTBI patients at one month postinjury., Conclusion: Pain could be associated with more pronounced sleep need in about one-third of mTBI patients during early recovery. Unalleviated pain, found in more than 60% of mTBI patients, should therefore be looked for in all mTBI patients reporting new onset of sleep disorder, not only in those with insomnia., (Copyright © 2016 Elsevier B.V. Purpose: We previously investigated the effects of team-based learning (TBL) on fixed prosthodontic education and reported that TBL could have higher efficiency with high student satisfaction than traditional lecture. In the current report, we introduced flipped classroom to the fixed prosthodontic education and compared their effectiveness based on the final examination score in addition to TBL., Methods: Participants were 41 students from Tokushima University School of Dentistry who attended a fixed prosthodontics course. The first six classes adopted the flipped classroom style while the latter eight classes adopted TBL. To evaluate the relationship between learning styles and their effectiveness, we compared results from the term-end examination between the curriculum covered by flipped classroom and TBL-style classes. To draw comparisons, a referential examination with the same questions was conducted to eight faculty members who had not attended any of these classes., Results: Term-end examination results showed that TBL classes had slightly higher scores than flipped classroom classes. Referential examination results also showed higher scores for the same curriculum and no significant interaction was found between class formats and the term-end and referential examination scores. Analysis revealed no noticeable difference in the effectiveness of the class formats., Conclusion: Our previous study reported that TBL had higher efficiency than traditional style lecture. In the current study, there was no statistical difference in the examination score between flipped classroom and TBL. Therefore, we conclude that both styles are highly effective than traditional style lecture and constitute valid formats for clinical dental education., (Copyright © 2016 Japan Prosthodontic Society. Patient: Observation of attrition patterns suggests that mandibular movement in sleep bruxism (SB) may be associated with current tooth attrition. The aim of this study was to confirm this phenomenon by investigating mandibular movement and masseter muscle activity. The subject was a healthy 21-year-old Japanese male. We recorded biological signals including mandibular movement and masseter electromyograms (EMGs) with a polysomnograph. Based on the EMG using Okura's criteria, SB events were classified into clenching, grinding and mixed types according to mandibular movement criteria. The close-open mandibular movement cycles (CO-cycles) during grinding and mixed type events were selected based on mandibular movement trajectories., Discussion: Fifty-eight CO-cycles were selected in seven grinding and three mixed types. We found that SB mandibular movements associated with current tooth attrition. Excessive lateral movements (ELM) beyond the canine edge-to-edge position were observed in the closing (10.3%) and opening (13.8%) phases of the CO-cycle. Total masseter muscle activity was significantly higher during voluntary grinding (VGR) than during CO-cycle including ELM (working side: P=0.036, balancing side: P=0.025). However, in the middle and late parts of the opening phase, working side masseter muscle activity was significantly higher during CO-cycle including ELM than during VGR (P=0.012). In the early part of the closing phase, balancing side masseter muscle activity was significantly higher during CO-cycle including ELM than during VGR (P=0.017)., Conclusion: These findings suggest that excessive forceful grinding during ongoing SB events may have caused canine attrition in this patient., (Copyright © 2016 Japan Prosthodontic Society. Purpose: We evaluated the effects of gum chewing on the response to psychological stress induced by a calculation task and investigated the relationship between this response and masticatory performance., Methods: Nineteen healthy adult volunteers without dental problems undertook the Uchida-Kraepelin (UK) test (30 min of reiterating additions of one-digit numbers). Before and immediately after the test, saliva samples were collected from the sublingual area of the participants. Three min after the UK test, the participants were made to chew flavorless gum for 3 min, and the final saliva samples were collected 10 min after the UK test. The experiment was performed without gum chewing on a different day. Masticatory performance was evaluated using color-changing chewing gum., Results: Salivary CgA levels at immediately and 10 min after the UK test were compared with and without gum chewing condition. Two-way repeated measures analysis of variance revealed significant interaction between gum chewing condition and changes in CgA levels during post 10 min UK test period. In patients who have lost natural occlusal contacts, the centric relation is usually estimated based on several anatomical factors such as the temporomandibular joint and masticatory muscles except dental arch. The clinical procedure to record the centric relation often depends on the clinician's expertise and technique; an objective method to determine proper occlusal position is desirable. The kinematic axis point (KAP) is kinematically estimated from sagittal movements and is an ideal posterior reference point that is used in dental articulators for reproducing jaw movement. Occlusal registration using the KAP may serve as a definite objective technique. The aim of this study is to investigate the effect of the exclusion range of sagittal jaw movement data from the intercuspal position (ICP) on the estimation of the KAP. The complete and incomplete sagittal border movement data of dentate subjects were used to estimate the KAPs. The locations of the estimated KAPs were compared. The results indicate that the incomplete sagittal border jaw movement data set does not include data points inside a 7 mm distance from the ICP can be used for estimation of the KAP. Numerous studies have demonstrated a correlation between hyaluronan expression and the malignant properties of various kinds of cancer, and inhibition of hyaluronan production causes decreased tumor growth. Hyaluronan oligosaccharides have been shown to inhibit several tumor cell types via disruption of receptor-hyaluronan interaction. However, few studies have addressed hyaluronan with respect to osteosarcoma. In this study, we examined the effects of exogenously added hyaluronan oligosaccharides on tumorigenicity of murine osteosarcoma cells, LM-8, and human osteoblastic osteosarcoma cells, MG-63. Moreover, the critical size of oligomers needed to inhibit malignant properties was defined. Fluorescent hyaluronan oligosaccharides accumulated both on the surface of cells and in the cytoplasm, and this retention was blocked by pretreatment with an anti-CD44 monoclonal antibody. Hyaluronan octasaccharides significantly inhibited cell viability and induced apoptosis as defined by cell proliferation and terminal deoxynucleotidyl transferase dUTP nick-end labeling assays, respectively. Octasaccharides also abrogated functional cell-associated matrices and significantly reduced the retention of endogenous hyaluronan. Further, octasaccharide treatment affected an inhibition of cell motility as well as cell invasiveness. Pretreatment of the cells with anti-CD44 antibody reduced the antitumor effect of the octasaccharides. In vivo, intratumoral injection of hyaluronan octasaccharides reduced the hyaluronan accumulation in local tumors, resulting in significant suppression of the formation of distant lung metastasis. Together these data suggest that hyaluronan oligosaccharides have potent antitumor effects functioning in part by the abrogation of hyaluronan-rich cell-associated matrices.

A 53-year-old man with Behçet's disease was admitted to our hospital for investigation of back and lower abdominal pain. Computed tomography (CT) showed a projecting saccular aneurysm below the right renal artery. We placed a stent-graft just below the right renal artery, successfully excluding the abdominal aortic aneurysm (AAA). His C-reactive protein level and white blood cell count remained elevated after stent-grafting. About 5 months later, he was readmitted with recurrent back and lower abdominal pain and CT showed progression of the AAA. Thus, we performed straight grafting using a woven Dacron graft just below the right renal artery. The patient had an uneventful postoperative course. We discuss the controversial issue of treating AAA in patients with Behçet's disease, focusing on the indications and timing of surgery.

We present a case of retroperitoneal neuroblastoma in a 27-year-old male with extensive bone marrow metastasis at the first presentation. After the simple excision of the tumor, adjuvant multi-drug chemotherapy, consisting of vincristine, actinomycin-D, ifosfamide, doxorubicin, carboplatin and etoposide, was carried out for 17 months, leading to complete remission. Ten months after completion of the chemotherapy, the tumor recurred with bone marrow metastasis. He further developed thoracic vertebral metastases resulting in paraplegia, and died of the disease 41 months after the presentation. The clinical course of this case, with its emphases especially on the effect of the chemotherapy, is described in this report. Since the clinical characteristics and treatment strategies for adult neuroblastoma have not yet been well established, they remain to be investigated in detail.

In a survey of pathologic agents in the wild animals of central Japan, we found a Hepatozoon sp. in the lungs of Japanese black bears in Fukui, Shiga, and Gifu Prefectures, Japan. Histopathologic examination of organs and tissues from the 18 bears inspected showed hepatozoonosis in all. Immature and mature meronts were found in all lobules and between alveoli, with a few found between pleura and in connective tissue. In the lungs, inflammatory cells were not found around meronts, merozoites, or tubercles made of macro-phages including zoites, but inflammatory cells were found around degenerating cells, zoites, and tubercles. A Hepatozoon sp. has not been reported as being detected in bears of any species before.
